Factors Associated with the Stability of Two-part Mini-implants for Intermaxillary Fixation

  • Two component orthodontic C-implants have been introduced as intermaxillary fixation (IMF) screws in cases of periodontal problems with bone loss, severely damaged teeth, or short roots. This retrospective research sought to investigate the complications and risk factors associated with the failure of two-part C-implants for IMF cases and to show the possible indications compared to one-component mini-implants. The study sample consisted of 46 randomly selected patients who had a total of 203 implants. Pearson chi-square tests of independence were used to test for associations among categorical variables. At least 19 of the total 203 implants failed (9.3%). There was no significant difference in implant failure due to gender, oral hygiene, and placement, although a significant difference due to soft tissue characteristics and root contact was observed. The two-component design of the mini-implant is reliable for difficult IMF cases. Note, however, that the factors influencing implant failure were found to be age, root damage, and condition of soft tissues.

Surgical management of edentulous/atrophic mandibular fracture: a report of two cases

  • According to Luhr's classification, a fracture in the mandible with a width of less than 15-20 mm is considered to be an atrophic mandibular fracture and its incidence is very rare. Because of the reduced cross-sectional area and smaller contact area of the fractured ends as well as the poorly vascularized bony structure and delayed bone healing, an atrophic mandibular fracture is a great challenge for oral and maxillofacial surgeons. Surgeons tend to perform closed reduction, because open reduction is considered a non-life-saving surgery among elderly patients. Thus, most of them have limited experience in surgical management. According to recent reports, open reduction yields a good result, and the Association for Osteosynthesis (AO) group has recommended open reduction. This is a case report of our two experiences of open reduction and rigid fixation of atrophic mandibular fractures by the AO principle. Articles were also reviewed here.

  • Florid osseous dysplasia (FOD) is an uncommon, benign, cemento-osseous lesion of the jaws. The etiology of FOD is still unknown. It is often asymptomatic and may be identified on routine dental radiographs. The classic radiographic appearance of FOD is amorphous, lobulated, mixed radiolucent/radiopaque masses of cotton-wool appearance with a sclerotic border in the jaws. In our case the lesion was found incidentally on routine periapical radiographs taken for restored teeth and edentulous areas. For further and detailed examination, a panoramic radiograph and cone-beam computed tomograph (CBCT) were taken. The panoramic radiograph and CBCT revealed maxillary bilateral and symmetrical, non-expansile, well-defined, round, radiopaque masses in contact with the root of the maxillary right second molar and left first molar teeth. Our aim in presenting this case report was to highlight the importance of imaging in diagnosis of FOD.

인공디스크에 대한 생체역학적 분석 (Biomechanical Analysis of the Artificial Discs)

  • Although several artificial disc designs have been developed for the treatment of discogenic low back pain, biomechanical change with its implantation was rarely studied. To evaluate the effect of artificial disc implantation on the biomechanics of functional spinal unit, nonlinear three-dimensional finite element model of L4-L5 was developed with 1-mm CT scan data. Two models implanted with artificial discs, SB $Charit\acute{e}$ or Prodisc, via anterior approach were also developed. The implanted model predictions were compared with that of intact model. Angular motion of vertebral body, force on spinal ligaments and facet joint, and the stress distribution of vertebral endplate for flexion-extension, lateral bending, and axial rotation with a compressive preload of 400 N were compared. The implanted model showed increased flexion-extension range of motion and increased force in the vertically oriented ligaments, such as ligamentum flavum, supraspinous ligament and interspinous ligament. The increase of facet contact force on extension were greater in implanted models. The incresed stress distribution on vertebral endplate for implanted cases indicated that additinal bone growth around vertebral body and this is matched well with clinical observation. With axial rotation moment, relatively less axial rotation were observed in SB $Charit\acute{e}$ model than in ProDisc model.

유리 복직근 및 복직근피판술을 이용한 사지의 재건술 (Reconstruction of the Extremity Injury using by Free Rectus Abdoninis Muscle or Myocutaneous Flap)

  • Severe upper and lower extremity trauma may result in soft tissue loss with exposed bone and the subsequence of risk of chronic osteomyelitis or malunion of fracture fragments. Such injuries present a major reconstructive problem. But Since the introduction of microsugical technique, free muscle and myocutaneous flaps were employed to provide coverage of severely injured defects. Since Tai and Hasegawa(1974) first reported a breast reconstruction using by rectus abdominis myocuraneous flap, the free rectus myocutaneous flap has been widely employed for breast reconstuction, head and neck reconstruction, and extremity reconstruction in these days. The authors present their successful experience with free rectus abdominis muscle and rectus abdominis myocutaneous flaps for upper and low extremity reconstruction. From Nov. 94, to May 95, Five cases of severely injured extremites due to trauma or contact burn were treated with free rectus abdominis muscle flap or free rectus abdominis myocutaneous flap. All flaps except 1 case were survived without severe complications. As free muscle or myocutaneous flap, the free rectus abdominis flap has the advantages of a reliable pedicle, easy dissection, and an acceptable donor site, so it seems logical to apply the free rectus abdominis flap to apply in upper and lower extremity reconstruction.

여성의 생활양식 변화를 통한 골다공증 예방 교육프로그램의 개발 및 효과 (Development of the Education Program and It Effect on Osteoporosis and Life Style among Women)

  • The purpose of this study was to develop the educational program based on the self-efficacy theory of Bandura(1986) and to identify the effect of the program among women. For this purpose a non-equivalent control group, and a pretest- posttest design was used between the experimental and the control group. The subjects in this study were female and were over the age 40, 37 in the experimental group and 46 in the control group. In this study, the educational program was developed to increase the level of osteoporosis self efficacy and to prevent osteoporosis. The program consisted of watching, videotapes, telephone contact, lectures, and small group discussions. This study was conducted to determine whether the 6 month educational program would increase osteoporosis self- efficacy, thus modifying life styles related to osteoporosis increas BMD. The instruments utilized in this study were the Lifestyle Questionnaire, and the Osteoporosis Self-Efficacy Scale. Also, bone marrow density (BMD) on the left wrist was measured by DTX-200. The findings are as follows: 1. A significant decrease in BMD was observed in the control group. By contrast, no significant change in BMD was observed in the experimental group. 2. The Osteoporosis Self-Efficacy was not significantly changed in both the experimental and control groups. 3. In the experimental group, the number of exercise participants and their exercise times were significantly increased. Also the amount of caffeine intake was significantly decreased.

  • When we establish treatment planning of facial asymmetry, we must predict each asymmetrical element that will be changed upon coronal, axial, sagittal plane. At the visual point, prediction of the change of coronal plane is most important. It is important difference between Rt. and Lt. mandibular angle belonging to posterior coronal plane, as well as anterior coronal plane, such as upper and lower incisor, or midline of chin point. Several methods for control bulk of mandibular angle are additional angle shaving after osteotomy, grinding contact area between proximal and distal segment for decrease the volume, or bone graft for increase the volume. But, at the point of bimaxillary surgery, transverse position of posterior maxilla is an important factor for control it. So, we would report transverse movement of posterior maxilla for decrease asymmetry on the posterior coronal plane of face, that is, asymmetry of mandibular angular portion.

Compatibility of Diazepam with Polypropylene Multilayer Infusion Container

  • Techflex$^{(R)}$, a polypropylene-lined, multilayer infusion bag, was studied for its compatibility with diazepam, in comparison to the conventional infusion bag, Safeflex$^{(R)}$, which is comprised of poly(vinyl chloride) (PVC). Diazepam was diluted in 0.9% sodium chloride isotonic solution and stored in the infusion bags for 24 h. To evaluate the sorption of diazepam into the infusion bags during storage, the concentration of the drug remaining in the bag was measured using gas chromatography-mass spectroscopy. The PVC bags exhibited a marked sorption of diazepam, with a drug loss reaching up to 90% of the initial concentration after 24 h of contact, whereas Techflex$^{(R)}$ inhibited the drug sorption, showing approximately 10%, under the same conditions. The differences in the sorption behaviors of the bags are discussed in terms of solubility parameters and crystallinities of the polymers.

요추부 다공성 추간체유합보형재의 개발 및 기계적 성능 평가 (Development and Mechanical Performance Evaluation of Lumbar Porous Interbody Fusion Cage)

  • Recently, porous additive manufactured(AM) cages have been introduced to provide more desirable stiffness and may be beneficial to bone ingrowth. They are designed to attempt to reduce the subsidence problem of traditional titanium cage and to get osseointegrative property that PEEK doesn't have. This study was performed to evaluate the mechanical performance of newly developed lumbar porous AM cages. Three types of mechanical tests were performed in accordance with the ASTM standards: Static compression, compression-shear, and subsidence tests. The porous AM cages with 60% porosity showed similar device stiffness and strength as the various products submitted to FDA 510(k), and their wider contact area improved the subsidence test results by about 50%. In conclusion, the porous AM cages developed in this study were considered mechanically safe and could be an alternative to solid PEEK cages.

Histologic analysis of resorbable blasting media surface implants retrieved from humans: a report of two cases

  • The purpose of this study is to evaluate the degree of osseointegration of resorbable blasting media (RBM) surface implants retrieved from humans. Three implants in the mandibular molar region that were surface-treated with RBM were retrieved from two patients. The implants were used to manufacture specimens in order to measure the bone-implant contact (BIC) ratio. The BIC ratios of the three implants were found to be an average of $69.0%{\pm}9.1%$. In conclusion, that RBM surface implants are integrated into the host environment with histological significance and the BIC ratio of the RBM surface-treated implant was not significantly different from that of other surface-treated implants.
