다낭신 환자의 요추 추간판탈출증에 대한 통합한방치료 1례 (A Case Report of Lumbar HIVD Treated with Korean Medicine on a Polycystic Kidney Patient)

  • Objectives: This research study aimed to determine the effect of Korean medicine treatments on a patient with lumbar disc herniation accompanying polycystic kidney disease. Methods: Acupuncture, herbal medicine, pharmacopuncture, spine decompression therapy, Motion Style Acupuncture Treatment (MSAT), and Chuna were preceded for treatment. We checked the patient's Oswestry Disability Index (ODI), numeric rating scale (NRS), and straight leg raise test (SLRT) on admission and discharge; we also used the NRS and SLRT to evaluate the patient's symptoms on every third day during the hospital stay. Because it is important to manage blood urea nitrogen (BUN), serum creatinine, and blood pressure during the early stage of polycystic kidney disease, BUN and serum creatinine levels were checked weekly while blood pressure was checked every morning. Results: Twelve days after admission, the NRS for lower back pain and right leg pain decreased from 7 to 3 and from 7 to 2, respectively. The ODI value also decreased from 56 to 20 while the SLRT value increased from 30/70 to 60/70. The BUN and serum creatinine levels and the blood pressure readings were all within normal range every time they were checked. Conclusions: The use of Korean medicine treatments resulted in improvements in NRS, ODI, and SLRT on a patient with a herniated lumbar disc herniated who had a past history of polycystic kidney disease; thus, the patient was able to maintaining kidney functioning. Herbal medicine, an alternative method of analgesic anti-inflammatory drugs that has been evaluated as relatively safe on liver and kidney function, could be suggested on a patient with a past history of polycystic kidney disease to maintain kidney function when renal function and blood pressure are monitored.

배에서 추출한 Phenolic Compound가 Streptozotocin으로 유발된 고혈당 생쥐에 미치는 영향 (Effects of Pear Phenolic Compound on the STZ-treated Mice for Induction of Diabetes)

  • 배에서 추출한 phenolic compound가 streptozotocin(STZ)응 투여하고 고혈당을 유발시킨 생쥐에 미치는 영향을 밝히고자, 생쥐의 혈당, 혈중 creatinine, BUN의 변화 및 insulin-면역 조직화학적 검색과 췌장섬 $\beta$-세포의 전자현미경관찰을 통한 미세구조 변화를 관찰하였다. 실험군은 정상적인 동물 사료를 식이토록 한 대조군, 사료에 phenolic compound(PA군, 13 mg/g/kg/day; PB군,90 mg/kg/day)를 혼합하여 6주 동안 섭식하게 한 실험군으로 구분하였다. 대조군의 혈당 농도는 4주부터 높게 나타났으며, PA군의 혈당은 대조군에 비하여 유의성 (p<0.05)있게 감소하였으며, 특히 PB군에서는 4주부터 감소하기 시작하여 6주까지 유의성 (p<0.05)있게 감소하였다. BUN과creatinine의 농도는 대조군에 비하여 실험군에서 다소 감소하였으나 유의성은 없었다. STZ을 투여한 대조군의 췌장섬은 대부분 파괴되어 insulin-면역조직화학적 반응을 보인 세포들이 거의 관찰되지 않았으나, PB군에서는 다수의 췌장섬이 관찰될 뿐만 아니라 인슐린-면역조직화학 반응이 양성으로 관찰되었다. 전자현미경관찰 결과 대조군의 $\beta$-세포에서는 인슐린 함유 과립들이 소수 관찰되었으나 PB군에서는 이들 과립들이 다수 관찰되었다. 이상의 결과로 보아 phenolic compound를 섭식한 실험군 생쥐는 STZ에 의해서 손상된 췌장섬이 회복 또는 재생되어 $\beta$-세포의 인슐린 분비가 복원되 어 가고 있다고 사료되었다.

A 24-Weeks Toxicity Study of Eryngium foetidum Linn. Leaves in Mice

  • Eryngium foetidum Linn. leaves (EF) are widely used in Thailand and many countries throughout Asia as a culinary seasoning and a traditional medicine. However, adverse effect of high dose consumption in long duration has not been evaluated. The aim of this study was to investigate chronic toxicity of EF in mice. Thirty-two ICR male mice were divided into 4 groups of 8 mice each. The mice were fed AIN-76 rodent diet, or AIN-76 rodent diet supplemented with ground freeze-dried EF at 0.8%, 1.6% and 3.2% that is equivalent to approximately 35, 73 and 155 times that of human consumption, respectively, at 97.5 percentile for a period of 24 weeks. At the end of experiment, the mice were euthanized and blood samples were collected for hematological and biochemical evaluations. Necropsy was performed while visceral organs such as lung, liver, kidneys, spleen etc. were collected, weighed and histopathologically examined. Blood urea nitrogen (BUN) results of mice in 1.6% and 3.2% EF diet groups were significantly higher than the BUN of control group. No significant difference was noted in other biochemical and hematological properties between the treatment groups and control; all results were within normal range. Histopathology of almost all visceral organs showed no significant changes. However, tubulonephrosis and chronic interstitial nephritis were observed in the groups treated with 1.6% and 3.2% EF diet. Body weight was reduced significantly at week 12 to week 20 when compared to the control group while relative kidney weights were significantly increased. In conclusion, the consumption of EF in diet at high doses illustrated the adverse effect on some biochemical parameters and histopathology in mice. Our findings suggested that EF daily consumption for 24 weeks, at higher doses than the 0.8% EF diet (35 times of human consumption), might cause adverse effect on kidney function in mice.

무비강장주의 면역증진 효과 (Immune-enhancing Effect of Mubigangjang-Ju)

  • Mubigangjang-Ju (MGJ) is a traditional wine, fermented extract of Cynanchum wilfordii, Angelicae gigantis and Epimedium koreanum etc. In the present study, we comparatively investigated the immune-enhancing effect of fermented extract (MGJ) and water extract (WE). Forced swimming test (FST) was performed as a model of activity test in mice and measured blood urea nitrogen (BUN), aspartate aminotransferase (AST), alanine aminotransferase (ALT), lactic dehydrogenase (LDH), glucose (Glc) and total protein (TP) in the serum. Each extracts were orally administered into mice, 10 ㎖/kg, once per day for 7 days using a feeding atraumatic needle. After 3 days, on FST, the immobility time was decreased in the MGJ-fed group (133.7±18.6 s) in comparison with the saline-fed group (155.8±16.6 s). After 7 days, the immobility time was significantly decreased in the MGJ-fed group (105.3±12.7 s) in comparison with the saline-fed group (171.3±8.1 s). In addition, the content of AST was significantly decreased and the contents of BUN, ALT and LDH in the blood serum was also decreased. Whereas, the content of Glc tend to increase and TP level was not changed. However, WE had no effect on all experiments. The present results suggest that fermented extract was more effective than water extract and it may be useful for the immune-enhancing agent.

Preoperative Levels of Hematological and Biochemical Indices Affect Perioperative Variables in Adult Patients with Coronary Artery Bypass Graft Surgery

  • The objective of this research was to evaluate the relationships of preoperative (Pre-OP) levels of hematological and biochemical indices to perioperative variables in patients that underwent coronary artery bypass graft surgery (CABG). Pre-OP levels of hematological factors [total white blood cells (T-WBC), erythrocytes, hemoglobin, hematocrit, glycohemoglobin A1c (HbA1c), or platelet] were negatively or positively related with biochemical indices [alanine aminotransferase (ALT), bilirubin, glucose, fructosamine, triglyceride, and high density lipoprotein cholesterol (HDL)]. Pre-OP levels of hematological factors and biochemical indices were negatively or positively correlated with echocardiographic variables. Pre-OP level of HbA1c had a relationship with C-reactive protein. Pre-OP levels of aspartate aminotransferase (AST), ALT, HDL, glucose, fructosamine, or blood urea nitrogen (BUN) were positively or negatively associated with Pre-OP levels of cardiac markers (brain natriuretic peptide, troponin-I, creatine kinase isoenzyme 2, or CRP). Pre-OP levels of hematological factors (excepting T-WBC) related with operation time (OPT), postoperative mechanical ventilation time (POMVT), intensive care unit-period (ICU-period) or hospitalization. Pre-OP levels of AST, ALT, bilirubin, triglyceride, HDL, low dwensity lipoprotein, fructosamine, or BUN were positively or negatively correlated with OPT, graft numbers, POMVT, ICU-period or hospitalization. Retrospective this study reveals that Pre-OP levels of hematological and biochemical markers are associated with echocardiographic variables, several cardiac markers and postoperative outcomes, suggesting that Pre-OP levels of hematological and biochemical markers may be useful predictors for the diagnosis and prognosis of coronary artery disease.

Change of Blood Magnesium Level in Diabetes Patients Undergoing Off-pump Coronary Artery Bypass Grafting

  • We carried out this study to investigate differences of physiological variables between patients with (DM group) and without type II diabetes mellitus (Non-DM group) undergoing off-pump coronary artery bypass grafting (OPCABG). Postoperative $Mg^{++}$ and $Ca^{++}$ levels were lower, whereas $Na^+$ level was higher in DM group than those in Non-DM group. ICU (intensive care unit) stay time in DM group was longer than that of Non-DM group. Postoperative platelet counts tended to decrease, whereas C-reactive protein (CRP) and cardiac troponin-I (cTNI) levels tended to increase in DM group compared with Non-DM group. Postoperative albumin level was lower, while blood urea nitrogen (BUN) and creatinine levels were greater in DM group than those in Non-DM group. DM group had higher incidence of post-operative arrhythmias than Non-DM group. These results reveal that type II DM patients undergoing OPCAB may have higher incidences of postoperative hypomagnesemia, hypocalcemia and arrhythmias, and increases of CRP, cTNI, BUN, and creatinine levels than in Non-DM patients undergoing OPCAB. The perioperative check and control (supplement) of $Mg^{++}$ levels should be considered in cardiovascular surgery combined with DM.

포공영 추출물이 납 및 수은에 노출된 랫드 혈청의 중금속 및 혈액지표에 미치는 영향 (Effect of Taraxacum Mongolicum Extract on Serum Heavy Metal Levels and Blood Parameters of Albino Rats Exposed to Lead and Mercury)

  • 본 연구에서는 포공영의 에탄올 추출물의 중금속 중독에 의한 손상 억제 효과를 측정하였다. 현대 사회에서 환경으로부터 오염이 우려되는 납과 수은을 실험동물에 노출시켰고, 납과 수은에 노출된 흰쥐에서 포공영이 이러한 중금속 노출로 영향을 받는 체중변화, 간 및 신장에 미치는 영향을 혈액세포분석 및 혈액 내의 생화학적 분자에 대한 조사하였다. 실험 결과, 포공영을 투여 받은 실험군은 중금속 투여로 인하여 발생하는 체중감소의 개선효과가 나타났고, 혈액세포의 변화에 있어서도 중금속 노출로 감소되었던 RBC의 수를 증가시켰고, 증가된 WBC 및 platelet의 수를 정상수준으로 회복시켰다. 간 및 신장의 손상 지표들로 측정한 Bilirubin, ALP, GOT, GPT, LDH, ceatinine 및 BUN에 있어서도 중금속 노출로 증가된 수치를 농도의존적으로 감소시켰다. 또한 혈액내에서의 중금속 잔존량을 측정한 결과, 포공영 추출물 투여로 인하여 농도의존적으로 그 잔존량이 감소하였다. 따라서, 포공영 에탄올 추출물은 체내 노출된 납과 수은에 대한 독성을 완화시켜주는 것으로 사료된다.

음곡에 시술한 석위약침이 Lipopolysaccharide로 유도된 흰쥐의 신장염에 미치는 영향 (The Effect of Pyrrosiae Herba Herbal-acupuncture at KI10 on Lipopolysaccharide Induced Nephritis in Rats)

  • Objectives : The purpose of this study is to investigate the effects of Pyrrosiae Herba herbal-acupuncture(PH-HA) at $KI_{10}$(Umgok) on nephritis induced by lipopolysaccharide(LPS) in rats. Methods : Rats were assigned to four groups: normal, LPS, saline and PH-HA. Rats in the saline and PH-HA groups were treated with saline injection and PH-HA respectively at $KI_{10}$, three times over the period of one week. All animals, except those in the normal group, were injected intra-peritoneally with LPS to induce nephritis. WBC, in blood, tumor necrosis factor alpha(TNF-${\alpha}$), cytokine-induced neutrophil chemoattractant 1(CINC-1), blood urea nitrogen(BUN), creatinine in serum, urinal volume, total protein creatinine in urine, and renal myeloperoxidase (MPO) were analyzed. Results : 1. PH-HA group showed significantly reduced levels of serum BUN, serum creatinine, TNF-${\alpha}$, and CINC-1 compared to the LPS group. Furthermore, a significant increase in urine output and more significant decreases in total protein in urine and MPO in renal tissue were observed in the PH-HA group when compared to the LPS group. 2. The PH-HA group showed significantly reduced levels of serum creatinine and renal MPO, and a more significant increase in urine output compared to the saline group. Conclusions : According to these results, it is postulated that PH-HA at $KI_{10}$ has anti-inflammatory and renal-protective effects on LPS-induced nephritis in rats, and both acupoint $KI_{10}$ and the herb Pyrrosiae Herba made contributions to these effects. Further studies on the interaction between acupoint $KI_{10}$ and the herb Pyrrosiae Herba may be needed.

단일 한약 복합 처방의 장기간 연용 투여가 신기능에 미치는 영향에 대한 전향적 연구 (A Study Of Effects on Renal Function from Continuous Long-Term Herbal Medication)

  • Background : Due to increased interest in herbal medicines is recent years, medical circles have made studies of toxicity and side effects of herbal medicines. Particularly the kidney is sensitive to toxicity. A few reports concerning the side effects and toxicity of herbal medicine have been presented recently. This has bought on some distrust in herbal medicines among patients and western doctors. Objectives : The purpose of this study is to determine what effects long-term prescription of one herbal medicine may have on renal function. Methods : Nineteen patients took herbal medicine for eight weeks. Tests of their Blood Urea Nitrogen(BUN), Creatinine of blood plasma, and urine (chemical and microscopic) were taken before taking medicine and at the 2nd, 4th, 8th weeks. Results : After taking a herbal medicine, BUN and Creatinine decreased significantly or remained the same in comparison with the prior interval. Chemical and microscopic examination of urine showed no changes. Conclusions : The results suggest that taking this herbal medicine for a long time does not induce nephrotixicity. Further study is needed for investigating safety and toxicity of herbal medicines.

인동(忍冬)이 Monosodium Urate로 유발(誘發)된 백서(白鼠)의 통풍(痛風)에 미치는 영향(影響) (The Effect of Yindong on Gout of Rats induced by Monosodium Urate)

  • In order to study the Effects of yindong(LC) on the variation of blood and joint value the gout induced by microcrystalline sodium urate in rats, for LC is one of the important medicine on gout. After pretreatment of LC(50, 500mg/kg) for 5days, the Effects of LC was evaluated on Serum albumin, Serum globulin, glutamate dxalacetate transminase(AST), glutanate pyruvate transminase(ALT), blood urea nitrogen(BUN), Serum creatitine, Serum uric acid, xanthine oxidase activity, Erythrocyte Sedimentation Rate(ESR), WBC, platelet were measured. The results were obtained as follows : Joint value Increase ratio was not significantly decreased in all LC taken groups compared with the control group. Serum albumin was significantly different in all LC taken groups compared with the control group and Serum globulin was significantly dicreased in 500mg/kgLC taken group compared with the control group. Serum AST, ALT were significantly dicreased in 500mg/kgLC taken group compared with the control group. Serum BUN was significantly decreased in all LC taken groups and Serum creatinine was significantly decreased in 500mg/kgLC taken group compared with the control group. Serum uric acid was significantly different in 500mg/kgLC taken group, and changes in xanthine oxidase activity was significantly decreased in 500mg/kg, 50mg/kgLC taken group. ESR was significantly decreased in all LC taken groups compared with the control group. WBC, platelet count were significantly decreased in 500mg/kgLC taken group compared with the control group. From above results it may be concluded that Yindong can be used for treatment and preventive medcine of gout induced by microcrystalline sodium urate in clinic.
