Effect of Sucralfate on Radiation-induced Esophageal Changes in Rat (방사선 조사에 따른 흰쥐 식도변화의 Sucralfate 효과)

  • The study was designed to evaluate the response of sucralfate against the esophageal damage of rat by irradiation. Sixty Sprague-Dawley rats weighing 200-250 gm were divided into six groups including only sucralfate group, the group of 10 Gy or 20 Gy radiation alone, the groups of 10 Gy or 20 Gy radiation-sucralfate combination, and control. Each group was sacrificed at 3, 7, 14, and 28 days after irradiation. For evaluation of the radiation-induced esophageal lesions, there were examined the change of body weight, barium-esophagogram and histopathologic findings. Mean body weight gain was decreased in the groups with 10 Gy or 20 Gy radiation alone and 10 Gy or 20 Gy radiation-sucralfate combination as comparing with control (p=0.03, p=0.02, p=0.04, p=0.02), but there was no difference between the groups of 10 Gy or 20 Gy radiation alone and the groups of 10 Gy or 20 Gy radiation-sucralfate combination. In the 10 Gy and 20 Gy radiation alone groups, barium esophagograms showed diffuse narrowing and stiffness through the entire esophagus from 7 days after irradiation, but there was similar pattern of esophagograms between the groups of 10 Gy radiation-sucralfate combination and the control. Histopathoiogic lesion scores of 10 Gy and 20 Gy radiation groups were significantly higher than the control (p=0.03, p=0.008). In the 10 Gy radiation group, there were significantly higher lesion scores at 3 and 7 days after irradiation as comparing with those in the group of radiation-sucralfate combination (p=0.04, p=0.03). These results suggest that sucralfate might be played as a role of protection from acute radiation-induced esophagitis within the limit of tolerable radiation doses.

The Study on Risk Factor of Metabolic Diseases in Pancreatic Steatosis (췌장지방증에서 대사성질환의 위험 요인에 관한 연구)

  • The body of the fat tissue increased in obese represented by risk factors such as cardiovascular diseases, diabetes, metabolic disease and dyslipidemia. Such metabolic diseases and the like of the cardiovascular and cerebrovascular disease, hypertension, dyslipidemia, increase in the adipose tissue of the pancreas is known to be a risk factor of these diseases. Study on the diagnosis and treatment of pancreatic cancer was conducted actively, case studies on pancreatic steatosis is not much. In this study, divided into a control group diagnosed with pancreatic steatosis as a result of ultrasonography to evaluation the physical characteristics and serologic tests and blood pressure and arterial stiffness. The control group and the test pancreas steatosis age and waist circumference, body mass index, total cholesterol, HDL cholesterol, LDL cholesterol, and systolic and diastolic blood pressure, fasting blood glucose, arterial elasticity is higher in pancreatic steatosis. And the lower ankle brachial stenosis and HDL-cholesterol were lower than the normal control group, so the pancreatic steatosis harmful to blood vessels.(P <0.05). The difference between the control group and it was confirmed that the pancreatic jibanggun statistically significant. In conclusion, pancreatic steatosis at abdominal ultrasound can predict the risk of metabolic diseases, and there was a correlation with cardiovascular disease.

Clinical Investigation of Fatigue Among Outpatients (피로를 호소하는 외래환자에 대한 임상적 관찰)

  • Objectives: Fatigue is a common symptom experienced by many people who visit Oriental medical clinics or hospital. However, there has been little study about the fatigue in the Oriental medical academic world. For this reason, we attempted to investigate the present status of fatigue of outpatients, and its relation with Health Practice Index(HPI). Methods: The subjects were 63 outpatients who visited the Tonification Clinic in Kyunghee Oriental Medical Center between January 1, 2001 and July 31, 2001. Their chief complaint was fatigue and they did not have any physical or mental problem. They were given a questionnaire which included questions reflecting general characteristics, fatigue degree and health habits. We measured degree of fatigue by Chalder scale et al. Health habits were investigated about 5 articles out of 'Breslow 7 Health habits'. Results: Among the subjects, 48 people(76.2%) were considered as 'fatigue patients' by the Chalder scale. Of this 48 fatigue patients, 27 people(56.3%) had manifested fatigue for more than 6 months. The average of scale II for all the patients was 14.05, which indicates moderate degree of fatigue. They complained fatigue, drowsiness and general weakness, dryness and discomfort of the eyes, headache, shoulder pain and neck stiffness, dizziness, heat in the upper part of the body, and poor concentration. There were no differences in degree of fatigue according to Health habits including exercise or not, smoking or not, the frequency of drinking, hours of sleeping, and body mass index. Conclusions: Many people complain fatigue symptom. Therefore doctors should have more interest in fatigue and care. This study can provide standards of prognosis of fatigue patients. Also prospective studies are needed to find relationship between health habits and fatigue degree.

  • Human's landing strategies have been explained through lower extremity kinetics in various conditions. However, how lower extremity kinetics respond when the two conditions between a load and landing height are combined is not yet understood. To achieve the purpose of this study, a total of 20 men and women were subjected to drop landing according to a load(No load, 10%, 20%, 30% of the body weight) at various landing heights(0.3 m, 0.4 m, 0.5 m). As a result of the study, the main effect of a load was not statistically significant in all variables. But increasing of the landing heights showed more flexion angle which was statistically significant in knee joint. In addition, as the landing height increased, the medial-lateral, anterior-posterior, vertical force, and loading rate increased, while time to peak vertical force decreased which was statistically significant. Thus, humans can successfully perform the landing motion even if the load is changed at various heights. However, it reacted more sensitively to the change in landing height than that load condition. The landing height can be prepared for recognition and shock absorption through visual information, but the weight level is difficult for the body to perceive and explains why it is more difficult to apply it to the landing strategy mechanism for shock absorption.

Arm Morbidity after Breast Cancer Treatments and Analysis of Related Factors (유방암 환자의 상지 부작용과 관련 요인)

  • Purpose : To evaluate the incidence of arm morbidity following breast cancer surgery including axillary dissection and to identify related factors. Materials and Methods : One hundred and fifty nine patients were studied using a self-report questionnaire and a clinical examination. Lymphedema, reduction of range of motion in shoulder joint and subjective symptoms (pain, impaired arm movement, numbness, stiffness) were evaluated. As related factors, demographic, oncologic characteristics and types of treatment were analysed. Results : The incidence of lymphedema ($\geqq$2 cm difference comparing to unaffected arm) was $6.3\%$, $10.7\%$, $22.5\%$ and $23.3\%$ at each 10 cm, 20 cm, 30 cm, and 40 cm from wrist. Reduction of range of motion in shoulder joint ($\geqq$ 20 degree difference comparing to unaffected arm) was noted In more than 1/3 patients for flexion, abduction and internal rotation. Especially the reduction of range of motion in internal rotation was severe ($>50\%$ reduction) in 1/3 patients. Approximately 50 to $60\%$ of patients complained impaired arm movement, numbness, stiffness and pain. Body mass index (BMI) was the significant risk factor for lymphedema. Conclusion : Lymphedema was present in 1/3 of patients and the common sites of edema were 30 cm 40 cm proximal from the wrist. Also most severe reduction of range of motion in shoulder joint was with internal rotation. There needs weight control for lymphedema because BMI was the significant risk factor for lymphedema. Also rehabilitation program for range of motion especially internal rotation In shoulder joint should be developed.

A Study on the causes of farmer's disease and greenhouse disease in a rural area of Kyungnam province (경남 일부지역의 농부증 및 하우스증 발생에 미치는 요인분석)

  • This study was carried out to elucidate the causes of farmer's disease and greenhouse disease in the rural area of Kyungnam province during 1996. 2,171 (69.1%) of the 3,140 adults above 20 years old who had lived in the selected 20 villages in Uiryong County of Kyungnam Province were selected in order to over both residents who live in the green house distracts and conventional rural area. The results were as follows: 1. Among the subjects, the male was 42.2% and female was 57.6%. The average age for the male was 52.3, 55.6 for the female. 2. The proportion of the farmers in the subjects was 81.5%. Among these 78.0% were engaged in the greenhouse farming. 3. Among the eight symptoms of Nofusho(japanese farmer's comples), lumbago was the most frequently complained and followed by shoulder stiffness and parenthesis of hand or foot. 4. The total score of farmer's disease was evidently higher in the female and the older the score was clearly bigger. 5. The prevalence of Nofusho(Japanese farmer's comples)was 26.8% and 43.15% was for the suspected Nofusho. In the male, the prevalence of Nofusho was 13.6% and 40.9% for the suspected Nofusho and in the female 36.5% and 44.7% were shown, comparatively. 6. According to the multiple classification analysis, sex, age, and occupation were selected as significant variables to explain the total score of Nofusho. 7. The correlations between the total score of Nofusho and the number of sick day, working years, and age were significant, comparatively. 8. According to the multiple classification analysis corolling interaction between independent variables, age was the only variable which was significant in the male and age, pesticide work in the female. 9. The score of greenhouse disease was highest in the group who engaged in greenhouse farming and conventional farming(2.76 for male, 3.77 for female) followed by the group who engaged only in greening house farming(2.66 for male, 3.49 for female) and by the group who engaged only in conventional farming(2.27 for male, 3.05 for female) 10. According to the multiple classification analysis with the total score of greenhouse disease as dependent variable, corolling interaction between independent variables, age and pesticide work were revealed as significant variables in the male, while, pesticide work and farming type were significant in the female. According to the above results, the following could be suggested. Because lumbago, shoulder stiffness, paresthesia of hand and foot were the most frequently complained symptoms in the respondent, the development of farming tool to reduce the body burden and periodical physical exercise and rest is highly recommended. It is revealed that both in the farmer' disease and greenhouse disease the score was higher in the female than in the male. So the reasonables measures is recommended to reduce the working hours of the female. Pesticide work was revealed as the significant variable in the female in farmer's disease and both in the male and the female in greenhouse disease. So the development of the safe method of pesticide spraying including safety education should be introduced. Particularly the female should be excluded in pesticide spraying.

  • The artificial ground freezing (AGF) method is a groundwater cutoff and/or ground reinforcement method suitable for constructing underground structures in soft ground and urban areas. The AGF method conducts a freezing process by employing a refrigerant circulating through a set of embedded freezing pipes to form frozen walls serving as excavation supports and/or cutoff walls. However, thermal expansion of the pore water during freezing may cause excessive deformation of the ground. On the other hand, as the frozen soil is thawed after completion of the construction, mechanical characteristics of the thawed soil are changed due to the plastic deformation of the ground and the rearrangement of soil fabric. This paper performed a field experiment to evaluate the freezing rate of marine clay in the application of the AGF method. The field experiment was carried out by circulating liquid nitrogen, which is a cryogenic refrigerant, through one freezing pipe installed at a depth of 3.2 m in the ground. Also, a piezo-cone penetration test (CPTu) and a lateral load test (LLT) were performed on the marine clay before and after application of the AGF method to evaluate a change in strength and stiffness of it, which was induced by freezing-thawing. The experimental results indicate that about 11.9 tons of liquid nitrogen were consumed for 3.5 days to form a cylindrical frozen body with a volume of about $2.12m^3$. In addition, the strength and stiffness of the ground were reduced by 48.5% and 22.7%, respectively, after a freezing-thawing cycle.

  • Ge-Fu(歌賦) means prose and poetry individually, and they both have a meaning of all the rhythmical poetrical compositions making it ease for people to remember the content. All the Ge-Fus used in oriental medicine are made in Yuan(元), Mine(明)and Qing(靑)dynasty, and they have been largely used in most of all the fields of medicine such as Ben-Cao(本草), Tang-Ye(湯液), Zhen-Jiu(鍼灸), Zhen-Duan(診斷). Zhen-Jiu-Ge-Fu(鍼灸歌賦) has about 90 poetries and 10 proses and they contain the names of meridian; courses of meridian streams; accurate positions of acupuncture points; functions; effects; meanings of the name of acupuncture points; usages and effects of special points; manipulations of reinforcing and reducing method; contraindications of acupuncturing; principles of selections and orders of acupuncture points in therapy; and eight diadgoses. Zhen-Jiu-Ge-Fu is subdivided into Jing-Xue(經穴歌), Zhi-Jiu-Ge(刺灸歌), Shu-Xue-Ge(輸穴歌), Zhi-Liao-Ge(治療歌). And In Zhi-Liao-Ge, the most brief and essential Ge-Fu-Si-Zong-Sue-Ge- contains theraputic designs using far apart acpuncture points from the right painful areas in the body. In this study, the author opinionated the Si-Zong-Sue-Ge can be the prototype of the distant needling; the research on this can open the importance of Ge-Fus. On conclusion, 1. "Upper and lower Abdomen - Zu-San-Li($S_{36}$) (肚腹三里留)" means when there are problems and disorders in upper and lower abdomen, distinctively, such as gastric pain, maldigestion, flatulence, abdominal pain, constipation, diarrhea, vomiting, menstrual disorer, knee pain and tonic functioning, 족삼리 can be a right choice for distant needling point for treating. 2. "Face and Eye-He-Gu($Li_4$) (面目合谷收)" means when there are problems and disorders in facial, eye, ear, nose, throat, mouth regions, distinctively, such as facial edema, toothache, headache, sore throat, rhinorrhea, frontal headache, abdominal pain, dizziness, He-Gu can be a right choice for distant needling point for treating. 3. "Upper and lower back - Wei-Zhong($B_{40}$) (腰背委中求)" means when there are problems and disorders in upper and lower back, distinctively, such as upper back pain, lumbargo, hamstring muscle pain, popliteal region pain, lower extremity compartment syndrom, Inguinal region pain, muscle twitch, vomiting and diarrhea, hemorrhoidal bleeding, skin rash, Wei-Zhong can be a right choice for distant needling point for treating. 4. "Head and neck - Lie-Que($L_7$) (頭項審列缺)" means when there are problems and disorders in capital and nuchal area, distinctively, such as migraine, frontal headache, rhinorrhea, asthmatic dyspnea, aphasia, coughing, neck stiffness, occipital headache, upper extremity pain, Lie-Que can be a right choice for distant needling point for treating.

Psychiatric Factors Associated with Farmers' Syndrome (농부중의 정신적 원인에 대한 연구)

  • Backgrounds : There has been many studies investigating the causes of farmers' syndrome. In some studies, psychologic stress is related to farmer's syndrome. And the diagnostic criteria of farmer's syndrome is similar to those of generalized anxiety disorder. So we carried out this study to investigate the psychiatric causes of farmers' syndrome. Methods : This study was done in some rural and urban areas of Kyoungsangnam Province, July, August, October and November of 1996. Those who came to free medical service and completed interview, medical examination and laboratory examination and 20-59 years old were 150 persons. And those who came to health center for health examination and completed only interview and 20-59 years old were 94 persons. The questionnaire was composed of sociodemographic factors, health risk factors, farmer's syndrome, work load, BEPSI(inventory to measure stress), Spielberger's state-trait anxiety inventory, self-rating depression scale. To examine statistical significance, we used X2-test, Mantel-Haenszel test for linear association, t-test, ANCOVA, correlation, multiple regression, logistic regression. Results : The prevalence of farmer's syndrome, adjusted for age and sex with population of Kyungsangnam Province of 1993 was 208 per 1,000(90 per 1,000 in men and 329 per 1,000 in women). In bivariate analysis, farmer's syndrome was significantly related to age, sex, job, income, smoking, alcohol drinking, work load, BEPSI, state anxiety, trait anxiety, depression, body mass index, Hwa-Byung, hypertension, anemia. However, when age and sex were adjusted, job was not significantly related to farmer's syndrome. The score of farmer's syndrome was significantly related to age, sex, work load, BEPSI, trait anxiety by multiple regression. Farmer's syndrome was significantly related to increasing age(odd ratio 1.079, 95% C.I. 1.060 - 1.099), sex(odds ratio of male 0.434, 95% C.I. 0.349 - 0.540), and BEPSI(odds ratio 1.231, 95% C.I. 1.148 - 1.320) by logistic regression. Results of logistic regression analysis of the component symptoms of farmer's syndrome were as follows. Shoulder stiffness was significantly related to increasing age, female sex and BEPSI. Lumbago was significantly related to increasing age, female sex and trait anxiety. Numb limbs and nocturia was significantly related to increasing age and female sex. Breathlessness was significantly related to work load, sleeplessness was significantly related to depression, dizziness was significantly related to job and state anxiety, and abdominal fullness was significantly related to female sex. Conclusion : Farmers' syndrome was related to work load, but was more related to psychiatric factors such as BEPSI and trait anxiety. And the occupation was not risk factor of farmers' syndrome in this study, so further study is needed to investigate the cause of farmers' syndrome.

Effects of Additives on Laying Performance, Metabolic Profile, and Egg Quality of Hens Fed a High Level of Sorghum (Sorghum vulgare) during the Peak Laying Period

  • This experiment was conducted to determine the effects of supplemental methionine, lysine, choline, and sulfur on laying performance, metabolic parameters, and egg quality of hens fed diets containing sorghum (Sorghum vulgare) during the peak laying period. Lohman layers (n = 144), 30-wk of age as 6 replicate cages of 4 hens, were allocated randomly to receive basal diets containing either 22% corn (B) or 22% sorghum (BS) and diets BS plus 0.57% methionine, 0.66% lysine, 0.47% choline, or 0.05% sulfur for 98 d. Feed intake (FI) and egg production (EP) were recorded daily, egg weight (EW) was measured bi-weekly, and body weight (BW) was measured monthly. A sample of 12 eggs from each experimental group was collected every month to evaluate egg quality. At the end of the experiment, blood samples were collected for metabolite concentrations. Data were analyzed using one-way ANOVA as repeated measures and significant differences between the experimental groups were assessed using Duncan's Multiple Range test. Partial replacement of corn with sorghum in the basal diet did not affect BW, EP, and FCR but increased FI by 5.7% and EW by 2.4%. The effects of additives on laying performance were variable. Except for serum total protein (STP) concentration, other metabolic parameters were not affected by partial replacement of corn with sorghum in the basal diet. Hens fed diet BS had lower SPT concentration than hens fed diet B. Except for methionine supplementation, other supplements ameliorated depression in STP concentration. The additives did not affect other metabolic parameters. Egg quality responses to the experimental diets were also variable. Partial replacement of corn with sorghum in the basal diet did not affect eggshell characteristics (both thickness and stiffness), whereas it had variable effects on inner egg quality parameters (increased yolk index, depressed yolk color, and unaltered albumen index and Haugh unit). In conclusion, laying hen diets could include low-tannin sorghum (0.26%) up to 22% without necessitating extra supplements to overcome compromised performance.
