One case Treated Cerebral Infarction with Aphasia by Jihwangumja (지황음자(地黃飮子)를 투여(投與)한 중풍(中風) 실어증(失語症) 환자 치험1례)

  • Aphasia is speech disorder caused by injuries on the speech nerve center. It usually occur due to a disease in the right cerebral cortex and is divided into the various aphasia such as Global aphasia, Broca's aphasia, Wernicke's aphasia, conduction aphasia, Anomic aphasia, etc. Jihwangumja is used the cerebral infarction with Aphasia due to deficiency syndrome of kidneys. The purpose of this study is to examine the efficacy of oriental treatment for cerebral infarction with aphasia by Jihwangumsa. In the hospital, the patient showed the symptoms of mental disorder, Rt. hemiparesis G3/G4, aphasia, chest discomfort, obstipation, frequent urination, etc. The case showed that acute infarction on Lt. fronto-temporo-parietal lobe in Brain-CT. We identified the patient's clinical conditions and treated accordingly. As a result of treatment, symptoms were markedly improved and he was discharged. Further elaboration of oriental diagnostic classification could possibly lead to the fundamental treatment.

Relationship of Weight Status and Physical Activity of Adolescents in Busan City (부산지역 일부 청소년의 체중상태와 신체활동량과의 관계)

  • The study was conducted to assess the association or physical activity, television watching and energy intake with body fat in 514 adolescents(218 boys, 296 girls) aged 14-17 years. Comparison of the weight status evaluated according to some indicators based on weight and stature was made. The quantitative food frequency was used to determine intakes of total energy and energy from each macronutrient. Television-watching time was used as an index of inactivity and time spent performing extra-curricular physical exercise or playing was used a index of activity, BMI(body mass index) was used as index of body fat. Weight status varied by the indicators used. Using BMI, Broca and Rohrer index, 54-59% of boys and 38-57% of girls were underweight decreased(23% of boys and 26% of girls) and the of overweight increased (9.3% of boys and 7.8% girls), when adolescents whose BMI are<=15th percentiles for age and sex, and >= 85th percentiles of standard growth charts of Korean Children and aldosescents(1998) were classified as underweight and overweight, respectively, Girls tended to get fatter compared to boys who showed greater skeletal development. There were no significant correlations between BMI and total energy intake, energy intakes from macronutrient and physical activity level. These lack of association seems to be related to indication that BMI reflects total body mass rather than fatness. Mpreover, the effect of growth may interact with diet intake or physical activity level. Although BMI was not related to physical activity, television-watching time was negatively correlated with physical activity. Television-watching time was positiviely correlated with energy intake in girls. In adolescents, criteria for weigh status based on weight and stature must be age- and sex-specific in that weight status varied by indicators whether gender and higher were taken into consideration or not. If follows that each hourly increment of television watching may be related to fatness of adolescents, because hours certainly reduce the opportunity to be active and increase th energy intake.(Korean J Nutrition 34(1):39-47, 2001)

Analysis of Brain Activation due to Mouth Shape during Grip Movement (잡기동작 수행 시 입모양에 따른 뇌활성화 분석)

  • Purpose : This study was performed to understand the relationship between hand and mouth shapes using functional magnetic resonance imaging(fMRI). Methods : Two healthy volunteers without any previous history of physical or neurological illness were recruited. fMRI was done that volunteers was 6 repeated of natural mouth, close mouth and open mouth while power grip and pinch grip movement. Results : Cerebral cortex activation was not well observed for the natural mouth during the power grip exercise. For the closed mouth, the temporal lobe, Broca's area, the prefrontal area related to thinking and judgment, the supplementary motor area, the auditory area and Wernicke's area were activated. For the open mouth, cortical activation was also observed in the temporal lobe, Wernicke's area, the prefrontal area related to thinking and the orbital frontal area related to visual sense. During the pinch grip exercise, cortical activation was observed for the natural mouth in the primary sensory area, Wernicke's area, the primary and supplementary motor area, and the prefrontal area. For the closed mouth, cortical activation was observed in the temporal lobe, Wernicke's area, the prefrontal area related to thinking, the secondary visual area, the primary sensory area and the supplementary motor area. In the case of the open mouth, cortical activation was observed in a few parts in the temporal lobe as well as Wernicke's area, the prefrontal area related to thinking, and other areas related to visual sense such as the primary visual area, the secondary visual area and the visual association area. Conclusion : Brain was more activation for close mouth and open mouth more than natural mouth movement.

The Effects of Auricular Acupressure in Children's Obesity (이침요법(耳鍼療法)이 아동의 비만도(肥滿度)에 미치는 효과(效果))

  • Objectives : The purpose of this study was to examine the effects of Auricular acupressure with semen sinapis in obese children over a specific period of time. Methods : The study subjects included 27 obese children who were evaluated as such using Broca's Index at an elementary school located in M city. The data was collected throughout March 2, to May 31, 2005. In order to collect data, as a pre-test, characteristics of obesity and general characteristics were examined. Experimental treatments were used on Shinmun, Kijeom, Wijeom, Bijeom and Naebunbijeom. Among the auricular spots, one pill of semen sinapis was placed and pressed for five seconds at a time with exerted force and the subjects endured pain. This procedure was conducted ten tunes every morning, noon and evening for a period of six weeks. As a post-test, the degree of obesity were examined two weeks, four weeks and six weeks after acupressure had been applied. The data was collected and analysed using the SPSS/WIN 10.0 program by number, percentage, mean standard deviation, and Repeated Measure ANOVA. Result : The first hypothesis that 'The obesity of the children who had auricular acupressure with semen sinapis will show no difference over time' (two weeks, four weeks and six weeks after acupressure had been applied) was rejected(p=.000). A statistically significant difference in the obesity of the children who had auricular acupressure with semen sinapis was shown over time. The children's obesity degree reduced from 35.85% before auricular acupressure with semen sinapis to 28.67% six weeks after acupressure. Thus after six weeks of acupressure the relative degrees of obesity were reduced by 7.18%. Conclusion : Bases on the above results, it can be determined that auricular acupressure with semen sinapis can be used as an effective method of treatment in Korea, for reducing degrees of obesity in children.

The Effect of Exercise Type on Cardiovascular Disease Risk Index Factors in Male Workers (일부 직장 남성들의 운동형태가 심혈관질환의 위험 예측인자에 미치는 영향)

  • Objectives : The purpose of this study was to gain an understanding of the effects of three exercise types on anthropometric and serum lipids and physiological index factors, which are known to be the three risk factors of cardiovascular disease in male workers. Methods : The experimental study period was 12 weeks. In this study, 30-40's males (N=31) were assigned to 3 experimental groups: regular aerobic(treadmill walking) exercise group, regular anaerobic(muscular endurance) exercise group, irregular aerobic & anaerobic exercise group and a control group using a stratified random assignment method. Results : In relation to anthropometric factors, the regular aerobic & anaerobic exercise groups showed significant decreases in Weight, Broca's index, WC, BMI, WHtR, WHpR and HRrest. With regard to the serum lipid factors, the TC was decreased, but the HDL-c increased among the regular aerobic & anaerobic exercise groups. However, no significant difference was found between the other groups in respect to the LDL-c and TG. Considering the physiological factors, the TC/HD-c, TC-HDL/HDL-c, LDL-C/HDL-c and NON-HDL-c ratios were decreased, but the HDL-c/TC ratio increased among the regular aerobic & anaerobic exercise groups. The TG/HDL-c and HDL-c/LDL-c ratios showed no significant differences between the groups. These results indicated that the positive change for each factor is much larger in the regular exercise groups, especially in the anaerobic exercise group. Conclusions : The results indicate that not only regular aerobic exercise, but also regular anaerobic(muscular endurance) exercise could be utilized in lessening the deleterious effects of the risk index factors for cardiovascular disease.

A Study of the Body Mass Index and Body Image among Female College Students (여자대학생의 비만지수와 신체상에 관한 연구)

  • This study aims at examining relationships between body mass index and perceived body shape and image. Using a purposive sampling method, 404 female college students were recruited from three universities located in Busan from April 2000 to June 2000. To classify the levels of obesity, Broca's method was used. Secord & Jourard's Body Cathexis Scale(1953) modified by Kim mi ok(1997) was also used to measure body image. The collected data were analyzed with the percentages, Mean, Standard deviation, T-test, and Pearson correlation Coefficient. The results of this study were as follows. 1. 56.4 % of subjects were under weight, and 42.1% were normal for BMI. 2. 55.9% of subjects were very satisfied with their body shape and 15.1% were satisfied. 3. The mean values of perceived body shape according to the BMI were 3.0 for the overweight group, 2.65 for the normal weight group, and 2.21 for the low weight group, the mean differences among three groups were significantly satisfied (F= 21.42, P= .000). 4. The mean scores of body image according to the BMI was 160.21 for low weight group, 155.57 for the normal weight group, and 140.0 for overweight group. The mean differences among the three groups were not significantly satisfied. 5. The mean scores of body image according to perceived body shape were 17819 for the satisfaction group, 161.08 for the moderate group and 150.82 for dissatisfied group. The mean differences among the three groups were significantly satisfied (F= 35.72, P=. 000). 6. There were high positive correlations between BMI and perceived body shape(r= .317) and between perceived body shape and body image (r=- .415).

Recording natural head position using an accelerometer and reconstruction from computed tomographic images

  • Objectives: The concept of natural head position (NHP) was first introduced by Broca in 1862, and was described as a person's stable physiologic position "when a man is standing and his visual axis is horizontal." NHP has been used routinely for clinical examination; however, a patient's head position is random during cone-beam computed tomography (CBCT) acquisition. To solve this problem, we developed an accelerometer to record patients' NHP and reproduce them for CBCT images. In this study, we also tested the accuracy and reproducibility of our accelerometer. Materials and Methods: A total of 15 subjects participated in this study. We invented an accelerometer that measured acceleration on three axes and that could record roll and pitch calculations. Recorded roll and pitch data for each NHP were applied to a reoriented virtual image using three-dimensional (3D) imaging software. The data between the 3D models and the clinical photos were statistically analyzed side by side. Paired t-tests were used to statistically analyze the measurements. Results: The average difference in the angles between the clinical photograph and the 3D model was $0.04^{\circ}$ for roll and $0.29^{\circ}$ for pitch. The paired t-tests for the roll data (P=0.781) and the pitch data (P=0.169) showed no significant difference between the clinical photographs and the 3D model (P>0.05). Conclusion: By overcoming the limitations of previous NHP-recording techniques, our new method can accurately record patient NHP in a time-efficient manner. Our method can also accurately transfer the NHP to a 3D virtual model.

18F-THK5351 PET Imaging in Nonfluent-Agrammatic Variant Primary Progressive Aphasia

  • Background and Purpose: To analyze $^{18}F-THK5351$ positron emission tomography (PET) scans of patients with clinically diagnosed nonfluent/agrammatic variant primary progressive aphasia (navPPA). Methods: Thirty-one participants, including those with Alzheimer's disease (AD, n=13), navPPA (n=3), and those with normal control (NC, n=15) who completed 3 Tesla magnetic resonance imaging, $^{18}F-THK5351$ PET scans, and detailed neuropsychological tests, were included. Voxel-based and region of interest (ROI)-based analyses were performed to evaluate retention of $^{18}F-THK5351$ in navPPA patients. Results: In ROI-based analysis, patients with navPPA had higher levels of THK retention in the Broca's area, bilateral inferior frontal lobes, bilateral precentral gyri, and bilateral basal ganglia. Patients with navPPA showed higher levels of THK retention in bilateral frontal lobes (mainly left side) compared than NC in voxel-wise analysis. Conclusions: In our study, THK retention in navPPA patients was mainly distributed at the frontal region which was well correlated with functional-radiological distribution of navPPA. Our results suggest that tau PET imaging could be a supportive tool for diagnosis of navPPA in combination with a clinical history.

A Study of Dietary Intake and Vitamin/Mineral Supplement Usage among Adolescents (청소년의 식이섭취와 비타민.무기질 보충제 복용에 관한 연구)

  • The relationship between dietary intake and vitamin/mineral supplement usage was examined in 706 adolescent girls who were high school students. 43.8% of subjects used vitamin/mineral supplements during one year. The higher the family income and parents education level, the higher percentage of vitamin/mineral supplement usage was. But there was no significant difference between grades and scores. And vitamin/mineral supplement usage was higher int he thin and obese groups than the average weight groups. Nutrition knowledge and food habit did not affect vitamin/mineral supplement usage. Calorie intakes of vitamin/mineral supplement users and nonusers were similar. However, independent of the supplements, the diets of supplement nonusers contained significantly more dietary protein, vitamin A, vitamin $B_2$, niacin, vitamin C, and calcium than the diets of the users. A considerable portion of both the users and nonusers had dietary intakes of less than 2/3 of the Recommended Dietary Allowances for vitamin $B_1$, vitamin $B_2$, niacin, calcium, and iron. Vitamin/mineral supplement nonusers generally consumed a more vitamin, mineral from diet. Reasons for taking supplements were to take energy, advice and illness.

The Health and Nutritional Status of Middle Aged Men at Worksite in Taejon (대전지역 직장 중년 남성의 건강 및 영양 상태 조사)

  • This study was conducted to investigate the health and nutritional status of 123 middle aged men at their worksite in Taejon. The results of this study on the factors that influence their health and nutritional status were as follows : 1) 74.8$\%$ of the subjects had history in the order of alimentary, heat, liver, diabetic and pulmonary diseases. 30.3$\%$ stopped smoking at 42.3% yrs. and 74.5$\%$ smoked more than 10 cigarets per day. Also 71.9$\%$ drank 2-3 times per week and 35.3$\%$ drank 1-2 times per week. 91.4$\%$ exercised more than 30min every day. 2) 54.4% showed concerns about their health whereas 20.3$\%$ were afraid that they night get sick. 3) 90.4$\%$ ate regularly and 54.5$\%$ worried about their cholesterol, salt, fat and MSG intakes. 48.7$\%$ ate out 1-2times per week and their favorite foods eaten outside were Korea. 4) 41.5$\%$ were classified as 'normal A', 30.9$\%$ 'normal B' group and high blood pressure and liber diseases in 'doubtful for disease' group were pointed out from their 1996 health check ups. 5) By Broca index, 39.8$\%$ were overweight and 9.8% were obese however by BMI only 23.6% were overweight. According to the relationship between calculated and self recognized obesity, 62.4$\%$ categorized themselves into the right weight range but 34.3$\%$ thought they were thinner than they were. 6) 43.9$\%$ were border line in cholesterol intake and 12.1$\%$ needed medical care for high blood cholesterol. 7) The Average energy intake was 1970.6㎉(80.9$\%$ RDA) with a 65 :19 : 16 ratio of carbohydrate : protein : fat. Protein, Fe, thiamin, riboflavin, niacin and Vit. A. 8) Occupation, regularity of meals, partner's job, income. smoking, alcohol drinking, health concerns and eating out were the factors that influenced the subject's nutrient intakes and health status. from this study, it was found that middle aged men needs to know their health and nutritional status and to be educated correct health and nutritional information through formal or informal channel. The worksite is the vest place to do this and we want these results to be used to develop the nutrition education program for middle aged men at the worksite.
