  • According as society looks graying trend gradually, more shaped system that can achieve measuring health-state of old people more harmoniously construction required in field of old people's welfare and medical treatment. Health-state is measured by two methods of contact and non-contact. The first, for example measurement of blood pressure or electrocardiogram, requires that measuring equipments are attached on human body but the second, for example X-Ray or MRI, is not. But both of methods are have some of defect, for example attaching equipments, needing of the special equipments or the necessary time, etc. Therefore desirable method of monitoring system must have minimum interrupt about daily life. This study suggest the system that can monitor the user, especially old people's outward aging and health-state by use the PAN TILTER and CCD camera.

  • Typically, the vital signs that are representing the state of human body, are the body temperature, sphygmus, respiration and blood pressure. The body temperature is the result of metabolic regulation and human steady-state body temperature is maintained from 35.9 to $37.4^{\circ}C$ by heat regulatory center. The body temperature is indicative of infection and especially it should be monitored to requiring intensive care patients or after surgical patients. But, measuring of body temperature to a heavy workload on nursing staff has been recognized. And, the health service of nurse is limited by simple tasks such as the measurement and record of vital sign. In this paper, the body temperature monitoring telemetry system was proposed to prove the recoding and transmission of body temperature patch system according the standard(ISO TS11073-92001). We proposed the transmission protocol to suit the MFER(medical waveform format encoding rules). The telemetry patch system was implemented and it was verified by experiments.

  • Pulsus paradoxus has been defined as a decrease in systolic blood pressure (SBP) of 10 mmHg or more during inspiration. This report describes pulsus paradoxus detected by pulse oximetry during dental procedure. Case: A 10 years old boy who had impacted mandibular premolar with malformation scheduled for extraction under intravenous sedation with Fentanyl and Propofol. The patient showed upper airway obstruction with stridor and pulsus paradoxus. Though pulsus paradoxus is generally critical condition, in this case, respiration and other vital sign was maintained comparatively well with care in administering oxygen and considerate monitoring of pulse oximetry and capnography. Discussion: Noninvasive continuous monitoring of pulse oximetry allows recognition of pulsus paradoxus which can lead to serious problems. Clinicians should know very well about it and be able to manage of this kind of situation.

  • Recently the number of the elderly who live alone are increasing more and more as the average life span is prolonged. The elderly are probably in danger at home without being helped due to external aggressions or sudden health problems. Accordingly, more and more interests are taken in medical welfare for the healthy life of the seniors. In this paper, we have developed a mobility and vitality signal monitoring system based on ZigBee-PSTN gateway for the elderly. This combination of ZigBee wireless sensor network and PSTN can be easily established even in the poor internet infrastructure as is usually common for the elderly, with the advantage of providing non-constrained monitoring feature. The research result can be extended to the future tele-medicine system.

  • Background: The existing data indicate that obstructive sleep apnea syndrome contributes to the development of cardiovascular dysfunction such as systemic hypertension and cardiac arrhythmias, and the cardiovascular dysfunction has a major effect on high long-term mortality rate in obstructive sleep apnea syndrome patients. To a large extent the various studies have helped to clarify the pathophysiology of obstructive sleep apnea, but many basic questions still remain unanswered. Methods: In this study, the influence of obstructive sleep apnea on systemic blood pressure, cardiac rhythm and urinary catecholamines concentration was evaluated. Over-night polysomnography, 24-hour ambulatory blood pressure and ECG monitoring, and measurement of urinary catecholamines, norepinephrine (UNE) and epinephrine (UEP), during waking and sleep were undertaken in obstructive sleep apnea syndrome patients group (OSAS, n=29) and control group (Control, n=25). Results: 1) In OSAS and Control, UNE and UEP concentrations during sleep were significantly lower than during waking (P<0.01). In UNE concentrations during sleep, OSAS showed higher levels compare to Control (P<0.05). 2) In OSAS, there was a increasing tendency of the number of non-dipper of nocturnal blood pressure compare to Control (P=0.089). 3) In both group (n=54), mean systolic blood pressure during waking and sleep showed significant correlation with polysomnographic data including apnea index (AI), apnea-hypopnea index (AHI), arterial oxygen saturation nadir ($SaO_2$ nadir) and degree of oxygen desaturation (DOD). And UNE concentrations during sleep were correlated with AI, AHI, $SaO_2$ nadir, DOD and mean diastolic blood pressure during sleep. 4) In OSAS with AI>20 (n==14), there was a significant difference of heart rates before, during and after apneic events (P<0.01), and these changes of heart rates were correlated with the duration of apnea (P<0.01). The difference of heart rates between apneic and postapneic period (${\Delta}HR$) was significantly correlated with the difference of arterial oxygen saturation between before and after apneic event (${\Delta}SaO_2$) (r=0.223, P<0.001). 5) There was no significant difference in the incidence of cardiac arrhythmias between OSAS and Control In Control, the incidence of ventricular ectopy during sleep was significantly lower than during waking. But in OSAS, there was no difference between during waking and sleep. Conclusion : These results suggested that recurrent hypoxia and arousals from sleep in patients with obstructive sleep apnea syndrome may increase sympathetic nervous system activity, and recurrent hypoxia and increased sympathetic nervous system activity could contribute to the development of cardiovascular dysfunction including the changes of systemic blood pressure and cardiac function.

Convergence study on Effects of Music Therapy in Patients Undergoing Prostatectomy with Spinal Anesthesia (척추마취 전립선절제술환자의 음악요법효과에 대한 융합연구)

  • The purpose of this convergence study was to identify the effects of favorite music therapy on anxiety, fatigue, and vital signs of patients undergoing prostatectomy with spinal anesthesia. This study used a nonequivalent control group design. A sample of 45 patients was included. The experimental group was given music therapy during operation. The data were collected using a structured questionnaire and monitoring at 30 min before operation, at 20 min and 40min undergoing operation, and at arrival recovery room after operation. Data were analyzed using descriptive statistics, ${\chi}^2-test$, Fisher's exact test, t-test, repeated measures ANOVA. The experimental group reported significantly lower anxiety and lower fatigue than the control group(p=.001; p=.020). However there were no significant differences in the systolic blood pressure, diastolic blood pressure and pulse rate between groups(p=.821; p=.473; p=.782). This findings indicate that the tailored favorite music therapy can be an effective nursing intervention for patient undergoing prostatectomy with spinal anesthesia to reduce anxiety and fatigue related to operation.

Effect of Desexualization Care guided by Dramaturgical Interaction on Women's Embarrassment during Cervical Cancer Screening (연극학적 상호작용에 의한 탈성화 간호가 자궁경부 세포진 수검 여성의 당혹감에 미치는 효과)

  • Purpose: The aim of this study was to examine the effect of Desexualization Care guided by dramaturgical interaction on women's embarrassment during cervical cancer screening. Method: This study was carried out in a nonequivalent control group non-synchronized post-test only design. 62 women who had cervical cancer screening were' conveniently recruited from a university hospital health promotion center. Embarrassment was measured under four distinctive sub-dimensions by the method of self-reported questionnaire and blood pressure and pulse rates monitoring. The data of control group had a conventional pap smear were collected in advance and then those of experimental group were gathered after completing data collection in the control group. Women in experimental group were provided with a newly developed cervical cancer screening programme in which interdisciplinary team conducted dramaturgical interaction. Result: There was no significant difference in the scores of VAS between the two groups. The score of physiological response of Embarrassment Measurement Scale was significantly lower in experimental group than in the control group (p <.05), while no significant difference was found in cognitive-emotional, non-verbal and verbal behavioral responses between the two groups. There was also no significant difference in blood pressure and pulse rates between the two groups during cervical s creening. Conclus ion: Desexualization Care guided by dramaturgical interaction during cervical screening was found to have positive effect on physiological response of women's embarrassment. Further research for identifying other main variables which might have influenced on women's embarrassment is needed.

  • The thesis presents a system that continuously collects the human body's physiological vital information at rest with sensors and ICT information technology and predicts diabetes using the collected information. it shows the artificial neural network machine learning method and essential basic variable values. The study method analyzed the correlation between heart rate measurements of BCG and ECG sensors in 20 DM- and 15 DM+ subjects. Artificial Neural Network (ANN) machine learning program was used to predictability of diabetes. The input variables are time domain information of HRV, heart rate, heart rate variability, respiration rate, stroke volume, minimum blood pressure, highest blood pressure, age, and sex. ANN machine learning prediction accuracy is 99.53%. Thesis needs continuous research such as diabetic prediction model by BMI information, predicting cardiac dysfunction, and sleep disorder analysis model using ANN machine learning.

A Comparative Study on Knowledge of Hypertension and Blood Parameters Between a Well-Controlled Hypertension Group and an Uncontrolled Hypertension Group Commuting to a Public Health Center (보건소의 혈압이 조절되는 고혈압 환자와 조절되지 않는 고혈압 환자의 고혈압관련 지식 및 혈액성상)

  • The purpose of this study was to compare knowledge on hypertension and blood parameters through blood pressure control of hypertensive patients commuting to a public health center. Hypertensive subjects were classified according to their blood pressures into a well-controlled group (SBP<140 mmHg and DBP<90 mmHg, n=44) and an uncontrolled group ($SBP\geq140mmHg\;or\;DBP\geq90mmHg$, n=41). All subjects were interviewed for clinical characteristics, effort for health maintenance and knowledge on hypertension and nutrition. Biochemical measurement of blood urea nitrogen, creatinine, triglyceride, total cholesterol, HDL-cholesterol and fasting blood glucose (FBG) were obtained from subjects. The knowledge about hypertension and effort for health maintenance were not significantly different between the groups. However, the nutrition knowledge score of the well-controlled group was significantly higher (p<0.05) than that of the uncontrolled group. On the test of nutrition knowledge, 97.7% of the well-controlled group answered correctly to the item, 'Medication is an easy way to control blood pressure'. This result was significantly higher (p<0.05) than that of the uncontrolled group of whom 82.9% answered correctly. Participants in both groups had only a little understanding about the importance of a balanced diet. Blood parameters were in the normal range and there was not a significant difference between the groups. However total cholesterol, triglyceride and fasting blood glucose levels of the uncontrolled group were higher than those of the well-controlled group. Therefore, education programs about the continuous control of hypertension through careful monitoring, medication, and exercise are required. The uncontrolled group was especially in need of strengthening education. Also more effective nutrition education programs about low salt, high fiber, low cholesterol, and balanced diets are required for hypertensive patients commuting to public health center.

A Case of Recurrent Ventricular Tachycardia after Pimozide and Haloperidol Overdose (Pimozide와 Haloperidol 과량 복용 후 반복적으로 발생한 심실 빈맥 1례)

  • Pimozide and haloperidol are typical antipsychotics. They share a similarity in pharmacotherapeutic and adverse effect profiles. Cardiovascular effects may be seen as alterations in heart rate, blood pressure, and cardiac conduction. Conduction disturbances may occur ranging from asymptomatic prolongation of the QT interval to fatal ventricular arrhythmia. So in the case of anti psychotics overdose, the patient must be carefully monitored by continuous electrocardiography (ECG). We experienced a 34-year-old woman of schizophrenia with recurrent ventricular tachycardia after pimozide and haloperidol overdose. Initially she was slightly drowsy, however her ECG showed normal sinus rhythm. After 6 hours on emergency department entrance, her ECG monitoring showed ventricular tachycardia and we successfully defibrillated. There were five times events of ventricular arrhythmia during the in-hospital stay. She was discharged 5 days later without any other complications.
