Leuconostoc mesenteroides를 이용한 브로콜리 발효물에 의한 Clostridium difficile의 생육 제어 (Growth Inhibition of Clostridium difficile by Fermented Broccoli with Leuconostoc mesenteroides)

  • 본 연구에서는 김치로부터 L. mesenteroides CJNU0041을 분리하여 16S rRNA 염기서열 분석을 통해, 동정하였으며, ${\beta}$-glucosidase 활성이 우수한 것으로 나타났다. L. mesenteroides CJNU0041를 이용한 브로콜리 발효 동안 생균수와 pH 및 ${\beta}$-glucosidase 활성에 대해 분석한 결과, 발효 시간은 48 hours이 적당한 것으로 판단되었다. 또한, 발효 후 생물 전환이 일어남을 HPLC 분석을 통해 확인하였다. 그리고, L. mesenteroides CJNU0041의 브로콜리 발효물에서 C. difficile에 대한 생육 억제 효과를 확인 할 수 있었다. 따라서, 본 연구를 통해 분리된 신규 L. mesenteroides CJNU0041을 C. difficile의 생육 제어를 위한 유산균 제재화가 가능할 것으로 판단되며, 브로콜리 발효물도 다양한 식품에 적용이 가능할 것으로 사료된다.

Clostridium difficile in Children: To Treat or Not to Treat?

  • Clostridium difficile infection has been increasing since 2000 in children and in adults. Frequent antibiotics use, comorbidity, and the development of hypervirulent strains have increased the risk of infection. Despite the high carriage rates of C. difficile, infants rarely develop clinical infection. Discontinuing antibiotics and supportive management usually leads to resolution of disease. Antibiotics use should be stratified depending on the patient's age and severity of the disease.

Detection, Characterization and Antibiotic Susceptibility of Clostridioides (Clostridium) difficile in Meat Products

  • Clostridioides (Clostridium) difficile is a Gram (+), anaerobic, spore forming, rod shaped bacterium that can produce toxin. The objective of this study is to reveal the presence of C. difficile in meat products, to analyze the ribotype diversity by PCR and to evaluate the antibiotic susceptibility of isolated strains. The organism was isolated in 22 out of 319 (6.9%) examined meat product samples and 9 out of 22 (40.9%) isolates were identified as RT027 and all isolates had the ability of toxin production. In terms of antibiotic susceptibility, all isolates were susceptive to amoxicillin-clavulanic acid, tetracycline and vancomycin and 21 (95.4%) isolates to metronidazole. On the other hand, imipenem and cefotaxim resistance was observed in all. In conclusion, the results of this comprehensive study conducted in Turkey deduced the presence of C. difficile in different meat products. Therefore, these products can be evaluated as a potential contamination source of C. difficile from animals to humans especially for elders, youngsters, long terms wide spectrum antibiotic used and immuno-suppressed individuals.

투여 항생제군과 Clostridium Difficile-Associated Diarrhea의 위험인자 분석 (Risk Factor Analysis of Clostridium Difficile Associated Diarrhea and Antibiotics Administration)

  • Background: Clostridium difficile is the primary reason of the nosocomial diarrhea. The antimicrobial therapy plays a central role in the pathogenesis of Clostridium difficile associated diarrhea (CDAD). Although nearly all classes of antimicrobial agents have been associated with CDAD, clindamycin and the third-generation cephalosporins have traditionally been considered to the greatest risk factor. Recent studies have also implicated fluoroquinolones as high-risk agents due to increasing use of the agents. This study was to determine the incidence and the risk factors of CDAD related to the administered antibiotics and to assess the therapeutic regimen of metronidazole or vancomycin based on the C. difficile toxin assay Methods: A retrospective study was performed in patients with Clostridium difficile toxin assay at I Hospital (Incheon, South Korea) during the period from January 2007 through December 2007. Administrative, laboratory, and pharmacy data were collected from Electronic Medical Databases. Results: The analysis included 129 reported C.difficile toxin assay results, with 42 positive cases and 87 negative cases. Significant antibiotic risk factors for CDAD included the use of the fourth-generation cephalosporinse (OR=5.97, 95% CI 1.37-25.98, P=0.017). Administration of metronidazole was protective against CDAD (OR=0.30, 95% CI 0.12-0.74, P=0.009). Prolonged antimicrobial therapy has been associated with an increased risk of CDAD. The third-generation cephalosporins (OR=3.81, 95% CI 1.08-13.41, P=0.037) and aminoglycoside (OR=5.50, 95% CI 1.43-21.10, P=0.013) demonstrated greater risk for CDAD over 15 days than 8days or less days of treatment duration. Conclusions: The fourth and third generation cephalosporin, aminglycoside were the significant risk factors compared with other antibiotics, whereas metronidazole appears to be protective. The longer duration of antiobiotic use increased CDAD.

Evaluation of the Usefulness of GDH & Toxin Test for the Diagnosis of Clostridioides difficile in a Tertiary Hospital in Seoul

  • The purpose of this study was to evaluate GDH & Toxin (GDT) tests for the identification of the presence of Clostridioides difficile (C. difficile) as well as to detect whether any toxin was present in the feces of patients suspected of diarrhea associated with C. difficile. Data related to the results of toxin and culture (TC) tests and GDT tests conducted on patients with diarrhea and suspected CDI between January 2017 and august 2018, positive test rates, patient ages and sexes, whether the patients were hospitalized, and turnaround time (TAT) were analyzed retrospectively. Of the 7,554 total tests conducted for CDI diagnosis, 1,010 TC tests (14.9%) were positive, while 92 GDT tests (12.0%) were positive. Of these positive cases, 815 (80.7%) identified through TC test and 80 (87%) identified through GDT test were inpatients. also, among the patients with positive test results, 497 (49.2%) diagnosed through TC test and 45 (48.9%) diagnosed through GDT test were aged 61 years or older. The total time required to complete a TC test was 83.6 hours, while the time required for a GDT test was 11.2 hours, equating to an approximately three-day difference between the two tests. The detection of toxin-producing C. difficile is important in CDI diagnosis, but the commonly used Enzymeimmunoassay (EIA) toxin tests with low sensitivity result in delayed CDI diagnosis time. Therefore, primary screening tests for CDI diagnosis using the GDT method and secondary tests using additional methods are considered most effective.

Synergistic Antimicrobial Effect of Achyranthes japonica Nakai Extracts and Bifidobacterium Supernatants Against Clostridium difficile

  • The synergistic antimicrobial effect of Achyranthes japonica Nakai (AJN) and Bifidobacterium extracellular factors against Clostridium difficile were measured using a turbidity method. Each broth supernatant of Bifidobacterium infantis ($68.8{\pm}0.02%$) and Bifidobacterium adolescentis ($33.2{\pm}0.2%$) obtained by adding ethyl acetate soluble fractionate from A. japonica Nakai ethanolic extracts (AJNEA, 100 ppm, no inhibition) showed high synergistic antimicrobial activity against C. difficile. In addition, the antimicrobial activity in a laboratory medium and yogurt products against C. difficile were evaluated. In yogurt prepared with a starter 5 (Lactobacillus acidophilus: Streptococcus thermophilus: B. adolescentis =1 : 1 : 1) and a starter 4 (L. acidophilus: S. thermophilus: B. infantis=1 : 1 : 1) and 0.5% AJNEA powder, high antimicrobial effects were recorded that measured 79.0 and 65.2%, respectively. The results indicated the potential of AJN extract for use as an antimicrobial agent. In addition, the efficiency of the antimicrobial activity of the extracts was further improved in combination with lactic acid bacteria, which suggests that they have the potential to be used as a highly effective antibiotic-tolerant microorganism prevention system. Such a strategy can be used for alternative drugs or functional food additives for treatment of antibiotic-associated diarrhea.

Beneficial Effect of Bifidobacterium longum ATCC 15707 on Survival Rate of Clostridium difficile Infection in Mice

  • Clostridium difficile infection (CDI) is the main cause of hospital-acquired diarrhea that can cause colitis or even death. The medical-treatment cost and deaths caused by CDI are increasing annually worldwide. New approaches for prevention and treatment of these infections are needed, such as the use of probiotics. Probiotics, including Bifidobacterium spp. and Lactobacillus, are microorganisms that confer a health benefit to the host when administered in adequate amounts. The effect of Bifidobacterium longum ATCC 15707 on infectious disease caused by C. difficile 027 was investigated in a mouse model. The survival rates for mice given the pathogen alone, and with live cells, or dead cells of B. longum were 40, 70, and 60%, respectively. In addition, the intestinal tissues of the B. longum-treated group maintained structural integrity with some degree of damage. These findings suggested that B. longum ATCC 15707 has a function in repressing the infectious disease caused by C. difficile 027.

In Vitro and In Vivo Anti-Clostridioides difficile Effect of a Probiotic Bacillus amyloliquefaciens Strain

  • Clostridioides difficile infection (CDI) is a significant cause of hospital-acquired and antibiotic-mediated intestinal diseases and is a growing global public health concern. Overuse of antibiotics and their effect on normal intestinal flora has increased the incidence and severity of infections. Thus, the development of new, effective, and safe treatment options is a high priority. Here, we report a new probiotic strain, Bacillus amyloliquefaciens (BA PMC-80), and its in vitro/in vivo anti-C. difficile effect as a prospective novel candidate for replacing conventional antibiotics. BA PMC-80 showed a significant anti-C. difficile effect in coculture assay, and its cell-free supernatant (CFS) also exhibited a considerable anti-C. difficile effect with an 89.06 ㎍/ml 50% minimal inhibitory concentration (MIC) in broth microdilution assay. The CFS was stable and equally functional under different pHs, heat, and proteinase treatments. It also exhibited a high sensitivity against current antibiotics and no toxicity in subchronic toxicity testing in hamsters. Finally, BA PMC-80 showed a moderate effect in a hamster CDI model with reduced infection severity and delayed death. However, further studies are required to optimize the treatment condition of the hamster CDI model for better efficacy and identify the antimicrobial compound produced by BA PMC-80.

소아에서 발생한 Clostridium difficile 관련 질환의 역학과 임상양상: 지역사회감염과 원내감염의 비교 (Epidemiology and Clinical Characteristics of Clostridium difficile-associated Disease in Children: Comparison between Community- and Hospital-acquired Infections)

  • 목 적: 최근 소아에서의 지역사회 획득 Clostridium difficile 관련질환(CDAD)은 증가하는 것으로 보고된다. 그러나 아직 소아에서 발생한 CDAD에 대한 정보는 부족한 실정이다. 저자들은 소아 CDAD환자에서 지역사회감염(CA-CDAD)과 병원감염(HA-CDAD)의 역학과 임상양상의 차이를 비교하여 알아보고자 하였다. 방 법: 2008년 4월부터 2010년 3월까지 가천의대 길병원 소아청소년과에 내원한 환자들의 의무기록을 후향적으로 분석하여 소화기계 증상을 가지고 있으면서 C. difficile 독소 A, B 또는 C. difficile 배양검사에서 양성으로 확인된 경우 CDAD로 진단하였다. 결 과: 총 61명(남 32, 여 29)의 환자가 연구에 포함되었고, 평균연령은 3.79${\pm}$4.54세였으며, 1세 미만이 22명(36.1%)으로 가장 많았다. 전체 중 23명(37.7%)의 환자만이 증상 발현 3개월 이내에 항생제를 투여 받은 적이있었다. 총 61명의 환자 중 CA-CDAD군이 41명, HA-CDAD군이 20명이었고, 나이, 성별, 증상, 검사실소견, 회복기간, 합병증 발생 및 재발률은 두 군 간에 유의한 차이가 없었으나 항생제 노출 여부는 HACDAD의 발생과 관련이 있었다. 결 론: 소화기계 증상을 가지고 외래에 내원한 영아와 어린 소아에서 항생제 노출력이 없는 CA-CDAD의 발생이 증가하고 있다. 신속하게 검사를 시행하는 것이 정확한 진단과 치료를 위해 요구된다.

등온증폭법을 이용한 Clostridium difficile 검출 (Detection of Clostridium difficile by Loop-Mediated Isothermal Amplification)

  • 본 연구는 loop-mediated isothermal amplification(LAMP)을 이용하여 Clostridium difficile을 검출하고자 하였다. LAMP 수행을 위하여 선택적인 타깃 유전자로 C. difficile의 16S ribosomal RNA를 타깃으로 하여 primer set를 구성하였다. 5개의 primer set(BIP, FIP, B3, F3, LF, PF)를 이용하여 TcdA와 TcdB toxin이 모두 양성인 균주, TcdA와 TcdB toxin이 모두 음성인 균주와 식품 분리균주를 효과적으로 검출할 수 있었다. LAMP는 80분 이내의 시간이 필요하며 thermocycler와 같은 장비를 필요로 하지 않고 또한 결과를 직접 눈으로 확인할 수 있기 때문에 식품 생산 현장에서 활용될 수 있을 것이라고 생각되었다.
