호흡 변이도의 최신 연구 동향 (A Review of Respiratory Variability)

  • Objectives In human, there is a diversity in the breathing pattern, for instance inspiratory and expiratory time, volume, breathing frequency, and breath-to-breath variation. Expecially, respiratory variability can provide important information about breathing regulation and physiological flexibility. it is significant to not only breathing index but also physiological index.Methods Thus this paper reviews the literature on respiratory variability with the aim of clinical application.Results We could find the interrelationships and respiratory variability between emotions, psychopathy, sighing, mental and physical activity.Conclusions As a result, respiratory variability can serve an important physiological index in the clinical area and reflects how our bodies act in diverse environments under various condition.

호흡근 훈련이 뇌졸중환자의 폐기능과 수면의 질에 미치는 영향 (Impact of Respiratory Muscle Exercises on Pulmonary Function and Quality of Sleep among Stroke Patients)

  • PURPOSE: This study attempts to examine the impact of respiratory muscle exercises on the respiratory function and quality of sleep among stroke patients. METHODS: A total of 20 stroke patients were randomly divided into a breathing-exercise training group(n=10) and a breathing-device-training group(n=10). Changes in pulmonary function, as well as the quality, were measured before and after the intervention. The breathing exercise was performed three times a week for a total of eight weeks. Breathing-device exercises made use of a lung-capacity-strengthening device and were performed for three times a week for eight weeks. One Flow FVC was used as a measurement tool for the pulmonary function test. The Pittsburgh Sleep Quality Index(PSQI) and a sleep measurement tool were used for sleep evaluation. RESULTS: In a comparison of changes in pulmonary function before and after the training, the breathing-exercise group and the breathing-device-training group showed a significant difference. In changes of sleeping measure and PSQI, the breathing-exercise group and the breathing-device-training group also showed significant differences. CONCLUSION: Diverse index analyses confirmed that breathing exercises and exercises using a lung-capacity-strengthening device, were effective in improving respiratory function and quality of sleep among stroke patients experiencing respiratory function disorders and sleep disorders.

중년여성의 혈중 지질농도, 신체증상, 헤모글로빈에 미치는 단전호흡운동과 걷기운동의 효과 비교 (Comparison of the Effects of Danjeon Breathing and Walking to Blood Lipids, Physical Symptoms and Hemoglobin in Middle Aged Women)

  • Purpose: The purpose of this research was to compare the effects of Danjeon Breathing exercise and walking exercise on blood lipids, physical symptoms and hemoglobin of middle-aged women to determine which of these two exercises was more beneficial. Methods: The Danjeon breathing exercise group(20) took health management counseling and performed an eighty-minute Danjeon breathing exercise three times a week for 12 weeks while the walking exercise group(25) took health management counseling and performed a walking exercise seven hours per week for 12 weeks. The control group(20) received only health management counseling. The groups were homogeneous on age, religion, marital status, education, profession, monthly income per household, menopause, smoking, drinking, fat intake and cholesterol intake. Likewise, the groups were homogeneous on total cholesterol, HDL-cholesterol,atherogenic index, hemoglobin and physical symptoms. The collected data were analyzed by using the SPSS PC program and $x^2$-test, ANOVA, repeated measures of ANOVA and LSD. Results: 1. Total cholesterol of the Danjeon breathing exercise group and the walking exercise group were lower than that of the control group. 2. HDL- cholesterol of the Danjeon breathing exercise group and the walking exercise group were not higher than that of the control group. 3. Atherogenic index of the Danjeon breathing exercise group was lower than that of the control group, but atherogenic index of the walking exercise group was not lower than that of the control group. 4. Hemoglobin of the Danjeon breathing exercise group and the walking exercise group were not higher than that of the control group. 5. Physical symptom score of the Danjeon breathing exercise group was lower than those of the walking exercise group and the control group, but Physical symptom score of the walking exercise group was not lower than that of the control group. Conclusion: Danjeon breathing exercise and walking exercise are effective for decreasing total cholesterol of middle-aged women. Danjeon breathing exercise is more effective for decreasing atherogenic index and physical symptoms of middle-aged women than walking exercise.

구호흡 어린이에서 수면호흡장애와 상기도와의 관계 (Relationship between Upper Airway and Sleep-Disordered Breathing in Children with Mouth Breathing)

  • 구호흡을 야기하는 가장 흔한 원인은 상기도의 폐쇄이다. 구호흡은 수면호흡장애의 병인론적 원인이 될 수 있으며, 어린이에서 수면호흡장애는 성장장애와 행동학적 문제를 일으킬 수 있다. 이 연구의 목적은 구호흡 어린이에서 수면호흡장애와 상기도와의 관계를 조사하고자 함에 있다. 초진 설문지에서 구호흡이 있다고 응답한 7 - 9세의 20명 남자 어린이를 대상으로 하였다. 편도평가, 구호흡 및 수면호흡장애와 관련된 설문지, 측모두부 방사선사진, 그리고 휴대용 간이수면검사를 시행하였다. 수면호흡장애에 대한 평가로 무호흡-저호흡 지수(apnea-hypopnea index, AHI), 산소불포화 지수(oxygen desaturation index, ODI)와 측모두부 방사선 사진을 통해 측정한 상기도 폭경과의 관계를 평가하였다. 상기도 부위 중 후구개 거리와 후설 거리는 무호흡-저호흡 지수와 산소불포화 지수가 커질수록 좁아지는 경향을 보였다(p = 0.002, p = 0.001). 또한, 편도의 크기와는 유의한 상관관계는 없었지만, 아데노이드 비대율의 경우 무호흡-저호흡 지수가 비정상군이 정상군에 비해 아데노이드가 비대하였다(p = 0.008). 이 연구결과를 통해 구호흡이 상기도에 영향을 미칠 수 있고 이는 수면호흡장애까지 연관될 수 있음을 확인할 수 있었다.

  • Sleep is associated with definite changes in respiratory function in normal human beings. During sleep, there is loss of voluntary control of breathing and a decrease in the usual ventilatory response to both low oxygen and high carbon dioxide levels. Especially, rapid eye movement (REM) sleep is a distinct neurophysiological state associated with significant changes in breathing pattern and ventilatory control as compared with both wakefulness and non-rapid eye movement (NREM) sleep. REM sleep is characterized by erratic, shallow breathing with irregularities both in amplitude and frequency owing to marked reduction in intercostal and upper airway muscle activity. These blunted ventilatory responses during sleep are clinically important. They permit marked hypoxemia that occurs during REM sleep in patients with lung or chest wall disease. In addition, sleep-disordered breathing (SDB) is more frequent and longer and hypoventilation is more pronounced during REM sleep. Although apneic episodes are most frequent and severe during REM sleep, most adults spend less than 20 to 25% of total sleep time in REM sleep. It is, therefore, possible for patients to have frequent apneas and hypopneas during REM sleep and still have a normal apnea-hypopnea index if the event-rich REM periods are diluted by event-poor periods of NREM sleep. In this review, we address respiratory physiology according to sleep stage, and the clinical implications of SDB and hypoventilation aggravated during REM sleep.

  • We performed the numerical analysis on the characteristics of indoor air quality and local supply index with a variation of supply · exhaust airflow rate. We analyzed the local supply index and carbon dioxide concentration at the room and breathing zone with respect to the variation of the supply · exhaust airflow rate. From the numerical results, we found that local supply index was affected but carbon dioxide concentration was hardly affected by the variation of the airflow rate in the room. And we also knew that carbon dioxide concentration was raised in despite of the increment of the supply airflow rate in the breathing zone. After this study it is necessary to analyze the local exhaust index when we evaluate the state of the ventilation in the room.

  • Objectives The objective of this study si to report clinical effect of korean medical treatments and Buteyko breathing technique for asthma patient with pneumonia. Methods The patient was treated by korean medical treatment including acupunture, moxibustion, herbal medication and Buteyko breathing technique. The improvement of clinical symptoms was observed by visual checking and X-ray, Pulmonary Function Test that index include TV, FVC, FEV1. Results After treatments, the patient was improved in visual checking and X-ray on chest, TV, FVC, FEV1. Conclusions We suggest that korean medical treatment and Buteyko breathing technique could be an effective method to improve the asthma patient with pneumonia.

중년여성의 건강증진을 위한 단전호흡 운동프로그램의 적용 효과 (The Effect of the DanJeon Breathing Exercise Program Applied to Health Promotion in Women in Midlife)

  • The purpose of this study was to develop a DanJeon Breathing Exercise Program for health promotion and to examine the effects of a DanJeon Breathing Exercise Program on the physical and emotional health promotion of women in midlife. The design utilized for this study was quasi-experimental with a nonequivalent control group pretest-posttest design. This study was done between February 1 and July 4, 2000 and the subjects of the study were 40 women in midlife living in Seoul. There were 20 members in experimental group who participated in the DanJeon Breathing Exercise Program and 20 members in the control group who were chosen as matched to members of the experimental group according to age, education and religion. The DanJeon Breathing Exercise Program was carried out for 80 minutes a day, 3 times a week for 12 weeks. The percent of body fat, back strength, flexibility (trunk flexion), balance and vital capacity were measured using the Health Management System developed by the Korea Physical Science Institution. Blood tests were done at D infirmary for total cholesterol, and the atherogenic index was calculated using an established formula. The scores of anxiety and depression were measured by a questionnaire with 10 questions on anxiety and 13 questions on depression. It was developed from the Korean Manual of Symptoms-Checklist-90 revision. The collected data were processed using the SPSS PC program and analyzed using $\chi^2$-test and t-test. The result of this study are as follows : 1. The percent of body fat for the experimental group who participated in the DanJeon Breathing Exercise Program was lower than that of the control group. 2. The degree of back strength, flexibility (trunk flexion) and balance of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 3. The vital capacity of the experimental group that participated in the DanJeon Breathing Exercise Program was higher than that of the control group. 4. The total cholesterol level and atherogenic index of the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. 5. The scores for anxiety and depression in the experimental group that participated in the DanJeon Breathing Exercise Program were lower than those of the control group. In conclusion, DanJeon Breathing Exercise Program promotes the physical and emotional health of women in midlife. The DanJeon Breathing Exercise Program can be utilized as a nursing intervention for the promotion of health in women in midlife.

마스크 착용시 스트레스 해소를 위한 "음~"발성호흡 연구 (On an "Um~" Vocal Breathing to Relieve Stress When Wearing a Mask)

  • 유행병 후 시대에 COVID-19는 아직 완전히 통제되지 않고 있다. 마스크 착용은 여전히 전염병 예방의 주요수단이며, 마스크로 인한 부정적인 영향은 계속되고 있다. 오랫동안 마스크를 착용하면 두 가지 문제가 발생할 수 있다. 첫 번째 문제는 저산소증이고 다른 하나는 심리적 스트레스의 증가이다. 마스크의 부정적인 영향을 줄이기 위해 이 논문에서는 새로운 호흡방식을 제안하였다. 저산소증과 스트레스 증가의 두 가지 문제를 동시에 해결하는 "음~"발성호흡이다. 이 논문에서 새로운 호흡 패턴으로 스트레스를 해소하는 원리를 파악하였고, HRV와 스트레스 지수의 관계를 설명하면서 SDNN을 지표로 사용하여 이 호흡 패턴의 효과를 스트레스지수로 측정하였다. 실험결과 "음 ~"발성호흡이 마스크 착용으로 인한 스트레스를 해소 할 수 있을 뿐만 아니라, 마스크를 쓰지 않을 때도 일상적인 스트레스를 해소해 주기도 한다. 따라서 누구나 쉽게 구현할 수 있는 이 방법은 앞으로 더 대중화될 것이다.

머신러닝을 활용한 통계 분석 기반의 수면 호흡 장애 중증도 예측 (Severity Prediction of Sleep Respiratory Disease Based on Statistical Analysis Using Machine Learning)

  • Currently, polysomnography is essential to diagnose sleep-related breathing disorders. However, there are several disadvantages to polysomnography, such as the requirement for multiple sensors and a long reading time. In this paper, we propose a system for predicting the severity of sleep-related breathing disorders at home utilizing measurable elements in a wearable device. To predict severity, the variables were refined through a three-step variable selection process, and the refined variables were used as inputs into three machine-learning models. As a result of the study, random forest models showed excellent prediction performance throughout. The best performance of the model in terms of F1 scores for the three threshold criteria of 5, 15, and 30 classified as the AHI index was about 87.3%, 90.7%, and 90.8%, respectively, and the maximum performance of the model for the three threshold criteria classified as the RDI index was approx 79.8%, 90.2%, and 90.1%, respectively.
