  • The word "Sajeung(死證)" in "Huangjenaegyeong(黃帝內經)" includes a warning to lead to death if it is treated wrongly as well as a definite diagnosis saying that it is impossible to care diseases. A disorder condition of the body means that the balance of Eum-yang(陰陽) are broken or O-haeng(五行) doesn't have a good circulation. The prediction to progress is very important as much as decision of whether it is Sajeung or not because it can be changed by the time of day or night and also by changes of the seasons. In addition, according to the relations between Sangsaeng(相生) and Sanggeuk(相克) of O-haeng patients' diseases fall into a dangerous condition at the time under control. But sometimes it can be a severe illness even they are full of vigor. When living and dying has to be determined, it is emphasized the significance of inspection, auscultation and olfaction, inquiring and palpation(望聞問切法). Especially this is the key point to study people's face and pulse.

  • The Temporomandibural joint(TMJ) is one of the most frequently used joint in the body as $1,500{\sim}2,000$ times per day for the activities of chewing, swallowing, talking, yawing and sneezing. The TMJ are formed by condylar process of mandible and mandible fossa of temporal bone, separated by an articular disc. This articular disc divides into two cavities as upper cavity and lower cavity. The gliding movement occurs in the upper cavity of the joint, whereas hinge movement occurs in the lower cavity. The movements that are allowed at the TMJ are opening, closing, protrusion, retraction and lateral movement. A cause of TMJ dysfunction are capsulitis, internal derangement, osteoarthritis, rheumatoid arthritis, infection and inflammation near the joint, trauma on joint, ankylosis, subluxation or dislocation of joint, injury of articular disc, myositis, muscle contracture or spasm, myofascial pain dysfunction syndrome, dyskinesia of masticatory muscles, developmental abnormality, tumor, connective tissue disease, fibrosis, malocclusion, swallowing abnormality, wrong habits such as bite nail or hair, bruxism, psycological stress and Costen syndrome etc. Assessment of TMJ dysfunction consist of interview, observation, functional examination, palpation, reflex test, joint play test, electromyography and radiologic examination and behavioral and psycological assessment etc.

  • Objectives : We perform this study for help to recovery of cervical syndrome by analyzing structural deviation and cervical curvature in upper cervical vertebrae and definiting functional change through cervical body heat change in cevical lesion which is estimated by DITI. Methods : From sep. 1st. 1998 to aug, 31th. 1999, I make an experiment with the 102 OPD patients of this hospital vertebral-joint center, which were judged as a cervical syndrome and confirmed in upper cervical deviation by motion palpation and then were taken cervical simple X-ray and cervical thermograpy. Results : In the classfication of upper cervical deviation, AS deviation took 98% and abnormal cervical curvature took 68%. in the Chief complain neck-shoulder pain took 78%. In the results of DITI, average value of all objects show $032{\pm}0.18$ as a meaning result. Conclusions : In medical treatment of cervical syndrome, this result shows necessity of keeping pace with remedy of upper cervical deviation and medical treatment of functional change and We can learn necessity of detailed assessment through continual clinical examination.

The Effect on Length Cognition Ability in Dominant Eye & Hand (우성안과 주동수가 길이 인지능력에 미치는 영향)

  • Background: Human body is formed of symmetric bilateral structures that are comprised of eye, upper arm, lower arm and etc. but, we are used only dominant components. The purpose of this study was to analysis length cognition ability in dominant eye & hand. Method: Total 180 persons (male 32, female 138) were participated in this study. They were tested with 'hole in the card' test for identification of dominant eye's side and the question for identification of dominant hand's side, then the length cognition ability was measured in right & left axillary level by describing 10cm line. Results: The results by independent t-test were as follows. In difference of length cognition ability in right axillary level between right dominant eyed group & left dominant eyed group, right dominant eyed group was superior to left dominant eyed group, but significant difference was not existed statistically(p>.05). In left axillary level, right dominant eyed group was superior to left dominant eyed group, but significant difference was not existed statistically(p>.05). In axillary level of dominant eye's side, non-crossed group was superior to crossed group, but significant difference was not existed statistically(p>.05). In axillary level of non-dominant eye's side, non-crossed group was superior to crossed group, but significant difference was not existed statistically(p>.05). Conclusion: These result can be applied to the learning of palpation & observation skill in physical therapy.

Solitary fibrofolliculoma on the nasal septum: a case report

  • Fibrofolliculoma is a benign, perifollicular, connective tissue tumor that usually arises in the form of multiple lesions; it is rarely seen as a solitary lesion. The lesions are clinically asymptomatic, 2 to 4 mm skin-colored, soft dome-shaped papules. Here, we report a patient who visited our hospital with a palpable lesion on the nasal septum. The lesion did not cause pain upon palpation, and nasal endoscopy confirmed an irregular wart-like lesion measuring 6×6 mm in the left anterior nasal septum near the columella. Other otolaryngology findings were normal, and there were no similar lesions in other parts of the body. None of the patient's family members were known to have had such lesions. An excisional biopsy was performed on the mass for removal of the lesion, and histological examination confirmed the lesion as fibrofolliculoma. We report the first case of solitary fibrofolliculoma in the nasal septum in a healthy 62-year-old woman along with a review of the relevant literature.

  • Palpation of the pulse has been used in Korean traditional medicine since ancient times to assess physical health. Pulse wave contour may be obtained by measuring arterial pressure or blood volume change of skin. The latter is called as Photoplethysmography(PPG) or digital volume pulse(DVP). The PPG signal is measured by a device comprising an infrared light sourece and a photodetector. Although less widely used, this technique deserves further consideration because of its simplicity and ease of use. The contour of the PPG is formed as a result of a complex interaction between the left ventricle and the systemic circulation. It usually exhibits an early systolic peak and an early diastolic peak. the first peak is formed mainly by pressure trasmitted along a direct path from the left ventricle to the finger. The second peak is formed in part by pressure transmitted along the aorta and large arteries to sites of impedance mismatch in the lower body. The contour of the PPG is sensitive to changes in arterial tone and is influenced by ageing and large artery stiffness. Measurements taken directly from the PPG or from its second derivative can be used to assess these properties. In some mathematical approaches, the extraction of periodic components using frequency analysis was tried to analysis of the PPG. But we don't understand yet what kind of factor in the cardiovascular system or human body is related with the respective specific Fourier components of PPG. This review describes the background to measurement principles, representative contour, contour analysis and frequency domain analysis of PPG, and current and future.

  • Eagle's syndrome is the term given to symtomatic elogation of the styloid process or mineralization of the stylohyoid or stylomandibular ligament. Since ossification of stylohyoid ligament at cadeva was fist described by De Manchetis in 1652 and Weinlecher described clinical symptom which produced by elongated styloid process and osteotomy of styloid process in 1872, Clinical symtom which include sensation of a foreign body on the pharynx, dysphagai, dysphonia, referred pain, and mouth opening disturbance was termed by Eagle as Eagle's syndrome. then, case reports of Eagle's syndrome are presented. in these cases, the patient's chief complaints included periauricular radiating pain, mouth opening disturbance, foreign body sensation, dysphagia, tenderness on the neck. Through vairous X-ray examination and palpation of tonsillar fossa, elongated styloid process were confirmed. Under the general anesthesia there were successfully removed out via transoral approach technique, described by Eagle. after resented styloid process, they were freed from the symptoms without further complication. Therefor we reported these cases treated by surgical resection of styloid process with good results.

Preparation and Management of Recipients in Bovine Embryo Transfer (수정란이식에 있어서 수란우의 준비와 관리)

  • Recipients are an integral part of embryo transfer and they are expensive to maintain as a good recipient. Recipient management is one of the most important components in a successful embryo transfer program. Management includes selection and subsequent care of the animals. A good recipient is basically on "open" cows or heffers whose reproductive tract is capable of receiving one or two embryos and incubating it to term. Potential recipients should be always be healthy and cycling normally ranging from 18 to 23 days. A thorough veterinary examination is recommended for candidate of recipients and cattle for questionable health should be eliminated from the recipient herd. Age and size of recipients are particularly important considerations when heifers are used, because of most embryos available for transfer are from large dams and sires. Body condition can influence a recipient's production, reproduction and health. Obese and underconditioned cattle should be avoided for use. Transfer of fresh embryos especially requires precise synchronization of donors and recipients. For estrus synchronization, PGF$_2$$\alpha$ is injected twice 10 to 12 days apart and short4erm progestagen treatment is applied to potential recipient cattle by coil into vagina (PRID) or ear implant (Synchro-Mate-B). The highest pregnancy results are achieved in recipients at exact synchrony with donors or 12 to 24 hr earlier than donors. Estrus detection is a major factor in breeding efficiency. High accuracy can be achieved by use of heat mount detection alds or by obserbing cattle for 30-minute peroids 3 times daily. Assay progesterone in milk can be used to discrIminate between pregnant and nonprenant recipients. Rectal palpation on day 35 to 70 after is an accurate and safe method of pregnancy diagnosis. Embryonic mortality in recipients may be associated with factors such as high environmental temperature and nutritional or lactational stress in early lactation period. Achievement of short calving interval requires concentrated management activity during the first 90 days following calving. Acceptable candidate for a recipient should be routinely vaccinated for infectious diseases. Proper nutritional programs according to NRC requirements and body condition scoring system for recipient cattles are vital to the ultimate success of an embryo transfer program.r program.

Reproduction-Nutrition Relationship in Dairy Buffaloes. I. Effect of Intake of Protein, Energy and Blood Metabolites Levels

  • Fifty one Nili-Ravi dairy buffaloes in their last two months of gestation were selected. After parturition, rectal examination of reproductive organs was carried out until the occurrence of the first oestrus (PEI). Milk samples were analyzed for milk progesterone levels (MPL). Ovulation (POI) was confirmed by rectal palpation and MPL. Feed and blood samples were collected fortnightly and analyzed. Body condition score (BCS) was recorded on a scale of 0 to 5. Crude protein (CP) intake varied among different seasons and correlated positively with serum urea levels, POI (p<0.01) and PEI (p<0.05). Excess CPI was lower in the group showing oestrus as compared to those remaining as anoestrus (p<0.05). The dietary ratio of crude protein - metabolizable energy (CP:ME) in the oestrus animals was narrow and constant, while the anoestrus animals had a widely fluctuating one. In normal breeding season (NBS) calvers, mean serum urea level (SUL) was lower than the low breeding season (LBS) calvers. SUL was positively correlated with PEI and POI (p<0.01). Up to six months postpartum, SUL were constantly higher in anoestrus than oestrus buffaloes. Mean metabolizable energy (ME) intake was lower in the NBS calvers than the LBS calvers (p<0.01). BCS and postpartum ovulation interval were correlated with ME intake (p<0.01). Prepartum ME intake was higher in oestrous as compared to anoestrous animals (p<0.05). Higher and lower ME intakes were associated with anoestrus, while a moderate energy intake was associated with a PEI of less than 75 days. Buffaloes with poor BCS belonged to the LBS calving group and most of the NBS calving buffaloes had good BCS. BCS was negatively correlated with PEI (p<0.01) and was higher in oestrous buffaloes than anestrus. It was concluded that excess intake of crude protein, associated with higher serum urea levels and low energy intake, associated with poor body condition, are the key factors for low reproductive efficiency. It may be corrected by adopting a proper feeding strategy.

  • Eagle's syndrome occurs when an elongated styloid process or calcified stylohyoid ligament causes recurrent throat pain or foreign body sensation, dysphagia. It is supposed that this symptoms and signs are due to the compression of the styloid process on some neural and vascular structures. More uncommonly, symptoms such as dysphagia, tinnitus, and otalgia may occur in patients with this syndrome. It may also cause stroke due to the compression of carotid arteries. The compression depends on the size, shape, and orientation of the ossified styloid process Besides, degenerative or inflammatory changes in the tendinous portion of the styloid ligament insertion or rheumatic styloiditis may also cause this syndrome. Diagnosis can usually be made on physical examination by digital palpation of the styloid process in the tonsiller fossa. Three-dimensional computed tomography can utilized for supporting diagnosis. The treatment of Eagle's syndrome is primarily surgical. The styloid process can be shortened through an intraoral and extraoral approach. We report a 62-years-old man with the symptomatology of Eagle's syndrome and literature reviews.
