고등급 표면(High Grade Surface) 골육종 - 증례 보고 - (High Grade Surface Osteosarcoma - Case Report -)

  • 고등급 표면(high grade surface) 골육종은 골의 표면에서 발생하는 골육종의 희귀한 아형으로, 발생빈도는 전체 골육종의 1%에 못 미친다고 한다. 몇 편의 증례 보고와 연구 논문에서 이 고등급 표면 골육종을 소개하였는데, 국내의 경우 127예의 골육종 중 1예가 고등급 표면 골육종이었음을 기술한 논문이 있으나, 환자 및 진단과 치료에 대한 정보가 포함되어 있지 않았다. 저자들은 66세 여자 환자의 대퇴골 전자하부에서 발생한 고등급 표면 골육종 1예를 진단하고, 수술 전 항암 화학 요법, 광범위 절제술과 사지 구제술 및 수술 후 항암 화학 요법을 시행하였다. 이 종양은 위치만 골 표면에서 발생할 뿐 조직학적으로 전형적인 골수강내 골육종과 같으며, 다른 표면 골육종과는 달리 치료나 예후에 있어 전형적인 골수강 내 골육종과 같기 때문에, 진단과 치료에 있어 방골성 골육종이나 골막성 골육종 등의 표면 골육종과 구별하여야 할 것으로 생각한다.

역분화 방골성 골육종 - 증례 보고 1 례 - (Dedifferentiated Parosteal Osteosarcoma - A case report -)

  • Parosteal osteosarcoma is characterized as a densely ossifying lesion, usually occurring on the surface near the metaphyses of a long bone. The histological pattern is a well- differentiated mature bone trabeculae with a hypocellular spindle-cell stroma. The cytological details are those of a low-grade malignant lesion. The natural history of this lesion is indolent local growth, late invasion of the underlying bone, and infrequently, distant metastasis. However, there is a significant risk of eventual dedifferentiation into a high-grade lesion. We report here-a case of parosteal osteosarcoma dedifferentiated into a high-grade lesion, which occurred in the left distal femur of a 40-years-old woman, and discuss the experience in detail.

Analysis of the Expression and Regulation of PD-1 Protein on the Surface of Myeloid-Derived Suppressor Cells (MDSCs)

  • Myeloid-derived suppressor cells (MDSCs) that are able to suppress T cell function are a heterogeneous cell population frequently observed in cancer, infection, and autoimmune disease. Immune checkpoint molecules, such as programmed death 1 (PD-1) expressed on T cells and its ligand (PD-L1) expressed on tumor cells or antigen-presenting cells, have received extensive attention in the past decade due to the dramatic effects of their inhibitors in patients with various types of cancer. In the present study, we investigated the expression of PD-1 on MDSCs in bone marrow, spleen, and tumor tissue derived from breast tumor-bearing mice. Our studies demonstrate that PD-1 expression is markedly increased in tumor-infiltrating MDSCs compared to expression in bone marrow and spleens and that it can be induced by LPS that is able to mediate $NF-{\kappa}B$ signaling. Moreover, expression of PD-L1 and CD80 on $PD-1^+$ MDSCs was higher than on $PD-1^-$ MDSCs and proliferation of MDSCs in a tumor microenvironment was more strongly induced in $PD-1^+$ MDSCs than in $PD-1^-$ MDSCs. Although we could not characterize the inducer of PD-1 expression derived from cancer cells, our findings indicate that the study on the mechanism of PD-1 induction in MDSCs is important and necessary for the control of MDSC activity; our results suggest that $PD-1^+$ MDSCs in a tumor microenvironment may induce tumor development and relapse through the modulation of their proliferation and suppressive molecules.

내과의 부분후방절골술을 통한 거골연골모세포종의 수술적 치료: 증례 보고 (Surgical Treatment of Talar Chondroblastoma via Partial Posterior Medial Malleolar Osteotomy: A Case Report)

  • During bone tumor resection, many cases require medial malleolar osteotomy to achieve adequate access to the operative field. Various osteotomy methods have been developed to address this issue, including oblique, transverse, reverse V-shape, and step-cut osteotomies. However, medial malleolar osteotomy has several drawbacks, such as the excessive disruption of the joint surface, unstable screw fixation when fixing the medial malleolus, and iatrogenic medial ankle joint arthritis due to articular displacement during the reduction of the osteotomy site. In addition, there is a possibility of injury to the posterior tibial artery, tibial nerve, or posterior tibialis tendon if the osteotomy range is too aggressive. Therefore, the authors propose a new osteotomy method, which has shown promising clinical results, namely, partial posterior medial malleolar osteotomy. This method minimizes articular involvement and provides adequate access to the operative field during talar body bone tumor resection.

역전사 중합효소 연쇄반응을 이용한 표면 적심성에 따른 골수유래 줄기세포의 생물학적 평가 (Biological Evaluation of Bone Marrow-Derived Stem Cells onto Different Wettability by RT-PCR)

  • 고분자 생체재료에서 세포부착과 성장은 재료의 적심성, 화학구조, 표면전하 및 거칠기 등의 표면 성질에 의존한다. 본 연구에서는 저밀도 폴리에틸렌 필름 (LDPE)의 표면 적심성과 골수유래 줄기세포의 증식 및 성장성을 측정하기 위하여 플라즈마 처리를 실시하였으며 개질된 필름 표면의 특성을 조사하였다. 또한 LDPE 필름에서의 세포부착과 증식률은 세포수 관찰과 역전사 중합효소 연쇄반응으로 확인하였다. 표면성질의 하나인 물 접촉각 측정 결과 플라즈마 처리 시간이 길어짐에 따라 필름표면의 접촉각이 감소하였으며 암형성 유전자와 암억제 유전자의 발현률이 60∼70$^{\circ}$ 사이에서 높음을 확인할 수 있었다. 또한 세포수 관찰을 통해 접촉각이 60∼70$^{\circ}$인 표면에서 세포 증식률이 우수하여 표면성질이 세포의 성장과 분화에 중요함을 확인하였다.

근위 경골의 골육종에 시행한 반관절성형술 (Hemiarthroplasty for Osteosarcoma of Proximal Tibia)

  • 목적: 근위 경골 육종의 절제 후 재건은, 특히 골성장이 완료되지 않은 환자의 경우 많은 문제점이 있다. 본 연구에서는 근위 경골 육종의 절제 후 재건에 따르는 문제점을 보완하기 위한 새로운 술식을 시도해 보고자 하였다. 대상 및 방법: 근위 경골에 발생한 골육종의 절제 후 반관절성형술을 시행한 4례를 대상으로 하였다. 평균 연령은 13세였으며 술후 평균 추시 기간은 64개월이었다. 전례에서 초고분자량 폴리에틸렌 삽입물(ultra-high molecular weight polyethylene liner)을 이용하여 관절면을 재건한 후 엔더정(Ender-nail)과 골시멘트를 이용하여 남아있는 경골에 고정하였다. 결과: 최종 추시상 MSTS 기준에 의한 기능적 점수는 23.5점(78.3%)이었다. 전례에서 술후 슬관절 동통, 불안정성은 관찰되지 않았다. 결론: 골성장이 완료되지 않은 소아 환자의 근위 경골 육종 절제 후 재건 방법으로서 반관절 성형술은 효과적인 술기로 판단된다.

  • Bone graft has been used to repair one defect caused by disease and trauma, congenital and acquired deformities. Graft materials are autogenous bone, allogenic bone, xenogenic bone, synthetics. Autogenous bone graft is the most superior to other materials for immunologic reaction, compatibility to host tissue, and revascularization. However, autogenous bone graft is required for additional operation and the amount of taking is limited. Autografts are obtained at own expense and also limited in size, shape. In order to compensate these problems, allogenic bone graft has been used increasingly. But allogenic bone graft encounters immunologic complications. Therefore, it has been used after freezing, lyophilization, or demineralization. Allogenic bone processed by only lyophilization includes potential antigenic properties on its surface, therefore it is demineralized to deplete immunologic reaction. Demineralized bone releases BMP and helps the mesenchymal cells transform to the chondroblast to produce cartilage and bone. This reaction is called osteoinducation. Many authors have reported that mineralized lyophilized bone had less antigenicity clinically and favorable bony consideration with host bone. In our department from 1995 to now, we have used banked allogenic bone graft that has been prepared from Wonkwang Bone Bank in 5 cases and mineralized lyophilized bone graft in 2 cases to reconstruct the maxillofacial bone defect after tumor resection and cyst enucleation and cleft alveolus. We will report with literature review that the result is favorable functionally and esthetically.

사지에 발생한 신경초종의 미세수술적 치료 (Microscopic Excision of Neurilemmoma of the Extremities)

  • Neurilemmomas are the most common benign tumor of the peripheral nerve trunks, and arises from the cells in the sheath of Schwann. Neurilemmomas are well encapsulated and may be separated easily from surrounding tissue and lie completely within a larger nerve trunk, with bundles of neurofibrils spread out over the surface of the tumor. A careful dissection and retraction of the nerve bundles will allow the tumor to be enucleated from the parent nerve without any significant interference with the function of the nerve. Resection of the involved nerve is seldom necessary and should be avoided if at all possible. Our aim in microscopic excision of neurilemmoma of extremities is to reduce any disturbance of the intact neurofibrils of the parent nerve. Thirteen cases of neurilimmomas were treated by microscopic excision at the Department of Orthopaedic Surgery, Korea University Hospital between January 1990 and March 1995. The results was as follows ; 1. The average age at surgical intervention was 40.1 years. Cases in fourth and fifth decades predominated. 2. In their anatomical distribution, 8 cases were in the upper extremity and 5 cases in the lower extremity. 11 cases were on the flexor surface. 3. On the operative field, all the tumors were well encapsulated, however 1 case of 13 was adherent to the periosteum of fibula. 4. In all cases, the tumor were enucleated from the parent nerve without any injury to nerve under high-power magnification, preserving individual fascicles, and sensory and motor function.

Cellular Mechanism of Newly Synthesized Indoledione Derivative-induced Immunological Death of Tumor Cell

  • Background: EY-6 is one of the newly synthesized indoledione derivatives to induce tumor cell-specific cell death. In this study, we investigated the mechanism of immunological death induced by EY-6 at mouse colon cancer cell as well as at the normal immune cell represented by dendritic cell. Methods: C57BL/6 mouse syngeneic colon cancer cell MC38 was treated with EY-6, and analyzed by MTT for viability test, flow cytometry for confirming surface expressing molecules and ELISA for detection of cytokine secretion. Normal myeloid-dendritic cell (DC) was ex vivo cultured from bone marrow hematopoietic stem cells of C57BL/6 mice with GM-CSF and IL-4 to analyze the DC uptake of dead tumor cells and to observe the effect of EY-6 on the normal DC. Results: EY-6 killed the MC38 tumor cells in a dose dependent manner (25, 50 and $100{\mu}M$) with carleticulin induction. And EY-6 induced the secretion of IFN-${\gamma}$ but not of TNF-${\alpha}$ from the MC38 tumor cells. EY-6 did not kill the ex-vivo cultured DCs at the dose killing tumor cells and did slightly but not significantly induced the DC maturation. The OVA-specific cross-presentation ability of DC was not induced by chemical treatment (both MHC II and MHC I-restricted antigen presentation). Conclusion: Data indicate that the EY-6 induced tumor cell specific and immunological cell death by modulation of tumor cell phenotype and cytokine secretion favoring induction of specific immunity eliminating tumor cells.
