Solitary Papilloma of the Right Upper Lobe Bronchus: Report of a Case and Review of the Literature (우측 폐상엽 기관지에 발생한 고립성 유두종: 1례 치험 및 문헌 고찰)

  • Papilloma of the upper respiratory tract, particularly larynx and vocal cords are relatively common disease. However, solitary papilloma of the bronchus is extremely rare condition and only a handful cases were reported in the literature. The patient is a 39 year old housewife who has been suffering from productive cough and occasional hemoptysis in the past one year. X-ray of the chest showed complete atelectasis of the right upper lobe. Bronchography revealed a hemispherical protruding mass in the right main bronchus with complete occlusion of the upper lobe bronchus. Bronchoscopy showed a whitish friable mass in the lumen of the right main bronchus biopsy of which was reported as benign papilloma. Right upper lobectomy together with wedge resection of the portion of right main bronchus to include the tumor was done. Cut-edges of the bronchus were stitched together with interrupted fine dacron sutures. During this procedure, right main bronchus was gently clamped with non-crushing Satinsky type clamp. Patient has had uneventful recovery from surgery and was discharged without symptom. Patientis doing well three months following the operation.

  • Left main bronchus compression occasionally occurs in patients with cardiac disease. A 19-month-old female patient weighing 6.7 kg was admitted for recurrent pneumonia and desaturation. S he had an atrial septal defect (AS D) with a right aortic arch. Her left main bronchus had been compressed between the enlarged right pulmonary artery (RPA) and the descending thoracic aorta for 14 months. We conducted ASD closure and RPA anterior translocation via sternotomy. The left main bronchus compression was relieved despite the medium-term duration of compression.

  • Dieulafoy's disease of the bronchus is rare but potentially life-threatening, and should be considered in patients with massive hemoptysis, especially from unknown etiology. We report a case of a patient with massive hemoptysis due to bronchial Dieulafoy's disease. He underwent bronchial artery embolization and surgical resection, and the post-operative specimen revealed dilated and tortuous arteries in the submucosa that presented as Dieulafoy's disease of the bronchus.

Surgical Repair of Acquired Benign Esophagobronchial Fistula - A case Report - (후천성 양성 식도기관지루 -수술치험 1례-)

  • Acquired esophagobronchial fistula is relatively rare disease. Its causes are malignancy of esophagus or bronchus, infection, trauma, and diverticulum of esophagus. Malignant esophagobronchial fistula is more frequent than benign origin. The patient was 21-year-old female and had typical Onos sign. On esophagogram, fistulous tract was identified between esophagus and left lower lobe bronchus. The cause was nonspecific inflammation of mediastinum. The fistulous tract was resected and reinforced by mediastinal pleura. Postoperative course was uneventful.

Clinical analysis of expandble metallic stent in benign tracheal & bronchial disease (양성 기관, 기관지 질환에서 확장성 금속 스텐트 사용에 관한 임상적 고찰)

  • Background Insertion of tracheal stent in the treatment of benign tracheal & bronchial disease has increased since the introduction of expandable metallic stent. Material & Methods : Between Jan, 1995 and Feb. 2004, eight patients who had benign tracheo-bronchial disease underwent insertion of expandable metallic tracheal stent. We retrospectively analyzed stent insertion indications, complications, and following the result. Results : Surgical indications were post-intubation tracheal stenosis (1 case), tracheal stenosis following tracheal surgery (2 cases), tracheo-esophageal fistula (2 cases), broncho-pleural fistula(1 case), left main bronchus stenosis following bronchoplasty (1 case), and left main bronchus stenosis due to mediastinal repositioning (1 case). Expandable metallic tracheal stent was inserted in five patients to resolve dyspnea caused by airway obstruction, and to prevent recurrent pneumonia in three patients. The complication developed in 6 patients $75\%$; 3 cases of distal stenosis due to growth of granulation tissue, and one case each of tearing of posterior membrane, aggravation of tracheo-esophageal fistula, and airway partial obstruction due to stent migration. The stent was removed in 5 patients and tracheal surgery (tracheal resection and end to end anastomosis with primary repair of esophagus, pericardial patch tracheo-bronchoplasty, tracheal repair and omental wrapping) was performed in 3 patients. Conclusion Insertion of self expandable metallic stent in benign tracheo-bronchial disease is an effective means of relieving dyspnea for only a short period, and it did not increase the long term survival. Better means of treatment of benign tracheo-bronchial stenosis in necessary.

  • Mucoepidermoid tumor arising from the bronchial tree as one of the bronchial adenoma is a extremely rare in incidence. And its clinical and histopathologic behavior has been reported as varying degree of benign to extremely malignant. Because symptoms are usually related to the bronchial obstruction or obstructive pneumonitis followed by endobronchial growth of tumor, frequently error is made in diagnosis of this tumor as entity of obstructive lung disease. We present a case of mucoepidermoid tumor with review of relevant literatures arising from the right middle lobe bronchus extending to intermediate bronchus in 47 years old housewife and was surgically removed by middle and lower Iobectomy.

Current Status of General Thoracic Surgery in Korea (한국의 일반 흉부수술 현황)

  • Overall 25,095 cases of general thoracic surgery were analysed, which were performed by 48 institutes in Korea during recent 6 years[242 hospital-years]. The proportions of tumorous disease and infectious disease to be operated were 6,864 cases[27.4%] and 6,775 cases [27.0%], The most common organ involved for operation was lung-bronchus 16,542 cases [69.5%], and remainders were pleura 2,500 [10.0%], esophagus 2,433[9.7%], mediastinum 1,902[7.6%], chest wall 1,297 [5.2%], and diaphragm 421 [1.7%] in order. Among 6,864 cases of tumorous diseases, the most common causes for operation were lung-bronchus tumor 3132 cases [45.6%] and most of them were lung cancer 2,731 cases [88.7%]. In the 2,019 cases of primary lung cancer with known cell type, squamous cell carcinoma 1,296 cases [64.2%] and adenocarcinoma 460 cases [22.8%] were the most. The common types in the 1,207 cases of mediastinal tumor with known cell type were neurogenic tumor 348 cases [28.8%], thymoma 311 [25.8%], and teratoma 252[20.9%]. The annual cases of operation for tumorous disease including malignant tumor were increased steadily. Operation for infectious lung diseases [including bronchiectasis and tuberculosis] were about twice common than infectious pleural disease [i.e. empyema], and operations for tuberculous disease occupied about half cases of infectious lung disease. In 11,456 cases of other disease entities, excluding tumorous and infectious disease, there were bullous lung disease 9,074 cases[79.2%], benign esophageal disease 484[4.2%], myasthenia gravis 356[3.1%], chest wall deformity 483[4.2%], and diaphragmatic lesion 421[3. 7%] in order. We propose that above results for inquiry can be used as the basic data of general thoracic surgery in Korea.

Bronchial compression in an infant with isolated secundum atrial septal defect associated with severe pulmonary arterial hypertension

  • Symptomatic pulmonary arterial hypertension (PAH) in patients with isolated atrial septal defect (ASD) is rare during infancy. We report a case of isolated ASD with severe PAH in an infant who developed airway obstruction as cardiomegaly progressed. The patient presented with recurrent severe respiratory insufficiency and failure to thrive before the repair of the ASD. Echocardiography confirmed volume overload on the right side of heart and severe PAH (tricuspid regurgitation [TR] with a peak pressure gradient of 55 to 60 mmHg). The chest radiographs demonstrated severe collapse of both lung fields, and a computed tomography scan showed narrowing of the main bronchus because of an intrinsic cause, as well as a dilated pulmonary artery compressing the main bronchus on the left and the intermediate bronchus on the right. ASD patch closure was performed when the infant was 8 months old. After the repair of the ASD, echocardiography showed improvement of PAH (TR with a peak pressure gradient of 22 to 26 mmHg), and the patient has not developed recurrent respiratory infections while showing successful catch-up growth. In infants with symptomatic isolated ASD, especially in those with respiratory insufficiency associated with severe PAH, extrinsic airway compression should be considered. Correcting any congenital heart diseases in these patients may improve their symptoms.

A Case of Endobronchial Aspergilloma Presented by Solitary Pulmonary Nodule (고립성 폐 결절 형태로 나타난 기관지 내 국균종 1예)

  • A 53 year-old woman visited to our hospital due to increased hemoptysis for 4 days. The chest X-ray showed solitary pulmonary nodule on right upper lobe and computed tomography of chest demonstrated mass on posterior segmental bronchus of right upper lobe. Bronchoscopic examination revealed that this segmental bronchus was completely obstructed by a yellow and brownish mud like mass, which was identified as an aspergilloma by pathologic examination. Patient had undergone bilobectomy because of persistent hemoptysis. Tracheobronchial apergillosis in an immunocompetent person is very rare disease. then, we report this case with review of the Korean literature.

A Case of Endobronchial Carcinoid Tumor Treated by Flexible Bronchoscopic Resection (기관지내시경 절제술로 치료한 기관지 카르시노이드 종양 1예)

  • Bronchial carcinoid tumor accounts for less than 5% of all primary lung tumors in adults. Although surgical resection is the treatment of choice, here we report a case of bronchial carcinoid tumor treated with flexible bronchoscopic resection. A 19-year-old-man presented with a history of wheezing with dyspnea for six months. A simple chest x-ray showed no abnormal findings, but a pulmonary function test showed a moderate obstructive lung disease pattern without a bronchodilator response. A computed tomogram of the thorax revealed an enhanced $15{\times}12$ mm nodule in the left main bronchus. Bronchoscopic examination showed a polypoid mass with a stalk in the left main bronchus, which almost completely occluded the left main bronchus. Histopathology of the resected specimen revealed a bronchial carcinoid tumor. We treated the carcinoid tumor with a flexible bronchoscopic resection. During the follow up period of 6 months, the previous tumor didn't relapse. Initial bronchoscopic resection should be considered when bronchial carcinoid tumor can be approached by bronchoscopy.
