상악골에 발생한 치성낭종에서 유래된 편평상피세포암 (SQUAMOUS CELL CARCINOMA OF THE MAXILLA ORIGINATED IN ODONTOGENIC CYST - A CASE REPORT -)

  • Primary intraosseous carcinoma(PIOC) is defined as a squamous cell carcinoma arising within the jaw, having no initial connection with the oral mucosa. The squamous cell carcinoma within the bone can be presumably developed from residues of the odontogenic epithelium, therefore, it is seen in the jaw only. Metastatic carcinoma from another primary site should be excluded in the diagnosis of Primary Intraosseous Carcinoma. This is a case of 62-year-old man, who initially diagnosed as odontogenic cyst on maxilla, but its pathologic examination was diagnosed as squamous cell carcinoma with odontogenic cyst. We treated this patient with partial maxillectomy, modified radical neck dissection(mRND), and postoperative radiation therapy.

전두동을 침습한 안와부 피부모양기형낭의 치험례 (A Clinical Experience of Direct Extension to Frontal Sinus of Orbital Dermoid Cyst)

  • The dermoid cyst is the one of common space occupying orbital lesion. This lesion is regarded as a non-invasive tumor, but infrequently causes destruction of adjacent bony structure and displacement of adjacent tissue. We experienced a characteristic ovoid orbital dermoid cyst that occupied in the frontal sinus and causes displacement of the eyeball with well-defined lining. This 55-year-old male presented a mass in left orbit, which rapidly increased in size for past 12 months, and patient could not open left eyelid. We removed this tumor totally then reconstructed the orbital roof and frontal sinus with an iliac bone graft and polyethylene sheet(Medpor Newnan, USA). This patient was followed up for 12 months and patient obtained satisfactory result without any complication suck as recurrence or infection.

An Intramedullary Neurenteric Cyst in the Conus Medullaris with Recurrent Meningitis

  • Neurenteric cysts are rare congenital lesions of the spine that are lined with endodermal epithelium. Their most common location is the cervico-dorsal region, and the mass usually lies ventral to the spinal cord. However the conus medullaris area location is an uncommon location. Neurenteric cysts are best treated by decompression and as near total excision of cyst membrane as possible. We report a case of a 7 year-old-girl with a neurenteric cyst in the conus medullaris. The patient had a history of meningitis and a gait disturbance. Magnetic resonance imaging [MRI] showed an intramedullary mass lesion in the conus medullaris with syringomyelia. There was no associated bone or soft-tissue anomaly. The mass was subtotally removed through a posterior approach. However 4 months later, meningeal irritation signs developed and MRI showed recurrence of the cyst. At the second operation, the cystic membrane was totally removed and the patient's neurological symptoms improved postoperatively. We reports a case of recurred neurenteric cyst occurred in unusual location with the review of literature.

  • Postoperative maxillary cyst (POMC) is relatively common complication among patients who underwent Caldwell-Luc surgery. Patients with POMC usually have no symptoms, although cyst extension can result in bone destruction or cystic infection with pain. The trigeminal nerve consists of the ophthalmic nerve, maxillary nerve, and mandibular nerve. Among these branches, the maxillary nerve runs to the lateral and frontal sides of the maxillary sinus wall. POMC can rarely lead to trigeminal neuropathy caused by cyst enlargement that compresses some branches of the trigeminal nerve. Recently, we experienced a case with trigeminal neuralgia due to POMC. The patient was successfully treated with inferior meatal antrostomy. We report this rare case with a literature review.

단방성 법랑모세포종 환아에 대한 증례 보고 (CASE REPORT OF UNICYSTIC AMELOBLASTOMA)

  • 단방성 법랑모세포종(Unicystic ameloblastoma)은 임상적, 방사선적 및 병리학적 소견과 치료방법 등이 법랑모세포종과 구별되어 따로 분류된 질환이다. 임상적, 방사선적으로는 치성낭종의 소견을 보이는 단방정 병소이며 조직학적으로 낭종의 소견과 함께 법랑모세포종의 소견을 보인다. 하악 구치부에 호발하며 악골에 무통성 종창을 야기 할 수 있으나 대개는 무증상인 경우가 많고 대부분 미맹출치 치관을 둘러싼 방사선 투과성 병소로 나타나며 방사선적으로 함치성낭(dentigerous cyst)이나 잔류낭(residual cyst)과 유사하다. 조직학적으로 낭종상피의 법랑모세포종성 변화(luminal type), 낭종강 내 종양결절의 돌출(intraluminal type), 결합조직 내로 법랑모세포종 세포의 증식(mural type) 등의 소견을 보인다. 본 증례는 좌측 안면부 종창을 주소로 내원한 8세 남자 환아에서 임상적, 방사선적 검사 후 낭종의 완전 적출술(enucleation)및 장골이식(iliac bone graft)을 시행하였으며 생검을 통해 단방성 법랑모세포종이라 확진되었고 양호한 치료 결과를 얻었기에 이에 보고하는 바이다.

  • Many alloplastic materials have been used as the bony substitute in large bony defects caused by fracture, periodontitis, & cyst, etc. Nowadays Hydroxyapatite(HAP) is the most usable material as the bony substitute. The reasonable properties of HAP are nontoxic, biocompatible to host tissues & have osteoconductivity. Other bioceramic materials are recommended as the bony substitute with high success rate. We have studied the clinical use of HAP as the bony substitute in the defected area caused by cyst. The reasonalbe & successful results are obtained. The results were as followed. 1. Better prognosis was obtained in the case of HAP & bone mixed graft than HAP graft only. And the best prognosis was obtained in the case of iliac bone graft. 2. Better prognosis was obtained in Mx. than in Mn. 3. It seems that the soft tissue ingrowth into the HAP granule play an important role in the success of the HAP graft. 4. Though the flap covering the HAP granules was perforated, the relative good prognosis was obtained by re-suturing the perforeated site.

Eosinophilic Granuloma Presenting as an Epidural Hematoma and Cyst

  • Langerhans' cell histiocytosis (LCH) is a rare immunologic disorder characterized by histiocyte proliferation in multiple organ systems. Eosinophilic granuloma, a benign bone lesion, represents a focal form of LCH. We experienced a case of Langerhans' cell histiocytosis in a patient who presented with intracranial epidural hematoma and cyst on the midline of the frontal skull. A 10-year-old boy presented with a rapidly growing large scalp mass on the midline frontal area after mild head trauma. The scalp mass was painless and immobile. Plain skull x-ray showed a punched-out bone lesion. Computed tomography and magnetic resonance imaging showed a non-enhancing osteolytic lesion presenting with an epidural hematoma and cyst on the midline of the frontal skull. The lesion of the skull was completely resected and the patient's recovery was uneventful. The acute presentation of a solitary eosinophilic granuloma of skull with an epidural hematoma has been described in only five cases in the literature and we report the first case of LCH presenting as an intracranial epidural hematoma on frontal area.

치근단병소에 관한 방사선학적 연구 (A RADIOGRAPHIC STUDY ON PERIAPICAL LESIONS)

  • The author studied the age distribution, etiology, affected site and several radiographic features of periapical granulomas, cysts, and abscesses. The material consisted of 928 films obtained from the patients who were diagnosed and treated under the diagnosis of periapical granulomas, cysts, and abscesses during the past 8 years (1979-1986) at the Infirmary of Dental School, Chosun University. The obtained results were as follows: 1. The order of incidence was as follows: periapical abscess (67.2%), granuloma, and cyst. 2. The age distribution revealed the highest incidence around the age of 30 and relatively higher incidence over the age of 60 in the case of periapical abscess and granuloma. 3. In the frequency of location: Periapical abscesses occured most frequently in the mandibular molars. Granulomas showed relatively higher incidence in maxilla than in mandible. Cysts were most common in the maxillary anterior teeth. 4. The mean diameter of dental granuloma was 5.9㎜, however, all dental granulomas were less than 9.3㎜ in diameter. The mean diameter of periapical cyst was l3.8㎜. 5. Periapical cyst revealed well circumscribed radiolucent lesions and 77.8% of the lesion showed white line. 86.0% of dental granuloma showed well circumscribed border, 54.5% sclerosis on surrounding bone and 38.5% partial white line. Periapical abscess revealed diffuse radiolucent lesion, 89.6% of the lesions had sclerosis on surrounding bone, and 38.0% sinus tract.

Risk factors for postoperative infection of odontogenic cysts associated with mandibular third molar

  • Background: Odontogenic cysts associated with lower third molar are common. The prognosis for surgical treatment is relatively good. However, postoperative infection discourages the clinicians. Hence, we would like to investigate the factors associated with infection after surgical treatment of cysts associated with the mandibular third molar. Methods: We retrospectively reviewed the medical and radiographic records of 81 patients who were diagnosed with dentigerous cyst or odontogenic keratocyst and underwent cyst enucleation. The factors affecting postoperative infection were divided into host factor, treatment factor, and cystic lesion factor. To identify the factors associated with postoperative infection, we attempted to find out the variables with significant differences between the groups with and without infection. Results: A total of 81 patients (64 male and 17 female) were enrolled in this study. There was no statistical relationship about the postoperative infection between all variables (gender, smoking, diabetes mellitus, age, bone grafting, related tooth extraction, previous marsupialization or decompression, type of antibiotics, cortical perforation associated with cystic lesion, preoperative infection, preoperative cyst size). Conclusions: The results of this study suggest that it is not necessary to avoid bone grafts that are concerned about postoperative infection.

하악골에 광범위하게 발생한 치성각화낭종 2례 (TWO CASES OF LARGE ODONTOGENIC KERATOCYST(OKC) IN THE MANDIBLE)

  • The odontogenic keratocyst(OKC) was used of term and described some of clinical, radiological and microscopic features by Philipsen 1956. The microscopic finding of OKC is characterized by the production of keratin, Which sometimes fills the lumen and thinning of epithelium of cyst. The most clinical importance of OKC is its extraordinary recurrence rate owing to the incomplete removal of the cyst wall. The final diagnosis of OKC should be evaluated by histologic findings and follow-up of any case of OKC with annual roentgenograms and clinical evaluations is essential for at least five years after surgery. This article is report of 2 cases of large OKC involved both molar portion and symphysis of mandible with facial asymmetry, severe bone destruction and paresthesia of lower lip on clinically, roentgenographically. By the postoperative findings the patients showed normal regeneration of bone defect area without recurrence signs by panorama films after surgery.
