노년기 농촌여성의 비타민 D 수용체 유전자형에 따른 칼슘 섭취량과 골밀도와의 관계 (Association of Calcium Intake and Bone Mineral Density by Vitamin D Receptor Genotype among Elderly Women Living in Rural a Area)

  • Recent studies have shown that environmental, biochemical, nutritional or genetic factors affect bone mineral density (BMD). The purpose of the present study was to investigate the effect of vitamin D receptor (VDR) gentotype and nutritional status on BMD of elderly women living in a rural area. Three hundred thirty five elderly women over 65 years in Asan participated the study. Data for demographic and nutrient intakes were obtained by survey with a two day 24 hr recall method. BMD was measured by broadband ultrasound attenuation (BUA) using quantitative ultrasound (QUS). VDR genotypes of the subjects analyzed with Bsm I restriction enzyme were bb (92%), Bb (7%) and BB (1%). No differences were found between genotypes bb and Bb/BB in age, menopausal age, body mass index and body fat. BUA of bb genotype was higher (62.5 $\pm$ 15.6 dB/MHz) than Bb/BB genotype (56.1 $\pm$ 17.6 dB/MHz) by Student's t-tests. Correlation analyses showed strong negative correlation of BMD and age, but positive correlation with BMI, energy and calcium intake. When subgroup analyses were conducted after stratification by the median calcium intake level (412.9 mg/d), the above median calcium intake group showed significant difference in BUA by VDR genotype while the lower median calcium intake group did not show significant difference. The current study confirmed interaction of calcium intake and VDR genotype in association with BMD. Further nutritional intervention will be needed to improve calcium status of the elderly women living in rural areas.

  • 이중에너지 X-ray 골밀도 측정기를 이용하여 폐경 전 여성들을 대상으로 골밀도에 영향을 규명하기 위하여 폐경기 이후에 증가하는 골다공증의 예방에 기여하고자 수행하였다. 연구 대상자의 골감소증은 20.2%였으며, 골밀도 수치를 예측할 수 있는 가장 중요한 인자는 연령 이었다. 연령이 높을수록 유의하게 낮은 골밀도를 보였다. 운동에 있어서는 운동을 안하는 것보다 운동을 하는 것이 골밀도에 더 좋은 영향을 미치며, 적정한 운동을 하는 것이 골밀도에 좋은 영향을 미치는 것으로 나타났다. 식생활에 따른 골밀도는 채식 위주의 식사보다는 육식위주의 식사를 선호하는 경우 골밀도에 유의한 영향을 미치는 것으로 나타났고, 생리주기가 짧을수록 골밀도가 유의하게 높았다. 골밀도와 관련요인에 대한 다중회귀분석에서는 연령이 증가할 유의하게 골밀도가 낮게 나타나 연령이 골다공증의 위험요인으로 나타났다.

폐경후 우리나라 여성의 영양 섭취 상태가 골다공증 발병에 미치는 영향 (Effects of Nutrients Intakes on Development of Osteoporosis in Korean Postmenopausal Women)

  • The relationship of nutrients intake and bone mineral density (BMD) was investigated in 285 Korean postmenopausal women (age 40-70 y) consisted of 65 normal women and 159 osteopenia patients who don't have other diagnosed disease. BMD was measured at the spine (vertebrae L2-4) and femur (neck, Ward's triangle and trochanter). Height, weight, body mass index(BMI), lean body mass(LBM), body fat(Bfat) and dietary intakes of animal calcium(animal Ca), protein and phosphorus per 1,000kcaI intake were correlated with BMD of the spine and the femur positively(p<0.05). Women with an animal calcium intake < 315mg/d (mean % animal Ca/total Ca intake > 50%) exhibited significantly lower BMD of the spine and Ward's triangle than that of women with an intake > 315mg/d, p<0.OS. When subjects were grouped by diagnosis into 3 groups (normal, osteopenia and osteoporosis group), animal Ca intake of osteoporosis group (mean animal Ca intake 261mg) was significantly lower than those of the other two groups (mean animal Ca intake 306mg and 297mg, respectively), p < 0.05. These results suggest that balanced nutrients intake and increased animal Ca in the diet is likely to be beneficial in reducing bone loss in postmenopausal women.

우유를 통한 칼슘의 공급이 일부 아산시 노인 여성의 골밀도에 미치는 영향 (Increased Calcium Intake through Milk Consumption and Bone Mineral Density of Elderly Women Living in Asan)

  • The objective of this study is to determine the effectiveness of 4-month milk consumption in the prevention of osteoporosis in elderly women living in Asan. Subjects included 277 women age over 65 years were divided into control (n=111) and milk (n=166) groups. For those in the milk group, one cup (200ml) of partially lactose-digested low-fat milk was provided everyday for 4 months. Each subject was interviewed to assess food intake by 24-h recall method before and after milk supplementation. Prevalence of osteoporosis was determined by WHO criteria with calcaneus bone mineral density (BMD) measured by quantitative ultrasound (QUS) on left heel. After 4 months, the nutrient intake levels of control did not change while intakes of energy, protein, calcium, phosphorous, riboflavin, pyridoxin, niacin and folic acid were significantly increased in milk group. No significant changes were observed in anthropometric, and BMD in both control and milk groups. T-score of milk group, however, was significantly increased after 4 month milk consumption. Prevalence of osteoporosis was increased (27% to 32%) in control group while that of milk group was decreased (32% to 30%). When BMD and t-score changes after 4 months of milk consumption were compared between those with low baseline calcium intake and high calcium intake subjects in the milk group, BMD and t-score were significantly improved in the low baseline calcium intake group. We conclude that one cup a day milk consumption for a relatively short period of 4 months can prevent further bone loss and significantly improve intakes of both macro and micro-nutrients of elderly women.

Effects of R. Glutinosa and E. Senticosus on Postmenopausal Osteoporosis

  • In this study, we investigated the therapeutic effects of a novel formulation of low-dose calcium and vitamin $D_3$ blended with Rehmannia glutinosa Libosch and Eleutherococcus senticosus Max (RE+), in postmenopausal women. The controls were given either a placebo or high dose calcium and vitamin $D_3$ (Ca + D). Bone mineral density (BMD) in the L2-3 lumber spines and femur regions was assessed, and serum osteocalcin, bone-specific alkaline phosphatase (BALP), and cross-linked N-telopeptide of type I collagen (NTx) were used as markers of osteoblast and osteoclast activity. Furthermore, all variables were measured before and after 6 and 12 months of treatment. The osteocalcin level was higher in the RE+ group, and BALP was almost the same in all groups. Serum NTx was significantly decreased in the RE+ group after 12 months (p<0.05). The NTx in the Ca + D and placebo groups showed no significant change. The decrease of femur BMD was further demonstrated in the placebo group, but significantly increased in the RE+ group after 6 and 12 months of treatment (p<0.05). There were significant differences in the percent changes of femur BMD between the placebo and RE+ groups (p<0.01) and Ca+D and RE+ groups (p<0.05). The decrease of spine BMD in the placebo group was inhibited both in the Ca + D and RE+ groups, however, there was significant difference only between the placebo and RE+ groups (p<0.05). These findings suggest that continuous oral therapy of the RE+ formulation reduces rapidly decreasing bone mineral density in postmenopausal women more effectively than high doses of calcium and vitamin $D_3$ alone by inhibiting osteoclastic activity. Therefore, it seems that the RE+ has its own antiosteoporotic effects. We suggest larger clinical studies to determine the most efficacious dosage and benefits of this novel treatment.

초음파방식으로 측정된 골밀도와 신체조성 및 배근력의 관계 (Relationship of Bone Mineral Density Measured by Ultrasound Bone Densitometry and Body Composition or Backmuscle Strength)

  • 본 연구에서는 20대 초반의 남, 여를 대상으로 초음파 방식으로 측정된 골밀도와 신체조성 및 배근력의 관계에 대해 알아보았다. D대학에 재학 중인 61명의 학생에 대해 IRB 승인과 참여자 동의 후 설문조사, 초음파방식 골밀도 및 신체조성, 배근력을 측정하였다. 골밀도는 주로 사용하는 측 종골을 측정하여 T-값을 두 그룹($T-score{\geq}-1.0$; T-score<-1.0)으로 나누었다. 생체전기 저항법을 이용하여 신체조성을 측정하였고, 배근력은 2회 측정하여 평균값을 구하였다. 모든 통계분석은 SPSS ver. 22.0 을 사용하여 실시하였다. 신체활동 또는 체질량지수가 증가할수록 골밀도 및 신체조성의 근육량도 증가하였다. T-값이 높은 그룹이 낮은 그룹 보다 신체조성의 근육량 및 골격근량, 신체발달이 높았지만 통계학적인 유의성은 없었다(p>0.05). 배근력에서도 T-값이 높은 그룹이 낮은 그룹 보다 높았다(101.1 vs. 92.1, p=0.308). 남, 여그룹에서 골밀도 T-값과 신체조성의 골격근과 근육량은 보통 수준의 유의한 양의 상관성을 보였고(p<0.05), 배근력은 신체조성의 골격근과 근육량, 신체발달에서 골밀도 보다 높은 유의한 양의 상관성을 보였다(p<0.001). 여자그룹이 남자그룹 보다 골밀도 T-값과 신체조성의 골격근과 신체발달에서 높은 상관성을 보였다. 남자 그룹에서 배근력과 골밀도의 T-값과는 유의한 상관성을 보이지 않았지만(r=0.176, p=0.319), 여자 그룹에서는 보통 수준의 유의한 상관성을 보였다(r=0.591, p=0.001). 이상과 같이 초음파 방식으로 측정된 골밀도와 신체조성, 배근력은 상호 유의한 관련성을 보였다.

장년기 여성의 영양섭취상태, 건강상태와 골밀도와의 관련성에 관한 연구 (Research on Relation of Nutrients Intake, Health Status, and Bone Mineral Density in Middle-aged Women)

  • This study was to investigate the relation of nutrients intake, health status, and bone mineral density in middle-aged women. Daily nutrients intake were analyzed by convenient method. The BMD of subjects were measured by Dual Energy X-ray Absorptiometry(DEXA). BMD was measured at the spine(vertebrae L2-L4) and femur(neck, Ward's triangle and trochanter). The nutrients intake of subjects were higher than recommended dietary allowances(RDA) except for calorie, iron, calcium, vitamin B1. Nutrients intake of vitamin A(p<0.05), iron(p<0.01), vitamin $B_2$(p<0.05), niacin(p<0.05), and vitamin C(p<0.05) between $\leq$49 yr group and $\geq$50 yr group. Mean daily intake of calcium was much less than the Korean RDA, 78.5% in $\leq$49 yr group and 77.3% in $\geq$50 yr group. 35.8% of the subjects under 50 years of age($\leq$49 yr group) and 77.4% of the subjects from 50 years up($\geq$50 yr group) were classified as osteopenia or osteoporosis. BMD of L2-L4 were positively correlated with height(p<0.05), weight(p<0.05) and BMI(p<0.05). BMD of femoral neck was positively correlated with BMI(p<0.05), BMD of Ward's triangle was positively correlated with weight(p<0.001). But BMD of L2-L4 and femoral neck were negatively correlated with age(p<0.05), menarche(p<0.05). BMD of Ward's triangle was negatively correlated with age(p<0.001). The BMD of L2-L4 were positively correlated with animal protein(p<0.05), calcium(p<0.05) and iron(p<0.05). The BMD of femoral neck was positively correlated with animal protein(p<0.05). The BMD of Ward's triangle was positively correlated with animal protein(p<0.001) and iron(p<0.001). The above results suggest that it should be difficult to prevent middle-aged women's bone destruction through nutrients intake. Further investigation is necessary to prove the mutual relations between BMD, exercise, and calcium intake. Therefore, middle-aged women will need proper exercise as well as Ca supplementation in order to prevent osteoporosis with aging.

An Effect of Dietary Chlorella on Bone Mineral Density in Postmenopausal Women

  • Age-related osteopenia and osteoporosis are common in postmenopausal women due to decrease in bone mass and ovarian function. A therapy for osteoporosis would depend on only drugs to inhibit bone loss, hormonal replacement therapy, exercise and dietary supplementation and it is very hard to fad an ideal therapy for osteoporosis as yet. Chlorella which is rich in minerals such as calcium magnesium fatty acids, vitamins and sterol, could be applicable for prevention and co-treatment of osteoporosis, but it has yet to be studied. The purpose of this study was to assess the relationship between the effect of dietary chlorella on bone mineral density (BMD) and nutritional improvement. BMD was measured in the femural neck and lumber spine portion. Nutritional and bone turnover markers from blood samples were assessed serum lkaline phosphatase, hemoglobin, number of erythrocytes and total protein. Studies for the femur neck measurement showed that normal BMD increased 2.1% for the group fed chlorella supplemented diet for four month and increased 6.6% fur group treated for one year when compared to the control group, and for the lumber spine measurements the few month group showed an increase of 9.1% over the control group, the one year group showed an increase of 64.2% over the control group. Hemoglobin content, number of erythrocytes and total protein showed similar increased patterns with BMD measurement, meanwhile, serum alkaline phosphatase increased 3% for the four month group and decrease 16% for the one year group compare to the control group. In conclusion, the postmenopausal women fed chlorella supplemented diet results in an increase in BMD. This is a marked increment in lumber spine, enhancement of nutritional state and stable bone turnover. This data showed a positive relationship between BMD and nutritional change with chlorella treatment, and suggested that chlorella dietary may lead to improving and preventing rapid loss of BMD in postmenopausal women.

The Relationship between Exercise, Bone Mineral Density and Antioxidant Enzyme Activity of Postmenopausal Women

  • This study was carried out to elucidate the relationship among exercise, bone mineral density and antioxidant enzyme activity of postmenopausal women. 60 women residing in the Iksan, Korea area were recruited. The questionnaires were designed to find out exercise habits. Bone mineral density (BMD) was measured by dual energy X-ray absorptiometry. Parameters of antioxidative capacity, including the activities of superoxide dismutase (SOD), glutathione peroxidase (GPx), catalase (CAT) and total antioxidant capacity (TA) were analyzed in fasting blood. The mean age, height, weight, and BMI of subjects were 65.0 years, 151.1 cm, 59.5 kg $26.0\;m/kg^2$, respectively. The mean BMDs of subjects were $0.85\;g/cm^2$ (lumbar spine), $0.6\;g/cm^2$ (Femoral neck), $0.49\;g/cm^2$ (trochanter), and $0.40\;g/cm^2$ (Ward's triangle). There was a significant difference in BMD among different age groups (50's, 60's and 70's) showing lower value with increasing age (p<0.05). The mean SOD, GPx, and CAT activities were 138.5 U/mL, 1,273.8 U/mL and 314.3 kU/L respectively, and TA was 1.16 mmol/L. TA of the group which exercised 3$\sim$4 times a week was significantly higher than those of the other exercise groups (p<0.05). The subjects with higher SOD activity also have a higher the T values in the lumbar spine, femoral neck, trochanter, and Ward's triangle. In conclusion, this study revealed that the levels of antioxidant enzyme activity were closely associated with the exercise status and bone mineral density in postmenopausal women.

여대생의 우유와 유제품 섭취실태 및 골밀도와의 관계 연구 (Relation between Milk Consumption and Bone Mineral Density of Female College Students in Korea)

  • The purpose of our study was to evaluate an association between milk consumption and bone mineral density (BMD) of female college students in Korea. The dietary data of 1,410 Korean college students living in the vicinity of Seoul area were collected through the Food Frequency Questionnaire. The mean age, height, weight and 8MI of the subjects were 20.4 years, 162.1 cm, 52.7 kg, and $20.0\;kg/m^2$ respectively. The mean intakes of energy, protein, and phosphorus were 2,398.7 kcal, 95.2 g, and 1,302.9 mg respectively, which were above their EER or Recommended Intake (RI), but the mean intake of calcium was 657.8 mg which was below its RI, and 44.0% of the subjects did not meet Estimated Average Requirement (EAR) of calcium. The daily milk consumption was 131.1 g, and the total consumption of milk and dairy product was 228.7 g. Forty five percent of the subjects consumed $50{\sim}200\;g$ of milk or dairy products. After the analysis of their dietary intakes, the subjects were divided into quintiles of total consumption of milk and dairy product, which were considered as major source of dietary calcium. Ninety two subjects from the upper quintile (UQ) and 85 subjects from the lower quintile (LQ) were selected and their bone mineral densities (BMD) of lumbar spine (L1-L4) and the femur were examined by the dual energy X-ray absorptiometry. The BMD $(g/cm^2)$of L1, L2, L3, and average of femur were 1.065, 1.150, 1.187, and 0.988 for the UQ subjects, which were significantly higher than those of the LQ subjects, 1.010, 1.090, 1.146, and 0.942 respectively. Moreover, the T-scores of the UQ were significantly higher than those of the LQ. The prevalence rates of osteopenia classified according to WHO's criterion were 12.0% in the UQ. and 20.0% in the LQ group which showed non significant. Blood levels of calcium, and phosphorus of UQ and LQ group were both within the normal range, and were not significantly different. Our results showed that UQ group presented the better status of bone mineral density and bone health. Therfore our results suggest that continuous consumption of more than one and a half cups of milk per day, which corresponds to the least amount of UQ group's consumption, would affect positively on the bone health of Korean adults.
