Neurological Effects of Bojungikki-tang and Bojungikki-tang-gamibang on Focal Cerebral Ischemia of the MCAO Rats

  • Objectives: This study demonstrates the neurological effects of Bojungikki-tang and Bojungikki-tang-gamibang on the focal cerebral ischemia of rats with ischemic damage caused by middle cerebral artery occlusion (MCAO). Methods: Rats were treated with Bojungikki-tang and Bojungikki-tang-gamibang extracts for about five days after MCAO, and the size and volume of cerebral infarction and the ratio of cerebral edema were observed. From the immunohistochemical view, significant changes of outbreak of Bax, Bcl-2, c-Fos, HSP72, and iNOS were observed in the brain tissues. Results: Bojungikki-tang repressed only brain edema and iNOS revelation led by focal cerebral ischemia, when considering significance. In contrast, Bojungikki-tang-gamibang demonstrated significant suppression of cerebral infarction, brain edema, Bax, c-Fos, HSP72, and iNOS induced by focal cerebral ischemia. Conclusions: Bojungikki-tang is considered functional treatment for cerebral ischemic damage; it can be effective to relieve secondary brain edema and immune response. Bojungikki-tang-gamibang can have a direct function to alleviate brain infarct and to control the natural death of nerve cells which cerebral ischemic damage brings about.

A Clinical Study of the Patients in Stroke Treated with Bojungikki-tang and Bojungikki-tang-gamibang (보중익기탕(補中益氣湯) 및 그 가미방(加味方)을 투여한 중풍환자에 대한 임상적 고찰)

  • Object: This study was designed to investigate the clinical and practical characteristics of stroke patients treated with the age-old herbal medicines, Bojungikki-tang and Bojungikki-tang-gamibang. Methods: The 80 patients studied were all diagnosed with stroke by neurological examination, B-CT and B-MRI scan. They were treated with Bojungikki-tang and Bojungikki-tang-gamibang over 5 days and symptoms were observed. There were inpatients and outpatients at the Oriental Medical hospital of Dong-eui University from January in 2000 to June in 2003. All patients were interviewed and medical charts examined. Result and Conclusion: The results were found: 1.Many were in their sixties. Average age was 64.54. 2.Cerebral infarction was diagnosed in 86.25% of all cases. 3.30% of stroke cases occurred in spring. 4.Average blood pressure was 129/84mmHg on the fifth day of treatment with Bojungikki-tang and Bojungikki-tang-gamibang and 135/86mmHg on the first day. 5.The white color were best in facial color about 85.29%. 6.There were many who returned for treatment within 6 to 10 days. 7.Bojungikki-tang and Bojungikki-tang-gamibang were used Rt. hemiplegia more than for Lt. hemiplegia.

The Current Trend of Research about Bojungikki-tang (보중익기탕의 최근 연구동향)

  • Objective : The purpose of this study was to provide the information about Bojungikki-tang by domestic papers and theses. Method : Domestic papers related to Bojungikki-tang were reviewed and analyzed. These papers were classified by year, research method, experimental method, and subject. Result : The following results were obtained in this study. 1. The study of Bojungikki-tang started from 1984 and continued steadily. 2. The domestic papers about Bojungikki-tang were 47 volumes. Among them, there were 26 volumes since 2000. 3. Classified by research method from the year 2000, 20 experimental studies, 4 clinical experiments, and 2 documentary researches. 4. Classified by experimental method from the year 2000, 10 studies used mouse in vivo experiment and 4 studies used rat in vivo experiment and 6 studies used each cell in vitro experiment. 5. Classified by subject from the year 2000, papers related to immune enhancing effect, nerve and mental system, radiation protection effect, anti-allergy, anti-inflammatory, anti-cancer, remedy of diabetes, skin protection and so on. Conclusion : Many papers of experimental studies, clinical experiments and documentary researches related Bojungikki-tang are included in domestic journals. Bojungikki-tang is being researched variously in immune enhancing effect, nerve and mental system, radiation protection effect, anti-allergy, anti-inflammatory, anti-cancer, remedy of diabetes, skin protection and so on. However, more clinical studies on Bojungikki-tang are needed.

Case Study of the Patient Considering Chronic Renal Failure Treated with Bojungikki-tang and Bojungikki-tang gamibang (만성신부전으로 의심되는 환자의 보중익기탕(補中益氣湯) 및 가미방(加味方) 투여 호전례)

  • Objectives : The purpose of this study is to report the clinical effect of herbal medicine on the patient considering chronic renal failure. The patient complained of hemiparesis, general weakness, anorexia, nausea and dizziness. Methods : According to the traditional Korean medicine syndrome differentiation, the patient was classified as Deficiency of Spleen Qi and prescribed Bojungikki-tang and Bojungikki-tang gamibang as well as acupuncture and moxibustion treatment. Changes of BUN, creatinine, VAS for genaral weakness, nausea, dizziness were compared before and after treatment for 2 weeks. Results : After treatment, the level of BUN and creatinine was decreased and main symptoms were improved. Conclusions : Herbal medicine Bojungikki-tang and Bojungikki-tang gamibang with acupuncture and moxibustion treatment would be efficient to the patient of chronic renal failure.

A Case Report of Chemotherapy-induced Alopecia Treated with Bojungikki-tang (보중익기탕 투여 후 화학항암제로 인한 전신쇠약감 및 탈모호전 증례보고)

  • This case study describes the effect of Bojungikki-tang on chemotherapy-induced alopecia. Alopecia is a well-documented cause of distress to patients undergoing cancer treatment, but no approved pharmacological treatment exists for chemotherapy-induced hair loss. A 70-year-old female diagnosed with a cholangiocarcinoma and liver metastasis received chemotherapy, including gemcitabine and cisplatin, every three weeks. As a result of the continuous chemotherapy, she suffered various toxicity-related side effects, including bone marrow suppression, general weakness, nausea, peripheral numbness, and hair loss. Bojungikki-tang was initially administered to improve the patient's general weakness and fatigue. After three months of treatment, the patient's hair loss and general condition improved, and the color of the new hair was dark, despite the chemotherapy. The treatment did not improve other symptoms, such as bone marrow suppression and peripheral numbness. This case suggests that Bojungikki-tang could have a beneficial effect on chemotherapy-induced alopecia.

The latest trend of research and clinical usage about Bojungikki-Tang in Japan -searching medical science articles published from 2003 to 2008- (일본에서 보중익기탕(補中益氣湯)에 대한 최신 연구와 임상 응용 동향 -2003년에서 2008년까지 학술논문 분석-)

  • Objective : The aim of this study was to analyse the latest trend of medical science articles about Bojungikki-Tang published in Japan. Methods : We searched medical science articles published from 2003 to 2008 in Japan on the web site, CiNii and then these articles were reviewed and classified according to experimental subject and method. Results : Fifty-two articles, related to Bojungikki-Tang were searched and these were classified into three classes, pharmacological signaling studies, clinical case studies and the others. As a result of classification, clinical case studies were most abundant by 38 cases, pharmacological signaling studies were 10 and the others were 4. This result suggested that the study of Bojungikki-Tang was mainly focused on clinical study rather than experimental study. Among the clinical case studies, clinical studies related to immune disease took the most greatest number and number of other clinical studies were evenly distributed over the various disease. Immune intensification effect of Bojungikki-Tang can be thought continuous with reinforce the middle Jiao Qi in company with improvement effect on growth hormone failure and male infertility. A fact that the one prescription, Bojungikki-Tang could be used to treat various diseases reflects a viewpoint of Oriental medicine that is based on the respect for and understanding of individual constitution and disposition while paying attention to the gender difference and individuality. The clinical usefulness of Bojungikki-Tang on obstructive sleep apnea syndrome and visceroptosis concerns with ascend Yang and treat Qi sinking. Pharmacological signaling studies were mainly focused on diseases that were treated Bojungikki-Tang in clinic, however, there was one study that experimented on changes of numerous gene expression according to the treatment of Bojungikki-Tang using DNA array. Conclusion : The therapeutic system of Oriental medicine is different in various aspects form that of Western medicine and appropriate harmonization of east and west is expected to improve the quality of medical care. In this regard, continuous investigation and analysis about herbal medical science papers in Japan can also be useful to make progress in Korean traditional medicine.

Two Cases of Improved Dizziness and Ataxia due to Pontine Stroke in Bojungikki-tang-gamibang (보중익기탕가미방(補中益氣湯加味方) 투여를 통한 뇌교병변 현훈과 보행실조 환자 치험 2례)

  • Bojungikki-tang-gamibang is a widely used herbal prescription in traditional medicine in Korea. The aim of this study is to investigate the effectiveness of Bojungikki-tang-gamibang for patients who have pontine stroke. They have problems with type of Qi deficiency(氣虛) in the rehabilitation stage. The type of Qi deficiency caused the dizziness and ataxia for the patients. In this study there are two types of patients. The first case is a patient with dizziness and ataxia because of potine hemorrhage. The second case is a patient who has the same symptoms as the first one but caused by pontine infraction. We treated the two patients with Bojungikki-tang-gamibang. The Improvement of symptom was evaluated by Korean vestibular disorders activities of daily living scale(K-VADL). As a result the degree of dizziness decreased noticeably while other symptoms improved as well. This study suggests that Bojungikki-tang-gamibang may be an effective treatment for patients who suffer stroke with type of Qi deficiency(氣虛) in the rehabilitations stage. However, more cases and methods of diagnosis are required to prove that the oriental medicine, Bojungikki-tang-gamibang can apply to patients universally.

Effects of the Bojungikki-tang gami on Headache (보중익기탕(補中益氣湯) 가미방(加味方)의 두통 치료 효과에 관한 연구)

  • This study was carried out to evaluate the effects of the Bojungikki-tang gami on patients with headache. This clinical study has been carried out with 32 patients, who visited the Department of Internal medicine, Donng-eui oriental medical center from January 2005 to September 2007, had been treated for headache with herbal medicine(Bojungikki-tang gami) and acupuncture. This study has been carried out by chart-review or telephone research. After treatment with Bojungikki-tang gami, 32 patients's headache was improved. (87.5%) After treatment with Bojungikki-tang gami, patients's other symptoms- fatigue, mild fever and feeling tight, anorexia, sweating, stiffness and numbness, dizziness -was improved. The treatment of headache with Bojungikki-tang gami is effective.

  • The following conclusions are produced by the consideration of Bojungikki-tang(補中益氣湯) from the view point of Hyungsang medicine. A review on the explanation and provisions of Bojungikki-tang stipulated in 'Donguibogam(東醫寶鑑)' results in the idea that the main purpose of Dongwon(東垣) and various application in clinic by Jeesan(芝山) cannot be understood without the deep comprehension of ki(especially Jungki and Wonki) and the fire. Jungki(中氣, the middle warmer ki) is the ki of stomach. Wonki(元氣, the original ki) is a combination of innate Jung(先天之精, Ki stored between the two kidneys) and acquired Jung(後天之精, Ki of the stomach) and the sources of five Jang and six Bu, twelve meridians and triple warmer. Yin fire is referred to the ministerial fire of spleen, liver and kidney. Excess or lack of the ministerial fire can cause a disease and especially the excess of it is harmful to original ki. Deficiency of original ki is thought to be a cause of allergic diseases. Damages on the original ki by the abdominal surgery lead to a disease. Pale complexion, long face and the long limbs are considered as the Hyungsang of deficiency of Jung ki by Jeesan. Clinical cases of Bojungikki-tang in Hyungsang medicine draw the following conclusions ; Bojungikki-tang is usually prescribed to the person who has one of the following Hyungsang : Bangkwang type, pale complexion, long face, short-sight, astigmatism and wrinkles on face. Bojungikki-tang can be applied to the every disease caused by the deficiency of original ki regardless of name and symptoms of disease.

Cases Report about Vascular Dementia Treated with Bojungikki-tang-gamibang (보중익기탕가미방을 투여한 혈관성 치매 환자 증례군 보고)

  • The purpose of this case series was to investigate the effectiveness of Bojungikki-tang-gamibang for vascular dementia. The case subjects were nine patients who were diagnosed as vascular dementia by Mini-Mental State Examination-Korean (MMSE-K) and Hachinski's ischemic score. They were inpatients at the Kumi Oriental Medical Hospitalof Daegu Haany University from February in 2008 to Aprile in 2009. They were treated with Bojungikki-tang-gamibang. Results of before and after therapy were evaluated and scored using Korean-version Dementia Rating Scale (K-DRS). The results were analyzed using statistical methods, such as Wilcoxon signed rank test and Mann-Whitney U test. The study showed that the K-DRS score significantly increased after treatment (P<0.05). In summary, this cases report suggests that Bojungikki-tang-gamibang appears to be effective in the treatment of vascular dementia.
