두경부(Head & Neck) CT 검사 시 장기의 유효선량 측정 (Effective Dose Determination From CT Head & Neck Region)

  • 두경부(Head & Neck) CT(Computed Tomography)검사에서 환자가 받는 피폭선량 측정을 위하여 인체등가물질로 만든 Rando phantom과 유리선량계를 이용하여 두경부 검사에 따른 환자의 흡수선량의 변화를 실험을 통하여 연구하였다. 인체두부모형을 안와신경(optic nerve), 교뇌(pons), 소뇌(cerebellum), 갑상선(thyroid)으로 나누어, 두경부(Head & Neck) 부위의 검사를 단독검사(Brain, 3D Facial, Temporal, Brain Angiography, 3D Cervical Spine)와 복합검사(Brain+Brain Angiography, Brain+3D Facial, Brain+Temporal, Brain+3D Cervical spine, Brain+3D Facial+Temporal, Brain+3D Cervical Spin+Angiography)로 구분하여 유효선량의 변화를 실험한 후 결과를 측정하였다. 단순 Brain검사와 Brain Angio검사에는 optic nerve에 유효 선량이 높게 분석되었으며, 또한 Temporal검사에는 Pons에, 3D facial 검사와 3D Cervical Spin검사에는 thyroid의 유효선량 값이 높게 나타났다. 복합적으로 이루어는 검사 중 두경부의 Brain+Brain Angio의 검사는 cerebellum의 부위, Brain+3D facial 검사와 Brain+3D Cervical Spin의 복합검사는 thyroid의 부위, Brain+Temporal의 검사에는 pon's 부위 유효 선량 값이 높게 나타났다. Brain +3D facial +Temporal의 복합검사와 Brain+3D Cervical Spin+Angio의 복합검사는 thyroid의 부위에 유효 선량 값이 높게 분석 되었다. 본 연구 결과 Brain+3D Cervical Spin+Brain Angio 복합검사인 경우의 유효 선량은 2.51858 mSv로 일반인의 연간 유효선량한도 1 mSv의 피폭을 초과하는 결과가 나왔다. 또한, Brain 단순 검사 시 optic nerve는 0.31312 mSv의 유효선량으로 향후 방사선학 검사가 이루어질 경우, 두경부의 일반인의 연간 유효선량을 훨씬 초과할 것이라 사료된다. 따라서 진료의 필요성에 의해서 시행되는 CT검사일지라도 질환 병변의 특성에 맞게 CT촬영조건 변화를 주면서 환자의 피폭선량을 최소한으로 할 수 있는 다양한 검사방법의 연구가 필요하다고 사료된다.

Macromolecular and Elemental Composition Analyses of Leuconostoc mesenteroides ATCC 8293 Cultured in a Chemostat

  • The cellular composition and metabolic compounds of Leuconostoc mesenteroides ATCC 8293 were analyzed after cultivation in an anaerobic chemostat. The macromolecular composition was 24.4% polysaccharide, 29.7% protein, 7.9% lipid, 2.9% DNA, and 7.4% RNA. Its amino acid composition included large amounts of lysine, glutamic acid, alanine, and leucine. Elements were in the order of C > O > N > H > S. The metabolites in chemostat culture were lactic acid (73.34 mM), acetic acid (7.69 mM), and mannitol (9.93 mM). These data provide a first view of the cellular composition of L. mesenteroides for use in metabolic flux analysis.

퇴행성 뇌질환 치료제의 연구개발 및 특허동향 분석 (A Study on the R&D Trend and Patent Analysis of Treatments for Degenerative Brain Diseases)

  • 현대사회가 급속히 고령화됨에 따라 알츠하이머질환, 파킨슨병 등 퇴행성 뇌질환의 발생속도도 급증하고 있다. 퇴행성 뇌질환 대부분은 아직도 근원적인 치료가 불가능한 채 발병원인도 명확하지 않은 상태이나 최근 세포분자학적 기술 수준의 향상으로 퇴행성 뇌질환의 원인 규명 및 치료제 개발에 활기를 띠고 있다. 본 연구에서는 퇴행성 뇌질환 기작별 최근 치료제의 연구개발 동향과 국내외 기술동향을 조사 분석 하였으며, 과학계량학적 특허정보 분석을 통해 국가별 기술수준을 파악하고 기술개발 전망을 제시하였다.

Experimental Study on Cerebral Hemodynamics during Observation of Plants

  • Psychological and physiological effects of plants were studied by investigating human responses while observing plants. Eighteen healthy adult male(aged between $19{\sim}25$ years) participated in this study. Semantic differential method(SD method) and multi-channel near-infrared spectroscopy(NIRS) were used to survey verbal and non-verbal response, respectively. Cerebral hemodynamics as a new evaluation index of brain activity was recorded for right brain hemisphere where visual information is mainly delivered. Thirty seconds of cerebral blood flow in forty seven channels were calculated when watching five types of picture images with different rates of hedge against gray block wall; 0:10, 3:7, 5:5, 7:3, 10:0. In the SD results, similar evaluations were found in all subjects. However, the change of cerebral hemodynamics as a non-verbal response varied among subjects. Largely two patterns of hemodynamics change were found with increasing plants rate in picture images; group A showed significant decreases of blood flow volume in many cortical regions, Group B had significant increase of blood flow volume in the occipital region for the scenes seen comparatively more plant. Our findings on the cerebral hemodynamics may indicate that there are two patterns of brain activity during observation of plants; group A in which brain areas associated with visual information and thinking work simultaneously to the visual stimuli; group B in which brain areas associated only with visual information work.

Magnetic Resonance-Guided Focused Ultrasound : Current Status and Future Perspectives in Thermal Ablation and Blood-Brain Barrier Opening

  • Magnetic resonance-guided focused ultrasound (MRgFUS) is an emerging new technology with considerable potential to treat various neurological diseases. With refinement of ultrasound transducer technology and integration with magnetic resonance imaging guidance, transcranial sonication of precise cerebral targets has become a therapeutic option. Intensity is a key determinant of ultrasound effects. High-intensity focused ultrasound can produce targeted lesions via thermal ablation of tissue. MRgFUS-mediated stereotactic ablation is non-invasive, incision-free, and confers immediate therapeutic effects. Since the US Food and Drug Administration approval of MRgFUS in 2016 for unilateral thalamotomy in medication-refractory essential tremor, studies on novel indications such as Parkinson's disease, psychiatric disease, and brain tumors are underway. MRgFUS is also used in the context of blood-brain barrier (BBB) opening at low intensities, in combination with intravenously-administered microbubbles. Preclinical studies show that MRgFUS-mediated BBB opening safely enhances the delivery of targeted chemotherapeutic agents to the brain and improves tumor control as well as survival. In addition, BBB opening has been shown to activate the innate immune system in animal models of Alzheimer's disease. Amyloid plaque clearance and promotion of neurogenesis in these studies suggest that MRgFUS-mediated BBB opening may be a new paradigm for neurodegenerative disease treatment in the future. Here, we review the current status of preclinical and clinical trials of MRgFUS-mediated thermal ablation and BBB opening, described their mechanisms of action, and discuss future prospects.

침습적 뇌자극기술과 법적 규제 - 뇌심부자극술(Deep Brain Stimulation)을 중심으로 - (Invasive Brain Stimulation and Legal Regulation: with a special focus on Deep Brain Stimulation)

  • 뇌에 전기적·자기적 자극을 가하는 뇌자극기술은 신경학적·정신학적 장애에 대해 다양한 범위에 걸쳐 상당한 치료 가능성을 보여준다. 뇌자극기술은 침습 여부에 따라 침습적 기술과 비침습적 기술로 구분되는데, 뇌심부자극술(이하, DBS)은 대표적인 침습적 뇌자극기술에 속한다. 현재 DBS는 식약처 고시인 "의료기기 품목 및 품목별 등급에 관한 규정"에 따라 4등급 의료기기로 분류되어 몇몇 질환에서 안정된 치료법으로 사용되고 있다. 동시에 날로 그 기술이 발전하여 다양한 방향에서 이용방법이 논의되고 있다. 반면, 이와 관련한 법적 규제에 대한 논의는 상대적으로 적은 편이다. 이러한 배경에서 본 글은 DBS의 기술 및 효과와 안전성을 간략하게 소개한 이후, DBS 이용에서 고려할 수 있는 주요한 법적 쟁점을 이용 목적별로, 즉 치료목적, 임상연구 목적, 표준적 치료법이 아니나 다른 치료법이 없는 경우, 향상 목적으로 구분하여 논의하고, 어떠한 목적의 이용이든 DBS 이용에 따른 법적 책임의 문제에서 새로이 공통적으로 부상하고 있는 쟁점-위험·이익평가, 의사의 설명의무, 환자의 동의능력, 기기의 조정, 보험의 보장-을 소개하고 논의한다.

China's Brain Gain at the High End: An Assessment of Thousand Youth Talents Program

  • While studies have viewed the effect of Chinese talent-attracting programs launched by government since reform and open door policy, little of them has assessed these programs empirically and pertinently. This article intends to assess an important program - the Thousand Youth Talents Program (TYTP). Frist, this paper proposed a transnational migration matrix of the academics to clarify the dynamic mechanism of academic brain gain at the high end. Then, the Kaplan-Meier analysis and Cox regression model are used to empirically analyze the policy effect of TYTP. The results show that, academic ability have double edged impacts on brain gain at the high end, some scholars whose last employer's academic ranking is world's Top100 have stronger willing to return, and the negative effect of academic ranking decreases with time passing; while scholars with a tenure-track position, a tenure position or a permanent position tend to stay overseas, and the hazard rate of staying increases with age. The older scholars have more intentions to go back China, while gender was not a significant factor influencing academic return at the high end. That is, the talent-attracting programs has partly succeeded in bringing back the academics at the high end.

Brain Stimulation of Elderly with Dementia Using Virtual Reality Home

  • The virtual reality (VR) is an immerging technology used in the serious games industry to treat psychological disorders like dementia. We created a system named as Virtual Reality Home (VRH) for the elderly who lived with Alzheimer's disease (or other form of dementia) and cognitive impairment using virtual reality technology. The purpose of our study is to measure the long-time immersion and retention of VRH on the moods and apathy, enhancement in physical and brain stimulation as well as a decision making with peoples of dementia and explore the experience of aged care home staff's member. The VRH shows a positive impact on the elderly participants and staff members. During the VRH experience, excitement and a great level of alertness were observed among the participants but few of them were feeling anxiety. Furthermore, we observed the improvement in physical, memory and brain stimulation, but the participants have a low focus on decision making because they wanted to explore all interactable objects in the VRH. This study suggests that the VR may have the potential to improve the quality of life, and these results can assist to expand the future development in the enhancement of efficiency of people with dementia.
