이하선의 심엽에 위치하며 안면신경의 볼가지를 침범한 타액관 암종 1예 (Salivary Duct Carcinoma in Parotid Deep Lobe, Involving the Buccal Branch of Facial Nerve : A Case Report)

  • Salivary duct carcinoma(SDC) is a highly malignant tumor of the salivary gland. The tumor is clinically characterized by a rapid onset and progression, the neoplasm is often associated with pain and facial paralysis. The nodal recurrence rate is high, and distant metastasis is common. SDC resembles high-grade breast ductal carcinoma. Curative surgical resection and postoperative radiation were the mainstay of the treatment. If facial paralysis is present, a radical parotidectomy is mandatory. Regardless of the primary location of SDC, ipsilateral functional neck dissection is indicated, because regional lymphatic spread has to be expected in the majority of patients already at time of diagnosis. If there is minor gland involvement, a bilateral neck dissection should be performed, because lymphatic drainage may occur to the contralateral side. The survival of SDC patient is poor, with most dying within three years. We experienced a unique case of SDC in parotid deep lobe. We report the clinicopathologic features of this tumor with a review of literature.

발병초기 Bell's Palsy 환자의 안면부 피부전기활동성 차이에 관한 연구 (Differences Between Facial Electrodermal Activities of Paralyzed Side and Those of Normal Side in Acute Stage of Bell's Palsy Patients)

  • Background and purpose: Bell‘s Palsy is a condition that causes the facial muscles to weaken or become paralyzed. It's caused by trauma to the 7th cranial nerve, and is not permanent. The aim of this study is to be convinced of differences between facial electrodermal activities of paralyzed side and those of normal side in acute stage of Bell's Palsy patients Methods: Electrodermal activity (EDA) was performed within 1 week after the onset of facial palsy and facial nerve electromyography (EMG) at 2 weeks after the onset. The recovery of facial nerve function was documented by House and Brackmann grading. All the patients were followed up weekly until recovery or up to 6 weeks. Results: There was significant differences (conductivity A: t=3.319, p=0.002; conductivity C: t=2.699, p=0.010) between facial electrodermal conductivities of paralyzed side and those of normal side in acute stage of Bell's Palsy patients (N=45). And the result showed that logarithmic scale of electrodermal conductivity A value ratio obviousely decreased with logarithmic scale of EMG zygomatic branch amplitude ratio (r=-0.472, p=0.143); logarithmic scale of capacitance B, logarithmic scale of EMG temporal branch amplitude ratio (r=-0.422, p=0.133); logarithmic scale of conductivity C, logarithmic scale of EMG buccal branch amplitude ratio (r=-0.545, p=0.083) (N=12). Conclusion: Electrodermal conductivities increased in paralyzed facial side in acute stage of Bell's Palsy patients.

안면 비대칭환자의 하악골체부의 시상 단면에 관한 연구 (Cross-sectional study of the mandibular body in patients with facial asymmetry)

  • Introduction: To correct the facial asymmetry by mandibular jaw surgery, it is important to know the anatomy of the mandible including the mandibular canal positioning of patients with facial asymmetry. This study was performed to evaluate the differences in the cross-sectional surface in the body of the mandible between the deviated side and opposite side in patients with facial asymmetry. Materials and Methods: The study was conducted on 37 adult patients composed of 2 groups, the asymmetry group (n=20) and non-asymmetry group (n=17). Using the cross-sectional computed tomography (CT) images, the distance from the buccal aspect of the mandibular canal to the outer aspect of the buccal cortex, distance from the buccal aspect of the mandibular canal to the inner aspect of the buccal cortex, distance from the inferior aspect of the mandibular canal to the inferior border of the mandible, thickness of the mandible, and cross-sectional surface area of the mandible were measured in each side of the mandible Results: The cross-sectional area of the mandible including the mandibular canal positioning in the deviated side was not statistically different from the opposite side in the asymmetry group. Only the distance from the inferior aspect of the mandibular canal to the inferior border of the mandible in the ramus area of the deviated side was significantly longer than opposite side. On the other hand, the bucco-lingual width of the asymmetry group was thinner than the non-asymmetry group. Conclusion: The cross-sectional area including the mandibular canal of the mandible did not appear to be modified by the facial asymmetry.

  • Herpes virus family is highly infectious to patients, their families and dentists. The diagnosis of herpes infection is based on the characteristic clinical appearance and the location of the lesions. Herpes Simplex Virus(HSV) usually acquired through direct contact with infected lesions or body fluids, and the prevalence of HSV infection increases progressively from childhood. Primary infections provoke herpetic gingivostomatis typically affects the tongue, lips, gingival, buccal mucosa and palate. Recurrent infections give rise to vesiculo-ulcerative lesions at vermilion border of lip(herpes labialis). In the form of chickenpox, Varicella Zoster Virus(VZV) usually is infected in childhood. VZV spreads in the affected primary afferent nerve to the skin and produces a vesicular rash and pain. Epstein-Barr Virus(EBV) infects B cells and cause infectious mononucleosis. Latent EBV infection has also been implicated in Burkitt lymphoma, nasopharyngeal carcinoma. Cytomegalovirus(CMV) is associated with immune-compromised patient such as organ transplantation and AIDS patients.

상악골 Le Fort I 골절 환자에서 경구 기관 내 삽관 하에서의 악간고정 및 정복: 증례보고 (Intermaxillary Fixation under Oral Intubation in a Patient with Le Fort I Fracture: a Case Report)

  • In order to reduce jaw fracture accompanied by basal skull or nasal fracture, submental intubation could be generally performed. Albeit submental intubation has been widely accepted, it could develop complications such as nerve injury, glandular duct injury, and orocutaneous fistula. Here, we suggest oral intubation for overcoming complications and providing more stable surgical environment in emergency case. Under oral intubation maintaining in retromolar triangle and buccal corridor space, intermaxillary fixation was successfully underwent in 38-years-old female patient with Le Fort I fracture accompanied by pneumocephalus.

  • The neurofibroma in oral cavity is typically associated with neurofibromatosis. The solitary neurofibroma is commonly observed in skin. It is relatively rare in oral cavity and usually observed in the tongue, buccal mucosa, and vestibule. The rare types of solitary neurofibromas have been reported as a case report and they were in the inferior alveolar nerve, infratemporal fossa, maxilla, and palatal ginviva. In our hospital, the presented case was the first case as reported as solitary neurofibroma in the oral cavity. The prognosis after excision and the review of literatures were presented.

악성 타액선 종양 114 예의 임상고찰 (Clinical Analysis of 114 Cases of Malignant Salivary Gland Tumors)

  • The authors reviewed 114 cases of malignant major and minor salivary gland tumors at Presbyterian Medical Center seen from February, 1963 to December, 1983. The results were obtained as follows; 1) Overall male and female sex ratio was 2:1. The peak age of patients with major and minor salivary gland tumor were both 5 th decade. 2) The ratio of benign and malignant tumor was 83:114. The incidence of malignancy in each group was 52% in parotid (50 patients), 75% in minor salivary gland (45 patients), 49% in submaxillary gland(18 patients) and 25% in sublingual gland (1 patient). 3) The incidence according to the anatomic primary site for minor salivary cancers was 10 cases in the nasal cavity, each 8 in the palate and the maxillary antrum, 7 in the tongue, 5 in the gum, 3 in the larynx and 2 in the buccal mucosa. 4) Adenoid cystic carcinoma was the most common cancer of minor salivary gland and malignant mixed tumor was the most common in major salivary glands, each comprising 34 cases (76%) of minor and 19 cases (28%) of major salivary gland tumors. 5) The incidence of cervical lymph node metastasis was 50% in the submaxillary gland cancers, 44% in the parotid gland cancers and 21% in malignant tumors of minor salivary glands. The highest incidence of lymph node metastasis according to histopathological classification was formed in high grade of mucoepidermoid (67%). 6) Nerve invasion was common in mucoepidermoid carcinoma. According to anatomic site, nerve invasion occurred most often in adenoid cystic carcinoma of the submaxillary gland (44%). 7) The lung was the commonest site for distant metastasis comprising 12 cases among 26 cases in which distant spread occurred. 8) The recurrence rate was 50% for major salivary gland cancer and 52% in cancer of the minor salivary gland. In accordance with pathological classification, adenocarcinoma most frequently recurred after excision. This being seen in 88% of patients undergoing definitive therapy. 9) The determinate 5 year survival rate was 78% in major salivary gland tumors, but 69% in minor salivary gland tumors.

Transconjuctival Incision with Lateral Paracanthal Extension for Corrective Osteotomy of Malunioned Zygoma

  • Background: Conventional correction of malunioned zygoma requires complete regional exposure through a bicoronal flap combined with a lower eyelid incision and an upper buccal sulcus incision. However, there are many potential complications following bicoronal incisions, such as infection, hematoma, alopecia, scarring and nerve injury. We have adopted a zygomaticofrontal suture osteotomy technique using transconjunctival incision with lateral paracanthal extension. We performed a retrospective review of clinical cases underwent correction of malunioned zygoma with the approach to evaluate outcomes following this method. Methods: Between June 2009 and September 2015, corrective osteotomies were performed in 14 patients with malunioned zygoma by a single surgeon. All 14 patients received both upper gingivobuccal and transconjunctival incisions with lateral paracanthal extension. The mean interval from injury to operation was 16 months (range, 12 months to 4 years), and the mean follow-up was 1 year (range, 4 months to 3 years). Results: Our surgical approach technique allowed excellent access to the infraorbital rim, orbital floor, zygomaticofrontal suture and anterior surface of the maxilla. Of the 14 patients, only 1 patient suffered a complication-oral wound dehiscence. Among the 6 patients who received infraorbital nerve decompression, numbness was gradually relieved in 4 patients. Two patients continued to experience persistent numbness. Conclusion: Transconjunctival incision with lateral paracanthal extension combined with upper gingivobuccal sulcus incision offers excellent exposure of the zygoma-orbit complex, and could be a valid alternative to the bicoronal approach for osteotomy of malunioned zygoma.

이부에 발생한 신경섬유종 (SOLITARY NEUROFIBROMA OF THE CHIN)

  • 말초 신경 조직에서 발생되는 종양은 신경섬유종, 신경초종, 파립세포종, 악성신경초종 등이 있다. 신경섬유종은 2가지 형태로 분류될 수 있다. 첫째 형태는 단독형 신경섬유종이고, 둘째는 신경섬유종 혹은 Von Recklinghausen's병으로 불리우는데 이는 주로 피부, 신경조직, 뼈, 연조직에 기형을 유발할 수 있고, 선천적이며, 가족력을 나타낸다. 단독형 신경섬유종은 양성이며, 경계는 명확하나 capsule에 의해 싸여 있지 않으며, 피부나 피하조직에 sessile 혹은 pedunculated 형태로 나타난다. 두경부의 어느 부위에나 나타날 수 있으며, 구강내의 호발 부위는 혀, 협점막, 구개 순이다. 단독형 신경섬유종은 방사선에 의한 치료에 효과가 적으며, 재발율이 낮고, 치료는 주로 외과적 절제술로 제거를 하고, 결손부를 삼각흉피판(D-P flap)과 설피판(tongue flap)으로 수복하여 현재까지 합병증 없이 양호한 결과를 얻었다.

콘빔 CT를 이용한 한국인의 하악 절치관 평가 (Assessment of mandibular incisive canal using cone-beam computed tomography in Korean population)

  • Purpose: This study was performed to investigate the characteristics of mandibular incisive canal (MIC) in Korean population. Materials and methods: A total of 97 subjects (60 males and 37 females) who underwent cone-beam computed tomography were included in the study. The anatomic features of MIC was assessed according to gender. Length, diameter and distance to inferior, lingual and buccal border were measured at the origin and the terminal. Also the distribution of MIC at each tooth position was evaluated. Results: Of 97 patients included, 75(77.3%) presented bilateral MIC and 13(13.4%) presented unilateral MIC. Of 194 hamimandibles, MIC was detected in 102(85%) sites in male and 61(82.4%) sites in female. Gender and side showed no statistically significant differences in detectability. The length, diameter and distance to adjacent structures were bigger in male than in female except the distance to lingual border. MIC travelled anteriorly in a slightly downward and lingual direction and usually terminated between the first premolar and the canine. On cross-sectional view, MIC showed individually scattered distribution both buccolingually and superoinferiorly. Conclusion: MIC is well detected with cone-beam computed tomography. Considered that the length and the location of MIC has large variations between individuals, its localization using cone-beam CT is highly recommended before performing surgical procedures such as implant placement and bone harvesting.
