A Comparative Analysis of the Metabolic and Coagulative Profiles in Patients with Idiopathic Scoliosis, Congenital Scoliosis and Healthy Controls: A Case-Control Study

  • Study Design: Single-center, observational, case-control study. Purpose: Comparison and analysis of the metabolic and coagulative profiles in patients with idiopathic scoliosis, patients with congenital scoliosis, and healthy controls. Overview of Literature: Serum melatonin deficiency has been a controversial topic in the etiopathogenesis of scoliosis. Low bone mineral density, low vitamin D3 levels, and high parathyroid hormone levels are common metabolic abnormalities associated with scoliosis that may be responsible for its pathogenesis. In addition to metabolic defects, several studies have shown coagulation defects that either persist from the preoperative period or occur during surgery and usually lead to more than the expected amount of blood loss in patients undergoing deformity correction for scoliosis. Methods: The study population (n=73) was classified into those having congenital scoliosis (n=31), those with idiopathic scoliosis (n=30), and healthy controls (n=12). After detailed clinicoradiological evaluation of all the subjects, 10-mL blood samples were collected, measured, and analyzed for various metabolic and coagulation parameters. Results: The mean serum melatonin levels in patients with idiopathic scoliosis were significantly lower than those in the healthy controls. Although the mean serum melatonin level in the congenital group was also low, the difference was not statistically significant. Serum alkaline phosphatase and parathyroid hormone levels were higher in the scoliosis groups, whereas the vitamin D level was lower. No differences were observed in the coagulation profiles of the different groups. Conclusions: Low serum melatonin levels associated with scoliosis can be a cause or an effect of scoliosis. Moreover, low bone mineral density, high bone turn over, and negative calcium balance appear to play an important role in the progression, if not the onset, of the deformity.

  • 어린이는 8-10세 경에 외상을 자주 받으며 구강 내에서는 상악 절치부가 호발하는 부위이다. 외상으로 인하여 상악 절치가 상실되면 어린이에게 합병증을 야기할 수 있으므로 가능한 한 상악 절치를 유지해야 하지만 조기 상실되는 경우가 있다. 상악절치의 조기 상실로 인한 합병증으로는 심미적 문제나 치조골의 수직적 높이와 수평적 너비의 감소, 인접치의 경사, 악궁 장경 감소 등이 있다. 그 중에서 치조골이 소실되면 보철적 수복 시에 기능성과 안정성, 심미적인 면에 영향을 준다. 이 증례는 외상으로 인하여 상악 중절치가 조기 상실된 9세 여아와 6세 남아에서 각각 5년 5개월, 3년 7개월 후의 치조골소실 정도를 cone beam computed tomography와 진단 모형 상에서 관찰하였기에 보고하는 바이다.

PDGF-BB와 IGF-I 혼합 투여가 비글견 인공 치근단 병소의 치유에 미치는 영향에 관한 연구 (THE EFFECT OF PDGF-BB AND IGF-I COMBINATION ON THE HEALING OF ARTIFICIAL PERIAPICAL LESIONS IN BEAGLE DOGS)

  • It is difficult to treat the endodontic apical perforation successfully. In this study, we hypothesized that the application of PDGF-BB and IGF-I into periapical perforation site may accelerate periapical healing and lead to bone deposition. And the specificity of osteonectin in periapical healing was investigated. The experiments were performed on the upper and lower 51 premolar teeth of 4 beagle dogs. The pulp chamber of each tooth was opened and the dental plaque was inserted into the canal for developing the periapical lesion for 5 weeks. Then, the roots were artificially perforated at the apex with the number 4 profile of .06 taper. In each step, standard periapical radiographs were taken to compare the size of lesion each other. The radiographs were scanned and analyzed by image analysis system. The mean and standard deviation of periradicular radiolucency ratios were calculated in each group. ANOVA was used for comparison. 51 premolars were grouped into 3 groups; control group, calcium hydroxide-treated group and calcium hydroxide plus growth factors-treated group. In the control group, the apical perforations were not sealed and obturated with gutta-percha and ZOE sealer by lateral condensation technique. In the experimental groups, the apical perforation were sealed with calcium hydroxide and with/without $4{\mu}g$ of PDGF-BB & IGF-I in cellulose gel and obturated by lateral condensation technique. Fluorescent bone markers were used to measure new bone formation. Following 2, 4, 12 weeks after experiment the dogs were sacrificed and histologic sections were prepared. Each tooth block including periapical lesion was sectioned mesiodistally. One half of the sections were decalcified with 6% nitric acid and processed by standard paraffin embedding technique. The sections were stained by hematoxylin and eosin, and immunostained for osteonectin. Histomorphometrical measurement of neoformed bone was performed using a light microscope. And the other half of the sections were prepared by undecalcified preparation, and confocal laser scanning microscopic investigations were done.

연 중독의 임상적 고찰 (Clinical evaluation on 5 cases of lead poisoning)

  • 5예의 연중독 환자를 경험하여 다음과 같은 결과를 얻었다. 1. 남자가 3예, 여자가 2예였다. 2. 납중독의 원인은 개인적 원인에 의한 환약복용이 3예, 직업성 중독이 2예였다. 3. 입원 당시 4예는 위장증상을 주소로 입원하였고 복통, 변비, 복부팽만을 호소하였으며 장 연동음의 감소를 관찰할 수 있었다. 1예는 위장증상 없이 빈혈을 주소로 입원하였다. 4. 전 예에서 빈혈과 망상적혈구의 증가(평균 4.7%)와 호염기성 반점을 관찰할 수 있었고 혈색소의 평균치는 9.2gm/dl이었고 정구성, 정색소성 빈혈이었다. 5. 골수천자를 시행한 4예 모두에서 적혈구제의 증식이 있었고 호염기성 반점을 관찰할 수 있었고 평균 M : E비는 0.72 : 1이었다. 6. 혈중연은 4예에서 증가되어 있었고, 24 시간 소변 연, ALA와 CPP는 전 예에서 증가되어 있었다. 7. 복통 및 빈혈은 Ca-EDTA 투여 후 호전되었다.

  • Dental clinicians commonly encounter extrusion of the anterior teeth due to localized alveolar bone loss in adult patients with chronic periodontitis. This pathologic migration of the anterior not only may cause loss of functional anterior guidance and continuous periodontal destruction, but also can lead to esthetic disaster. In fact, most frequently reported chief complaint of the patients with extruded anterior teeth is the 'loss of esthetics', not the periodontal problem itself. The utilization of the expertise and skills in the various dental disciplines is thought to be the best treatment modality for the ultimate functional and esthetic attainment. Interdisciplinary therapy, involving periodontics, orthodontics, and restorative dentistry, was performed for the highest levels of patient and provider satisfaction in this case.

  • Although breast implants have been in clinical use for almost 6 decades and have undergone considerable development during this time, implant rupture is still a dreaded long-term complication. Some obvious external factors, such as trauma, can lead to implant rupture, but many studies have reported a high rate of "spontaneous" implant rupture. Herein, we present two cases with the aim of raising awareness of a new possible cause of "spontaneous" implant rupture: mechanical irritation by bony protrusions.

  • 식도이물의 합병증은 희소하나, 간혹 예리한 골편, 예리한 금속성리물 및 이물의 장기체류에 의해 식도손상을 일으키는 이물자체에 의한 경우, 식도질환 환자의 진단, 치료 목적으로 식도경검사 때의 조그마한 부주의에 의한 경우 및 식도의 위치관계 등의 원인으로 합병증을 일으키어 자칫 잘못하면 치명적인 상태까지 야기시키는 수도 있다. 그러므로 여기에 대한 조기진단과 신속한 치료을 가하지 않으면 높은 사망률과 병변을 초래하기도 한다. 이러한 의미에서 저자는 식도이물자체에 의한 합병도 및 식도경검사시의 부주의에 의한 합병증 27례를 경험하였기에 문헌적고찰과 함께 보고하는 바이다.

  • Bizarre parosteal osteochondromatous proliferation of bone (BPOP) was first described in 1983 by Nora et. al., who reported 35 cases of a bizarre proliferative lesion involving the small bones of the hands and less often the feet. These lesions simulate osteochondromas and conlain cartilage cap exhibiting atypical chondrocytes often undergoing ossification. Radiologically, they differ from osteochondromas. The lesion recurs in a significant number of cases after local excision, and the disturbing cytologic features may lead to a mistaken diagnosis of malignancy. The purpose of this report is to describe a case of bizarre parosleal osteochondromatous proliferation of the foot in a patient without traumatic history.

  • Hematopoiesis has a pivotal role in the maintenance of body homeostasis. Ironically, several hematological disorder caused by chemicals, drugs, and other environmental factors lead to severe bone marrow failure. Current treatments like stem cell transplantation and immunosuppression remain ineffective to ameliorate this diseases. Therefore, a newtreatment to overcome this entity is necessary, one of them by promoting the usage of medicinal plants. Thus, this study aimed to evaluate the hematopoiesis potency of S. javanica berries and leaves extracts in chloramphenicol (CMP)-induced aplastic anemia mice model. In this present study, several types of blood progenitor cell such as $TER-119^+VLA-4^+$ erythrocytes lineage, $Gr-1^+$ granulocytes, and $B220^+$ B-cell progenitor cells were evaluated by flow cytometry analysis. Accordingly, we revealed that S. javanica berries and leaves extracts significantly promoted $TER-119^+VLA-4^+$ erythrocytes lineage and $Gr-1^+$ granulocytes after exposed by CMP. Thus, these results suggested that S. javanica berries and leaves extracts might have hematopoiesis activity in CMP-induced aplastic anemia mice model.

Treatment of the cemental tear

  • Cemental tears are uncommon form of root fracture that can lead to rapid localized periodontal attachment loss. Studies have described periodontal breakdown as being associated with the separation of the cementum from the underlying tooth structure. The aim of this case report is to assess the outcome of treatment of cemental tear with several surgical treatment regimens. Three patients with cemental tear were treated with different surgical method. In all three cases, the cemental tear occurred on maxillary right central incisors. In each case, the root fragment were removed, the localized defect was treated using different surgical methods including guided tissue regeneration and bone graft followed by scaling and root planting. In all three cases, symptoms subsided after the treatment and clinical attachment level was improved up to 2 mm at 3 month after surgery. Both conventional and regenerative periodontal surgery could achieve successful outcomes.
