Complex open elbow fracture-dislocation with severe proximal ulna bone loss: a case report of massive osteochondral allograft surgical treatment

  • We report a case of a 69-year-old right-dominant man who had an open Monteggia-like lesion of the right elbow (Gustilo-Andersen IIIA) with severe proximal ulna bone loss associated with an ipsilateral ulnar shaft fracture due to a motorcycle accident. The patient underwent two-stage surgery. Wound debridement and bridging external fixation were performed at first. Three months later, a frozen massive osteochondral ulnar allograft was implanted and fixed with a locking compression plate. A superficial wound infection appeared 5 weeks after the second surgery. Superficial wound debridement, negative pressure therapy, and antibiotics were administered for 3 months, achieving infection healing. At 3 years post-surgery, the elbow range of motion was satisfactory with a Disabilities of the Arm, Shoulder and Hand (DASH) score of 16.7. Radiographs and computed tomography scans showed good allograft-bone integration without allograft reabsorption or hardware loosening. Although not complication-free, massive ulna osteochondral allograft implantation can be considered a valid option in cases of open Monteggia-like lesions associated with ulnar shaft fracture and severe bone loss in active patients, whenever osteosynthesis or joint replacement is not a proper solution. This type of bone stock restoration allows for future surgery, if needed.

Crystal Structure of Cytochrome cL from the Aquatic Methylotrophic Bacterium Methylophaga aminisulfidivorans MPT

  • Cytochrome cL (CytcL) is an essential protein in the process of methanol oxidation in methylotrophs. It receives an electron from the pyrroloquinoline quinone (PQQ) cofactor of methanol dehydrogenase (MDH) to produce formaldehyde. The direct electron transfer mechanism between CytcL and MDH remains unknown due to the lack of structural information. To help gain a better understanding of the mechanism, we determined the first crystal structure of heme c containing CytcL from the aquatic methylotrophic bacterium Methylophaga aminisulfidivorans MPT at 2.13 Å resolution. The crystal structure of Ma-CytcL revealed its unique features compared to those of the terrestrial homologues. Apart from Fe in heme, three additional metal ion binding sites for Na+, Ca+, and Fe2+ were found, wherein the ions mostly formed coordination bonds with the amino acid residues on the loop (G93-Y111) that interacts with heme. Therefore, these ions seemed to enhance the stability of heme insertion by increasing the loop's steadiness. The basic N-terminal end, together with helix α4 and loop (G126 to Y136), contributed positive charge to the region. In contrast, the acidic C-terminal end provided a negatively charged surface, yielding several electrostatic contact points with partner proteins for electron transfer. These exceptional features of Ma-CytcL, along with the structural information of MDH, led us to hypothesize the need for an adapter protein bridging MDH to CytcL within appropriate proximity for electron transfer. With this knowledge in mind, the methanol oxidation complex reconstitution in vitro could be utilized to produce metabolic intermediates at the industry level.

  • Escherichia coli AcrAB-TolC is a multidrug efflux pump that expels a wide range of toxic substrates. The dynamic nature of the binding or low affinity between the components has impeded elucidation of how the three components assemble in the functional state. Here, we created fusion proteins composed of AcrB, a transmembrane linker, and two copies of AcrA. The fusion protein exhibited acridine pumping activity, suggesting that the protein reflects the functional structure in vivo. To discern the assembling mode with TolC, the AcrBA fusion protein was incubated with TolC or a chimeric protein containing the TolC aperture tip region. Three-dimensional structures of the complex proteins were determined through transmission electron microscopy. The overall structure exemplifies the adaptor bridging model, wherein the funnel-like AcrA hexamer forms an intermeshing cogwheel interaction with the ${\alpha}$-barrel tip region of TolC, and a direct interaction between AcrB and TolC is not allowed. These observations provide a structural blueprint for understanding multidrug resistance in pathogenic Gram-negative bacteria.

PVA와 CMC 첨가가 커튼 코팅용 GCC 도공액의 유변 특성 및 커튼 안정성에 미치는 영향 (Effects of PVA and CMC addition on Rheological Characteristics and Curtain Stability of GCC Based Curtain Coating Colors)

  • Curtain stability without curtain contraction is critical for a successful operation in curtain coating, and this can be influenced by the change in particle dynamics and rheological properties of coating colors. In this study, polyvinyl alcohol (PVA) and carboxymethyl cellulose (CMC) were added to control the rheology of GCC based curtain coating colors. Surface tension was increased slightly with increasing content of cobinder. Shear-thinning of viscosity was more pronounced for the CMC containing GCC coating colors. Complex modulus decreased when small amount of PVA was used as a cobinder, but it increased in other coating colors. Extensional viscosity was increased with increasing of the cobinder content, but CMC was more effective. Results indicate that pigment interaction with PVA is different from that with CMC. Dispersibility of coating colors was improved due to steric stabilization when small amounts of PVA was used, but flocculation occurred by bridging when the amount of PVA was increased. Dispersibility of coating colors was improved when small amount of CMC was added, while flocculation was observed by depletion effect when the concentration of CMC was increased in coating colors. Addition of cobinders at proper levels gave positive effects both in rheological properties and curtain stability. On the other hand, excessive amount of cobinders caused particle flocculation and this resulted in rheological and curtain stablity problems.

Syntheses of Polysiloxane-Bridged Dinuclear Metallocenes and Their Catalytic Activities

  • The polysiloxane-bridged dinuclear metallocenes $[(SiMe_2O)_n-SiMe_2(C_5H_4)_2][(C_9H_7)ZrCl_2]_2$ (n=1 (7), 2 (8), 3 (9)) have been generated as a model complex for the immobilized metallocene at silica surface by treating the respective disodium salts of the ligands with 2 equivalents of $(C_9H_7)ZrCl_3$ in THF. All three complexes are characterized by $^1H$ NMR and measurement of metal content through ICP-MS. It turned out that the values of ${\Delta}{\delta}=[{\delta}_d-{\delta}_p]$, the chemical shift difference between the distal $({\delta}_d)$ and proximal $({\delta}_p)$ protons, for the produced dinuclear compounds (0.47 for 7, 0.49 for 8, and 0.5 for 9) were larger than the Δδ value of the known ansa-type complex holding the same ligand as a chelating one, that is just the opposite to the normal trend. In order to compare polymerization behavior of the dinuclear metallocene with the corresponding mononuclear metallocene, (Cp)$(C_9H_7)ZrCl_2$ was separately prepared. To investigate the catalytic properties of the dinuclear complexes and mononuclear metallocenes ethylene polymerization has been conducted in the presence of MMAO. The polymerization results display the typical activity dependence on polymerization temperature for all complexes. The most important feature is that the polymers from the dinuclear metallocenes represent enormously improved molecular weight compared with the polymer from the corresponding mononuclear metallocene. In addition, the influence of the nature of the bridging ligand upon the reactivities of the dinuclear metallocenes has also been observed.

2-아닐리노 피리딘을 배위자로 하는 이핵 로듐착물의 두 산소첨가 생성물에 대한 합성 및 전기화학적 성질 (Synthesis, ESR and Electrochemical Characterization of Dioxygen Binding to Dirhodium Complexes with 2-anilinopyridinato Bridging Ligand)

  • 두 개의 아닐린계 질소와 두 개의 피리딘계 질소가 각각의 로듐이온에 트란스형으로 결합된 $Rh_2(ap)_4$(2,2-trans) 이성질체는 -0.40 V vs SCE에서 디 옥시젼의 한 전자를 환원시킬 수 있음을 보여주고 있다. 123K에서의 ESR 스펙트럼에 의하면 한 로듐이온에 산소 1분자가 측쇄 결합하여 $[Rh_2(ap)_4(O_2)]^-$이온을 형성하고, 착물은 $Rh_2^{III}$ 산화수를 가짐을 알 수 있다. 이 착물은 시안화메틸/염화메틸렌 혼합용액에서 Rh-$O_2^-$결합의 분열없이 [$Rh_2(ap)_4(O_2)(CH_3CN)]^-$을 형성한다. -0.25, 0.55V에서 산화되었을 때 [$Rh_2(ap)_4(O_2)]^-$$Rh_2(ap)_4(O_2)$와 [$Rh_2(ap)_4(O_2)]^+$을 형성하기 위하여 두 단계의 한 전자 산화과정이 일어난다. 두 화학종은 모두 초산소가 측쇄 결합되어 있지만, 전자는 $Rh^{II}Rh^{III}$, 후자는 $Rh_2^{III}$의 산화수를 갖는다. 반면에, ESR 스펙트럼과 $CH_3CN$ 부가 연구에서 보면 후자 착물이 로듐이온에 위치한 부대전자와 함께 [$Rh^{II}Rh^{III}(ap)_4(O_2)]^+$로 기술되고 그 착물은 측쇄 배위결합된 산소분자를 가지고 있음을 보여주고 있다. 또한 전기화학적, ESR 연구는 디 옥시젼의 활동도가 전기화학적 산화환원전위와 관계 있음을 보여주고 있다.

변형경화형 시멘트 복합체를 사용한 프리캐스트 끼움벽의 내진성능 (Seismic Performance of Precast Infill Walls with Strain-Hardening Cementitious Composites)

  • 지진이 빈번하게 발생하는 지역에서는 비내진상세구조물은 지진 발생시 연약층을 형성하고 취성적 붕괴를 일으키게 된다. 그러나, 기존 구조물을 해체하고 내진상세 구조물을 신축하는 방법은 건설폐기물, 환경오염 및 민원 등 여러가지 문제들을 가지는 등 비경제적 방법이라 할 수 있다. 따라서 기존 구조물이 내진성능을 만족하도록 내진보강에 관한 많은 연구가 이루어졌으며, 이러한 내진보강방법에는 끼움벽, 철골브레이스, 연속벽, 부벽, 날개벽, 기둥/보의 자켓팅 등이 있다. 이 중 끼움벽 골조는 큰 변형과 접합부에서의 회전이 발생하는 골조와, 비교적 작은 변형에서도 전단파괴를 야기하는 끼움전단벽 등 복합적인 거동특성을 나타낸다. 따라서, 이러한 시스템의 거동특성은 개개의 골조나 벽에서 나타나는 거동특성과 매우 다르게 된다. 본 연구에서는 끼움벽의 내진성능을 평가하고자 하였으며, 손상에너지의 효과적 흡수를 위해 변형경화형 시멘트 복합체 (SHCC)를 사용하였다. 실험은 1/3 축소모형의 끼움벽을 반복가력하는 것으로 계획하였다. 실험 결과, SHCC 끼움벽에서는 섬유의 가교작용을 통해 시멘트 복합체 내 응력을 재분배함으로써 미세균열이 발생하였으며, 강도 및 에너지소산능력이 우수한 것으로 나타났다.

TiO2(110) 표면에 흡착된 물분자의 결합 활성화에 관한 MO 연구 (Molecular Orbital Anaysis of Water Activation on TiO2(110) Surface)

  • $TiO_2$(110) 표면에서 물의 OH 결합 활성화가 어떤지 전자적 메카니즘에 의해 이루어지는 extended Hiickel 방법을 통해 알아보았다. 물분자는 $3a_1$오비탈의 시그마 상호작용 겨로가로 5배위 $Ti^{4+}$원자바로위에 수직으로 흡착한다. 이 결합구조에서는 물분자의 H원자가 $TiO_2$의 2배위 bridging $O^{2-}(O_b)$원자와 너무 멀리 떨어져 있으므로 OH 결합 해리를 촉진시키는 수소결합 상호 작용을 할 수 없으므로 물분자를 $O_b$ 원자쪽으로 기울여 수소결합이 형성되도록 한다. 이 경우 $O_b$ P 오비탈로부터 흡착 물분자의 LUMO $2b_1$반 결합성 오비탈로 전자밀도의 이동이 일어나고 또 물의 $3a_1$오비탈(약한 결합성)로부터 $Ti^{4+} 3d_{z2}$오비탈로 전자밀도의 이동이 일어남으로써 물의 OH 결합이 상당히 약화됨을 확인할 수 있었고 그 결과 OH와 H로 해리할 것이라는 해석이 가능하다.

Clinical Importance of the Smear layer

  • A number of investigations have shown that the presence of bacteria is prerequisite for developing pulpal and/or periradicular pathosis. Depending on the stage of pulpal pathosis, various species of bacteria can be cultured from infected root canals. Kakehashi et al. showed that exposure of pulpal tissue in germ-free rats was characterized by minimal inflammation and dentinal bridging while exposure of pulpal tissue in conventional rats with normal oral flora was characterized by pulpal necrosis, chronic inflammation, and periapical lesions. Currently used methods of cleaning and shaping, especially rotary instrumentation techniques, produce a smear layer that covers root canal walls and the openings to the dentinal tubules. The smear layer contains inorganic and organic substances that include fragments of odontoblastic processes, microorganisms, their by products and necrotic materials. Because of its potential contamination and adverse effect on the outcome of root canal therapy, it seems reasonable to suggest removal of the smear layer for disinfection of the entire root canal system. Presence of this smear layer prevents penetration of intracanal medications into the irregularities of the root canal system and the dentinal tubules and also prevents complete adaptation of obturation materials to the prepared root canal surfaces. Removal of the smear layer by an intracanal irrigant and placement of an antibacterial agent in direct contact with the content of dentinal tubules should allow disinfection of this complex system and better outcome for the root canal therapy. A new solution, which was a mixture of a tetracycline, an acid, and a detergent(MTAD), was developed in the Department of Endodontics, Dental School. Lorna Linda University, USA. It has been demonstrated that MTAD was an effective solution for the removal of the smear layer and does not significantly change the structure of the dentinal tubules when used as a final irrigant in conjunction with 1 % NaOCl as a root canal irrigant. Studies are in progress to compare the anti- microbial properties of this newly developed solution with those of sodium hypochlorite and EDTA that are currently used to irrigate the root canals and remove the smear layer from the surfaces of instrumented root canals.canals.

비파괴검사법을 이용한 복합재료의 파괴인성 평가법 개발 (Development of Fracture Toughness Evaluation Method for Composite Materials by Non-Destructive Testing Method)

  • 연속섬유강화 복합재료의 경우 보강섬유의 파괴, 모재의 파괴, 섬유와 모재의 분리, 층간파괴 등의 복합적인 파괴현상이 동반되고 특히 균열성장과 균열성장 정지가 균열가교 현상 때문에 반복되므로 안정성장과 불안정성장이 불규칙하게 반복된다. 따라서 주균열 성장의 개시점과 불안정 파괴점에서의 파괴인성치를 정확하게 결정한다는 것은 매우 어려운 것이다. 본 연구에서는 CFRP에 대하여 파괴인성 실험과 병행하여 실시간으로 결함을 검출하는 새로운 방법인 AE분석법 및 비디오 마이크로 스코프를 이용하여 파괴과정을 기록하여 검토, 분석함으로서 손상의 정도와 파괴기구를 규명하였을 뿐만 아니라 주균열 성장의 개시점, 균열가교 역할을 하는 섬유다발의 파단점, 균열의 불안정 파괴 개시점을 찾아 이를 기초로 균열진전 저항곡선에 의한 파괴인성치를 평가하여 신뢰성 있는 파괴인성 측정법을 제시하였다.
