A Case of Extraadrenal Pheochromocytoma Showed a Transient Oral Captopril Test Positive Result (Captopril 검사 양성반응을 보인 부신외 갈색세포종 (Extraadrenal Pheochromocytoma) 1예)

  • There have been some case reports of the coexistence of extraadrenal pheochromocytoma and renal artery stenosis. Some reporters proposed that their coexistence may be associated through a common pathophysiological mechanism mediated by catecholamine secretion. Also some noted that trasient renal artery stenosis due to a spasm was induced by the catecholamines released from pheochromacytoma. We report a case of left paraaortic extraadrenal pheochromacytoma that had a transient oral captopril test positive result. After 5 days ${\alpha}$-antagonist reduced the vasospastic response of catecholamines. After surgical removal of the tumor, plasma catecholamines and urinary vanillyhnandelic acid concentrations as well as the blood pressure level, were restored to normal.

Urgent Intracranial Carotid Artery Decompression after Penetrating Head Injury

  • We describe a case of intracranial carotid artery occlusion due to penetrating craniofacial injury by high velocity foreign body that was relieved by decompressive surgery. A 46-year-old man presented with a penetrating wound to his face. A piece of an electric angular grinder disc became lodged in the anterior skull base. Computed tomography revealed that the disc had penetrated the unilateral paraclinoid and suprasellar areas without flow of the intracranial carotid artery on the lesion side. The cavernous sinus was also compromised. Removal of the anterior clinoid process reopened the carotid blood flow, and the injection of glue into the cavernous sinus restored complete hemostasis during extraction of the fragment from the face. Digital subtraction angiography revealed complete recanalization of the carotid artery without any evidence of dissection. Accurate diagnosis regarding the extent of the compromised structures and urgent decompressive surgery with adequate hemostasis minimized the severity of penetrating damage in our patient.

Successful Treatment of Lead Poisoning in a Whooper Swan (Cygnus cygnus) and a Cinereous Vulture (Aegypius monachus) in Korea

  • We described the successful treatment of cases of lead poisoning in a Whooper swan (Cygnus cygnus) and a Cinereous vulture (Aegypius monachus) in Korea. The swan and vulture rescued were diagnosed with lead poisoning based on clinical signs, the presence of suspected lead-like particles in the ventriculus detected by radiography and elevated blood lead levels. Endoscopic retrieval for the swan and gastric lavage for the vulture were used to remove the lead respectively. After the removal of lead particles, a course of chelation treatment was administrated. This would be the first report of successfully treating wild birds with lead poisoning in Korea.

Mycotic Abdominal Pseudoaneurysm due to Psoas Abscess after Spinal Fusion

  • A 36-year-old man, who had undergone thoracoscopic anterior spinal fusion using the plate system and posterior screw fusion three months previously, presented to our hospital with left flank pain and fever. Computed tomography indicated the presence of a psoas muscle abscess. However, after two days of percutaneous catheter drainage, a mycotic abdominal aortic pseudoaneurysm was detected via computed tomography. We performed in situ revascularization using a prosthetic graft with omental wrapping. Methicillin-resistant Staphylococcus aureus was identified on blood and pus culture, and systemic vancomycin was administered for one month. Although the abscess recurred, it was successfully treated with percutaneous catheter drainage and systemic vancomycin administration for three months, without the need for instrumentation removal. The patient remained asymptomatic throughout two years of follow-up.

  • A 54-year-old male farmer residing in Chunchon, Korea. complaining of blood tinged discharge and tinnitus in the left ear for two days. was examined in August 16, 1996. Otoscopic examination revealed live maggots from the ear canal. The patient did not complain of any symptoms after removal of maggots. Five maggots recovered were identified as the third stage larvae of Lucilia sericata (Diptera: Calliphoridae). This is the first record of aural myiasis in Korea.

  • Essential metabolic functions have been identified for seven macrominerals (calcirum, phosprorus, magnesium, sodium, potassium, chloride, and sulfur), and eight microminerals (cobalt, copper, iodine, iron, manganese, molybdenum, selenium, and zinc). Major functions for each of these minerals are summarized. Considerable research suggests that chromium is also essential and that it functions by facilitating insulin activity. Studies are reviewed which indicate that chromium supplementation of animal diets may: 1) increase glucose removal from blood, 2) reduce carcass fat and increase lean in nonruminants, 3) alter egg cholesterol content, and 4) enhance immunity and disease resistance in ruminants. A number of other minerals including nickel, boron, vanadium, arsenic, silicon, lithum, and lead have been reported to be essential, but specific metabolic functions have not been defined for any of these elements. Limited research in poultry suggests that boron may be of practical significance in some instances.

  • The asymptotic solution using the Tailor series has been given explicit form for the solute concentration and overall solute removal in hemodiafilter using one dimensional model. The numerical solutions have been calculated within 0.001% error by the Romberg integration method. Compared with the numerical solutions, the oneterm asymptotic solutions were found to be within 3% error for the condition > 3.0 and three-terms asymtotic solutions were required for the condition >0.7 where denotes measure of convection over diffusional transport and a the ratio of blood flow rate over dialysate flow rate.

Intrathoracic Goiter (흉곽내 갑상선종 1예 보고)

  • Substernal goiter may be defined as any thyroid enlargement that has 50 to 100 % of its mass inferior to the thoracic inlet. Ectopic substernal goiters are rare, and most substernal gaiters arise from cervical thyroid gland. Fifteen to fifty percent of these patients are asymptomatic. Symptoms, when present, are usually the result of tracheal or esophageal compression. Symptoms are often positional. Standard chest roentgenograms are often diagnostic, but computed tomographic or radioactive iodine scans may be helpful. In symptomatic patients or those in whom explorations are undertaken for diagnostic purpose or exclude carcinoma, surgical removal is indicated. Although cervical thyroids with substernal extension may be safely and successfully removed through a cervical incision, primary substernal goiters by definition derive their blood supply from within the thorax and are better approached by splitting the sternum or through a posterolateral thoracotomy. Recently we experienced a child fist sized secondary posterior mediastinal goiter in 55-year old female. The mass was completely removed through right posterolateral thoracotomy without any complications. The postoperative courses were uneventful.

  • Mucoepidermoid carcinoma is an uncommon lesion that accounts for approximately 1% of primary malignant bronchial gland tumors and less than 0.2% of all lung neoplasm. This tumor presents with symptoms of bronchial irritation or obstruction. Distant metastasis is uncommon, therefore complete surgical resection is the treatment of choice. The prognosis of tumor correlates with on the histologic grade of tumor. We experienced mucoepidermoid carcinoma in a 15 year-old girl with symptoms of cough and blood tinged sputum. The patient underwent successful removal of tumor by bilobectomy via explorothoracotomy after chest CT and bronchoscopic biopsy.

Vascular Remodeling with a Microvascular Anastomotic Coupler System: A Case Report

  • Despite increased utilization of microvascular anastomotic coupler (MAC) devices, the consequences have yet to be fully explored in terms of vascular regeneration. Removal of an exposed venous coupler is described herein, documenting normal circulatory flow through the remodeled site of application. A 25-year-old man who underwent open reduction and rigid fixation elsewhere for traumatic calcaneal fracture ultimately presented with a necrotic postoperative wound. The debrided defect was treated by free thigh perforator flap, incorporating a MAC device. Three months later, the flap remained viable, but the MAC itself was exposed. Structural integrity of the vessel and blood flow were sustained as the device was carefully removed, confirming true vascular remodeling in this example of MAC usage.
