Evaluation of electron dose distribution obtained from ADAC Pinnacle system against measurement and Monte Carlo method for breast patients

  • Introduction: With the development of dose calculation algorithms for electron beams, 3D RTP systerns are available for electron beam dose distribution commercially. However, no studies evaluated the accuracy of dose calculation with ADAC Pinnacle system for electron beams. So, the accuracy of the ADAC system is investigated by comparing electron dose distributions from ADAC system against the BEAMnrc/DOSXYZnrc. Methods: A total of 33 breast cancer patients treated with 6, 9, and 12MeV electrons in our institution was selected for this study. The first part of this study is to compare the dose distributions of measurement, TPS and the BEAMnrc/DOSXYZnrc code in flat water phantom at gantry zero position and for a 10 ${\times}$ 10 $\textrm{cm}^2$ field. The second part is to evaluate the monitor unit obtained from measurement and TPS. Adding actual breast patient's irregular blocks to the first part, monitor units to deliver 100 cGy to the dose maximum (dmax) were calculated from measurement and 3D RTP system. In addition, the dose distributions using blocks were compared between TPS and the BEAMnrc/DOSXYZnrc code. Finally, the effects of tissue inhomogeneities were studied by comparing dose distributions from Pinnacle and Monte Carlo method on CT data sets. Results: The dose distributions calculated using water phantom by the TPS and the BEAMnrc/ DOSXYZnrc code agreed well with measured data within 2% of the maximum dose. The maximum differences of monitor unit between measured and Pinnacle TPS in flat water phantom at gantry zero position were 4% for 6 MeV and 2% for 9 and 12 MeV electrons. In real-patient cases, comparison of depth doses and lateral dose profiles calculated by the Pinnacle TPS, with BEAMnrc/DOSXYZnrc code has generally shown good agreement with relative difference less than +/-3%. Discussion: For comparisons of real-patient cases, the maximum differences between the TPS and BEAMnrc/DOSXYZnrc on CT data were 10%. These discrepancies were due in part to the inaccurate dose calculation of the TPS, so that it needs to be improved properly. Conclusions: On the basis of the results presented in this study, we can conclude that the ADAC Pinnacle system for electron beams is capable of giving results absolutely comparable to those of a Monte Carlo calculation.

Dose Reduction of the Adolescent Female Breast during Scoliosis Radiography (청소년기 여성의 척추측만증 검사에서 유방입사선량 저감효과)

  • The purpose of this study was to investigate quantitative data on the difference in breast entrance surface dose with changes in focus-film distance, patient posture (anteroposterior-posteroanterior), thoracic wall thickness, rib bone thickness, lung tissue thickness, tube voltage, and high-voltage rectification method in Whole Spine Scanography, which is necessary for the treatment of scoliosis patients. Given a tube voltage of 90 kVp, kerma of 0.1 mGy, focus-film distance of 260 cm, tube voltage ripple rate of 0, filter thickness of 3.5 mm, and thickness of patient's thoracic wall of 120 mm as an X-ray exposure condition, from the simulation results using the Simulation of X-ray Spectra program to confirm the reduction effect of breast entrance surface dose according to the patient's posture (AP and PA), there was a dose reduction effect in aluminum filter thickness of 2.6 times at 3.5 mm, 25.7 times the thoracic wall thickness at 120 mm, 1.43 times higher tube voltage, and 0 to 1.14 times the tube voltage ripple rate. The total dose reduction effect was about 109 times. In order to confirm the dose reduction effect of RANDO phantom posture (AP and PA), from the results of the measurements taken under the conditions that the focus-film distance was 260 cm, the tube voltage was 90 kVp, the tube current was 270 mA, the exposure time was 0.31 sec, and the tube voltage ripple rate of X-ray generators was 0, the entrance surface dose reduction effect of the breast in the PA position was found to be 20.56 times lower than that of the AP position.

Actual condition on accuracy control of mammography equipment in Kyeongsangbuk-do (경상북도 유방촬영장비의 정도관리에 대한 실태조사)

  • The breast cancer has the highest occurrence rate among the female cancers, and as the living style changes, the occurrence is increasing gradually. For the breast cancer test among women, who comprises up to 50% of the total population, the mammography is mainly used as the screening test, and the accuracy control is the most important aspect of the testing. Therefore this research divided the northern part of Kyeongsangbuk-do into 4 regions and investigated the accordance ratio of examination field and light examination field, the total focus using the optical density and compression rate, and the overall maintenance of mammography within the regions. The equipments of 11 hospitals were investigated, and the 7 hospitals passed the standard level of the accordance ratio of examination field. 6 hospitals passed the standard optical density, and 7 hospitals had the passing performance in the compression rate. Fibers, group of specks, and masses within the Mammographic Accreditation Phantom scored 10, being within the standard range. However, only 3 hospitals were equipped with private development processor and illumination. The result reflects the fact that the image quality of breast is not correctly being maintained. Moreover, only 27.27% satisfied all the three categories of compression fitting, accordance ratio of examination field, and phantom image evaluation at the same time. The accuracy control must be maintained more precisely for the accurate diagnosis of breast cancer.

Evaluation of Absorbed Dose according to the Use of Bolus in Opposite Breast during Radiation Therapy of Breast Cancer using VMAT (VMAT를 이용한 유방암 방사선치료 시 반대편 유방의 Bolus 사용에 따른 흡수선량 평가)

  • Although the development of radiation therapy techniques has made the treatment more precise, exposure by radiation is unavoidable beyond the treatment site. In this study, we wanted to evaluate the absorbed dose according to the thickness of the bolus on the opposite side of the treatment in radiation treatment for breast cancer and to analyze the effect of dose reduction. An experiment was conducted on Rando phantom using VMAT treatment methods. Five points of A, B, C, D, and E were selected for the breast opposite the treatment, and when the dosimeters of 5, 10, 15, and 20 mm were used. The highest absorbed dose at point D closest to the treatment point was measured and lowest at point B furthest from the treatment point. The mean absorbed dose was 8.61 cGy When the bolus is not used and 8.10, 7.94, 8.06, and 8.10 cGy Depending on the thickness of the bolus. In this study, bolus was intended to be used to analyze the dose-reducing effects of breasts on the other side of the treatment. The results of the study showed the effect of dose reduction and the appropriate bolus thickness should be set up to reduce the dose in normal tissues.

Evaluation of Ovary Dose for woman of Childbearing age Woman with Breast cancer in tomotherapy (가임기 여성의 유방암 토모치료 시 난소선량 평가비교)

  • Purpose : The aim of this study is to evaluate unwanted scattered dose to ovary by scattering and leakage generated from treatment fields of Tomotherapy for childbearing woman with breast cancer. Materials and Methods : The radiation treatments plans for left breast cancer were established using Tomotherapy planning system (Tomotherapy, Inc, USA). They were generated by using helical and direct Tomotherapy methods for comparison. The CT images for the planning were scanned with 2.5 mm slice thickness using anthropomorphic phantom (Alderson-Rando phantom, The Phantom Laboratory, USA). The measurement points for the ovary dose were determined at the points laterally 30 cm apart from mid-point of treatment field of the pelvis. The measurements were repeated five times and averaged using glass dosimeters (1.5 mm diameter and 12 mm of length) equipped with low-energy correction filter. The measures dose values were also converted to Organ Equivalent Dose (OED) by the linear exponential dose-response model. Results : Scattered doses of ovary which were measured based on two methods of Tomo helical and Tomo direct showed average of $64.94{\pm}0.84mGy$ and $37.64{\pm}1.20mGy$ in left ovary part and average of $64.38{\pm}1.85mGy$ and $32.96{\pm}1.11mGy$ in right ovary part. This showed when executing Tomotherapy, measured scattered dose of Tomo Helical method which has relatively greater monitor units (MUs) and longer irradiation time are approximately 1.8 times higher than Tomo direct method. Conclusion : Scattered dose of left and right ovary of childbearing women is lower than ICRP recommended does which is not seriously worried level against the infertility and secondary cancer occurrence. However, as breast cancer occurrence ages become younger in the future and radiation therapy using high-precision image guidance equipment like Tomotherapy is developed, clinical follow-up studies about the ovary dose of childbearing women patients would be more required.

A Comparative Study on Image Quality of Breast Image Tests using ACR Phantom (ACR 팬텀을 이용한 시스템별 유방검사 영상의 비교 연구)

  • Recently, interests and utilization on Computed Radiography(CR) and Digital Radiography(DR) tends to increase owing to an introduction of Picture Archiving and Communication System(PACS) and an accuracy control of special medical equipment for a breast imaging examination. This study was intended to compare and analyze a detector's imaging quality by each system to be used for the breast imaging examination by using ACR Phantom used at the accuracy control. As an evaluation method, a significance and reliability of image's value using the ACR Phantom was analyzed by using SPSS program. The results are followed. 1. For the fiber, there was 3.9 score in Screen-Film, 4.2 score in CR($50{\mu}m$), 3.2 score in CR($100{\mu}m$), and 4.2 score in DR. There was the high score in the order of CR($50{\mu}m$), DR, Screen-Film, and CR($100{\mu}m$)(P<0.05). 2. For the calcification, there was 2.7 score in Screen-Film, 2.5 score in CR($50{\mu}m$), 2.0 score in CR($100{\mu}m$), and 2.9 score in DR. There was the high score in the order of DR, Screen-Film, CR($50{\mu}m$), and CR($100{\mu}m$).(0.025(P<0.05). 3. For Mass, there was 3.8 score in Screen-Film, 3.8 score in CR($50{\mu}m$), 3.6 score in CR($100{\mu}m$), and 4.5 score in DR. There was the high score in the order of DR, CR($50{\mu}m$), Screen-Film, and CR($100{\mu}m$) (P<0.1). 4. As the total score, there was 10.4 score in Screen-Film, 10.6 score in CR($50{\mu}m$), 8.7 score in CR($100{\mu}m$), and 11.3 score in DR. There was the high score in the order of DR, $CR(50{\mu}m$), Screen-Film, and $CR(100{\mu}m$). As shown in the above results, it can be known that DR and Screen-Film System has higher image quality than CR. But, DR has unstability caused by element, and Screen-Film has the low image quality caused by artifact as disadvantages. When Dual-Side CR($50{\mu}m$) was used among CR systems which had the problem of low image quality, it was indicated that there was no difference with Screen-Film System. Because the radiation imaging examination tends to become digitalized, each system for the breast imaging examination will need to be developed and supplemented.

  • The aim of this study is evaluation of dose distribution on radiation therapy planning system with the CT image of high-density material inserted phantom. Gammex 467 Tissue Characterization Phantom is used to acquire an image similar to the human tissues and insert a Titanium to generate metal artifact. The acquired images were reconstructed with Metal Artifact Reduction for Orthopedic Implants (O-MAR). By using the treatment planning system, the volume was analyzed and dose distribution was extracted. Photon dose distribution in linear accelerator was measured by the $MapCHECK^{TM}$ and compared with planned and measured dose distributions. In result of the comparative analysis, when artifact is generated by Titanium, The volume applied O-MAR was increased 6.8% to BR-12 Breast and 40.2% to LV 1 Liver. After O-MAR was used, Dose distribution was higher 1.4 to 1.6% than before. Consequently, The artifact caused by metal objects should be removed if possible, and after that used in the radiotherapy treatment plan can be considered to reduce errors.

Development of a Wireless Gamma-ray Probe for Diagnosing and Evaluation of its Effectiveness (진단용 무선 감마선 프로브 개발 및 유용성 평가)

  • We developed a wireless gamma-ray probe based on radiation photon counting method to diagnose and detect remaining lesions after surgery, and its effectiveness was evaluated using calibration sources and a phantom. The probe was designed and miniaturized using a semi-conductor-based radiation sensor, and a Bluetooth remote communication module was used to implement the wireless diagnosis and detection system. Moreover, a remote monitoring system was implemented to monitor affected areas during diagnosis and surgery. To assess the effectiveness of the developed probe in this study, calibration sources $^{57}Co$, $^{133}Ba$, $^{22}Na$ and $^{137}Cs$ and a chicken breast phantom were used. Furthermore, the probe's detection response to gamma ray was confirmed through evaluation. Its clinical applicability was verified by assessing the response linearity and detection direction according to gamma-ray intensity, as well as the detection efficiency according to the depth of the gamma source in the phantom.

Effect of Gd-based MR contrast agents on CT attenuation of PET/CT for quantitative PET-MRI study

  • We evaluate the influence of MR contrast agent on positron emission tomography (PET) image using phantom, animal and human studies. Phantom consisted of 15 solutions with the mixture of various concentrations of Gd-based MR contrast agent and fixed activity of [$^{18}F$]FDG. Animal study was performed using rabbit and two kinds of MR contrast agents. After injecting contrast agent, CT or MRI scanning was performed at 1, 2, 5, 10, and 20 minutes. PET image was obtained using clinical PET/CT scan, and attenuation correction was performed using the all CT images. The values of HU, PET activity and MRI intensity were obtained from ROIs in each phantom and organ regions. In clinical study, patients (n=20) with breast cancer underwent sequential acquisitions of early [$^{18}F$]FDG PET/CT, MRI and delayed PET/CT. In phantom study, as the concentration increased, the CT attenuation and PET activity also increased. However, there was no relationship between the PET activity and the concentration in the clinical dose range of contrast agent. In animal study, change of PET activity was not significant at all time point of CT scan both MR contrast agents. There was no significant change of HU between early and delayed CT, except for kidney. Early and delayed SUV in tumor and liver showed significant increase and decrease, respectively (P<0.05). Under the condition of most clinical study (< 0.2 mM), MR contrast agent did not influence on PET image quantitation.

Evaluation of the Breast plan using the TLD and Mosfet for the skin dose (열형광선량계(TLD)와 MOSFET을 이용한 유방암 방사선치료계획에 대한 피부선량 평가)

  • Purpose : The measurement of skin dose is very important that treatment of breast cancer. On account of the cold or hot dose as compared with prescription dose, it is necessary to analyse the skin dose occurring during the various plan of the breast cancer treatment. At our hospital, we want to apply various analyses using a diversity of dosimeters to the breast cancer treatment. Subjectss and Methods : In the study, the anthropomorphic phantom is used to find out the dose difference of the skin(draining site), scar and others occurring from the tangential treatment plan of breast cancer. We took computed tomography scan of the anthropomorphic phantom and made plans for the treatment planing using open and wedge, Field-in-Field, Dose fluence. Using these, we made a comparative analysis of the dose date points by using the Eclipse. For the dose comparison, we place the anthropomorphic phantom in the treatment room and compared the measurement results by using the TLD and MOSFET on the dose data points. Results : On the central point of treatment planing basis, the upward and downward skin dose measured by the MOSFET was the highest when the fluence was used. The skin dose of inner and outer was distinguished from the figure(5.7% ~ 10.3%) when the measurements were fulfilled by using TLD and MOSFET. The other side of breast dose was the lowest in the open beam, on the other hand, is highest in the Dose fluence plan. In the different kinds of treatment, the dose deviation of inner and outer was the highest, and so this was the same with the TLD and MOSFET measurement case. The outer deviation was highest in the TLD, and the Inner'was highest in the MOSFET. Conclusion : Skin dose in relation to the treatment plan was the highest in the planing using the fluence technique in general and it was supposed that the high dose had been caused by the movement of the MLC. There's some differences among the all the treatment planning, but the sites such as IM node occurring the lack of dose, scar, drain site are needed pay close attention. Using the treatment planning of dose fluence is good to compensate the lack of dose, but It increases the dose of the selective range rather than the overall dose. Therefore, choosing the radiotherapy technique is desirable in the lights of the age and performance of the patient.
