Ankle Arthrodesis Using Auto Fibula Bone Graft: Two Cases Report (자가 비골이식술을 이용한 족관절 유합술: 2예 보고)

  • Recently, development and improvement in joint replacement therapy, the need for arthrodesis has been decreasing. However, result of joint replacement is not always satisfactory, and most cases are rather indicative to ankle arthrodesis than ankle replacement. Often, ankle arthrodesis can be more beneficial salvage method to treat cases with failure in joint replacement therapy, talar avascular necrosis with massive bone defect, talus fracture with severe comminution and bone defect and ankle dislocation. In cases with large bone defect that need to be treated with ankle arthrodesis using internal fixation, it is difficult to fill the defect with conventional auto-iliac bone or all-bone graft. Thus, we make a report on our experience in treating 2 cases with ankle arthrodesis using auto-fibular bone graft and plate fixation.

  • A composite of $Ca_3$(PO$_4$)$_2$ and MgAl$_2$O$_4$ spinel is biologically active and has enduring strength. Its strength depends on the spinel phase. The flaws in the spinel depend on the grain size of the calcium phosphate phase and are not altered by dissolution. The calcium phosphate, ${\alpha}$ tri-calcium phosphate, controls the tissue response. Bone bonds to the implant. A design for a bone graft as a replacement for a section of the diaphysis of a canine femur provides for tensile, compressive, torsional and bending load; and for the physiological processes of bonding and remodeling. A bone plate, used to stabilize the implant at time of surgery was removed after about one year. Over seven years of service have been achieved without internal or external fixation.

Longterm Benefit and Unmet Needs in Enzyme Replacement Therapy of Gaucher (고셔병 환자에서 효소대체치료의 장기적 효과와 한계)

  • Gaucher disease is a multisystemic disorder arising from a deficient activity of the lysosomal enzyme glucocerebrosidase, which leads to accumulation of glycosylceraide and other glycolipids in the regiculoendothelial system. The characteristics of Gaucher disease are anemia, thrombocytopenia, hepatosplenomegaly, and skeletal disease. Enzyme replacement therapy (ERT) has been proven to prevent progressive manifestations of Gaucher disease and effective in improving anemia, thrombocytopenia, bone markers and biomarkers. However, some patient needs still remain unmet because of the inaccessibility of certain sites including brain, bone and various organs. ERT could not Improve the irreversible lesion such as liver fibrosis, hepatopulmonary syndrome, and necrosis or infarction of bone and other organs. Adult patients with Gaucher disease should be screened for longterm complication such as bone disease, pulmonary hypertension, gallstone, and cancer, especially in patients with splenectomy. Parkinsonism and polyneuropathy was also reported among patients with type 1 Gaucher disease, but ERT does not improve neurological function. We need to review the benefits and unmet needs of ERT in Gaucher disease.

Two-Dimensional Finite Element Analysis of Bone Resorption from the Artificial Hip Replacement (인공고관절 골흡수로 인한 응력분포 변화의 2차원 유한요소 해석)

  • Clinically, proximal bone resorption in the femur is frequently seen postoperatively on the follow up XI-rays after total hip replacement (THR). We developed the finite element model of cementless THR. The model is two dimensional side plate model, whereby the three dimensional structural integrity of the bone can be accounted for by a separate two dimensional mesh, a side plate. The subject of this article is the development and application of this two dimensional side plate FEM to study the reverse effect of the various degree of bone resorption of femur after THR. The results of this study indicates that 1) two dimensional side plate model is good and simple alternative to complex three dimensional model and 2) the severity of the proximal bone resorption has the effect of more increasing stress on the cortex at the level of femoral stem tip.

  • In clinical orthopaedics, bone resoption in the cortex is often seen post operatively on X-rays or bone densitometry after total hip replacement (THR) in the form of cortical osteoporosis or atropy. Stress shielding of bone occurs, when a load, normally carried by the bone alone, is shared with an implant as a result, the bone stresses are abnormal and with remodelling analysis this may cause extensive proximal bone resoption, possibly weakening the bone bed to the point of failure. The author made finite element models of the cemented and non-cemented type implanted femoral stem with bone resorption of the proximal medial femur and studied the feed back effect of the various degree of bone resoption to THR system by parametric analysis on the stress of the femoral stem and interface. The results of the present finite element analysis implied that the extent of proximal bone resorption has the effect of more increasing stress on the distal stem tip, cement mantle and interface in both type of femoral stem and this high distal stress possibly can cause the mechanical failure of loosening or failure after THR.

  • Periprosthetic fracture after total elbow replacement surgery is a difficult complication to manage, especially when it comes together with implant loosening. If stem revision and internal fixation of the periprosthetic fracture are performed simultaneously, this would be a very challenging procedure. Most of total elbow replacement implants are cemented type. Cement usage at periprosthetic fracture site may interfere healing of fractured site. Authors underwent internal fixation with use of locking plate and cerclage wire for periprosthetic fracture, allogenous fibular strut bone inserted into the humerus intramedullary canal allowing the fractured site to be more stable without cement usage. At 10-month follow-up, the complete union and good clinical outcome was achieved. We present a novel technique for treating periprosthetic fracture with implant loosening after total elbow replacement surgery, using intramedullary allogenous fibula strut bone graft.

  • We treated three cases of bone tumors-giant cell tumor, chondroblastoma and malignant fibrous histiocy toma-with a custom-made prosthetic replacement. The patients were followed from 10 months to 18 months, postoperatively. The results of these study are as follows : 1. satisfactory anatomic restoration 2. early ambulation 3. good function 4. biomechanically sound reconstruction.

  • The purpose of this study is to evaluate the effect of $HTR^{TM}$ (hard tissue replacement, Bioplant Inc, U.S.A) polymer on short-term healing as a grafting material for alveolar ridge augmentation. A 48-year-old female presented insufficient bone height and width for the placement of implants. $HTR^{TM}$ polymer was used for ridge augmentation. Bone biopsy was harvested 8 months after the ridge augmentation procedure. $HTR^{TM}$ polymer displayed rapid bone regeneration and mature lamellar and trabecular bone redevelopment. Clinical and histologic observation from the treatment of the patient presented suggest that $HTR^{TM}$ polymer seems to be a appropriate material for alveolar ridge augmentation.

  • The nanoparticle-hydrogel complex as a new bone defect replacement matrix, which is composed of the nanoparticles for the sustained release of BMP and the hydrogel for filling the bone defect site and playing a role as a matrix where new bone can grow, is presented. In vivo evaluation of bone formation was characterized by soft X-ray, MT staining, and calcium assay, based on the rat calvarial critical size defect model. The effective bone regeneration was achieved by the BMP-2 loaded nanoparticles in fibrin gel, compare to bare fibrin gel, the nanoparticle-fibrin gel complex without BMP-2, or the BMP-2 in fibrin gel, in terms of the new bone area and the gray level in X-ray, the bone marrow are, and the calcium content in the initial defect site. These findings suggest that the BMP-2 loaded nanoparticle-fibrin gel complex can a promising candidate for a new bone defect replacement matrix.
