Plain radiographic analysis is the initial imaging modality for suspected small bowel obstruction. Among the many features that affect the diagnosis of small bowel obstruction (SBO), the presence of gas-filled or fluid-filled small bowel loops is the most salient feature that can be automatized by computer vision algorithms. In this study, we compare three frequently applied pixel-clustering algorithms for extracting gas-filled areas without human intervention. In a comparison involving 40 suspected SBO cases, the Possibilistic C-Means and Fuzzy C-Means algorithms exhibited initialization-sensitivity problems and difficulties coping with low intensity contrast, achieving low 72.5% and 85% success rates in extraction. The Adaptive Resonance Theory 2 algorithm is the most suitable algorithm for gas-filled region detection, achieving a 100% success rate on 40 tested images, largely owing to its dynamic control of the number of clusters.
Purpose: The purpose of this study was to compare differences in the time when bowel sounds were heard and gas was passed in women who had an abdominal hysterectomy and were treated for 5 minutes (experimental group A) or 10 minutes (experimental group B) with San-Yin-Jiao (SP-6) acupressure. Method: The design of this study was a nonequivalent control group non-synchronized post test only design. The participants included 142 women, 39 in experimental group A, 30 in experimental group B, and 73 in the control group. Data was collected using a structured questionnaire which included items on general characteristics and a self report of time when gas was passed. Differences for the three groups as to time when bowel sounds were heard and gas was passed were analyzed using ANOVA. Result: The time when bowel sounds were heard was statistically significantly shorter in both experimental groups compared to the control group(F=10.29, p=.000). The time when gas was passed was statistically significantly shorter in experimental group B(10 min) compared to the control group(F=4.68, p=.011). Conclusion: It could be concluded that SP-6 acupressure of 10 minutes was effective in shortening the time until bowel sounds were heard and gas was passed for women who had had an abdominal hysterectomy. Replication of the study with a larger number of participants is necessary in order to be able to generalize the results.
Purpose: This study was aimed to investigate the effects of gum-chewing on the recovery of bowel movement and oral cavity discomfort after abdominal surgery. Method: The nonequivalent control group, non-syncronized repeated treatment design was used. A total of 99 patients were participated in the study. The 44 patients were in the experimental group and the rest in the control group. The experimental group chewed gum three times a day until they passed gas. As the patient reported gas-passing, bowel movement time, subjective symptoms of oral cavity, and oral status were recorded precisely. The frequency, percentage, $x^2$-test and t-test were analyzed by SPSS PC 12.0. Results: There were significant differences in bowel movement, gas passing, oral cavity symptoms, and oral status scores between the experimental and the control group. Conclusion: The findings of the study demonstrated the fact that gum-chewing helps early recovery from post-operative ileus and thirst. It is because gum chewing stimulates bowel mobility and secretion of saliva. Thus gum-chewing seems to be an effective nursing intervention in reducing post-operative side effects for patients with abdominal surgery.
Objectives : The purpose of this study was to compare differences in bowel movement in women who had an abdominal hysterectomy and were treated for 10 minutes with San Yin Jiao (SP6) acupressure at three different time periods. Methods : The design was a nonequivalent control group non-synchronized post test only design. The participants included 123 women, 30 in experimental group 1 (post anesthesia 30 min), 22 in the experimental group 2 (post anesthesia 4 hours), 23 in experimental group 3 (Post anesthesia 24 hours), and 48 in a control group. Data were collected using a structured questionnaire consisting of general characteristics, self reported time when gas was first passed. Differences in the time when first gas was passed among four groups were analyzed using ANOVA. Results : There was no a statistical difference among the 4 groups for the time when gas was first passed. Conclusion : It is necessary to replicate the study with sonographic data for bowel movement.
Hepatic portal venous gas (HPVG) is a rare radiographic finding associated with severe intra-abdominal disease and fatal outcome. Most cases of HPVG are historically related to mesenteric ischemia accompanied by bowel necrosis. The current spread of computed tomography scan promotes not only the early detection of related severe diseases but also the identification of other causes of HPVG. It has been reported in many non-fatal conditions, such as inflammatory bowel disease, intra-abdominal abscess, bowel obstruction, paralytic ileus, endoscopic retrograde cholangiopancreatography and endoscopic sphincterotomy, and gastric dilatation. Among these, paralytic ileus is a very rare condition, with no case yet reported in South Korea. Reported herein is a case of HPVG in paralytic ileus, which was treated well internally and was promptly resolved.
Pneumatosis cystoides intestinalis and portomesenteric venous gas are uncommon radiological findings, but are found commonly in cases of bowel ischemia, or as a result of various non-ischemic conditions. A 72-year-old man visited an emergency center with altered mental status 2 hours after ingestion of an unknown pesticide. On physical examination, he showed the characteristic hydrocarbon or garlic-like odor, miotic pupils with no response to light, rhinorrhea, shallow respiration, bronchorrhea, and sweating over his face, chest and abdomen. Laboratory results revealed decreased serum cholinesterase, as well as elevated amylase and lipase level. We made the clinical diagnosis of organophosphate poisoning in this patient based on the clinical features, duration of symptoms and signs, and level of serum cholinesterase. Activated charcoal, fluid, and antidotes were administered after gastric lavage. A computerized tomography scan of the abdomen with intravenous contrast showed acute pancreatitis, poor enhancement of the small bowel, pneumatosis cystoides intestinalis, portomesenteric venous gas and ascites. Emergent laparotomy could not be performed because of his poor physical condition and refusal of treatment by his family. The possible mechanisms were believed to be direct intestinal mucosal damage by pancreatic enzymes and secondary mucosal disruption due to bowel ischemia caused by shock and the use of inotropics. Physicians should be warned about the possibility of pneumatosis cystoides intestinalis and portomesenteric venous gas as a complication of pancreatitis following anticholinesterase poisoning.
This study was conducted to identify the effectiveness of meridian acupressure for enhancement of post operative bowel movement to the patient with absolute bed rest who having surgery under general anesthesia. This study used a qusai experimental, nonequivalent control group post test only design. This subject were 44 patients, 22 for the experimental and 22 for the control group, who were admitted at KyungHee University hospital, neurosurgical unit A and B ward, assigned by matched sample by the name of operation who having microvascular decompression and laminectomy. Date were collected from May 1, 2001 to June 30, 2001 by auscultation, self report and by using 7 point face scale. The recovery of bowel sound were measured every 4 hours until gas out for 1 minute auscultation on lower abdomen after 4 hours having surgery. The time of gas out were measured by self report, the severity of nausea were measured every 4 hours for 7 point face scale and also the degree of satisfaction of nursing care were measured after 2 days having surgery with same scale. Data were analyzed with $X^2$, t-test, repeated measures ANOVA and ANOVA. The result of this study were as follows; 1. The experimental group which were implemented with meridian acupressure showed shorter time the recovery of bowel sound after having surgery than control group(t=-5.112, p=.0001). 2. The experimental group which were implemented with meridian acupressure showed shorter time of gas after having surgery than control group(t=-4.010, p=.0001) 3. The experimental group which were implemented with meridian acupressure showed decreased level of nausea score according to time interval than control group(F=21.995, p=.0001). 4. The experimental group which were implemented with meridian acupressure showed higher the degree of satisfaction of nursing care than control group(t=-4.010, p=.0001). These finding indicate that a meridian acupressure could be a effective nursing intervention for enhancement of post operative bowel movement to the patient with absolute bed rest who having surgery under general anesthesia.
This study was conducted to identify the effect of meridian acupressure on defecation of patients with post spinal operation. The nonequivalent control group posttest only design was used. The data were obtained from 77 post spinal operative patients, 34 in the experimental 43 in control group in Y Hospital, Seoul, Korea. The neurosurgical unit A and B ward, assigned by matched sample by the name of operation such as laminectomy and posterior lumbar interbody fusion are performed. Meridian acupressure meant the method that an examiner presses response points distributed in the pass of energy vessel. In this study, meridian acupressure program was performed on as points in order of Hegue (LI-4), Zhigou (TE-6), Zusanli (S-36), Shangjuxu (S-37), Xiajuxu (S-39), Tianshu (S-25), Taichong (L-3) which was known to be related to large intestine. Data were collected from 1, July 2003 to 1, September 2003. Meridian acupressure program was carried out for 20 minutes 4 hours after operation twice daily. In order to evaluate the effect of meridian acupressure intervention, they were asked time of bowel recovery, gas passing, and defecation though questionnaire method. Data were analyzed by the SPSS/ WIN 11.0 program. The results of this study were summarized as follows; 1. Homogeneity tests of general characteristics and operation related characteristics of the experimental group and the control group were performed. General characteristics included age, sex, defecation habit, eating pattern, fluid intake, life style, activity, usage of laxative and etc. 2. Hypotheses were verification as follows; 1) Recovery of bowel sound of the experimental group who received meridian acupressure intervention was faster than that of the control group after spinal operation (t=-6.770,P=.000). 2) Time of gas passing of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-8.003, P=.000). 3) Time to defecation of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-9.026, P=.000). 4) Abdominal discomfort due to defecation of the experimental group who received meridian acupressure program was lesser than that of the control group after spinal operation (t=-3.431, P=.001). From these results, meridian acupressure intervention was effective for recovery of bowel sound, reduce time to gas passing, time to defecation and lessen abdominal discomfort due to defecation on post spinal operative patients. And therefore this intervention can probably considered on clinical practice.
Objectives The purpose of this study is to report the case of fecal impaction in a child without any abdominal symptoms treated by taking herbal medicine. Methods We examined a 7-year-old girl who had normal bowel movement and defecation per day, and had no particular abdominal symptoms. Abdominal radiography was taken, and unexpected severe fecal impaction was observed throughout the abdominal cavity, filled with intestinal gas and feces. According to the subject's parents, she had no generalized symptoms, such as abdominal pain or distension, and had on a regular diet and normal bowel movement daily. She was treated with herbal medicine (Daeseunggi-tang) for 23 days. While she was on the therapy, numbers, doses, bowel movements, and radiography were checked and recorded. Results During the treatment, her stool was softened, and fecal impaction was relieved as showed by abdominal radiography. Conclusions We have identified that there are cases where subjects have no symptoms of abdominal pain, despite presence of severe fecal impaction. In addition, it was found that Daeseunggi-tang is effective in fecal impaction in childhood.
4개월령의 수컷 잡종견이 교통사고에 의한 외상, 기립불능, 혼수, 복부통증으로 내원하였다. 방사선 검사에서 복수, 장 확장, 우측 장골 골절, 소심증이 확인되었다. 복부 초음파 검사에서 내강에 무 에코의 액체와 장벽 점막 내 가스를 함유하고 있는 심하게 확장된 소장 분절이 관찰되었다. 이러한 초음파적 특징은 장 기종 (pneumatosis intestinalis)에서 관찰되며, 이것은 외상에 따른 장 분절의 혈관 혈류 이상에 의한 것으로 의심된다. 환축이 폐사 한 후 부검한 결과, 일부 장 분절이 요추하 근육에 의해 교액되어 변색 및 괴사한 상태임을 확인하였다. 초음파는 장벽 내 에코가 있는 가스를 확인하여 장 기종을 진단할 수 있는 유용한 수단이다.
