  • Purpose: To investigate whether or not acupuncture of GB34 produces a significant response of the modulation of somatomotor areas by functional magnetic resonance imaging (fMRI) study. Methods: The acupoint, GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MRI scanner, functional MR imaging of the whole brain was performed in 12 normal healthy subjects during two stimulation paradigms; acupuncture manipulation on GB 34 and sham points. This study investigates the activation of the motor cortex elicited by a soft and an intensified stimulation of GB 34. Three different paradigms were carried out to detect any possible modulation of the Blood Oxygenation Level Dependent (BOLD) response in the somatomortor area to motor stimulation through acupuncture. Results: Group analysis from seven individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. Conclusions: The present study shows that acupuncture fMRI study can be safely conducted in 3T MRI environment, and acupuncture stimulation in GB34 modulates the cortical activities of the soma- to motor area in human. The present findings may shed light on the CNS mechanism of motor function by acupuncture and form a basis for future investigations of motor modulation circuits in the stroke patients. • Objective : There is a rich literature confirming the default mode network found compatible with task-induced deactivation regions in normal subjects, but few investigations of alterations of the motor deactivation in patients with intracranial lesions. Therefore, we hypothesized that an intracranial lesion results in abnormal changes in a task-induced deactivation region compared with default mode network, and these changes are associated with specific attributes of allocated regions. Methods : Blood oxygenation level dependent (BOLD) functional magnetic resonance imaging (fMRI) during a motor task were obtained from 27 intracranial lesion patients (mean age, 57.3 years; range 15-78 years) who had various kinds of brain tumors. The BOLD fMRI data for each patient were evaluated to obtain activation or deactivation regions. The distinctive deactivation regions from intracranial lesion patients were evaluated by comparing to the literature reports. Results : There were additive deactivated regions according to intracranial lesions : fusiform gyrus in cavernous hemangioma; lateral occipital gyrus in meningioma; crus cerebri in hemangiopericytoma; globus pallidus, lateral occipital gyrus, caudate nucleus, fusiform gyrus, lingual gyrus, claustrum, substantia nigra, subthalamic nucleus in GBM; fusiform gyrus in metastatic brain tumors. Conclusion : There is increasing interest in human brain function using fMRI. The authors report the brain function migrations and changes that occur in patients with intracranial lesions.

  • Purpose: This study is to compare the distinct brain activation between implicit and explicit memory retrieval tasks using a non-invasive blood-oxygenation-level-dependent (BOLD) functional magnetic resonance imaging(fMRI). Materials & Methods: We studied seven right-handed, healthy volunteers aged 21-25 years(mean;22 years) were scanned under a 1.5T Signa Horizon Echospeed MR imager(GE Medical Systems, Milwaukee, U.S.A.). During the implicit and explicit memory retrieval tasks of previously teamed words under the conceptual processing, we acquired fMRI data using gradient-echo EPI with 50ms TE, 3000ms TR, 26cm${\times}$26cm field-of-view, 128${\times}$128 matrix, and ten slices(6mm slice thickness, 1 mm gap) parallel to the AC-PC(anterior commissure and posterior commissure) line. By using the program of statistical parametric mapping(SPM99), functional activation maps were reconstructed and quantified.

  • Purpose: The purpose of this study was to identify cerebral cortices related with sexual arousal fro visual sexual stimulation in healthy males using BOLD-based functional MR imaging Method: Sixteen male volunteers with sexually potent(mean age:24) were examined for thi study. Functional MRI was performed on a 1.5T MR scanner(GE Signa Horizon) with birdcage-type head coil. In this study, blood oxygenation level dependent(BOLD) technique was utilized to create fMR image reflecting local brain activities. The BOLD-based fMRI d were obtained from 7 oblique planes using gradient-echo EPI with $90^{\circ}$flip angle, 50ms TE 6000ms TR, $26cm{\times}26$ cm FOV, $128{\times}128$ matrix, and 10mm slice thickness. The sexual stimulation paradigm consisted of two alternating periods of rest and activati and it began with a 1 minute rest, followed by a 2 minute stimulation by a documentary a erotic video film. Brain activation maps were generated by cross-correlation of imag acquired during rest and activation periods. The index of activation was used to compare t number of pixels activated by each task in each volunteer, where the significance of th differences was evaluated by using Students t-test.

  • Recently, functional MRI has been used to investigate the neurobiological mechanisms of acupuncture and the specificity of acupoint. The group data tend to be regarded as more important than the individual data in the most of previous studies. This study was designed to investigate the effect of the variability of individual data on the group results. A functional MRI (fMRI) of the whole brain was performed in fifteen healthy subjects during placebo and acupuncture stimulations at the ST36 acupoint. After remaining at rest for 30 seconds, the acupuncture was inserted and twisted at the rate of 2 Hz for 45 seconds and then the acupuncture was removed immediately. This process was repeated three times. Individual and group analyses were performed by voxel-based analyses using SPM2 software. Visual inspections of the activation and deactivation maps from individual sessions have shown the large variability across fifteen subjects. This means that the group data reflected the brain activation responses of only a few subjects. We suggest that the individual data should be presented to demonstrate the effect of acupuncture.
