Influence of Midazolam and Glycopyrrolate on Intra-operative Body Temperature in Abdominal Surgical Patients

  • Purpose: influence of benzodiazepine (midazolam)or cholinergic inhibitor (atropine or glycopyrrlate) on intra-operative body temperature remains unclear and controversial. This study compares intra-operative body temperature in 50 abdominal surgical patients under general anesthesia between the administration of midazolam and glycopyrrolate in combination, or glycopyrrolate alone. Methods: Patients who underwent abdominal surgery were recruited from September 2008 through October 2009 at Gachon University Gil hospital in incheon. Core body temperature was measured in the right ear using a tympanic membrane thermometer at induction of general anesthesia and at 1 hr, 2 hr, and 3 hr after induction. Results: There were no differences in core body temperature at any measurement point between either patient group (F=1.08, $p$=.377). Core body temperature decreased throughout the 3 hr after induction in both groups (F=9.22, $p$ <.001). Specially, core temperatures at induction of general anesthesia (p<.001), 1 hr (p<.001), 2 hr ($p$ <.001), and 3 hr ($p$ <.001) after induction were lower than before administration of midazolam and glycopyrrolate, or glycopyrrolate alone. Conclusion: We conclude that a cholinergic inhibitor (glycopyrrolate, 0.1 mg) therefore seems not to affect intra-operative body temperature of patients given a benzodiazepine (midazolam, 0.04 mg $kg^{-1}$), and not to increase body temperature in patients not given a benzodiazepine during the 3 hr after the induction of general anesthesia. Intra-operative warming therefore is needed to prevent hypothermia in surgical patients who receive pre-operative administration of midazolam and/or glycopyrrolate.

하복의 온열생리학적 기초연구 (The Basic Studies of Thermal Physiology for Summer Wears)

  • In the experiment with a basic material for the design of summer wear that comfort can be obtained in temperature, to get individual differences, clothing styles (slacks, skirt), material differences(T/C, cotton), and the contrast between when naked and dressed, when two healthy females were dressed four kinds of summer wear as an object of experiment under the regular warm temperature environmental condition (24, 28, 32, $36^{\circ}C$, $60\pm10\%$ RH), the measurement of physiological, phychological change was taken and the result goes as follow; 1. Mean skin temperature rose by clothing, body weight loss decreased below $32^{\circ}C$, thermal sensation changed toward low temperature by $2\~3^{\circ}C$. 2. Mean skin temperature, body weight loss, the lowest blood pressure above $32^{\circ}C$, under-clothing temperature, and thermal sensation increased when in slacks to be compared with when in skirt. 3. Ambient temperature had a great effect on mean skin temperature, body weight loss, respiration, clothing surface temperature, under-clothing temperature and humidity, thermal sensation, etc. 4. It was admitted that pulse, thermal sensation, comfort show different individuality. 5. It was recognized that the lowest blood pressure, clothing surface temperature, under. clothing humidity differ according to the kinds of clothing. 6. A comfortable ambient temperature in clothing summer wear was about $27\~28^{\circ}C$.

근적외분광분석법을 이용한 생쥐꼬리에서의 비침습 혈당 정량시 장기간 측정에 따른 변이 요인의 보정 (Compensation of Variation from Long-Term Spectral Measurement for Non-invasive Blood Glucose in Mouse by Near-Infrared Spectroscopy)

  • Non-invasive blood glucose measurement from mouse tail was performed by near-infrared (NIR) spectroscopy. Three groups; normal, type I diabetes (insulin dependent diabetes mellitus, IDDM), type II diabetes (non-insulin dependent diabetes mellitus, NIDDM) group, were studied over a 10 weeks period with the collection of near-infrared (NIR) spectra. Spectral variations from long-term measurement (10 weeks) from dramatic and nonlinear changes in the optical properties of the live tissue sample were compensated by chemometrics techniques such as principle component analysis (PCA) and partial least squares (PLS) regression. The effect from mouse body temperature changes on NIR spectral data was also considered. This study showed that the compensation of variations from long-term measurement and temperature changes improved calibration accuracy of non-invasive blood glucose measurement.

전신마취 복부 수술 후 적극적인 가온요법이 통증, 체온 및 체온불편감에 미치는 효과 (The Effects of Active Warming on Pain, Temperature, and Thermal Discomfort in Postoperative Patients after General Anesthesia for Abdominal Surgery)

  • Purpose : This study investigated the effects of active warming using a Warm Touch warming system or a cotton blanket in postoperative patients after general anesthesia for abdominal surgery. Methods : This quasi-experimental study utilized two experimental groups and one control group: a cotton-blanket group (n = 25) were warmed with a cotton blanket and a sheet; a forced-air warming group (n = 24) were warmed with a Warm Touch warming system, a cotton blanket, and a sheet; and a control group (n = 25) were warmed with a sheet. Measurement variables were postoperative pain, body temperature, and thermal discomfort. Data were analyzed using a one-way ANOVA, ${\chi}^2-tests$, Fisher's exact test, and a repeated measures ANOVA. Results : The effects of active warming using a Warm Touch warming system and a cotton blanket on postoperative patients was significant in reducing pain (F = 13.91, p < .001) and increasing body temperature (F = 12.49, p < .001). Conclusion : Active warming made a significant difference in pain and body temperature changes. Active warming methods may help patients' postoperative recovery and prevent complications. Further research is needed to explore the effects and side effects of active warming on recovering normothermia.

  • Recently, domestic animal disease caused tremendous damage to farmhouses and the damage stretched in nationwide with the spread of epidemic disease. To prevent animal diseases from happening again, the system development to easily measure the temperature of sick animals and identify of them is needed, thereby quickly treat them, reducing losses of farmhouses. However, a lack of related equipment and human resource hampered its effort to minimize its losses. This study tries to develop diagnosis system as part of measures to curb these domestic animal diseases. This paper present the 센서 node based on IEEE 802.15.4 which can be attached to the animal body for real-time temperature measurement. We design and implement tiny chip-type that can be attached to the body of animals. Then, we use available power only when measuring temperatures in a long term-basis. In this paper, the 센서 node was applied to horse's neck. We measure the horse's body temperature between $32.2^{\circ}C{\sim}33.7^{\circ}C$ and analyze phenomenon data for 4 months.

  • Using industrial robots and sensors, we developed an inline car body inspection system which proposes high flexibility and sufficient accuracy. Car Body Inspection(CBI) cell consists of two industrial robots, two corresponding carriages, camera vision system, a process computer with multi-tasking ability and several LDS's. As industrial robots guarantee sufficient repeatabilities, the CBI cell adopts the concept of relative measurement instead of that of absolute measurement. By comparing the actual measured data with reference data, the dimensional errors of the corresponding points can be calculated. The length of the robot arms changes according to ambient temperature and it affects the measuring accuracy. To compensate this error, a robot arm calibration process was realized. By measuring a reference jig, the differential changes of the robot arms due to temperature fluctuation can be calculated and compensated.

부자탕과 초오약침을 병행한 한방복합치료로 호전된 복합부위통증 증후군 제1형 환자의 말초체온 상승 및 통증 경감 효과에 관한 증례 보고 (A Case Report on the Effect of Peripheral Body Temperature Increase and Pain Relief in a Patient with Complex Regional Pain Syndrome Type 1 Improved by Buja-tang and Aconitum ciliare Decaisne Pharmacopuncture)

  • The aim of this study was to investigate the effect of complex Korean medicine treatment especially Buja-tang and Aconitum ciliare decaisne pharmacopuncture on patients with type 1 complex regional pain syndrome caused after a traffic accident. The patients was treated with Buja-tang, Aconitum ciliare decaisne pharmacupuncture, acupuncture and physical therapy during 20 days of hospitalization. Numerical rating scale (NRS), short form McGill pain questionnaire (SF-MPQ), body temperature measurement of the foot, manual muscle test (MMT) were used for assessment. After treatments, NRS of burning pain in the left ankle decreased from 8 to 4-5 and NRS of burning pain in the right ankle decreased from 7 to 4-5. Also, SF-MPQ results, foot temperature measurement and MMT showed improvement during hospitalization. This study shows that complex Korean medicine treatment especially Buja-tang and Aconitum ciliare decaisne pharmacopuncture are effective in alleviating symptoms in patients with type 1 complex regional pain syndrome.

다 채널 직결 온도측정 시스템의 설계 및 제작 (Development of Multichannel On-line Thermometry System)

  • 저자들은 다양한 진료 환경과 연구 수행에 폭 넓게 이용할 수 있는 새로운 온도측정 시스템을 개발하기 위하여 임상에서 필요한 기능과 연구에 요구되는 기능을 가진 간편하고 편리한 온도측정 시스템을 개발하였다. 이 측정 시스템은 일정한 지점의 온도를 정확하고 신속하게 측정하여 기록할 수 있는 장치로서 실용 가능성을 다양하게 검증해 본 결과 기존의 평면적인 열 분포도보다 진보된 등온곡선을 도출할 수 있었으며, 기존의 체온 측정장치보다 더욱 편리하게 진료의 각 분야에 효과적으로 이용할 수 있음을 확인하였다. 본 연구결과에 의한 기대효과 및 향후 활용방안으로는 1) 환자의 체온 변화를 신속하게 진단할 수 있어서 치료효과의 극대화와 부작용의 최소화에 기여할 수 있다. 2) 체온의 변화를 일으키는 다양한 질환의 진단 및 치료성과 판정에 이용할 수 있다. 3) phantom을 이용한 다양한 실험적 연구를 수행할 수 있다. 4) 장기 입원중인 환자들을 대상으로 중앙통제 방식으로 환자들의 상태를 지속적으로 감시할 수 있어서 간호인력의 노동력 절약에 많은 도움을 줄 수 있다. 5) 현재의 기능을 더욱 발전시키면 다양한 질환의 환자들에게 두루 적용할 수 있으며, 환자 감시체제를 위한 필수장비로 이용할 수도 있다.

적외선 촬영으로 측정한 편도질환에서의 통증변화 (The Change of Pain in Tonsillar Disease Estimated by DITI)

  • Backgroud and Objectives: Because the pain is subjective in human, we frequently need the objective tool for estimation of pain. The Visual Analog Scale (VAS) is a method for pain grading, but it is also a subjective method. The purpose of this study is to seek an objective method for measurement of pain. Author suggests that the estimation of local body temperature may be associated with the severity of pain in tonsillar disease and the change of it measured by Digital Infrared Thermographic Imaging (DITI), may also be correlated with the change of pain. Materials and Method : Four groups were selected and measured for VAS and body temperature in DITI. Group A and B were post-tonsillectomy groups divided by age. Group C was acute tonsillitis group. Group D was peritonsillar abscess group. Results : In Group A and B, the subjective VAS was significantly correlated with objective local body temperature (correlation coefficient r=0.673, 0.647) and significant correlation was also present in acute tonsillitis and peritonsillar abscess groups(r=0.596,0.642). Conclusion : The change of pain was strongly associated with that of local body temperature in tonsillar disease. DITI is a useful method for objective grading of pain and can be used for the study of postoperatve pain and effectiveness of pain control.
