A Trial of 6-MV Linear Accelerator Radiation Therapy (RT) for Breast Cancer (6MV 선형가속기를 사용한 유방암 치료)

  • Radiation therapy(RT) has been used in the treatment of breast cancer for over 80years. Technically, it should include a part or all of such areas as chest wall or breast, axilla, internal mammary nodes (IM) and supraclavicular nodes (SCL). Authors tried three-field technique for the treatment of breast cancer using 6-MV linear accelerator, exclusively the department of radiology. Kang-Nam St. Mary's Hospital, at Catholic Medical College. The field junction was checked by a Phantom study and radiation doses measured by film densitometry and TLD. The 3 fields we used in this study were two isocentric opposing tangential fields encompassing the breast, chest wall and occasionally IM and one single anterior field encompassing the axilla and SCL. sing appropriate beam blocks and boluses, we were able to avoid unwanted intrinsic divergency of photon beam. Blocking also enabled us to set-up precise radiation field with ease.

Usefulness of Breast Lymphoscintigraphy after Whole Body Bone Scan (유방암 환자에서 전신 뼈 검사 후 감시림프절 위치 파악 검사의 유용성)

  • Purpose: Breast cancer is known to be more vulnerable to bone metastasis and lymph node metastasis than other types of cancer, and nuclear examinations whole body bone scan and lymphoscintigraphy are performed commonly before and after breast cancer operation. In case whole body bone scan is performed on the day before lymphoscintigraphy, the radiopharmaceutical taken into and remaining in the bones provides anatomical information for tracking and locating sentinel lymph nodes. Thus, this study purposed to examine how much bone density affects in locating sentinel lymph nodes. Materials and Methods: The subjects of this study were 22 patients (average age $52{\pm}7.2$) who had whole body bone scan and lymphoscintigraphy over two days in our hospital during the period from January to December, 2009. In the blind test, 22 patients (average age $57{\pm}6.5$) who had lymphoscintigraphy using $^{57}Co$ flood phantom were used as a control group. In quantitative analysis, the relative ratio of the background to sentinel lymph nodes was measured by drawing ROIs on sentinel lymph nodes and the background, and in gross examination, each of a nuclear physician and a radiological technologist with five years' or longer field experience examined images through blind test in a five-point scale. Results: In the results of quantitative analysis, the relative ratio of the background to sentinel lymph nodes was 14.2:1 maximum and 8.5:1 ($SD{\pm}3.48$) on the average on the front, and 14.7:1 maximum and 8.5:1 ($SD{\pm}3.42$) on the average on the side. In the results of gross examination, when $^{57}Co$ flood phantom images were compared with images containing bones, the score was relative high as 3.86 ($SD{\pm}0.35$) point for $^{57}Co$ flood phantom images and 4.09 ($SD{\pm}0.42$) for bone images. Conclusion: When whole body bone scan was performed on the day before lymphoscintigraphy, the ratio of the background to sentinel lymph nodes was over 10:1, so there was no problem in locating lymph nodes. In addition, we expect to reduce examination procedures and improve the quality of images by indicating the location of sentinel lymph nodes using bone images as body contour without the use of a source.

Evaluation of Scattered Dose to the Contralateral Breast by Separating Effect of Medial Tangential Field and Lateral Tangential Field: A Comparison of Common Primary Breast Irradiation Techniques (유방암 접선조사 치료 방법에 대한 반대쪽 유방에서의 산란선량 평가)

  • Purpose: The concern of improving the quality of life and reducing side effects related to cancer treatment has been a subject of interest in recent years with advances in cancer treatment techniques and increasing survival time. This study is an analysis of differing scattered dose to the contralateral breast using common different treatment techniques. Materials and Methods: Eclipse 10.0 (Varian, USA) based $30^{\circ}$ EDW (Enhanced dynamic wedge) plan, $15^{\circ}$ wedge plan, $30^{\circ}$ wedge plan, Open beam plan, FiF (field in field) plan were established using CT image of breast phantom which in our hospital. Each treatment plan were designed to exposure 400 cGy using CL-6EX (VARIAN, USA) and we measured scattered dose at 1 cm, 3 cm, 5 cm, 9 cm away from medial side of the phantom at 1 cm depth using ionization chamber (FC 65G, IBA). We carried out measurement by separating effect of medial tangential field and lateral tangential field and analyze. Results: The evaluation of scattered dose to contralateral breast, $30^{\circ}$ EDW plan, $15^{\circ}$ wedge plan, $30^{\circ}$ wedge plan, Open beam plan, FIF plan showed 6.55%, 4.72%, 2.79%, 2.33%, 1.87% about prescription dose of each treatment plan. The result of scattered dose measurement by separating effect of medial tangential field and lateral tangential field results were 4.94%, 3.33%, 1.55%, 1.17%, 0.77% about prescription dose at medial tangential field and 1.61%, 1.40%, 1.24%, 1.16%, 1.10% at lateral tangential field along with measured distance. Conclusion: In our experiment, FiF treatment technique generates minimum of scattered dose to contralateral breast which come from mainly phantom scatter factor. Whereas $30^{\circ}$ wedge plan generates maximum of scattered doses to contralateral breast and 3.3% of them was scattered from gantry head. The description of treatment planning system showed a loss of precision for a relatively low scatter dose region. Scattered dose out of Treatment radiation field is relatively lower than prescription dose but, in decision of radiation therapy, it cannot be ignored that doses to contralateral breast are related with probability of secondary cancer.

  • Purpose As breast cancer patients continue to increase every year, cases of BSGI are on the rise with a heavier reliance on it. However, BSGI protocol in hospitals was not studied enough despite it was covered by hospital's condition and recommendation of manufacturers. The objective of the study was an examination of methods to be applicable to BSGI protocols, putting the self-development phantom to use in quality assessment of the images. Materials and Methods Dilon 6800 (Dilon Technologies Inc, Newport News, USA) was used in the study and five different sizes of sphere were distinctively produced in the phantom. The study used $^{99m}TcO_4$. The cases were classified in to three categories that background radioactivity to region of interest as ratio of 2: 4: 8, They were acquired images for 5, 7, 10mins. The acquired image was set region of interest according to the size of sphere, and We analyzed quantitative and qualitative analysis. The acquired data statistically analyzed with SPSS ver.18.0. Results As the result of quantitative and qualitative analysis, count rate of each sphere in accordance with difference of injection dose showed that higher count rate as injection dose and sphere size increased (P<0.005). Count rate of each sphere in accordance with difference of acquisition time showed that higher count rate as acquisition time and sphere size increased (P<0.005). Contrast noise ratio of each sphere in accordance with difference of injection dose showed that higher contrast noise ratio as injection dose increased. Particularly, Contrast noise ratio of eight times ratio images was the highest among. Contrast noise ratio of each sphere in accordance with difference of acquisition time showed that higher contrast noise ratio as acquisition time increased. And, Contrast noise ratio of seven minute image was the highest among (P<0.005). Conclusion There was significant change of Contrast noise ratio through quantitative and qualitative analysis. Moreover, We found usefulness of phantom. If Institutions identified image through the phantom study and they made BSGI protocol, We expected to help the improvement of diagnostic value of the images.

The evaluation of contralateral breast's dose and shielding efficiency by breast size about breast implant patient for radiation therapy (인공 유방 확대술을 받은 환자의 유방암 치료 시 크기에 따른 반대 측 유방의 피폭 선량 및 차폐 효율 평가)

  • Purpose : To evaluate the dose on a contralateral breast and the usefulness of shielding according to the distance between the contralateral breast and the side of the beam by breast size when patients who got breast implant receive radiation therapy. Materials and Methods : We equipped 200 cc, 300 cc, 400 cc, and 500 cc breast model on the human phantom (Rando-phantom), acquired CT images (philips 16channel, Netherlands) and established the radiation treatment plan, 180 cGy per day on the left breast (EclipseTM ver10.0.42, Varian Medical Systems, USA) by size. We set up each points, A, B, C, and D on the right(contralateral) breast model for measurement by size and by the distance from the beam and attached MOSFET at each points. The 6 MV, 10 MV and 15 MV X-ray were irradiated to the left(target) breast model and we measured exposure dose of contralateral breast model using MOSFET. Also, at the same condition, we acquired the dose value after shielding using only Pb 2 mm and bolus 3 mm under the Pb 2 mm together. Results : As the breast model is bigger from 200 cc to 500 cc, The surface of the contralateral breast is closer to the beam. As a result, from 200 cc to 500 cc, on 180 cGy basis, the measurement value of the scattered ray inclined by 3.22 ~ 4.17% at A point, 4.06 ~ 6.22% at B point, 0.4~0.5% at C point, and was under 0.4% at D point. As the X-ray energy is higher, from 6 MV to 15 MV, on 180 cGy basis, the measurement value of the scattered ray inclined by 4.06~5% at A point, 2.85~4.94% at B point, 0.74~1.65% at C point, and was under 0.4% at D point. As using Pb 2 mm for shield, scattered ray declined by average 9.74% at A and B point, 2.8% at C point, and is under 1% at D point. As using Pb 2 mm and bolus together for shield, scattered ray declined by average 9.76% at A and B point, 2.2% at C point, and is under 1% at D point. Conclusion : Commonly, in case of patients who got breast implant, there is a distance difference by breast size between the contralateral breast and the side of beam. As the distance is closer to the beam, the scattered ray inclined. At the same size of the breast, as the X-ray energy is higher, the exposure dose by scattered ray tends to incline. As a result, as low as possible energy wihtin the plan dose is good for reducing the exposure dose.

Utilization of Mammogram in the Tomosynthesis (토모신테시스의 유방촬영에서의 활용)

  • This study evaluated the diagnostic value and compares the Mammogram Tomosynthesis, and as compared to the AGD, was studied with respect to utilization of Tomosynthesis. During January 2015 one month were enrolled patients admitted to 62 people present. The ACR phantom was used. AEC was set to be. kVp is fixed and given a step-by-step changing the mAs analyzed AGD. Tomosynthesis was superior to the distinction of breast lesions when compared with Mammogram showed a noticeable difference in contrast. AGD(Average Glandular Dose) was higher 0.33 mGy. However, in the long run, the dose was reduced. Tomosynthesis is therefore increase the diagnostic value of the breast, a examination that can reduce the dose.

  • Magnetic Resonance Imaging(MRI) has chemical shift phenomenon between fat and water, and the phenomenon has influence on structure enclosed by fat. Strong signals emitted from fat often generate false artefact, which reflects the importance of fat suppression techniques. There have been a number of researches on fat suppression techniques, but using fat suppression method alone in MRI can cause difficultproblems in diagnosis. This paper aims to study a fat suppression method by Chemical Shift Selective saturation(CHESS). This research describes the theoretical background and the experiment on water and fat phantom with MR instruments. In the experiment, CHESS pulse was designed by utilising Matlap program, and the pulse diagram was generated for the Pre-saturation process. The experiment using water and fat phantom was applied to C-spine, L-spine and Breast, and produced successful fat suppression results. This experiment has proved that the CHESSpulse fat suppression is a very helpful technique in diagnosing medical imaging.   • Mechanical property of tissue is closely related to diseases such as breast cancer, prostate cancer, cirrhosis of the liver, and atherosclerosis. Therefore measurement of tissue mechanical property is important for a better diagnosis. Ultrasound elastography has been developed as a diagnostic modality for a number of diseases that maps mechanical property of tissue. Optical coherence elastography (OCE) has a higher spatial resolution than ultrasound elastography. OCE, therefore, could be a great help for early diagnosis. In this study, we made tissue phantoms and measured their compressive moduli with a rheometer measuring the response to applied force. Uniaxial strain of the tissue phantom was also measured with OCE by using cross-correlation of speckles and compared with the results from the rheometer. In order to compare stiffness of tissue phantoms by OCE, the applied force should be measured in addition to the strain. We, however, did not use a load cell that directly measures the applied force for each sample. Instead, we utilized one silicone film (called as reference phantom) for all OCE measurements that indirectly indicated the amount of the applied force by deformation. Therefore, all measurements were based on displacement, which was natural and effective for image-based elastography such as OCE.

Comparative Analysis According to Acquisition Type by Using the Resolution Phantom for Mammography Equipment (유방촬영의 영상획득 방법에 따른 해상력 차트의 비교 분석)

  • Nowadays, diseases related with breast are increasing rapidly and because of this high quality of resolution images is required to get clear detail specially for early detection and diagnosis. It has a tendency to use digital equipments than analog one in clinic. In this experiment, DR, CR, and Film are used for the resolution applied by AEC. Resolution phantom in DR was $7LP{\cdot}mm^{-1}$ in both verticality and horizontality. In CR, however, it was $6LP{\cdot}mm^{-1}$ in both which was lower this standard. The resolution stayed in range of standard in Film but it showed differences between $11{\sim}14LP{\cdot}mm^{-1}$ Overall, the difference of resolution was displayed Film, DR and CR, in order, which means the study is needed for more high quality of digital images.

Comparison and validation of Brass mesh bolus using tissue equivalent bolus in the breast cancer radiotherapy (유방암 방사선치료시 조직등가보상체와의 비교를 통한 Brass mesh bolus의 유용성 평가)

  • Purpose: In breast cancer radiotherapy, brass mesh bolus has been recently studied to overcome disadvantage of conventional bolus. The purpose of this study is to investigate the stability of first introduced the brass mesh in the country, and evaluate the skin surface dose of that. Materials and Methods: The measurement of skin surface dose was evaluated to verify similar thickness of the Brass mesh bolus that compared conformal tissue equivalent bolus with 5 mm thickness. We used 6 MV photons on an ELEKTA VERSA linear accelerator and optically stimulated luminescent dosimeter (OSLD). In addition, two opposed beam using IMRT phantom was applied to comparative study of brass mesh bolus between tissue equivalent bolus. Results: The results showed that similar thickness of the Brass mesh bolus was 3 mm compared with 5 mm tissue equivalent bolus by measuring the skin surface dose of solid phantom. The surface dose for IMRT thorax phantom using 3 mm brass mesh bolus was about 1.069 times greater than that using tissue equivalent bolus. Conclusion: In this study, we found that the brass mesh bolus improved better reduction of skin sparing effect and dose uniformity than tissue equivalent bolus. However evaluation for various clinic cases should be investigated.
