Synthesis and Biocompatibility Study of Hydrogel for Patch Sensor in Non-invasive Glucose Monitoring System (무채혈 혈당 측정시스템의 Patch Sensor용 수화젤의 합성 및 생체적합성에 관한 연구)

  • This study aims to verify for humans the suitability of the enzyme-fixed hydrogel used for the patch sensor of the blood sugar testing system without blood sampling, which utilizes reverse iontophoresis. Using acrylate monomers, hydrogel was synthesized to which a certain unit of enzyme is fixed. In order to analyze the material property of the synthesized hydrogel, a structural analysis was performed using FT-IR spectroscopy, while the DSC was used to verify the thermal stability. In addition, with the UV-Vis spectrophotometer, it was verified that the degree of active enzyme is at least 50% greater than the standard product. The SEM was used to verify secure fixation of the enzyme onto the surface. As a result, it was observed that the enzyme is successfully fixed to the surface. Since the hydrogel makes direct contact with a patient's skin, it is essential to evaluate the toxicity when making direct contact with the skin. For that purpose, various sets of tests were undertaken according to the ISO 10993-cytotoxicity, intracutaneous reactivity, skin irritation test and maximization sensitization. Consequently, it was successfully verified that the enzyme-fixed hydrogel have bioavailability.

Spontaneous Intracranial Hypotension : Clinical Presentation, Imaging Features and Treatment

  • Objective : In the present study, the authors investigated the clinical and imaging features as well as the therapeutic outcomes of SIH (spontaneous intracranial hypotension) patients. Methods : A retrospective review of 12 SIH patients was carried out. The diagnostic work-up included lumbar tapping and measurement of CSF opening pressure, radioisotope cisternography, brain and spinal magnetic resonance imaging (MRI), and computed tomography (CT) myelography. Autologous epidural blood patching was performed in patients who did not respond to conservative therapies, including analgesics, steroids, hydration and rest. Results : Typical postural headache was found in 11 (91%) patients. Nine (75%) patients showed pachymeningeal enhancement on their initial T1-weighted MR images. The CSF opening pressure was less than 60 mm$H_2O$ in 9 of 11 patients. Autologous epidural blood patching was performed in 7 patients, and all of them showed good responses. Conclusion : SIH can present with various clinical presentations and neuroimaging findings. Autologous epidural blood patching is thought to be the treatment of choice for patients with SIH.

Pulmonary Atresia with Intact Ventricular Septum (둔형 폐동맥 폐쇄증 1)

  • Pulmonary atresia with intact ventricular septum is a extremely rare congenital cardiac anomaly. With the history of cyanosis and failure to thrive, this anomaly should not be excluded and emergency management is necessary. Our patient was 69 day-old male with pulmonary atresia which was confirmed by cardiac angiography. Prostaglandin E1 was used for maintenance of pulmonary blood flow preoperatively. Right ventricular outflow reconstruction with pericardial patch and concomitant pulmonary valvotomy were done on beating heart for palliation. With this method, growing of right ventricle and tricuspid annulus are highly expected.

  • We experienced 2 years and 5 months old male patient with partial anomalous pulmonary venous return of the left lung into the coronary sinus without atrial septal defect. After incising the atrial septum and the wall between the left atrium and the coronary sinus, we made the roof of the coronary sinus and closed the artificial atrial septal defect, with using patch, then we could change the direction of the blood flow from the coronary sinus into the left atrium. The patient was discharged on the 13th postoperative day after uneventful postoperative course.

Repair of the Coarctation of the Aorta Using the Subclavian Artery as a Flap and Preservation of Arterial Blood Flow to the Left Arm (상지혈류를 보전한 쇄골하동맥피판 대동맥성형술)

  • From April 1990 through June 1990, three patients underwent subclavian flap aortoplasty for relief of the coarctation of the aorta. The age of the patients were 13 days, 7 months and 39 months and their weights were 3.3kg, 6.5kg, and 11kg, respectively. Two patients had persistence of the ductus arteriosus and all patients had associated intracardiac anomalies. We used the technique devised by Mendonca, namely, repair of the coarctation of the aorta using the subclavian artery as a flap and preservation of the arterial blood flow to the left arm. In one patient with long narrowing of isthmus, significant residual pressure gradient was remained by this technique and we added patch aortoplasty. There were no hospital deaths and follow-up over a one year period shows all patients in good condition.

  • Immunositmulation effects of the mice fed with the cell lysate of Lactobacillus plantarum isolated from Kimchi were studied. The mice group fed with cell lysate was different from the control group on the degree of immune responses, e.g. 1) proliferation of splenocytes and Peyer's patch cells, 2) production of nitric oxide (NO) by peritoneal macrophages, 3) production of intestinal secretory lgA (slgA), 4) variation of TNF-${\alpha}$ and IL-2 concentration in blood, 5) production of specific lgG against sheep red blood cells. A general enhancement in enteric and systemic immune responses was observed with a simple oral administration of immunostimulators. With the oral feeding of L. plantarum, not only the total amount of gut secretion antibody, but also the binding capacity of antibodies to the enteric microorganisms including L. plantarum was increased. These experimental results clearly showed that the oral feeding of immunostimulators gave multifunctional effects on the mucosal and systemic immune systems of mice.

  • Detection of cerebrospinal fluid leakage or exact localization of leakage site after spinal surgery is difficult on conventional imaging studies. We report two patients with surgery-related spinal CSF leakage detected on magnetic resonance (MR) myelography. They presented with severe headache after spinal surgeries, lumbar discectomy and excision of spinal meningioma, respectively. The sites of spinal CSF leakage in the patients were detected accurately on MR myelography, and the patients recovered from the postoperative CSF leakage after being treated with an epidural blood patch or reoperation. MR myelography may be effective in demonstrating the exact site of surgery-related spinal CSF leakage.

Autologous Epidural Blood Patch for the Treatment of Headache in Post-Dural Puncture -Report of 37 cases- (요부경막천자후 발생된 두통치료 -자가혈액봉합 37예-)

  • We performed the autologous epidural blood patch (AEBP) for the relief of headache and other related symptoms following dural punctures of the lumbar region during 8 years from 1981 to 1988. The total of 37 patients with the patching consisted of 9 cases in 3007 of spinal anesthesia, 12 accidental dural punctures in 4283 cases of lumbar epidural anesthesia, 12 cases in 4747 of epidural analgesia for back pain control, 3 cases of myelography and a case of diagnostic lumbar puncture. The headaches were relieved completely in 35 cases following the first AEBP and the remaining two were also relieved following the second AEBP. We think that the AEBP for postdural-puncture headache is the treatment of choice.

Effect of Propofol, an Intravenous Anesthetic Agent, on $K_{ATP}$ Channels of Pancreatic ${\beta}-cells$ in Rats

  • ATP-sensitive potassium channels ($K_{ATP}$ channels) play an important role in insulin secretion from pancreatic beta cells. We have investigated the effect of propofol on $K_{ATP}$ channels in cultured single pancreatic beta cells of rats. Channel activity was recorded from membrane patches using the patch-clamp technique. In the inside-out configuration bath-applied propofol inhibited the $K_{ATP}$ channel activities in a dose-dependent manner. The half-maximal inhibition dose (ED50) was $48.6{\pm}8.4\;{\mu}M$ and the Hill coefficient was $0.73{\pm}0.11.$ Single channel conductance calculated from the slope of the relationship between single channel current and pipette potential $(+20{\sim}+100\;mV)$ was not significantly altered by propofol $(control:\;60.0{\pm}2.7\;pS,\;0.1\;mM\;propofol:\;58.7{\pm}3.5\;pS).$ However, mean closed time was surely increased. Above results indicate that propofol blocks the $K_{ATP}$ channels in the pancreatic beta cells in the range of its blood concentrations during anesthesia, suggesting a possible effect on insulin secretion and blood glucose level.

  • To protect the patient's internal organs when a patient with cardiovascular disease occurs, it is important to reduce the elapsed time by providing emergency medical services. Decisions for conducting cardiopulmonary resuscitation are mainly made using the carotid palpation method, which directs the pulse of the carotid artery, which can diagnose the patient's condition according to one's own subject and cause cerebral blood flow to be blocked by excessive pressure in the carotid due to the weaken cardiopulmonary function. In this study, we developed a multimodal patch-type probe based on multi-channel ultrasound Doppler pairs and oxygen saturation measurement modules which can monitor cardiopulmonary functions. From the in-vivo experiments, the developed probe can be utilized as a novel tool that can increase the survival rate of cardiovascular disease patients by objectively monitoring the cardiopulmonary function of the patient quantitatively and promptly in an emergency situation.
