Background: Although the survival outcomes of childhood cancer patients have improved, childhood cancer survivors suffer from various degrees of immune dysfunction or delayed immune reconstitution. This study aimed to investigate the effect of Korean Red Ginseng (KRG) on T cell recovery in childhood cancer patients who underwent autologous hematopoietic stem cell transplantation (ASCT) from the perspective of inflammatory and senescent phenotypes. Methods: This was a single-arm exploratory trial. The KRG group (n = 15) received KRG powder from month 1 to month 12 post-ASCT. We compared the results of the KRG group with those of the control group (n = 23). The proportions of T cell populations, senescent phenotypes, and cytokine production profiles were analyzed at 1, 3, 6, and 12 months post-ASCT using peripheral blood samples. Results: All patients in the KRG group completed the treatment without any safety issues and showed a comparable T cell repopulation pattern to that in the control group. In particular, KRG administration influenced the repopulation of CD4+ T cells via T cell expansion and differentiation into effector memory cell re-expressing CD45RA (EMRA) cells. Although the KRG group showed an increase in the number of CD4+ EMRA cells, the expression of senescent and exhausted markers in these cells decreased, and the capacity for senescence-related cytokine production in the senescent CD28- subset was ameliorated. Conclusions: These findings suggest that KRG promotes the repopulation of CD4+ EMRA T cells and regulates phenotypical and functional senescent changes after ASCT in pediatric patients with cancer.
연구배경: 외인성 천식은 TH2세포에 의한 매개체의 주요 원인중 하나임이 밝혀지고 있다. 연구자 등은 증상 악화로 내원한 기관지천식 환자와 만성기관지염 환자 사이에 T 림프구 아형의 변화와 싸어토카인 (cytokine)들의 변화에 차이가 있는지 연구하고자 하였다. 방 법: 기관지 천식으로 치료중인 환자 중에서 천식 악화로 내원한 외인성 천식 15예와 만성기관지염 환자 12예, 그리고 정상인 5예를 대상으로 하였다. 환자의 병력과 임상소견, 피부반응검사, 그리고 특이 IgE 측정을 시행하고 단일항체인 CD62L를 이용하여 flow cytometer로 림프구아형을 분석하고 ELISA kit(Quantikine IL-4, IFN-$\gamma$)를 이용하여 IL-4, IFN-$\gamma$을 측정하였다. 결 과: CD4+ T 림프구는 기관지천식환자에서 $40{\pm}7.2%$ 만성기관지염 환자 $43{\pm}19.8%$, 정상인에서 $41{\pm}14%$로 기관지 천식환자와 다른 군간에 의미있는 차이는 없었다(p=0.49, p=0.75). CD62L(L-selectin) 양성 T 림프구의 세포 백분율은 기관지천식환자(n=7) 에서 $24.8{\pm}23.6%$였고, 만성기관지염환자(n=5) $17.0{\pm}16.9%$, 정상인(n=5) $16.7{\pm}16.4%$으로 기관지천식환자와 다른 군간에 의미있는 차이는 없었다(p=0.32, p=0.22). 혈청 IL-4 의 활성도는 기관지천식환자에서 $3.6{\pm}0.9pg/ml$ 만성기관지염 환자 $2.0{\pm}1.2pg/ml$, 정상인에서 $0.7{\pm}1.1pg/ml$로 기관지 천식환자에서 만성기관지염 환자군에 비하여 증가되어 있었으며 (p=0.02) 정상인보다 증가되어 있었다(p=0.006)(Fig. 4). 혈청 INF-$\gamma$의 활성도는 증가되지 않았다. 결 론: 결론적으로 CD62L 양성 T 림프구의 세포 백분율은 기관지천식환자에서 증가되어 있지 않았으며 혈청 IL-4의 활성도는 기관지천식환자에서 증가되어 있었다.
팽연왕겨(ERH)가 혼합된 상토를 이용하여 '설향' 딸기를 육묘하면서 용기 내 수분보유 특성, 그리고 모주 및 자묘의 생장에 미치는 영향을 구명하고자 본 연구를 수행하였다. 용기용수량 상태의 수분 함량은 팽연왕겨가 20-23% 범위로 딸기전용상토 60-66%, 양질사토 30-35% 및 마사토 30-34%와 비교할 때 매우 낮았다. 수분 함량은 관수 8시간 후에 10-12%로 급격히 낮아졌으며 연결포트의 각 셀당 수분편차가 크게 나타났다. ERH와 코이어 더스트(CD), 사양토(SL), 펄라이트(PL) 그리고 마사토(SMM)의 4종류 물질을 혼합하여 조제한 모든 상토는 pH 6.7-7.1, EC $0.03-0.08dS{\cdot}m^{-1}$ 범위로 측정되었고, 관수 20일과 40일 후에 뚜렷한 차이를 보이지 않았다. 자묘의 생육은 혼합상토 중 ERH + SL(55:45, v/v)에서 가장 우수하였다. ERH + CD 상토의 경우 CD의 혼합비율이 30% 이상일 때 생육이 저조한 경향을 보였고, ERH + PE는 혼합 상토 중 지상부와 근권부 생육이 가장 저조하였다. 팽연왕겨가 혼합된 상토에 정식한 모주의 생육은 정식 30일 후 ERH + PE와 ERH + CD 모두 PE + CD(50:50, v/v) 상토보다 저조하였다. 모주정식 90일 후의 생육은 ERH + CD 상토는 CD의 혼합 비율이 높을수록 생육이 우수하였지만 정식 30일 후와 유사한 경향을 보였고, PE + CD(50:50, v/v)보다 저조하였다. 본 연구결과는 팽연왕겨를 포함한 혼합상토 조성과 이를 이용한 육묘과정의 수분 관리에 관한 기초자료로 활용될 수 있을 것으로 기대된다.
Purpose: In this study, we investigated the effects of GDB(Gidaebang) on the immune response to establish the treatment mechanism of vaginitis. Methods: We examined the effects of GDB on the DCs(Dendritic cells) phenotypic and functional maturation. iDCs were cultured in the presence of GM-CSF and the generated iDCs were respectively stimulated by GDB or LPS as the control group for 24 hours. To evaluate the DCs phenotypic and functional maturation, we used flow cytometric analysis, RT-PCR and ELISA. Results: 1. GDB upregulated the expression of class II MHC and CD40 on DCs. 2. GDB upregulated the expression of CD80 and CD86 on DCs. 3. GDB induced cytokine IL-12 production and mRNA expression in DCs. Conclusion: These results suggest that GDB is able to improve the antigen-presenting capacity of DCs through the upregulation of their maturation, and might induce proliferation of T cells. In conclusion, this immunomodulatory properties of GDB may be useful in the treatment of vaginitis.
Purpose: To describe the clinical characteristics of celiac disease (CD) among Saudi children and to determine the adherence rate to gluten free diet (GFD) and its determinant factors among them. Methods: A cross-sectional study was conducted, in which all the families registered in the Saudi Celiac Patients Support Group were sent an online survey. Only families with children 18 years of age and younger with biopsy-confirmed CD were included. Results: The median age of the 113 included children was 9.9 years, the median age at symptom onset was 5.5 years and the median age at diagnosis was 7 years, the median time between the presentation and the final diagnosis was 1 year. Sixty two of the involved children were females. Ninety two percent of the patients were symptomatic at the diagnosis while eight percent were asymptomatic. The commonest presenting symptoms included: chronic abdominal pain (59.3%), poor weight gain (54%), abdominal distention, gases, bloating (46.1%) and chronic diarrhea (41.6%). Sixty percent of the involved children were reported to be strictly adherent to GFD. Younger age at diagnosis and shorter duration since the diagnosis were associated with a better adherence rate. Conclusion: CD has similar clinical presentations among Saudi children compared to other parts of the ward; however, the adherence to GFD is relatively poor. 본 연구에서는 부산물(석분 슬러지, 고로슬래그, 제강슬래그, 폐콘크리트 슬러지, 바텀애시) 5종과 클링커 내 중금속 분석을 위한 레이져 유도 플라즈마 분광법(LIBS) 적용성과 재현성 검토를 실험적으로 수행하였다. 클링커를 포함한 6종의 부산물 시료에 대한 ICP-MS, XRF, LIBS 분석 결과, 정량적 분석에서 ICP와 XRF의 차이가 확인되었으나, LIBS 분석은 표준분석과 일치하지 않고 시료의 정성적인 분석만 가능하였다. LIBS를 통한 정량적 분석을 위하여 Cd, Pb, Hg에 대하여 LIBS 분석 파장(Cd - 214.44nm, Pb - 405.78nm, Hg - 253.65nm)을 설정하였고, 농도별(1~1000ppm) 용액에 함침하여 건조시킨 멤브레인 표면에 레이져를 조사한 결과, 스펙트럼 강도와 농도 사이의 상관관계를 확인할 수 있었다.
본 연구에서는 대심도 풍화대에 시공되는 2차로 도로터널을 기준으로 터널의 굴착방법과 지반보강그라우팅에 의한 지반변위 및 지반응력의 분석을 통하여 최적의 시공방안을 찾고자 하였다. 터널 직경은 약 12m를 적용하였으며, 상부 강관다단그라우팅 및 측벽 보강그라우팅을 적용하였다. 검토된 굴착방법은 링컷 굴착방법과 중벽분할 굴착방법이다. 터널 천단의 소성율과 연직응력비를 분석한 결과, 풍화대지반에 굴착되는 터널에서 토피고(H)가 터널직경(D)의 4배 이상일 경우(H>4D) 대심도 터널로 판단할 수 있었다. 해석결과 중벽분할 굴착방법이 링컷 굴착방법에 비하여 대심도 풍화대 NATM터널에서 안정성이 높은 굴착방법으로 판단할 수 있었다. 또한, 측벽 보강그라우팅 실시로 터널 내공변위 및 침하 발생을 억제할 수 있었다.

Forkhead box P3-positive (Foxp3+)-inducible Tregs (iTregs) are readily generated by TGF-β1 at low TCR signaling intensity. TGF-β1-mediated Foxp3 expression is further enhanced by retinoic acid (RA) and lactoferrin (LF). However, the intensity of TCR signaling required for induction of Foxp3 expression by TGF-β1 in combination with RA and LF is unknown. Here, we found that either RA or LF alone decreased TGF-β1-mediated Foxp3 expression at low TCR signaling intensity. In contrast, at high TCR signaling intensity, the addition of either RA or LF strongly increased TGF-β1-mediated Foxp3 expression. Moreover, decreased CD28 stimulation was more favorable for TGF-β1/LF-mediated Foxp3 expression. Lastly, we found that at high signaling intensities of both TCR and CD28, combined treatment with TGF-β1, RA, and LF induced robust expression of Foxp3, in parallel with powerful suppressive activity against responder T cell proliferation. Our findings that TGFβ/RA/LF strongly generate high affinity Ag-specific iTreg population would be useful for the control of unwanted hypersensitive immune reactions such as various autoimmune diseases.
Mesenchymal stem cells (MSCs) are effective in treating autoimmune diseases and managing various conditions, such as engraftment of allogeneic islets. Additionally, autologous and HLA-matched allogeneic MSCs can aid in the engraftment of human allogeneic kidneys with or without low doses of tacrolimus, respectively. However, HLA alloantigens are problematic because cell therapy uses more HLA-mismatched allogeneic cells than autologous for convenience and standardization. In particular, HLA-mismatched MSCs showed increased Ag-specific T/B cells and reduced viability faster than HLA-matched MSCs. In CRISPR/Cas9-based cell therapy, Cas9 induce T cell activation in the recipient's immune system. Interestingly, despite their immunogenicity being limited to the cells with foreign Ags, the accumulation of HLA alloantigen-sensitized T/B cells may lead to allograft rejection, suggesting that alloantigens may have a greater scope of adverse effects than foreign Ags. To avoid alloantigen recognition, the β2-microglobulin knockout (B2MKO) system, eliminating class-I MHC, was able to avoid rejection by alloreactive CD8 T cells compared to controls. Moreover, universal donor cells in which both B2M and Class II MHC transactivator (CIITA) were knocked out was more effective in avoiding immune rejection than single KO. However, B2MKO and CIITA KO system remain to be controlled and validated for adverse effects such as the development of tumorigenicity due to deficient Ag recognition by CD8 T and CD4 T cells, respectively. Overall, better HLA-matching or depletion of HLA alloantigens prior to cell therapy can reduce repetitive transplantation through the long-term survival of allogeneic cell therapy, which may be especially important for patients seeking allogeneic transplantation.

Alcoholic liver cirrhosis (ALC) is caused by chronic alcohol overconsumption and might be linked to dysregulated immune responses in the gut-liver axis. However, there is a lack of comprehensive research on levels and functions of innate lymphocytes including mucosal-associated invariant T (MAIT) cells, NKT cells, and NK (NK) cells in ALC patients. Thus, the aim of this study was to examine the levels and function of these cells, evaluate their clinical relevance, and explore their immunologic roles in the pathogenesis of ALC. Peripheral blood samples from ALC patients (n = 31) and healthy controls (HCs, n = 31) were collected. MAIT cells, NKT cells, NK cells, cytokines, CD69, PD-1, and lymphocyte-activation gene 3 (LAG-3) levels were measured by flow cytometry. Percentages and numbers of circulating MAIT cells, NKT cells, and NK cells were significantly reduced in ALC patients than in HCs. MAIT cell exhibited increased production of IL-17 and expression levels of CD69, PD-1, and LAG-3. NKT cells displayed decreased production of IFN-γ and IL-4. NK cells showed elevated CD69 expression. Absolute MAIT cell levels were positively correlated with lymphocyte count but negatively correlated with C-reactive protein. In addition, NKT cell levels were negatively correlated with hemoglobin levels. Furthermore, log-transformed absolute MAIT cell levels were negatively correlated with the Age, Bilirubin, INR, and Creatinine score. This study demonstrates that circulating MAIT cells, NKT cells, and NK cells are numerically deficient in ALC patients, and the degree of cytokine production and activation status also changed. Besides, some of their deficiencies are related to several clinical parameters. These findings provide important information about immune responses of ALC patients.
