Purpose: To perform a comparative analysis of the palatal bone thickness in Thai patients exhibiting class I malocclusion according to whether they exhibited a normal or open vertical skeletal configuration using cone-beam computed tomography (CBCT). Materials and Methods: Thirty CBCT images of Thai orthodontic patients (15-30 years of age) exhibiting class I malocclusion with a normal or open vertical skeletal configuration were selected. Palatal bone thickness was measured in a 3.0-mm grid pattern on both the right and left sides. The palatal bone thickness of the normal-bite and open-bite groups was compared using the independent t-test. The level of significance was established at P<.05. Results: The palatal bone thickness in the normal-bite group ranged from $2.2{\pm}1.0mm$ to $12.6{\pm}4.1mm$. The palatal bone thickness in the open-bite group ranged from $1.9{\pm}1.1mm$ to $13.2{\pm}2.3mm$. The palatal bone thickness was lower at almost all sites in patients with open bite than in those with normal bite. Significant differences were found at almost all anteroposterior sites along the 3 most medial sections (3.0, 6.0, and 9.0 mm lateral to the midsagittal plane)(P<.05). Conclusion: Class I malocclusion with open vertical skeletal configuration may affect palatal bone thickness, so the placement of temporary anchorage devices or miniscrew implants in the palatal area in such patients should be performed with caution.
Purpose: This study aimed to introduce a novel method to evaluate the alveolar bone and interdental septum in the anterior mandible using cone-beam computed tomography (CBCT). Materials and Methods: Fifty-six CBCT scans from adult patients were selected. The CBCT scans were obtained before and after orthodontic treatment. The following measurements were taken: width of the alveolar bone and the interdental septum, height of the interdental septum, height of the bone plates, distance between the cementoenamel junction and marginal bone crests, and vertical positioning of the mandibular incisor, using the lingual plane as a reference. To test the reproducibility and the stability of the lingual plane, a triangle was traced in the anterior mandible. The intra-class correlation coefficient(ICC) was used to determine intra- and inter-examiner agreement. The paired Student t-test was used to evaluate the area of the triangle and the reproducibility of all measurements. Results: The ICC was excellent for the alveolar bone and dental measurements (0.9989 and 0.9977, respectively), as well as for the interdental septum (0.9987 and 0.9961, respectively). The area of the triangles showed stability in the lingual plane (P>0.05). For the alveolar bone, mandibular incisor, and interdental septum measurements, no statistically significant differences were found between the 2 examiners(P>0.05), confirming the technical reliability of the measurements. Conclusion: The method used in this study provides a valid and reproducible assessment of alveolar bone dimensions in the anterior mandible measured on CBCT images.
Purpose: This study investigated whether the relationship between the maxillary sinus and the root of the maxillary premolar is correlated with the root position and whether there is a difference in the long axis angle of premolars and the buccal bone thickness according to the sinus-root relationship and root position. Materials and Methods: Cone-beam computed tomographic images of 587 maxillary first premolars and 580 second premolars from 303 patients were retrospectively reviewed. The maxillary sinus floor-root relationship was classified into 4 types, and the root position in the alveolar bone was evaluated as buccal, middle, or palatal. The long axis angle of the maxillary premolars in the alveolar bone and the buccal bone thickness were measured. The correlation between these parameters was analyzed. Results: The maxillary sinus floor-root relationship showed a statistically significant correlation with the root position in the alveolar bone. Most maxillary first premolars were buccally located, and more than half of the second premolars had their roots in the middle. The long axis angle of the premolars was significantly larger in buccal-positioned teeth than in middle-positioned teeth, and the buccal bone was thinner. Conclusion: When the root of the maxillary premolar was separated from the sinus floor, the premolar was often located on the buccal side. Most of the maxillary first premolars had a thinner buccal bone and larger inclination than the second premolars. It is recommended to evaluate the root position, sagittal angle and buccal bone thickness using CBCT for implant treatment planning.
2007년부터 2011년까지 부산대학교 병원 구강내과를 내원한 측두하악장애 환자 중 임상 검사 및 방사선 검사, cone beam computed tomography(CBCT) 영상 촬영 결과 측두하악장애 골관절염으로 진단된 환자 114명 228개의 관절을 대상으로, 치료 후 6~35.5개월(평균 10개월)후 CBCT를 재촬영하고, 임상 검사를 실시하였다. 구강내과 전문의와 구강악안면방사선과 전문의가 영상 진단을 하였고, 임상 검사 결과와 CBCT의 영상 결과를 추적 연구 비교하여 다음과 같은 결과를 얻었다. 1. 측두하악관절 관절염의 골변화 양상은 다음과 같다. 1) 관절염의 골변화 진행 양상은 대부분 초기 상태를 유지하였다. 2) 침식은 침식, 편평화, 정상 관절, 골경화, 골증식체 순으로 골변화 진행 양상이 나타났다. 3) 편평화는 편평화, 골증식체, 정상 관절 및 골경화 순으로 진행 양상이 나타났다. 4) 골증식체는 골증식체, 침식, 골경화, 편평화 순으로 진행 양상이 나타났다. 5) 골경화는 골경화, 골증식체, 침식, 정상 관절 순으로 진행 양상이 나타났다. 2. 골변화에 따른 임상 증상의 변화는 다음과 같다. 1) 침식은 침식으로 유지된 경우 통증, 관절음, 개구제한, 최대 편이 개구량 모두 증상 개선이 있었고, 편평화로 이행된 경우 통증, 개구제한, 최대 편이 개구량에서, 정상 관절면으로 이행된 경우 통증, 관절음, 개구제한에서 증상 개선이 있었다. 침식에서 침식으로 유지된 경우 보다 침식에서 편평화로 이행된 경우 최대 편이 개구량에 있어 개선 효과가 높았다. 2) 편평화는 편평화로 유지된 경우 통증, 관절음, 개구제한에서 증상 개선을 보여 주었고, 편평화가 골경화로 이행된 경우, 개구제한에 있어 유의성있는 증상 개선을 보여주었다. 3) 골증식체가 골증식체로 유지된 경우 통증, 개구제한, 최대 편이 개구량에서 증상 개선 효과가 있었다.
본 연구는 수평적, 수직적 안면 골격 유형에 따른 하악 전치부 치조골의 형태학적 차이를 알아보기 위하여 시행 하였다. 연구 재료 및 방법: 40명의 Cone-beam computed tomography (Cone-beam CT)를 선별하여, 4개 군으로 분류하였다. Cone-beam CT 자료를 이용하여 하악 전치부 치조골의 부피($mm^3$), 하악 4절치 치축 기준 시상단면의 단면적(총 단면적, 해면골 단면적: $mm^2$), 백악법랑경계(cemento-enamel junction: CEJ) 2 mm, 3 mm 아래 순, 설측 치조골 두께를 측정하였다. 통계분석은 GLM, Kruskal-Wallis test and Tukey HSD를 사용하였다. 결과: 측절치의 백악법랑경계 2 mm, 3 mm 하방 설측 치조골 두께가, Class I low angle군이 나머지 3군 보다 두꺼웠다 (P < 0.05). 하악 전치 치조골의 부피, 전체 치조골 및 해면골의 단면에서의 통계적으로 유의한 차이는 없었다. 결론: Class I low angle군은 Class II high angle군에 비해 하악 전치 치조골의 설측 부위가 더 두껍다.

Objective: This study aimed to propose clinical guidelines for placing miniscrew implants using the results obtained from 3-dimensional analysis of maxillary anterior interdental alveolar bone by cone-beam computed tomography (CBCT). Methods: By using CBCT data from 52 adult patients (17 men and 35 women; mean age, 27.9 years), alveolar bone were measured in 3 regions: between the maxillary central incisors (U1-U1), between the maxillary central incisor and maxillary lateral incisor (U1-U2), and between the maxillary lateral incisor and the canine (U2-U3). Cortical bone thickness, labio-palatal thickness, and interdental root distance were measured at 4 mm, 6 mm, and 8 mm apical to the interdental cementoenamel junction (ICEJ). Results: The cortical bone thickness significantly increased from the U1-U1 region to the U2-U3 region (p < 0.05). The labio-palatal thickness was significantly less in the U1-U1 region (p < 0.05), and the interdental root distance was significantly less in the U1-U2 region (p < 0.05). Conclusions: The results of this study suggest that the interdental root regions U2-U3 and U1-U1 are the best sites for placing miniscrew implants into maxillary anterior alveolar bone.

Purpose: This study was performed to determine the buccal alveolar bone thickness following rapid maxillary expansion (RME) using cone-beam computed tomography (CBCT). Materials and Methods: Twenty-four individuals (15 females, 9 males; 13.9 years) that underwent RME therapy were included. Each patient had CBCT images available before (T1), after (T2), and 2 to 3 years after (T3) maxillary expansion therapy. Coronal multiplanar reconstruction images were used to measure the linear transverse dimensions, inclinations of teeth, and thickness of the buccal alveolar bone. One-way ANOVA analysis was used to compare the changes between the three times of imaging. Pairwise comparisons were made with the Bonferroni method. The level of significance was established at p<0.05. Results: The mean changes between the points in time yielded significant differences for both molar and premolar transverse measurements between T1 and T2 (p<0.05) and between T1 and T3 (p<0.05). When evaluating the effect of maxillary expansion on the amount of buccal alveolar bone, a decrease between T1 and T2 and an increase between T2 and T3 were found in the buccal bone thickness of both the maxillary first premolars and maxillary first molars. However, these changes were not significant. Similar changes were observed for the angular measurements. Conclusion: RME resulted in non-significant reduction of buccal bone between T1 and T2. These changes were reversible in the long-term with no evident deleterious effects on the alveolar buccal bone.

Objectives: The purpose of this study was to evaluate the proximity of the mandibular molar apex to the buccal bone surface in order to provide anatomic information for apical surgery. Materials and Methods: Cone-beam computed tomography (CBCT) images of 127 mandibular first molars and 153 mandibular second molars were analyzed from 160 patients' records. The distance was measured from the buccal bone surface to the root apex and the apical 3.0 mm on the cross-sectional view of CBCT. Results: The second molar apex and apical 3 mm were located significantly deeper relative to the buccal bone surface compared with the first molar (p < 0.01). For the mandibular second molars, the distance from the buccal bone surface to the root apex was significantly shorter in patients over 70 years of age (p < 0.05). Furthermore, this distance was significantly shorter when the first molar was missing compared to nonmissing cases (p < 0.05). For the mandibular first molars, the distance to the distal root apex of one distal-rooted tooth was significantly greater than the distance to the disto-buccal root apex (p < 0.01). In mandibular second molar, the distance to the apex of C-shaped roots was significantly greater than the distance to the mesial root apex of non-C-shaped roots (p < 0.01). Conclusions: For apical surgery in mandibular molars, the distance from the buccal bone surface to the apex and apical 3 mm is significantly affected by the location, patient age, an adjacent missing anterior tooth, and root configuration.

Objective: To assess the labial and lingual alveolar bone thickness in adults with maxillary central incisors of different inclination by cone-beam computed tomography (CBCT). Methods: Ninety maxillary central incisors from 45 patients were divided into three groups based on the maxillary central incisors to palatal plane angle; lingual-inclined, normal, and labial-inclined. Reformatted CBCT images were used to measure the labial and lingual alveolar bone thickness (ABT) at intervals corresponding to every 1/10 of the root length. The sum of labial ABT and lingual ABT at the level of the root apex was used to calculate the total ABT (TABT). The number of teeth exhibiting alveolar fenestration and dehiscence in each group was also tallied. One-way analysis of variance and Tukey's honestly significant difference test were applied for statistical analysis. Results: The labial ABT and TABT values at the root apex in the lingual-inclined group were significantly lower than in the other groups (p < 0.05). Lingual and labial ABT values were very low at the cervical level in the lingual-inclined and normal groups. There was a higher prevalence of alveolar fenestration in the lingual-inclined group. Conclusions: Lingual-inclined maxillary central incisors have less bone support at the level of the root apex and a greater frequency of alveolar bone defects than normal maxillary central incisors. The bone plate at the marginal level is also very thin.
Objective: Rapid maxillary expansion (RME) is used to expand the maxilla and increase arch perimeter; yet, there are few reports on its effects on the sphenoid bone. With cone-beam computed topography (CBCT), it is possible to visualize sphenoid bone changes. The purpose of this study was to investigate sphenoid bone changes observed in conjunction with RME treatments, using CBCT. Methods: Sixty patients (34 women and 26 men, aged 11-17 years) underwent RME as part of their orthodontic treatment. Patients were randomly assigned to one of three groups: a tooth-anchored group, a bone-anchored group, or a control group. Initial CBCT scans were performed preceding the RME treatment ($T_1$) and again directly after the completion of expansion ($T_2$). Statistical analysis included ANOVA, descriptive statistics, and the intraclass correlation coefficient (ICC). Results: The reliability of the landmark location was at least 0.783, and the largest ICC mean measurement error was 2.32 mm. With regard to distances, the largest change was 0.78 mm, which was not statistically significant (p > 0.05). Statistical significance was established in patient groups of the same sex and treatment type for the following distance measurements: right anterior lateral pterygoid plate to the right edge of the hypophyseal fossa ($d_2$), anterior distance between the medial pterygoid plates ($d_4$), and anterior distance between the left medial and lateral plates ($d_8$). Conclusions: In this study, there were no clinically significant changes in the sphenoid bone due to RME treatments regardless of sex or treatment type.
