Studies on the Disposition of Brazilin in rats (천연색소 Brazilin의 체내동태에 관한 연구)

  • The disposition of Brazilin including plasma concentration-time profiles, excretions via urine and bile, and plasma protein binding was investigated after intravenous or oral administration of radio labeled Brazilin ($^3H-Brazilin$) to male Wistar rats. The main pharmac:okinetic parameters were as follows; $t\;_{ 1/2}$, 13.71 hr; AUC, $53.38\;\mu\textrm{g}{\cdot}hr/ml$; AUMC, $1013.4I\;\mu\textrm{g}{\cdot}hr^2/ml$, MRT, 18.95 hr; Vss, 17778 mllkg and CL, 936.77 ml/hr.kg. The 2nd peak was found in the plasma concentration-time profiles indicating potential enterohepatic circulation. The enterohepatic circulation was supported by the bile excretion. After oral administration, about 64.4 % of administered radioactivity was excreted into the bile within 10 hours and its excretion rate reached maximum at 3 hours after administration. The Vss was extremely high, 17.8 l/kg indicating distribution of brazilin in most organs (tissues) with high concentration of brazilin in some organs. Brazilin was distributed into most of organs (spleen, adrenal, pancreas, kidney, thymus, lung, heart, liver, prostate, epididymus, testis, fat, muscle and done) except brain. High concentration of Brazilin was detected especially in liver, kidney, epididymus and testis. Approximately, 62.9% and 44.1% of the dose was excreted for intravenous and oral administration, respectively. About 80% of the dose eventually excreted into urine was excreted within 24 hr after dosing. Plasma protein binding of brazilin resulted in $40\;{\pm}\;4%$ by ultrafiltration method.

  • The possibility of metaverse system to be a catalyst for hyper-connected society will be dependent on the speed of connected technological development and its social utilization in the same manner as AI technology. Putting these technical realization processes in brackets, this paper focus on some philosophical-political issues in connection with cognitive-ecological changes in the future cinema which will be influenced by the complexive techno-socio couples of accelerated development of metaverse system. Generally speaking, essence of metaverse system seems to be the degree of immersion by technical accuracy, but is not true. In perspective of cognitive-ecology, flow degree of a picture or photograph is relied not on 'accuracy of representation' but on its message's contextual link-up. In this aspect, real potentiality of metaverse system shall be understood in the context of cognitive-ecological changes of human brain's multi-intelligence networking abilities(intersection of augmentation-simulation and outside-inside) which will be activated in the new structure of natural-social-technological coupling of metaverse system. These cognitive-ecological potentialities have been partially actualized in the cinematic process of tripod mimesis for the longest time, [real contradiction/conflicts (Mimesis-1) -->fictional solutions of cinema (Mimesis-2) --> selective interpretation of spectator's wish fulfillment (Mimesis-3) --> real change (Mimesis-1')]. Therefore metaverse's real potentiality must be considered to be dependent on the possibility of deepening and extending of cinematic circulation between real seperation/problems and ideal connection/solutions. In this context, advanced metaverse system can be compared as a modern technical version of ideal circulation of physics and metaphysics

Control Simulation of Left Ventricular Assist Device using Artificial Neural Network (인공신경망을 이용한 좌심실보조장치의 제어 시뮬레이션)

  • In this paper, we present a neural network identification and a control of highly complicated nonlinear left ventricular assist device(LVAD) system with a pneumatically driven mock circulation system. Generally, the LVAD system needs to compensate for nonlinearities. It is necessary to apply high performance control techniques. Fortunately, the neural network can be applied to control of a nonlinear dynamic system by learning capability. In this study, we identify the LVAD system with neural network identification(NNI). Once the NNI has learned the dynamic model of the LVAD system, the other network, called neural network controller(NNC), is designed for a control of the LVAD system. The ability and effectiveness of identifying and controlling the LVAD system using the proposed algorithm will be demonstrated by computer simulation.

New paradigms on siRNA local application

  • Small interfering RNA (siRNA) functions through pairing with specific mRNA sequences and results in the mRNA's degradation. It is a potential therapeutic approach for many diseases caused by altered gene expression. The delivery of siRNA is still a major problem due to its rapid degradation in the circulation. Various strategies have been proposed to help with the cellular uptake of siRNA and short or small hairpin RNA (shRNA). Here, we reviewed recently published data regarding local applications of siRNA. Compared with systemic delivery methods, local delivery of siRNA/shRNA has many advantages, such as targeting the specific tissues or organs, mimicking a gene knockout effect, or developing certain diseases models. The eye, brain, and tumor tissues are 'hot' target tissues/organs for local siRNA delivery. The siRNA can be delivered locally, in naked form, with chemical modifications, or in formulations with viral or non-viral vectors, such as liposomes and nanoparticles. This review provides a comprehensive overview of RNAi local administration and potential future applications in clinical treatment.

Saccular Aneurysm at the Anterior Communicating Artery Complex Associated with an Accessory Middle Cerebral Artery : Report of Two Cases and Review of the Literature

  • Accessory middle cerebral artery (MCA) is an infrequent vascular anomaly of the brain. Cerebral aneurysms associated with this anomalous artery are also very rare. To our knowledge, there have only been ten previous reports of an aneurysm associated with accessory MCA. The authors present two patients with accessory MCA-related aneurysms. A 38-year-old male and a 59-year-old female both presented with sudden-onset severe headache. In both patients, computed tomography (CT) scan revealed subarachnoid hemorrhage. A subsequent angiogram demonstrated an accessory MCA arising from the anterior cerebral artery (ACA) and a saccular aneurysm at the anterior communicating artery (ACoA) complex associated with an accessory MCA. Surgical clipping allowed for complete exclusion of the aneurysm from the arterial circulation. Based on our review of the ten cases of aneurysms associated with accessory MCA documented in the literature, we suggest that accessory MCA-related aneurysms can be classified according to whether the accessory MCA originates from the proximal A1 segment or from the ACoA complex. We also emphasize the importance of precise interpretation of preoperative angiograms and intraoperative precaution in determining the presence of this anomalous artery prior to temporary clip placement.

Successful Damage Control Resuscitation with Resuscitative Endovascular Balloon Occlusion of the Aorta in a Pediatric Patient

  • Resuscitative endovascular balloon occlusion of the aorta (REBOA) is considered an emerging adjunct therapy for profound hemorrhagic shock, as it can maintain temporary stability until definitive repair of the injury. However, there is limited information about the use of this procedure in children. Herein, we report a case of REBOA in a pediatric patient with blunt trauma, wherein the preoperative deployment of REBOA played a pivotal role in damage control resuscitation. A 7-year-old male patient experienced cardiac arrest after a motor vehicle accident. After 30 minutes of cardiopulmonary resuscitation, spontaneous circulation was achieved. The patient was diagnosed with massive hemoperitoneum. REBOA was then performed under ongoing resuscitative measures. An intra-aortic balloon catheter was deployed above the supraceliac aorta, which helped achieved permissive hypotension while the patient was undergoing surgery. After successful bleeding control with small bowel resection for mesenteric avulsion, thorough radiologic evaluations revealed hypoxic brain injury. The patient died from deterioration of disseminated intravascular coagulation. Although the patient did not survive, a postoperative computed tomography scan revealed neither remaining intraperitoneal injury nor peripheral ischemia correlated with the insertion of a 7-Fr sheath. Hence, REBOA can be a successful bridge therapy, and this result may facilitate the further usage of REBOA to save pediatric patients with non-compressible torso hemorrhage.

Keyhole Approach and Neuroendoscopy for Cerebral Aneurysms

  • Treating diseases in the field of neurosurgery has progressed concomitantly with technical advances. Here, as a surgical armamentarium for the treatment of cerebral aneurysms, the history and present status of the keyhole approach and the use of neuroendoscopy are reviewed, including our clinical data. The major significance of keyhole approach is to expose an essential space toward a target, and to minimize brain exposure and retraction. Among several kinds of keyhole approaches, representative keyhole approaches for anterior circulation aneurysms include superciliary and lateral supraorbital, frontolateral, mini-pterional and mini-interhemispheric approaches. Because only a fixed and limited approach angle toward a target is permitted via the keyhole, however, specialized surgical devices and preoperative planning are very important. Neuroendoscopy has helped to widen the indications of keyhole approaches because it can supply illumination and visualization of structures beyond the straight line of microscopic view. In addition, endoscopic indocyanine green fluorescence angiography is useful to detect and correct any compromise of the perforators and parent arteries, and incomplete clipping. The authors think that keyhole approach and neuroendoscopy are just an intermediate step and robotic neurosurgery would be realized in the near future.

Development of work uniform design for people with disabilities applying a universal design concept (유니버설 디자인 개념이 적용된 장애인을 위한 근무복 디자인 개발)

  • The purpose of this study was to develop a workwear design that takes into account the characteristics of people with severe developmental disabilities who can engage in vocational activities. The aim was to identify needs according to the specific characteristics of people with severe disabilities to design work clothes and develop products according to universal design guidelines. This research method was conducted through representative interviews from a company employing people with severe dis- abilities in Daegu to determine the requirements for workers-related work clothes, and then applied universal design guidelines to perform appropriate design. The results of the study show that the hygiene and warmth of clothing are important for people with developmental disabilities. Therefore, the use of bright materials is required. Second, people with brain lesions often have low body temperature due to difficulties with blood circulation, for which warmth is a required factor. Third, people with severe developmental disabilities should not be differentiated in comparison to people without disabilities, therefore, it was important to use nondiscriminatory designs. Accordingly, it was more efficient to modify and supplement clothing designed for non-disabled people with hidden functions to suit specific characteristics, rather than to develop specialized clothing. These demands were found to conform to what is referred to as a universal design concept, through which three nondiscriminatory shirt designs and two easy-to-use pants were designed.

Preoperative Evaluation and Significance of Carotid Resection in Advanced Cervical Metastatic Cancer (진행된 경부 전이암에서 경동맥 절제를 위한 술전검사와 절제술의 의의)

  • Objectives: We studied what is the role of elective carotid artery resection in the management of advanced cervical metastatic cancer. Materials and Methods: 5 patients with elective carotid artery resection in advanced metastatic cervical cancer were reviewed retrospectively. The patients underwent complete neuroradiologic evaluation, including CT/MRI. angiography, duplex doppler, balloon occlusion test with EEG, and brain SPECT for determination of compatible collateral circulation after carotid artery resection. Results: Perioperative complication were appeared in 2 patients those were middle cerebral arterial infarction and mediastinal bleeding. Postoperative mortality rate was 20%. 4 patients recurred within 1 year. Conclusion: Preoperative collateral study rarely provide whether resection carotid artery or not. Elective carotid artery resection cannot provide locoregional control of tumor and don't promote survival.

Effect of Ginkgo biloba extract in treatment of cerebral infarction (腦硬塞 患者에서 銀杏葉椎出物의 治療效果)

  • Background: Ginkgo biloba extract is used in disorser of cerebral and peripheral blood circulation, dysfunction of brain, atherosclerosis etc., but there are little study about GbE in oriental medicine. We wished to assessthe efficacy of GbE for the treatment of cerebral infarction Method : The study group comprised 40 patients who arrived at hospital during 48 hours after attack. All patient were devided into two group. The control group was treated with Uhuangcheongsimhuan, Seonghyangjeonggisan, acupuncture therapy only, while the GbE group was treated with above therapy plus 5 days of administration of GbE(40mg three times per day). Result: 1. Symptom improve scores did not showed significant difference between control and GbE group. 2. Vasoreactivity of carotid siphon increased significantly in GbE group after treatment (in the left only : p<0.05). 3. Vasorcactivity of radial artery increased significantly in GbE group after treatment(in the right only ; p<0.05). 4. PT, a-PTT, Fibrinogen did not showed significant changes between before and after treatment in both group. Conclusion: These findings suggest that vasoreactivity increasing effect of GbE may be useful in the prevention and treatment of cerebral infarction. But the vasoreactivity increasing effect of GbE may be different from symptom imroving.
