Micro-computed tomography evaluation of the effects of orthodontic force on immature maxillary first molars and alveolar bone mineral density of Sprague-Dawley rats

  • Objective: To investigate changes in the immature teeth of Sprague-Dawley rats during orthodontic treatment and to explore the changes in the peri-radicular alveolar bone through micro-computed tomography (CT). Methods: Twenty-five 26-day-old male Sprague-Dawley rats were included. The maxillary left first molar was moved mesially under a continuous force of 30 cN, and the right first molar served as the control. After orthodontic treatment for 7, 14, 21, 28, and 42 days, the root length, tooth volume, and alveolar bone mineral density (BMD) around the mesial root were measured through micro-CT. Results: The immature teeth continued to elongate after application of orthodontic force. The root length on the force side was significantly smaller than that on the control side, whereas the differences in the volume change between both sides were not statistically significant. Alveolar bone in the coronal part of the compression and tension sides showed no difference in BMD between the experimental and control groups. The BMD of the experimental group decreased from day 14 to day 42 in the apical part of the compression side and increased from day 7 to day 42 in the apical part of the tension side. The BMD of the experimental group decreased in the root apex part on day 7. Conclusions: The root length and volume of immature teeth showed continued development under orthodontic forces. Alveolar bone resorption was observed on the compression side, and bone formation was observed on the tension side.

The Precision Test Based on States of Bone Mineral Density (골밀도 상태에 따른 검사자의 재현성 평가)

  • Purpose: ISCD (International Society for Clinical Densitometry) requests that users perform mandatory Precision test to raise their quality even though there is no recommendation about patient selection for the test. Thus, we investigated the effect on precision test by measuring reproducibility of 3 bone density groups (normal, osteopenia, osteoporosis). Materials and Methods: 4 users performed precision test with 420 patients (age: $57.8{\pm}9.02$) for BMD in Asan Medical Center (JAN-2008 ~ JUN-2008). In first group (A), 4 users selected 30 patient respectively regardless of bone density condition and measured 2 part (L-spine, femur) in twice. In second group (B), 4 users measured bone density of 10 patients respectively in the same manner of first group (A) users but dividing patient into 3 stages (normal, osteopenia, osteoporosis). In third group (C), 2 users measured 30 patients respectively in the same manner of first group (A) users considering bone density condition. We used GE Lunar Prodigy Advance (Encore. V11.4) and analyzed the result by comparing %CV to LSC using precision tool from ISCD. Check back was done using SPSS. Results: In group A, the %CV calculated by 4 users (a, b, c, d) were 1.16, 1.01, 1.19, 0.65 g/$cm^2$ in L-spine and 0.69, 0.58, 0.97, 0.47 g/$cm^2$ in femur. In group B, the %CV calculated by 4 users (a, b, c, d) were 1.01, 1.19, 0.83, 1.37 g/$cm^2$ in L-spine and 1.03, 0.54, 0.69, 0.58 g/$cm^2$ in femur. When comparing results (group A, B), we found no considerable differences. In group C, the user_1's %CV of normal, osteopenia and osteoporosis were 1.26, 0.94, 0.94 g/$cm^2$ in L-spine and 0.94, 0.79, 1.01 g/$cm^2$ in femur. And the user_2's %CV were 0.97, 0.83, 0.72 g/$cm^2$ L-spine and 0.65, 0.65, 1.05 g/$cm^2$ in femur. When analyzing the result, we figured out that the difference of reproducibility was almost not found but the differences of two users' several result values have effect on total reproducibility. Conclusions: Precision test is a important factor of bone density follow up. When Machine and user's reproducibility is getting better, it’s useful in clinics because of low range of deviation. Users have to check machine's reproducibility before the test and keep the same mind doing BMD test for patient. In precision test, the difference of measured value is usually found for ROI change caused by patient position. In case of osteoporosis patient, there is difficult to make initial ROI accurately more than normal and osteopenia patient due to lack of bone recognition even though ROI is made automatically by computer software. However, initial ROI is very important and users have to make coherent ROI because we use ROI Copy function in a follow up. In this study, we performed precision test considering bone density condition and found LSC value was stayed within 3%. There was no considerable difference. Thus, patient selection could be done regardless of bone density condition.

  • Irradiation is widely used for the treatment of malignant diseases, and possibly cause the osteoporosis. The densitometry and bone scintigraphy are valuable when used to monitor the patients longitudinally to access the progression of osteoporosis and risk of osteoradionecrosis. To evaluate the osteoporosis after irradiation of Cobalt-60 gamma ray on the lumbar spines of New Zealand white rabbits, bone densitometry by dual photon absorptiometry and bone scintigraphy were performed weekly. The decrease of bone density began at the first week after irradiation, and were in the nadir at 4-6th week. The osteoblastic activity measured by bone scintigraphy decreased in the first week, and was in the nadir at 4-6th week. The severity of these changes were related to the radiation dose. In conclusion, the osteoporosis before the presentation of the osteoradionecrosis can be diagnosed early with the dual photon absorptionmetry and bone scintigraphy.

  • Purpose : The research was done to investigate the effectiveness of 2D bony morphometry and microstructure of micro-computed tomography (micro-CT) on the osteoporotic bony change. We performed the bone morphometric analysis of proximal femur in ovariectomized rabbits with BMD and micro-CT examination. Materials and Methods : Twenty-one female (Newzealand, about 16 weeks old, 2.9-3.4kg) rabbits were used. Three rabbits were sacrificed on the day when experiment began (Baseline). The remaining 18 rabbits were divided into two groups. One group was ovariectomized bilaterally (OVX) and the other animals were subjected to sham operation (Sham). Bone specimens were obtained from the right and left femur of sacrificed rabbits. At intervals of 1, 2, 3, 5, 6 months respectively, BMD tests were performed on the proximal femur by using PIXlmus 2 (GE Lunar Co. USA), 2-dimensional bone morphometric analysis by custome computer program and 2D/3D bone structure analysis by micro-CT (Skyscan 1072, Antwerpen, Belgium). Statistical analysis was carried out for the correlation between bone morphometry, micro-CT and BMD Result : BV/TV, Tb.Th, Tb.N of micro-CT parameters showed higher values in sham group than OVX group. N.Nd/Ar.RI, N.NdNd, N.NdTm, N. TmTm, PmB/Ar.RI, 3-D BoxSlope of 2D morphometric parameters showed higher values in Sham group than OVX group. The micro-CT parameters of Tb.Sp, Tb.N were statistically significant correlated with BMD respectively. Several 2D morphometric parameters were statistically significant correlated with BMD respectively. Conclusion : Several parameters of 2D bony morphometry and micro-CT showed effective aspects on the osteoporotic bony change.

The Study on the Factors which are Related to Bone Mineral Density of Male (남성 골밀도와 관련 요인에 대한 연구)

  • Objectives: Osteoporosis is the most common metabolic disease of the bone, and is one of the most important major public health problems world wide. It is more occurred in female than male, but as the osteoporosis of men is increasing, therefore bone fractures of men are increasing. So we investigated the factors which are related to Bone Mineral Density(BMD) of male for prevention of osteoporosis. Methods: We measured the Bone Mineral Density(BMD) of lumbar spine($L_2$-$L_4$) and femoral neck in 5198 male, using dual energy X-ray absorptionmetry(DEXA; DPX-alpha). And then we analysed the 8 factors - age group, bone mass index(BMI), amount of smoking, drinking, exercise, and fast blood sugar, gastric disease, thyroid disease - which are related to BMD of male. Results: 1. In age group according to ${\ll}$Hwangjaenaekyong Somun, 黃帝內徑 素問${\gg}$, T-score was the highest at 17-24(三八歲) years group and decreased rapidly after 57-64(八八歲) years group in both lumbar spine($L_2$-$L_4$) and femoral neck. Therefore we concluded that T-score of male in lumbar spine($L_2$-$L_4$) and femoral neck change according to age group in ${\ll}$Hwangjaenaekyong Somun, 黃帝內徑 素問${\gg}$. 2. In BMI(body mass index), T-score of lumbar spine($L_2$-$L_4$) and femoral neck were the highest in obese group than non-obese group. In comparison of age group according to BMI, T-score of lumbar spine($L_2$-$L_4$) was significant difference in 17-72 years group and T-score of femoral neck was in 25-72 years group. 3. In exercise, T-score of lumbar spine($L_2$-$L_4$) and femoral neck was increasing as exercising more. In comparison of age group according to exercise, Both T -score of lumbar spine and femoral neck were significant difference in 25-72 years old. 4. T-score of lumbar spine($L_2$-$L_4$) was the highest in men who have taken exercise daily, and T-score of femoral neck was the highest in men who have taken exercise 1-3 times for a week. Conclusions : The age group in ${\ll}$Hwangjaenaekyong Somun, 黃帝內徑 素問${\gg}$ is related to BMD of men. And risk factors - BMI, exercise - are related to BMD of men. Therefore we expect that this study will help for prevention of osteoporosis of men.

Effect of Soymilk and Exercise on Bone Mineral Density and Bone Metabolism Related Markers in Underweight College Women with Low Bone Density

  • The objective of this study was to investigate the effect of soymilk and exercise on bone mineral density (BMD) in underweight college women of 19-22 years of age, who had lower bone mass. The BMD of the lumbar spine and femoral neck was measured for 52 underweight college women. Among them, 33 subjects, whose t-score value was below -1, were selected. Questionnaire survey, anthropometrical measurements, dietary recall, analysis of BMD, fasting serum osteocalcin and urinary deoxypyridinoline (DPD) were conducted before and after the 10 week study. The 33 subjects were divided into 2 groups: soymilk group (n=19), and soymilk + exercise group (n=14). The soymilk group was given 400$m\ell$ soymilk containing 60mg of isoflavones on a daily basis and the soymilk + exercise group exercised three times a week with a daily intake of 400$m\ell$ soymilk for 10 weeks. The average ages of the soymilk group and the soymilk + exercise group were 21.1 years and 20.4 years, respectively and, there were no significant differences between the soymilk group and the soymilk + exercise group in the areas of height, weight or Body Mass Index (BMI). At the baseline, the mean daily energy intake of the soymilk group and the soymilk + exercise group was 1,597.9kcal (79.43% of RDA) and 1,704.2kcal (85.2% of RDA), respectively. The mean calcium intake of the soymilk group (408.3mg) was not significantly different from that of the soymilk + exercise group (389.4mg). Despite the 400$m\ell$ soymilk supplementation, there were no significant changes of nutrient intake in either group after treatment. However, there were significant increases in BMD's of lumbar spine and femoral neck in both groups. There were some increases in the serum osteocalcin level and decreases in the urinary deoxypyridinoline level as well. BMD change of the soymilk group was not significantly different from that of the soymilk + exercise group. In conclusion, supplementary intake of soymilk (containing 60mg of isoflavones) resulted in a significant increase in the BMD's of the lumbar spine and femoral neck in underweight college women with low bone mass. However, exercise did not result in any significant changes in the BMD's, implying the necessity for more intensive and specific long-term physical training for any substantial changes. Further investigation is necessary to determine the exercise that most strongly affects BMD.

THREE DIMENSIONAL FINITE ELEMENT ANALYSIS ON THE MINIMUM CONTACT FRACTION OF BONE-IMPLANT INTERFACE (골조직과 임플랜트 계면의 최소접촉분율에 관한 삼차원 유한요소분석적 연구)

  • In order to find the degree of osseointegration at bone-implant interface of clinically successful implants, models including the 3.75mm wide, 10mm long screw type $Br{\aa}nemark$ implant as a standard and cylinder, 15mm long, 5.0mm wide, two splinted implants, and implants installed in various cancellous bone density were designed. Also, the amount of load and material of prostheses were changed. The stress and minimum contact fraction were analyzed on each model using three-dimensional finite element method(I-DEAS and ABAQUS version 5.5). The results of this study were as follows. 1. 10mm long, 3.75mm diameter-screw type implant had $36.5{\sim}43.7%$ of minimum contact fraction. 2. Cylinder type implant showed inferior stress distribution and higher minimum contact fraction than screw type. 3. As implant length was increased, minimum contact fraction was increased a little, however, maximum principal stress was decreased. 4. Implants with a large diameter had lower stress value with slightly higher minimum contact fraction than standard screw type. 5. Two splinted implants showed no change of minimum contact fraction. 6. The higher bone density, the lower stress value. 7. The material of occlusal surface had no effect on the stress of the bone-implant interface.

Effect of Tower Climbing Exercise, Aerobic Exercise & Alpha Lipoic Acid Administration on Bone Metabolism Factor in Ovariectomized Rats (자율적 등반운동과 유산소성 운동시 alpha lipoic acid 처치가 난소절제 흰 쥐의 골 대사 변인에 미치는 영향)

  • The purpose of this study was to investigate the change of bone metabolic parameters between climbing exercise training and treadmill running training with alpha lipoic acid supplementation in ovariectomized rats. Fifty-six Sparague-Dawley, 8 weeks of age, were assigned into nine groups: normal control at pre-test (Pre-NC), normal control at post-test (NC), sham-sedentary (SS), ovariectomized-control (OVX-Con), ovariectomized-ALA inject (OVX-ALA), ovariectomized-climbing exercise (OVX-CE), ovariectomized-ALA inject+climbing exercise (OVX-ACE), ovariectomized-treadmill exercise (OVX-TE) and ovariectomized-ALA inject+treadmill exercise (OVX-ATE). The climbing exercise group voluntarily climbed the 200 cm tower to drinking water from the bottle set at the top of it. The treadmill exercise group was performed the 60 min treadmill running with grade 6-8%, and velocity 10-15 m/min. The frequency of exercise training was performed 6 days per wk in both training intervention. For the comparison of bone metabolic responses following to different training intervention, the strength, length and weight of femur, total bone density, bone mineral contents, deoxypyridioline, osteocalcin and parathyroid hormone, estradiol and lutenizing hormone were measured in this study. The body weight was higher in the OVX-Con, OVX-ALA and OVX-TE groups as compared to NC group. The total bone mineral density of OVX-ACE showed a higher value than SS, OVX-Con, OVX-ATE and OVX-TE. But urine and blood metabolic parameters showed no significance among groups. In conclusion, this study results suggested that climbing exercise training and alpha lipoic acid supplementation may an effective intervention for prevention of osteoporosis in ovriectomized rats.

The Relationship between Bone Mineral Density and Physical, Obstetric Characteristics in Middle-aged Women (중년 여성의 골밀도와 신체적, 산과적 특성과의 관계 연구)

  • Purpose: This study were to investigate BMD of middle-aged women and to examine the relationships between BMD and Physical, Obstetric characteristics Method: The data was collected from 119 healthy women who were 40-60 years old. they were examined for BMD at 4 regions(forearm, lumbar, femur, whole body), %fat by DEXA and investigated physical, obstetric characteristics using scale, questionnaire from January to March, 2001. Result: 1) According to bone diagnostic results by WHO classification, 95.8% of forearm and whole body BMD were normal but 21.8-48.7% of lumbar and femur BMD(neck, trochanter, ward's triangle) were diagnosed osteoporosis or osteopnea. 2) The bones were significantly positive correlations of each other (r=.19-.69, p=.04-.00) and there were significant correlations between BMD and physical, obstetric characteristics such as age (r=-.22, p=.02), weight(r=.36~.48, p=.00), height(r=.22, p=.02), %fat(r=.19, p=.04) and age of first delivery(r=-.28, p=.00). Conclusion: Based on this study, healthy middle-aged women were also exposed to risk of osteoporosis related to aging, change of physical conditions or hormonal release. Further research to develop nursing interventions for the purpose of preventing osteoporosis by modifying risk factors is suggested.

Effectiveness of Mini-Implant for the Reduction of Mandibular Fracture

  • Purpose: This study sought to verify the usefulness of mini-implant and surgical steel wire in the treatment of mandibular fracture through the objective identifi cation of the change of bone structure and bone density before and after reduction by evaluating radiological change through fractal analysis when mandibular fracture is treated using mini-implant and surgical wire. Materials and Methods: This study looked at 45 patients (males: 38, female: 7) diagnosed with mandibular fracture in the oral and maxillofacial surgery division of Chung-Ang University Dental Hospital and who received open reduction and intra-osseous fi xation. Result: The average fracture dimension values were higher for the group of the patients who had mini-implants and surgical wire treatment. Conclusion: Based on the results of the study on the usefulness of the reduction technique using mini-implant and surgical steel wire in the treatment of mandibular fracture through the fractal analysis method, the reduction technique using mini-implant and surgical steel wire is regarded as an effective method of minimizing the gap between mandibular fracture fragments.
