Triglycerides and C-peptide are Increased in BMI over than $23kg/m^2$ Diabetic Patients (BMI $23kg/m^2$ 이상의 비만 당뇨병 환자에서 중성지방과 식후 2시간 C-peptide 증가)

  • The purpose of this study to compare of clinical profile between obese and nonobese type 2 diabetic patients. The subjects were consist of 111 obese (50 male, 61 female) and 159 non obese (79 male, 80 female) type 2 diabetic patients underwent fasting blood glucose, 2-hour postprandial blood glucose, $HbA_1c$, total cholesterol, triglyceride, high density lipoprotein, microalbuminuria, fasting C-peptide and 2-hour postprandial C-peptide were measured. Diabetes was diagnosed according to the American Diabetes Association (ADA) criteria. Obesity was defined as body mass index (BMI, kilograms per meters squared) ${\geq}23$. Data analyses were t-test, chisquare test in SAS program. The results were as follows : 1) Triglycerides and 2-hour postprandial C-peptide were significant higher in obese than non-obese patients. 2) Systolic blood pressure, Diastolic blood pressure, fasting blood sugar, 2-hour postprandial blood glucose, $HbA_1c$, total cholesterol, high-density lipoprotein, microalbuminuria and fasting C-peptide were no difference between obese and non-obese groups. These data indicate that obesity is a risk factor for the development of coronary heart disease (CHD) in diabetic patients. Therefore, weight reductions have beneficial effects on insulin action and glycemic control in obese type 2 diabetic patients.

  • Aim To evaluate blood pressure, blood glucose and serum lipid level in obese and nonobese type 2 diabetic patients. Methods 206 obese(76 male, 130 female) and 442 nonobese(208 male, 234 female) type 2 diabetic patients underwent fasting blood glucose, 2-hour postprandial blood glucose, $HbA_1c$ total cholesterol, triglyceride, high density lipoprotein, microalbuminuria, blood urea nitrogen, creatinine and C-peptide were measured. Diabetes was diagnosed according to the American Diabetes Association(ADA)criteria. Obesity was defined as body mass index(BMI, kilograms per meters squared)${\geq}25$. Results In male, systolic blood pressure, triglycerides, microalbuminuria and C-peptide were significant higher in obese than nonobese patients. Fasting blood glucose were significantly lower in obese than nonobese patients. Diastolic blood pressure, 2-hour postprandial blood glucose, $HbA_1c$, total cholesterol, high density lipoprotein, blood urea nitrogen, and creatinine were no difference between 2 groups. In female, triglycerides and C-peptide were significant higher in obese than nonobese patients, Blood pressure, fasting blood glucose, 2-hour postprandial blood glucose, $HbA_1c$, total cholesterol, high density lipoprotein, microalbuminuria, blood urea nitrogen, and creatinine were no difference between 2 groups. Conclusion Our present study supports that increased triglycerides play a major role in increasing the risk of coronary heart disease(CHD) in obese women type 2 diabetic patients.

A Study on Food Intake and Family Support in Patients with Coronary Artery Disease (관상동맥질환자의 식품섭취와 가족지지)

  • Purpose: This study was to examine the food intake and family support of patients with Coronary Artery Disease(CAD). Methods: 90 hospitalized patients who received CAD medical treatment were randomly selected. The food intake frequency and family support instrument were utilized. Collected data was analyzed by SPSS/WIN 14.0: t-test, $x^2$-test and Pearson's correlation coefficient. Results: The mean score of food intake frequency was 5.65(total mean score 97.50) which was relatively healthy food intake based on the CAD treatment guidelines. Undesirable food intake items were fruit, fish, milk etc. The mean score of family support was 2.44(range: 1 - 4). Food intake showed a significant(p < .001) positive correlation with family support. There was significant difference(p < .05) on family support according to exercise. There was significant difference(p < .05) on LDL level, one of the CAD risk factors, between first admission patients and readmission patients. Conclusion: These findings suggest that nursing intervention education programs should be developed to appropriately care for CHD patients for needed changes.

  • Complex breakup behavior in the revival wave has been observed in the Belousov-Zhabotinsky(BZ) reaction system containing 1,4-cyclohexanedione (1,4-CHD) in the dish divided into two compartments with a sliding window. A same reaction mixture is poured into the two compartments individually with time difference. Wave propagation exhibited different behavior in the revival wave of the reaction system. This was largely dependent on the progress time prior to the pouring into each compartment and on the gap between the times of pouring into the two compartments. The revival wave in the reaction system is induced spontaneously as a new wave train with a long time lag after the disappearance of the initially induced wave. A thoroughgoing study of the chaotic breakup of propagating chemical wave train was to be possible since the revival wave has a longer wavelength, clearer wave-train patterns, and longer duration period.

  • 1. Objective The purpose of this study was to prove the correlation between risk factor of cardiovascular disease and sasang constitution. 2. Methods We analyzed the data of 200 patients who visited Cheonan oriental medical hospital from Apr 1, 2007 until September 31, 2007, for medical examination, which includes sasang constitution examination, blood chemistry test and physical examination. And we classified their results into 3 groups based on with NCEP-ATP III diagnostic criterions. 3. Results and Conclusion The results were as follows : 1. The frequency of dyslipoproteinemia(Hypertriglycemia) was significantly higher in Teaumin group than other constitutions. 2. The frequency of Metabolic syndrome was significantly higher in Taeumin group than other constitutions. 3. The risk of coronary heart disease was higher in Teaumin than other constitutions.

Pattern and Management of Dyslipidemia in Type 2 Diabetes Patients in Korea (제 2형 당뇨환자에서 지질이상 유형 및 관리)

  • Dyslipidemia is an important CHD risk factor in diabetic patients. We conducted this study to assess the pattern of dyslipidemia in type 2 diabetes patients, to examine the demographic and clinical factors associated with dyslipidemia and to evaluate attaining within the lipid target goals and treatment strategies. A retrospective analysis was conducted among patents diagnosed type 2 diabetes at outpatient clinic in endocrinology between January 2003 and December 2004. Clinical history and physical examination were reviewed and laboratory data including blood glucose, HbAlc, lipid levels were recorded sequentially at least 1 year. In 882 patients with type 2 diabetes, 437 patients (49.6%) have dyslipidemia and 73% of them (319 patients) received lipid-lowering agents. 244 patients (94 males, 150 females, mean age 60 years old) were susceptible to analyses. The most frequent pattern of dyslipidemia is high LDL level and high TG levels (28%). Metabolic syndrome and macrovascular complication were significant negative independent association with lipid levels within the target goals (p<0.05). Only 15.2% (19 males, 18 females) attained within the lipid tar- get goals. Patients with diabetic dyslipidemia need maximization of lipid-lowering agents, increasing the fibric acid derivatives prescription and the effort to correction of low HDL and/or high TG.

  • To estimate the incidence rate of coronary heart disease in Korea, of all residents in Taegu city aged 25 or above, those who had an acute MI or a fatal coronary event between 1 July 1996 and 30 June 1997 were registered. Seven hundreds and eight patients were registered during the study period(685 were identified at hospital and 23 were autopsy cases). Age-standardized annual incidence rate of men in city area was 93 per 100,000(95% CI; 61-142) and 35% CI; 16-67) in women(100 in men and 20 in women aged 35-64). The incidence was rapidly increased after age 40 in men, however, in women after age 60. Twenty-eight-days case fatality rate was 45% in men and 47% in women. However, in the age group of 45-59 case fatality rate in women was two times higher than that in men. In conclusion, crude annual incidence rate of CHD in city area was 73 per 100,000 in men and 33 in women. The age-standardized annual incidence of CHD in men(93 per 100,000) was 2 times higher than that in women (33 per 100,000) in Korea.

  • A newborn girl with a partial anomalous pulmonary venous connection, coarctation of the aorta, and ventricular and atrial septal defects underwent a complete repair successfully at 49 days of age. In this case, the left upper pulmonary vein was connected to the left innominate vein via an atypical vertical vein.

  • Objective : The purpose of this study is to investigate the current trends of children patients with convulsion. Children convulsion patients who visited the Pediatric Department of the Oriental Medicine hospital were investigated and analyzed. Method : 90 children complaining convulsion who visited the Department of Pediatrics of Kyung Hee University Oriental Medicine Hospital, between March, 2005 and October 2005 were statistically analyzed. Results : The percentage of boys outnumbered the girls. The percentage of children under the age of 5 18 66.7%. The percentage of children with family history is 43%. The majority of children had visited the hospital twice (n=21). As for intercurrent diseases, there were URI, C.P., ADHD, CHD, sequela of encephalitis and etc. As for the result of EEG examinations, 25 children were normal and 18 were abnormal. As for the appearance of seizures, Tonic-Clonic seizure was the most common seizure among the children (n=13). As for the type of convulsion, simple febrile convulsion was the most common (n=35). As for the curative effect. the percentage of children who showed improvement to treatment was 41.2%. Jangdambosintang, sunbangpaedoktang, gamichodeungeunm, gamichungeumchowitang and etc were the most frequently prescribed medicines.

Heart Diseases Prevalence of Elementary School Children in Kyonggi Province (경기도 초등학교 아동의 심장질환 유병률)

  • Objective : The heart diseases are known as a major cause of sudden death, as well as a cause of poor life-quality of school-age children. But there have been few mass screening of heart diseases in these children in Korea. This study was done to estimate the prevalence of heart diseases of these population. Methods : We screened all elementary students(grade 1) in 12 cities and 16 counties(Gun) in Kyonggi province from 1992 to 1955. The first screening was done by auscultation of doctors and simultaneously by checking using 'auto-interpreter of EKG-cardiac sound' (Fukuda Densi ECP 50A). We conducted luther examinations to whom classified as being abnormal condition in first screening, by using EKG, chest x-ray, doppler echocardiograpy(if needed). Results : The total number of examined students was 161,308(92% of the population), the male were 83,238 and female were 78,070. The congenital heart diseases(CHD) patients were 290(18 per 10,000) - male 155(18.6 per 10,000) and female 135(17.3 per 10,000). The most frequent disease was ventricula septal defect(VSD, 45.5%), Atrial septal defect(ASD, 14.8%), Tetralogy of Follot(TOF, 11.7%), and Patent Dutus Arteriosis(PDA, 7.6%) in order. In female, the order was VSD(48.1%), ASD(13.3%), TOF(11.1%), and PDA(10.4%). The total number of EKG abnormality were 433(62.7 per 10,000) among 69,056 screened children in 1995. The complete right bundle branch block(CRBBB) and paroxymal ventricular contraction(PVC) were frequent(26.6%, 26.3% in each), and incomplete right bunddle branch block(IRBBB, 14.6%), paroxymal atrial contraction(PAC, 6.7%), abnormal Q(5.8%), Wolf-Pakinson-White syndrom (5.5%) in order. In female, the most frequent abnormality was PVC(29.8%), and CRBBB(19.9%) in order. Conclusion : We could present the stable prevalence of the rare heart disease. The prevalence of congenital heart diseases was 18.0 per 10,000 and of EKG abnormality was 62.7 per 10,000 among school children.
