Peripheral Vestibular Syndrome in a Cat with Foreign Body Otitis Media/Interna

  • A 15-month-old, intact male, domestic short hair cat was presented with ataxia, protrusion of the right third eyelid, and anorexia. Clinical signs were firstly noted after ear cleaning at home. Symptoms of the Homer's syndrome were evident. However, postural and proprioceptive reaction deficits were not detected on neurologic examination. Otoscopic examination revealed foreign body in right ear canal. A diagnosis of peripheral vestibular syndrome was made based on results of physical examination including neurologic and otic examination, blood work, and magnetic resonance imaging (MRI). Based on these examinations, the present patient was definitely diagnosed as otitis medial interna induced by foreign body. The vestibular signs were resolved 10 days after removal of cotton tips in right ear canal. This case report indicates that home-care cotton swab can iatrogenically induce otitis media/interna in cats.

Metastatic lipid-rich mammary carcinoma in a dog

  • An adult female dog was presented for evaluation of mammary gland masses. Complete blood count and serum chemistry data were within normal limits. Fine-needle aspiration cytology of the mammary masses revealed clusters of malignant epithelial cells with clear cytoplasmic vacuoles. Based on histopathological findings, a diagnosis of lipid-rich mammary carcinoma was made. Approximately 5 weeks after surgical removal, the tumor recurred at the surgery site and metastasis to the tibia was detected. Due to the poor prognosis and deterioration of the condition, the dog was euthanized.

급성 화농성 심낭염 14례 보 (Surgical treatment of acute purulent pericarditis: report of 14 cases)

  • Acute purulent pericarditis, though not common in incidence after introduction of antibiotics, is still potentially life treating isease. Since 1971, we have experienced 14 cases of acute purulent pericarditis with successful treatment. Among these 14 cases, 9 cases were male and they had high occurrence on their third to fifth decades in age distribution. Isolation of causative organisms were obtained in 11 cases through the bacterial culture of infectious source which was mainly pericardial effusion or blood, and the most frequently recovered organism was the staphylococcus aureus. Pre-existing inflammatory disease preceding to pericarditis, named as antecedent disease, were proved in 12 cases, and among which contiguous extension from the intrathoracic infection such as pneumonia or empyema accounted for the majority of antecedent disease. Pericardiocentesis with administration of antibiotics were tried in all cases, but result in recovery in 1 patient only. Remaining 13 cases had persistent picture of pericarditis and necessitated surgical drainage procedure. Ten of these 13 cases were underwent the open pericardial window using a mode of anterior approach in 4 and subxiphoid approach in 6 cases respectively. Two cases of subxiphoid group were reoperated by the anterior interphrenic pericardiectomy, due to insufficient drain of too thick effusion. In remaining 3 cases, anterior interphrenic pericardiectomy was performed initially because of purulent effusion already changed into fibrinopurulent peel with thickened pericardium. Through the experience of this series, we recommended that pericardiectomy should not be reluctant in purulent pericarditis as a initial surgical procedure for advantage of complete removal of infected space and avoidance of late constrictive pericarditis.

편타성 손상후 발생한 뇌진탕증 2례 (Two cases of concussion after whiplash injury)

  • Object : This study was focused on finding out the better method of medical treatment of concussion after whiplash injury. Methods : This study has been carried out for two cases of concussion after whiplash injury which had been hospitalized at the Kangnam Oriental medical Hospital of the Dongguk University Results and conclusions : The concussion is caused by whiplash injury about $47\%. In the symptoms of concussion, there are lightheadedness, dizziness, neck pain, headache, photophobia, phonophobia, anamnesis disorder, concentration disorder, tinnitus, irritability, etc. It is considered more important that the treatment of nausea and vomiting than the removal of stagnation of blood. Oriental medical treatments including of herb-medicine, acupuncture, moxibustion, cupping, chuna, taping have more economic favor and rapid effects than the others. so we report on two cases.

자기공명촬영에서 침강 레벨을 나타낸 뇌하수체 선종 - 증 례 보 고 - (A Sedimentation Level in Pituitary Adenoma on Magnetic Resonance Imaging - Case Report -)

  • We present a rare case of a pituitary adenoma revealing a sedimentation level on MRI, which has not been previously documented. This 55-year-old woman was referred with the diagnosis of craniopharyngioma. She presented with four-month history of progressive headache and visual dimness. Neurological examination revealed a bitemporal hemianopsia and decreased visual acuity. Laboratory data including endocrine examination were unremarkable. An additional three-dimensional MRI was taken for further evaluation, and demonstrated a sedimentation level within the tumor. The patient underwent transcranial removal of the tumor. About 12cc of dark-red blood was aspirated from the tumor. Histological examination revealed a pituitary adenoma with hemorrhage. Postoperatively, the patient showed gradual improvement of visual function. Considering that the pituitary adenoma is one of more common tumors that cause tumoral bleeding, a cystic sellar tumor that has a sedimentation level should be sought first for a pituitary adenoma rather than a craniopharyngioma. This may have an important impact when deciding surgical approach.

감마선조사에 의한 미역 추출물 항염증제제의 갈색 색상 제거 (Gamma-irradiation Elimination of Brown Color from the Anti-inflammatory Agent Containing an Undaria pinnatifida Extract)

  • This study was conducted to examine the bleaching of an anti-inflammatory agent containing Undaria pinnatifida extract (AIAU). Brown-colored AIAU was irradiated with $^{60}CO$ gamma rays at doses ranging from 10-200 kGy. Discoloring of the anti-inflammatory agent was achieved by gamma irradiation, with significantly removal by 50 kGy irradiation. The main active compound of the agent consisted of 4% U. pinnatifida extract, which inhibited inflammatory symptoms in mouse ear edema by 93%. There was no change in effects against edema, erythema, blood flow and radical scavenging activity by irradiation.

Linezolid Treatment for Osteomyelitis due to Staphylococcus Epidermidis with Reduced Vancomycin Susceptibility

  • Limited therapeutic options are available for vancomycin intermediate-resistant Staphylococcus epidermidis (VISE) infections and no optimum therapy has been established. We report a case of VISE skull osteomyelitis that was successfully treated with linezolid. The patient was a 53-year-old man who presented with headache, nausea and dysphasia. Brain computerized tomography (CT) demonstrated a subdural hematoma in the left hemisphere. Craniotomy and hematoma evacuation was performed and he showed good recovery despite a scalp wound infection caused by methicillin-resistant Staphylococcus aureus (MRSA). The organism isolated from the scalp wound was sensitive to vancomycin. The patient was treated with intravenous vancomycin for 44 days. However, he showed a high fever, persistent positive methicillin-resistant Staphylococcus epidermidis (MRSE) blood cultures, and a deteriorating clinical status. He underwent infected skull bone flap removal and linezolid treatment for 35 days. During one year of follow up, he has not had any further episodes of osteomyelitis or fever. Linezolid has shown to be effective agent to eradiate osteomyelitis caused by VISE.

  • 윌름씨 종양은 주로 1-5세에 복부에 생기는 종양으로 약 25% 이상에서 고혈압을 동반하지만 심각한 고혈압에 의하여 심부전을 보이는 경우는 매우 드물다. 저자들은 3세된 여자 아이가 윌름씨 종양으로 인한 고레닌 혈증으로 다음, 다뇨, 저나트륨성 고혈압과 심부전을 보였으나 수술적으로 종양을 제거한 후 증상 호전을 보였기에 문헌 고찰과 함께 보고하는 바이다.

Intravascular Papillary Endothelial Hyperplasia (Masson tumor) of the Skull : Case Report and Literature Review

  • A 10-year-old female patient presented with a rapidly growing nodular mass lesion on her right frontal area. On skull radiography and computed tomography (CT) imaging, this mass had a well-demarcated punch-out lesion with a transdiploic, exophytic soft tissue mass nodule on the frontal scalp. Magnetic resonance (MR) imaging revealed the presence of a $1.5{\times}1.2{\times}1$ cm sized calvarial lesion. This lesion was hypointense on T1 and heterogenous hyperintense on T2 weighted MR images, and exhibited heterogeneous enhancement of the soft tissue filling the punch-out lesion after intravenous administration of gadolinium. En block removal of the tumor with resection of the rim of the normal bone was performed. The pathological diagnosis was intravascular papillary endothelial hyperplasia (IPEH). After surgery, no recurrence was found for 8 months. IPEH is a rare and benign reactive lesion usually found in thrombosed subcutaneous blood vessels. Involvement of skull bone is rare. In this article, we present a case of IPEH involving the calvarium, in a 10-year-old woman.

Glossopharyngeal Neuralgia Caused by Arachnoid Cyst in the Cerebellopontine Angle

  • Glossopharyngeal neuralgia is a relatively rare condition characterized by severe, paroxysmal episodes of lancinating pain in the tongue, throat, ear, and tonsil. This disorder is assumed to be due to compression of the glossopharyngeal nerve by vascular structures. A 47-year-old woman complaining of sharp and lancinating pain in the right periauricular and submandibular areas visited our hospital. Swallowing, chewing, and lying on her right side triggered the pain. Her neurologic examination revealed no specific abnormalities. The results of routine hematologic and blood chemistry studies were all within normal limits. Carbamazepine and gabapentin were given, but her symptoms persisted. Her pain was temporarily relieved only by narcotic pain medication. MRI showed an arachnoid cyst located in the right cerebellomedullary cistern extending to the cerebellopontine cistern. Cyst removal was performed via a right retrosigmoid approach. Lateral suboccipital craniotomy was performed using the right park-bench position. After opening the dura and cerebellopontine angle, the arachnoid cyst was exposed. The arachnoid cyst was compressing the flattened lower cranial nerves at the right jugular fossa. Her symptoms resolved postoperatively. Two months after the operation, she was completely free from her previous symptoms.
