  • Aneurysmal bone cyst is a benign, highly vascular bony lesion of unknown origin that may present difficult diagnostic and therapeutic problems. It is usually located on long bones, but rarely on the spine. The lesion is usually treated successfully by curettage and bone grafting when it involves bones of the extremities, but there are special considerations in the management of the lesion when located on the spine. We report a case of aneurysmal bone cyst in on the 8th thoracic spine treated with surgical treatment.

Clinical evaluation of sinus bone graft in patients with mucous retention cyst

  • Background: Mucous retention cyst refers to a cyst made by expansion due to the blockage of the salivary gland near the maxillary sinus, and it is surrounded by epithelial cells. Most of them are small; therefore, they cannot be found well and are frequently with antral polyp. The aim of this study was to evaluate the clinical prognosis of sinus bone graft in patients with mucous retention cyst. Methods: This study was performed retrospectively on 23 patients who had sinus bone graft. Group 1 was 8 patients (10 sinuses) who had a mucous retention cyst, and group 2 was 15 patients (17 sinuses) who had no pathologic history about the maxillary sinus. For these patients, sinus bone graft was performed using the lateral approach technique. The total 51 implants were placed 6.22 weeks on the average after sinus bone graft. Sinus membrane perforation during operation, postoperative complications, marginal bone loss after restorative function, implant success rate, and survival rate were analyzed. Results: There was no complication in group 1, and there were three complications in group 2. In group 2, two cases of implants failed. The types of postoperative complications consisted of two minor infections and one wound dehiscence. Two implants of total 51 implants were removed, and the survival rate of implants was 96.08 % (group 1 100 %, group 2 93.5 %). The total success rate of implants was 92.2 % (group 1 95 %, group 2 90.3 %). Conclusions: The clinical prognosis was not affected by the presence of mucous retention cyst.

Aneurysmal Bone Cyst of the Orbit : A Case Report with Literature Review

  • Aneurysmal bone cyst (ABC) is benign vascular lesion destructing the cortical bone by the expansion of the vascular channel in the diploic space that usually involve long bone and spine. Orbital ABC is rare and the clinical symptoms deteriorate rapidly after initial slow-progression period for a few months. A 12-year-old female patient visited ophthalmologist due to proptosis and upward gaze limitation of the right eye, and orbital mass was noted in the upper part of right eye on orbital MRI. Five months later, exophthalmos was worsened rapidly with other features of ophthalmoplegia. Orbital mass was enlarged on MRI with intracranial extension. Surgery was done through frontal craniotomy and intracranial portion of the tumor was removed. Destructed orbital roof and mass in the orbit was also removed, and surrounding bone which was suspected to have lesion was resected as much as possible. Histopathological diagnosis was aneurysmal bone cyst. Postoperative course was satisfactory and the patient's eye symptoms improved. Authors report a rare case of orbital ABC with review of the literature. Exact diagnosis by imaging studies is important and it is recommended to perform surgical resection before rapid-progressing period and to resect the mass completely to prevent recurrence.

  • The authors describe a case of aneurysmal bone cyst of a thoracic vertebra. A 34-year-old woman presented with posterior neck and upper back pain. Radiological examination demonstrated an aneurysmal bone cyst involving the posterior elements of the second thoracic vertebra. The spinal cord was compressed severely also. Satisfactory results were obtained after complete resection of the lesion.

  • An extremely unusual case of epidermoid cyst showing diffuse parakeratosis and aggressive clinical behavior is presented. A destructive bone lesion with surrounding ill-defined soft tissue lesion was found by computed tomography in a 63 year-old man complaining of painful swelling of the right buttock. He had a history of surgical excision twice for epidermoid cysts of soft tissue of the right hip during recent one year On aspiration cytology, the aspirate was highly cellular and mostly composed of desquamated nucleated squamous cells. Operation finding revealed that the iliac bone was Irregularly destroyed and filled with gray-white cheesy material and necrotic bone bedris. Adjacent gluteus muscle showed scattered gray-white lesions. The curettage specimen showed bone necrosis and desquamated squamous cells filling the marrow spaces. The lesion within muscle revealed epidermoid cyst with diffuse parakeratosis.

Frontonasal Bone Flap for Treating a Paranasal Sinus Cyst Diagnosed by CT in a Thoroughbred Racehorse

  • A 3-year-old Thoroughbred gelding was referred to the Jeju National University Equine Hospital with prolonged unilateral nasal discharge for several months. A sinus cyst was suspected based on the history, symptoms, endoscopic findings, and radiographic results. Computed tomography (CT) scan revealed that the cyst (7.5 × 8.0 × 10.0 cm) was located between the rostral maxillary sinus and the frontal sinus, causing distortion and remodeling of the overlying bones, swelling of the overlying cutaneous tissues, and a slight deviation of the nasal septum. Standing surgery under sedation was decided to remove the cyst. A frontonasal bone flap was performed using an oscillating bone saw, and the cyst filled with pus was removed. To support the approach to the maxillary sinus, trephination was performed. The horse was treated with sinus lavage via catheterization, nebulization, antibiotics, and nonsteroidal anti-inflammatory drugs. The horse was discharged 18 days after the surgery without signs of any complication. This case showed that a CT scan could be a valuable tool for the diagnosis and subsequent surgical management of paranasal sinus cysts in horses. Also, the frontonasal bone flap was useful for exposing and removing the large-sized of cyst in the paranasal sinus.

Incidentally detected odontoma within a dentigerous cyst

  • Odontoma is an asymptomatic slow-growing odontogenic tumor. It is usually found by chance in the maxilla or mandible on radiography, or when it deforms the adjacent teeth. It is commonly found in patients who are 30 years of age or younger. We report our encounter with an odontoma within a dentigerous cyst found incidentally in a 56-year-old man. He presented with abnormal fullness in the right infraorbital area of the cheek. During the evaluation of the mass, we incidentally detected the odontogenic tumor within a dentigerous cyst in the patient's maxilla. Under general anesthesia, complete surgical drainage of the infraorbital cystic mass was performed. Enucleation of the odontogenic tumor and a bone grafting from the iliac bone were also performed. The final diagnosis was odontoma. After 2 years of follow-up, there was no sign of recurrence of the tumor. In case of odontogenic tumors, even in old patients, it is important to suspect an odontoma. When odontoma accompanies a dentigerous cyst, surgical excisional biopsy should be performed to rule out malignancy. In case of a large bony defect after enucleation, autogenous bone grafting is essential for reconstruction.

Bone regenerative effect of recombinant human bone morphogenetic protein-2 after cyst enucleation

  • Background: The aim of this study is to quantitatively evaluate the effect of rhBMP-2 for repair of bone defects after cyst enucleation using the osteogenesis index (OI). Methods: Under general anesthesia, 10 patients (12 lesions) underwent oral or maxillofacial surgery for cyst enucleation. Postoperatively, 12 lesions were divided into two groups: group A (six lesions) was treated with absorbable collagen sponge (ACS) in combination with rhBMP-2, and group B (six lesions) was treated with ACS alone. After 3 months, cone-beam computed tomographic scans were obtained to measure changes in the volume of the lesions. We then calculated the OI of each group at two different Hounsfield units to determine any statistically significant difference between these two groups (Mann-Whitney U test). Results: As tested at the level of new bone, the mean OI was 72.37 % in group A and 55.08 % in group B -a statistically significant difference (p = 0.041). As tested at the level of mature bone, the mean OI was 27.47 % in group A and 18.88 % in group B, but the difference was not statistically significant (p = 0.394). Conclusions: The application of rhBMP-2 after maxillofacial cyst enucleation accelerated new bone formation in the bone defects. Thus, the use of rhBMP-2 in combination with ACS may be considered an alternative to conventional bone grafting in some patients with postoperative bone defects.

프랙탈 분석을 이용한 낭종 적출술 후 결손부 치유 양상에 관한 연구 (Study on bone healing process following cyst enucleation using fractal analysis)

  • Introduction: Bone regeneration of cystic defects of the jaws after a cyst treatment requires lengthy healing periods. Generally, the bony changes are observed periodically through a visual radiographic reading as well as by the clinical opinion and radiographic images (panorama, periapical view), but it is difficult to compare the objective bony changes using only the radiographic density. In addition, it is difficult to observe minute bony changes through a visual radiographic reading, which can lead to a subjective judgment. This study exmined the bone density after the enucleation of a jaw cyst by fractal analysis. Materials and Methods: Eighteen patients with a cystic lesion on the jaw were assessed. Panoramic radiographs were taken preoperatively, immediately postoperatively, and 1, 3, 6 and 12 months after cyst enucleation. The images were analyzed by fractal analysis. Results: The mean fractal dimensions increased immediately after surgery and 3, 6 and 12 months postoperatively. The postoperative 6 and 12 months fractal dimension was similar to the controls. Conclusion: Fractal analysis was used to overcome the limit of a subjective reading during an assessment of bone regeneration after cyst enucleation.

단순 골 낭종의 보존적 치료 (Conservative Management in Simple Bone Cyst)

  • 목적: 단순 골 낭종에 대해 관찰 요법 또는 국소적 스테로이드 주입술을 통한 보존적 치료를 시행한 결과에 대하여 보고하고자 한다. 대상 및 방법: 1990년 2월부터 2010년 5월까지 단순 골 낭종으로 진단된 153명 중 보존적 치료만을 시행한 126명을 대상으로 하였다. 진단시 평균 연령은 14세(${\pm}9$세, 2-60세)였으며 평균 추시 관찰 기간은 41개월(${\pm}37$개월, 12-159개월)이었다. 보존적 치료의 치료 결과에 대해 변형 Neer 분류로 나누어 비교하였다. 결과: 단순 골 낭종으로 진단되어 보존적 치료를 실시한 환자 중 관찰 요법만 실시한 경우는 31예(24.6%)였고, 스테로이드 주입술을 실시한 경우는 95예(75.4%)였다. 보존적 치료 후의 결과는 변형 Neer 분류상 치유에 해당하는 Grade I, II가 102예(80.9%)였다. 결론: 단순 골 낭종의 일차적 치료로 보존적 치료를 고려하여야 한다.
