The Result of Mandible Fracture Fixations with Biodegradable Materials (생체흡수성 재료를 이용한 하악골절 치료의 결과)

  • Purpose: Traditionally, titanium miniplate has been used for rigid fixation of mandible fractures. However, the limitations of metal plate have been reported such as hypersensitivity, interference with the cranio-facial growth of growing child, secondary bone resorption around the plate, foreign body reaction, declination of primary callus formation, and bone atrophy, and so forth. Recently, biodegradable miniplate has been introduced and used as an alternative to the metal plate despite of its lower strength. This study evaluated the usefulness and stability of biodegradable plate and screw for treatment of mandible fractures. Methods: In this study, 61 patients(92 areas) diagnosed as mandible fracture in the last 2 years have been reviewed. We used titanium plate and screw in 32 patients, and biodegradable plate and screw($INION^{(R)}$) in 29 patients. Stability of plates and screws, bony healing process and its side effects were observed by clinical and radiographic assessment. Results: In the titanium material group, one of malocclusion, two of mouth opening limitation, three of pain, three of palpation were shown. The plate of six patients involved in these complications were removed. In the biodegradable group, two of mouth opening limitation, two of pain, one of localized wound infection were shown and one plate was removed secondarily. Conclusion: There was no statistical difference between two groups in bony healing and complication rates. Biodegradable implants show efficient stability during initial bone healing and low side effects in long-term follow up periods.

  • The authors have treated a squamous-cell carcinoma occurred in molar region of the right mandible in 52-year old woman by means of bony resection and banked tibial bone graft, and made the normal appearance of her face. The result as follow: 1) The healing of a graft is similar to the healing of an uninfected fracture except that it will take considerably longer. It is of the utmost importance that it should be well immobilized, otherwsie there is danger of absorption of bone and fibrous union. 2) Until the graft acquires a blood supply it is easily infected, for it has no defence against organisms, so it is most important to prevent wound infection set in.

Clinical Application of Recombinant Human Bone Morphogenetic Protein-2 for Delayed Fractures in Dogs (개에서 골절의 지연유합에 대한 rhBMP-2의 임상적 적용 2례)

  • A 1-year-old, castrated male Yorkshire terrier (case 1) and a 7-year-old female Poodle (case 2) presented with delayed union fractures. In case 1, the dog had a fractured right distal radius and ulna. In case 2, the dog had a fractured left distal tibia and fibula. A physical examination and radiographs performed in both dogs revealed delayed union fractures with large gaps. The fracture sites were fixed by bone plate and screws. Autogenous cancellous bone graft was applied into the fracture gap. To encourage rapid bone union, we used matrigel containing $20{\mu}g$ of recombinant human bone morphogenetic protein-2 (rhBMP-2) in the fracture site. Radiographs were taken postoperatively to monitor healing. Rapid bone union was noted in both dogs in long-term radiographs. In case 1, the radiographs revealed that the fracture gaps of the radius and ulna were bridged at 2 weeks. Fracture lines were not observed and normal appearance was restored at 20 weeks. In case 2, the radiographs showed that fracture repair had progressed at 11 weeks. The fractures healed faster than expected in these two cases. The results indicate that rhBMP-2 and matrigel may be effective and useful materials to enhance healing of delayed fractures.

Assessment of neovascularization during bone healing using contrast-enhanced ultrasonography in a canine tibial osteotomy model: a preliminary study

  • Blood perfusion of skeletal muscle and callus was evaluated using contrast-enhanced ultrasonography (CEUS) in a canine osteotomy model to determine the applicability of CEUS in the assessment of neovascularization during fracture healing and to compare the vascular signals on CEUS between external skeletal fixation and cast-applied dogs. In 6 Beagle dogs, a simple transverse osteotomy was performed at the left tibial shaft and external skeletal fixation (n = 3) or a cast (n = 3) was applied. Radiography, power Doppler ultrasonography (power Doppler), and CEUS were performed until complete union was achieved. On CEUS, vascular changes were quantitatively evaluated by measuring peak intensity (PI) and time to PI in the soft tissue and callus and by counting the vascular signals. Vascular signals from the soft tissue were detected on power Doppler and CEUS on day 2. Significantly more vascular signals were detected by CEUS than by power Doppler. On CEUS, PI in the surrounding soft tissue was markedly increased after the fracture line appeared indistinctively changed on radiography in all dogs. In the cast-applied dogs, vascular signals from the periosteal and endosteal callus were detected on CEUS before mineralized callus was observed on radiography. CEUS was useful in assessing the vascularity of soft tissue and callus, particularly in indirect fracture healing, and provided indications of a normally healing fracture.

THE EXPERIMENTAL STUDY ON THE EFFECTS OF CYCLOPHOSPHAMIDE ON THE HEALING PROCESS OF THE FRAGURE IN THE MOUSE (항암제투여가 골절치유에 미치는 영향에 관한 실험적 연구)

  • The purpose of this study is to determine the effects of Cyclophosphamide on the healing process of fractures of the mice. Tweny-one young adult male mice were used. All of them were experimentally fractured in the left mandibular body areas, and Cyclophosphamide 0.1mg. per 30gr. body wt. was administrated intramuscularly to the experimental group. the mice of both control and experimental groups were sacrificed 1,3,5,7,10,14, and 21st day after operations and microscopic slides were made. The author has observed the histopathological findings. The results were as follows; 1. There were no specific difference between the experimental and control group in the early stage (1~5th day after operation) of the healing process of the mandibular fracture. 2. In the healing process of fractures 7-10th day after operation, the fibrous tissure formation and osteoblastic activity were poor in the experimental groop compared with the control group. 3. In the healing process of fractures 14-21th day after operation, the connective tissue and new bone formation were very poor in the experimental group compared with the control group. 4. On the whole, cyclophosphamide affected the experimentally fractured wound to delay healing in the jaw bone of the mice.

Effects of Yuhyangjeongtong-san on Fracture Healing in Rats

  • Objectives: The purpose of this study is to determine the effect of Yuhyangjeongtong-san on the recovery of tibial fractures in rats. Methods: In this study, osteocalcin and Calcitonin, CTX-2, TGF-β and BMP-2, which are used as indicators of bone formation, were analyzed after hematologic fractures using experimental rats. In addition, the fracture union process was confirmed using X-rays. Results: Osteocalcin, Calcitonin and BMP-2 showed a significant increase compared with the control at 4 weeks. CTX-2 and TGF-β showed a significant increase compared with the control at 3 weeks. On X-ray, YJS treated group, as the experiment progressed, the boundary line became blurred, the bone outline was clearly visible, and the fracture recover was progressing. Conclusion: The findings suggest that YJS can play a significant role in the repair of fractures. Therefore YJS is likely to be used to treat fractures.

Guided Bone Regeneration in Comminuted Long-Bone Fractures Using Recombinant Human Bone Morphogenetic Protein-2 and a Collagen Membrane

  • A dog aged two years and seven months and a cat aged seven years were referred owing to fractures of long bones. Preoperative radiographs revealed comminuted bone fractures close to joints. Conventionally, long-bone fractures are treated using intramedullary pins, plate and screw systems, or an external fixator system. In cases of non-reducible fractures, various graft materials have been used in fracture treatments to stimulate bone repair. Here, recombinant human bone morphogenetic protein-2 (rhBMP-2) and a collagen membrane were applied. Four weeks after surgery, fractured bone fragments began to unite and the bone union was observed using radiography four months after surgery. No complications occurred related to grafted materials. We successfully applied rhBMP-2 and collagen membranes in two different species to support the healing process of comminuted fractures, according to the concept of guided bone regeneration.

A Method and Effect for Tibial Defect Treatment Using Interstitial Low Level Laser (경골 손상 치료에서의 침습형 저출력 레이저 치료법 및 효과)

  • Tibial defect, or fracture is very routine musculoskeletal case which brings fully uncomfortable and painful situations to patient. Moreover, it has long hospitalization period because of its risk of non-union. There are many studies using ultrasound, vibration, and laser for bone regeneration to figure out fast bone healing. Among them, Low Level Laser Therapy (LLLT) is already known that it is very easy to treat and may have positive effect for bone regeneration. However, LLLT has uncertain energy dose because of scattering and absorption of laser in tissue. In this study, we used interstitial LLLT to treat tibial defect in animal study. The Interstitial LLLT can overcome some limitations caused by laser scattering or absorption in tissue medium. The results were evaluated using u-CT which can calculate X-ray attenuation coefficient and bone volume of bone defect area. These results showed that interstitial LLLT may affect fast bone healing process in early phase.

Effect of electrical stimulation and vitamin AD3E on fracture healing in a rat model (전기자극과 vitamin AD3E가 랫드 골절치유에 미치는 영향)

  • Electrical stimulation and vitamin $AD_3E$ administration have been shown to enhance the repair of biological tissues such as bone, ligament and tendon, The objective of this study were (a) to investgate the therapeutic effects of different levels of electrical stimulation and vitamin $AD_3E$ administration on fracture healing in a rat model and (b) to identify the most effective voltage level. Ninety Sprague-dawley rats were divided into electrical stimulation group and vitamin $AD_3E$ administration group. Electricla stimulation group was divided into four groups on the basis of the level of current delivered. The experimental groups received current (which varied by group), while the controls, with identical electrodes, received no current. In vitamin $AD_3E$ administration group, experimental group was injected 2,500 IU of vitamin A, 1,250 IU of vitamine $D_3$, 1mg of vitamine E intramuscularly. And in control group 0.1ml of saline was injected intramuscularly. After time periods (7-day, 14-day, 21-day for stimulation all rats were tested with combination of biochemical, roentgenologic and histomorphological methods. The results obtained were as follows ; In electrical stimulation groups, serum calcium and inorganic phosphorus level of experimental and control groups showed non specific change within normal physiological ranges. In vitamin $AD_3E$ administration group, serum calcium level of experimental and control groups showed non specific changes within normal physiological ranges, while experimental group showed slightly higher serum inorganic phosphorus level. According to roentgenologic and histomorphological examination, 2V 25Hz electrical stimulation group and vitamine $AD_3E$ administration group showed statistically significant improvements in bone density and ossification reaction until day 14. The terapeutic effect of stimulation on fracture healing was similiar to that of vitamin $AD_3E$ administration. In this study stimulation of 2V 25Hz was the most effective level of electrical stimulation for the healing of fracture of rats.

Treatment of Metacarpal Bone Fracture Using Biodegradable Plates and Screws (흡수성 고정판과 나사를 이용한 중수골 골절의 치료)

  • Purpose: Metacarpal fractures are common hand injury that may require operative intervention to ensure adequate reduction and stabilization. Traditionally, titanium miniplate was used for rigid fixation of bone fractures. However, the use of permanent plate lends itself to multiple complications such as infection, exposure of the hardware, tendon adhesions, tendon rupture, prolonged pain, bony atrophy and osteoporosis (stress shielding), metal sensitization, and palpation under the skin. This study evaluated the usefulness and stability of biodegradable plates and screws for treatment of metacarpal bone fractures. Methods: There was 17 patients who had surgery for metacarpal bone fracture from April 2007 to June 2010. All patients had open reduction and internal fixation. We used absorbable plates and screws (Inion CPS$^{(R)}$) for internal fixation. Postoperative results were assessed with x-ray. Stability of plates and screws, healing process and its complications were observed by clinical and radiographic assessment. Results: All patients were successfully reduced of bone fracture, and fixations with absorbable plates and screws were stable. The mean follow up period was 7.1 months. 2 patients complained postoperative pain, but they were relieved with analgesics. All patients experienced transient stiffness, but they were relieved with active assistive range of motion after removal of splint. No patients suffered complications which could be occurred by using metallic plate. Conclusion: There was no critical complications such as re-fracture or nonunion among patients. No patients suffered side effects related with metallic implants. Biodegradable implants can offer clinically stable and attractive alternative to metallic implants to stabilize metacarpal bone fractures in the hand.
