전신 뼈 스캔은 핵의학에서 가장 많은 비중을 차지하는 검사 중 하나이다. 기본적으로 전면상과 후면상을 동시에 획득하는데, 때때로 전면상과 후면상만으로는 병소를 분간하기 어려울 때가 있다. 이러한 경우 SPECT/CT나 추가 정적 영상을 통한 병소의 정확한 위치 확인이 중요하며, 추가 촬영 여부에 대한 최초 판단은 주로 방사선사에 의해 이루어진다. 이에 본원에서는 방사선사의 업무 능력 함양을 위한 다양한 개선활동이 이루어져 왔고, 본 연구에서는 방사선사의 교육 및 표준화된 업무 프로세스 적용이 전신 뼈 스캔 오류 감소에 어떠한 영향을 미치는지 확인하고자 한다. 새로운 프로세스 적용을 위해 몇 가지 시스템이 순차적으로 도입되었다. 첫 번째는 의국 교육 및 테스트의 시행, 두 번째는 추가 촬영이 예상되는 환자를 분류하여 촬영 전에 방사선사가 미리 확인할 수 있는 사전 여과 시스템 도입, 마지막으로 판독의가 방사선사에게 직접 촬영 영상에 대해 피드백 할 수 있는 NMQA라는 커뮤니케이션 시스템을 적용하였다. 결과 확인을 위해 2014년 1월부터 2016년 12월까지 서울아산병원 핵의학과를 내원한 전신 뼈 스캔 환자를 대상으로 조사하였다. 조사 기간 동안 전체 전신 뼈 스캔 대비 NMQA가 전송된 검사 건수를 백분율로 산출하였다. 연간 발생량은 2014년 141건, 2015년 88건, 2016년 86건으로 집계되었고 NMQA 발생률은 2014년 0.88%, 2015년 0.53%, 2016년 0.45%로 감소하였다. 새로운 프로세스가 적용된 2014년 이후 NMQA 발생률이 감소하는 경향을 보였다. 다만 통계적으로 유용성을 확인하기까지는 데이터가 부족하여 향후에도 지속적인 데이터 축적이 필요할 것으로 생각한다. 본 연구를 통해 전신 뼈 스캔 영상 질 향상을 위해 표준화된 업무와 교육의 필요성을 확인하였고 향후에도 지속적인 연구와 관심으로 업데이트가 필요하다고 사료된다.
Image fusion is fast catching attention as Wagner pointed out in his 2006 version of the recent progress and development presented at the annual meeting of Society of Nuclear Medicine. Prototypical fusion of bone scan and radiograph was already attempted at in 1961 when Fleming et al. published an article on strontium-85 bone scan. They simply superimposed dot scan on radiograph enabling simultaneous assessment of altered bone metabolism and local bone anatomy. Indeed the parallel reading of images of bone scan and radiography, CT, MRI or ultrasonography has been practiced in nuclear medicine long since. It is fortunate that recent development of computer science and technology along with the availability of refined CT and SPECT machines has permitted us to open a new avenue to digitally produce precise fusion image so that they can readily be read, exchanged and disseminated using internet. Ten years ago fusion was performed using Bresstrahlung SPECT/CT and it is now achievable by PET/CT and SPECT/CT software and SPECT/CT hardware. The merit of image fusion is its feasibility of reliable assessment of morphological and metabolic change. It is now applicable not only to stationary organs such as brain and skeleton but also to moving organs such as the heart, lung and stomach. Recently, we could create useful fusion image of cardiac SPECT and 64-channel CT angiograph. The former provided myocardial metabolic profile and the latter vascular narrowing in two patients with coronary artery stenosis and myocardial ischemia. Arterial stenosis was severe in Case 1 and mild in Case 2.
Purpose: A missed fracture is a very common occurrence in the Emergency Department (ED) and can have serious results because of delays in treatment, resulting in long-term disability. It is also one of the most common causes leading to medical legal issues. We analyzed the causes of missed fractures by using a bone scan which is known to be an effective tool for diagnosing bony lesions. Methods: We reviewed the medical records of trauma patients who underwent a bone scan after being discharged the ED from September 2006 to March 2008. Cases of missed fractures were identified by using electronic medical records to review each diagnosis. Definition of missed fracture was read after bone scan by radiologist. We decided that there was no fracture if we read 'trauma-related lesion' or 'cannot rule out fracture' on a bone scan read by a radiologist. Enrolled patients were analyzed by age, sex, time until bone scan and Injury Severity Score (ISS). Patients were divided into two groups, alert mentality and not-alert mentality, so there were split between a diagnosis group and a missed fracture group. ISS was also used in determining the severity of the patient's injury upon discharge from the ED. Results: A total of 532 patients were enrolled in this study. Of those, 487 patients were in the diagnosis group, and 45 patients (8.4%) were discovered to have had a fracture. Of the 45 missed fracture patients, 34 patients (6.4%) had one-site fractures, 8 patients (1.5%) had two-site fractures, and 3 patients (0.6%) had three-site fractures. The most commonly missed fracture was multiple rib fractures (18 patients, 30.5%), followed by lumbosacral (LS) spine fractures (10 patients, 16.9%), thoracic spine fractures (8 patients, 13.6%), and clavicle fractures (6 patients, 10.2%). Mean age was $50.12{\pm}18.54$ years in the diagnosis group and $57.38{\pm}16.88$ years in the missed fracture group. For the diagnosis group, the mean ISS was $9.03{\pm}8.26$, but in the missed fracture group it was $17.53{\pm}9.69$. Missed fractures were much more frequent in the not-alert mentality (p<0.01) and in the high (ISS$ ISS{\geq}16$) group (p<0.01). Conclusion: Missed fractures occur most frequent in patients of old age, not-alert mentality, and high ISS. Multiple rib and spine fractures were found to be the most frequent missed fractures, regardless of trauma severity. This study also shows a high possibility of clavicle and scapula fractures in patients with severe trauma.
We studied four patients with muscle necrosis associated with acute renal failure to evaluate the diagnostic value of the bone scan in this disease. The illness followed carbon monoxide poisoning in two patients, acute physical exertion in one and contaminated intramuscular injection in the other. Whole-body rectilinear bone scans using technetium 99m-methyldiphosphonate were done. In all patients, increased muscle labelling at the regions of suspected muscle injury was showed, and in one, it was after normalization of serum muscle enzyme levels. In one patient, the bone scan was rechecked 8 months later and showed no residual abnormality. Above all, the site and precise extent of muscle injury could be detected and the degree of muscle labelling seemed to correlate with the severy of muscle injury. These findings suggest that isotope scanning may be useful in the diagnosis of patients with acute muscle necrosis.
Juvenile dermatomyositis is a common inflammatory muscle disease of childhood, characterized by weakness in proximal muscles and specific skin rash. In case of juvenile dermatomyositis without characteristic clinical features, non-invasive imaging tools such as $^{99m}Tc$-HDP three-phase bone scan are very helpful in diagnostic workup of myopathies. We report a case of 13-year old female with juvenile dermatomyositis, in which $^{99m}Tc$-HDP three-phase bone scan was useful in diagnosis and assessing therapy response.
목적 : 골 및 관절질환의 예민한 진단방법으로 이미 잘 알려진 골스캔의 측두하악관절장애의 진단 및 관절원판변위와 관절염의 감별진단에 있어서 임상적 유용성을 알아보고자 하였다. 방법 : 임상적으로 진단이 가능한 측두하악관절원판 변위환자 21예(일측성 13, 양측성 8), 관절염 환자 26예(일측성 23예, 양측성 3예)와 측두하악장애의 병력과 증상이 없는 대조군 39예에서 골스캔을 시행하고, 10만 계수의 두경부의 측면영상에서 측두하악관절부위?l 섭최계수를 구하여 각 측두하악관절의 섭취율과 양측측두하악관절 섭취율의 차이를 구하여, 임상증상과의 일치정도를 확인하고, 환자군에서는 경두개 및 파노라마 방사선 촬영결과와 비교 분석하였다. 결과 : 1) 측두하악관절의 단순섭취율은 정상대조군에서 $1.084{\pm}0.172$였고, 관절염으로 이환된 관절에서 $1.673{\pm}0.606$으로 가장 높았으며, 관절원판변위로 이환된 관절에서는 $1350{\pm}0.351$로 대조군에서 보다 증가되어 있었다. (p<0.01). [대조군의 평균치+2표준편차]인 1.428%이상 증가된 경우는 일측성으로 이환된 관절원판변위 환자에서 21.3%, 일측성관절염 환자에서 69.6%였다. 2) 양측 측두하악관절 단순섭취율의 차이는 대조군에서 $0.062{\pm}0.063$이었고, 일측성 관절염 환자에서 $0.608{\pm}$0.533으로 가장 높았으며, 관절원판변위 환자에서는 대조군과 차이가 없었다.(p<001). [대조군의 평균치+2표준편차]인 0.188이상 증가된 경우는 일측성 관절염환자의 87%, 일측성 관절원판변위 환자의 23%였다. 3) 방사선 촬영결과 골변화가 나타났던 관절과 골 변화가 없었던 관절사이의 섭취율 차이는 없었고, 방사선 촬영결과의 골변화와 골스캔상의 섭취율의 증가가 임상상과 일치한 경우는 일측성 관절원판변위 환자 13예 중 각각 0예와 3예, 일측성 관절염환자 23예 중 12예와 16예였다. 결론 : 측두하악관절의 장애가 유발된 경우 골스캔상 측두하악관절의 섭취가 증가되며, 이는 관절원판변위 환자보다 관절염 환자에서 더욱 현저히 관찰된다. 양측 측두하악관절 섭취율의 차이도 일측성 관절염환자에서 일측성 관절원판변위 환자보다 더욱 현저히 증가되어 보인다. 따라서 골스캔은 특히 일측성으로 이환된 측두하악관절장애의 진단 및 관절원판변위와 관절염의 구별에 도움을 줄 수 있는 유용하고 간편한 진단방법으로 생각된다.
Acute osteomyelitis is usually shown as a localized area of increased activity (hot uptake) in the infectious focus on bone scintigraphy. In our patient, absence of radioactivity (cold lesion) was noted in the distal metaphyseal and diaphyseal regions of his right femur. Initial x-ray was interpreted as normal except for mild soft tissue swelling in the right thigh. The lesion was confirmed as an acute osteomyelitis with subperiosteal abscess on surgery. Staphylococcus aureus was the etiologic organism. We describe a case of acute osteomyelitis in a one-year-old boy shown as a cold lesion on bone scan.
[목적] 뼈 검사는 골격계 질환에 대한 예민도와 높은 해상력의 핵의학 영상 검사이다. 본 연구는 추가적인 방사선피폭없이 뼈 검사 영상과 골반 X선 영상의 결합을 시행하고 영상의 해부학적 정보와 관용도를 증가시켜 환자 병변 진단에 도움이 되고자 한다. 2015년 11월에서 2016년 8월까지 본원에서 뼈 검사를 실시한 환자 중 골반 X선 촬영을 시행한 236명을 대상으로 진행하였다.(남자 64명, 여자 172명, 평균연령 $50.96{\pm}15.39$세) 검사장비는 감마카메라 Symbia Ecam(SIEMENS, Germany)을 사용하였고 디지털 엑스레이는 DRS-800(Listem, Korea)을 사용하였다. 영상의 결합 및 분석프로그램은 Osirix version 3.8.1(Osirix, USA), 통계프로그램은 Stata/SE 버전 14.0(StataCorp, USA)를 사용하였다. 환자에게 $^{99m}Tc-DPD$ 740 MBq을 정맥주사한 후 2~4시간이 지난 뒤에 검사를 진행하였다. 감마카메라 영상 획득 조건은 Matrix size $256{\times}1024$, Zoom 1.00, 검사 속도 17 cm/min로 하였다. 디지털 엑스레이는 Collimator size $14^{{\prime}{\prime}}{\times}17^{{\prime}{\prime}}$, 77 kVp(60~97kVp), 평균 30 mAs(20~48)로 전상장골극과 치골결합 사이 수직입사를 하였다. Osirix 상에서 골반 뼈의 윤곽 및 가상의점을 선택하여 결합하고 획득한 영상을 핵의학과 5년 이상 근무한 방사선사 3명에 의해 평가 되었으며, 모두 동일할 경우 일치, 하나라도 동일하지 않을 경우 불일치로 평가하였다. 전체 236명 중 일치하는 대상은 216명 (91.53%) 이다. 연령의 중위수 및 범위(최소값~최대값)는 일치하지 않는 그룹에서 67(46~81)세, 일치하는 그룹에서 52(22~87)세로, 일치하지 않는 그룹의 연령이 더 높은 것을 확인하였고, 두 군 간에 연령이 다른지 Wilcoxon rank-sum test를 수행해 본 결과, 통계적으로 유의하게 두 군 간에 나이가 차이나는 것을 확인할 수 있었다(p < 0.0001). 남자는 64명 중 60명(93.75%), 여자는 172명 중 156명(90.70%)가 일치하였고, 골반 병변이 없던 환자는 54명 중 54명(100.00%), 골반 병변이 있던 환자는 182명 중 162명(89.01%) 일치하였고, 골반 병변 여부에 따른 일치율은 통계적으로 유의한 차이를 보였다(p = 0.0094). 뼈 검사와 골반 X선 영상을 결합하는데 있어서 여러 가지 변수가 있을 수 있었고 환자의 연령과 골반병변이 영상의 결합에 일부 영향을 줄 수 있을 것으로 생각된다. 본 연구를 통하여 방사선 피폭 없이 소아의 골반 골육종 진단에 도움이 되리라 사료되고 이와 같은 영상의 결합 연구를 통해서 핵의학 영상발전에 도움이 될 것으로 기대된다.
골육종에서 드물게 발생하는 도약전이 병소는 불량한 예후 인자로서 중요한 연관성이 있기 때문에 골육종 환자의 광범위 절제술 시에 절제범위에 포함되어야 한다. 따라서 수술 전 검사에서 골육종의 골수 내 침범에 대한 정밀한 평가가 필수적이며, 골스캔 검사는 도약전이의 발견에 있어서는 높은 위음성률을 보일수 있으며 자기공명영상 검사는 종양이 위치한 골 전체를 촬영하지 않는 경우 임상적인 증거가 없으면 간과하기가 쉽다는 단점이 있다. 이에 저자들은 주 종양 부위만을 포함한 국소 자기공명영상 촬영과 골스캔에서 발견되지 않았으나 FDG-PET/CT에서 도약 전이가 진단된 대퇴 원위부의 골육종 증례에 대해 문헌 고찰과 함께 보고하고자 한다.
