Clinical Test of prototype Oxygen Concentrator (국산 산소농축기 시제품의 임상시험)

  • Background : Oxygen concentrators are convenient to operate and economical for patients with chronic obstructive pulmonary disease (COPD). However, oxygen concentrators are not manufactured domestically and the COPD patients are currently treated with imported oxygen concentrators. To evaluate the efficacy and safety of domestically developed prototype oxygen concentrator before clinical application, the efficacy and safety of the domestic oxygen concentrator were evaluate by comparing with the imported one. Material and Methods : The clinical tests were performed on 36 hyperhydrosis patients from April 1999 to August 1999. Domestic and imported oxygen concentrators were in turn applied to the same patient, who inspired oxygen for 60 minutes at a rate of 3 liters per minute through nasal prong. The oxygen concentrator, which was applied first, was randomly allocated. The arterial partial oxygen pressure ($PaO_2$) was estimated to compare the efficacy; and the carboxy hemoglobin(COHb), pH, arterial $CO_2$ partial pressure, pulse rate, blood pressure, and respiration rate to compare the safety before and after applying each oxygen concentrator. A student t-test was used to analyze the results. Result : In respect to efficacy, the difference in the change of $PaO_2$ before and after the application between two concentrators was not statistically significant. In respect to safety, the differences in the changes of COHb, pH, partial pressure of arterial $CO_2$, pulse rate, blood pressure, respiration rate between two concentrators were also not statistically significant. Conclusion : The domestically developed oxygen concentrator, showed satisfactory efficacy and safety when compared with the imported one.

Management of Diabetic Mellitus in Low-income Rural Patients (일개 농촌지역 저소득층 당뇨병환자의 관리 상태)

  • Objectives : Knowledge about the management status of diabetic melitus (DM) is essential to improve diabetic management. Moreover, low income is associated with poor adherence to treatment and increased mortality. This study was performed to evaluate the management status of DM in low-income patients in a rural area. Methods : We enrolled 370 patients with type 2 DM living in Gokseong county, JeollaNamdo. A well-trained examiner measured the height, weight, waist circumference, blood pressure, total cholesterol, triglyceride, high density lipoprotein cholesterol, fasting blood sugar and glycosylated hemoglobin (HbA1c) levels. Carotid ultrasonography was used to measure carotid artery carotid artery intima media thickness (IMT) and plaque. anklebrachial index (ABI) was used to evaluate peripheral artery disease. A fundoscopic examination was performed to evaluate diabetic retinopathy. A history of diabetes complications and health-related questionnaires were also completed. Results : The age of diabetic subjects was 68.7$\pm$8.7 years and the duration of diabetes was 8.9$\pm$8.2 years. Most (63.5%) had hypertension, and 45.7% had triglycerides below 150 mg/dl, 38.1% had low density lipoprotein cholesterol (LDL) cholesterol below 100 mg/dl, 48.7% had urine albumin to creatinine ratio (UACR) below 30 mg/g. Less than half (45.9%) achieved the goal of HbA1c less than 7% suggested by the American Diabetes Association (ADA). 10.6% had peripheral vascular disease, 11.9% had retinopathy, and 60.8% had chronic kidney disease. Conclusions : DM management in low income patients is very poor and requires further work to improve.

Comparison of Physiological Variables by Age Group in Drinking Men (음주남성의 나이대별 생리학적 변수들의 비교)

  • This study was designed to investigate the differences of physiological variables by age group in alcohol-consuming men. The thirty year old-age group had the lowest waist-hip ratio (WHR), right and left cardio-ankle vascular index (R-CAVI and L-CAVI), right and left ankle-brachial index (R-ABI and L-ABI), amylase, high sensitivity C-reactive protein (hs-CRP), and creatine phosphokinase (CPK) levels, and the highest body weight (BW), interleukin-6 (IL-6), right and left intraocular pressure (R-IP and L-IP), hematocrit (Hct), hemoglobin (Hb), erythrocyte, red blood cells distribution width (RDW), alanine aminotransferase (ALT), total cholesterol (TCH), triglyceride (TG), and ferritin levels. The forty year old-age group was the highest in hs CRp and CPK levels, but the lowest in ferritin level. The fifty year old-age group possessed the highest WHR, diastolic blood pressure (DBP), R- and L-ABI, eosinophil, and amylase levels. The sixty year old-age group had the highest R- and L-CAVI, mean corpuscular volume (MCV), ALP, $\gamma$-glutamyltranspeptidase ($\gamma$-GTP), rheumatoid factor (RF), $\alpha$-fetoprotein (AFP) and prostate specific antigen (PSA) levels and the lowest BW, DBP, R-IP, L-IP, Hct, Hb, leukocyte, platelet, RDW, eosinophil, monocyte, ALT, amylase, TG, and CPK levels. These findings indicate that there may be differences of physiological variables depending on age group in alcohol-consuming men. Further studies should be focused on the physiological differences between alcohol-consuming men and women.

Study on the Relationship between Pro-Brain Natriuretic Peptide and Routine Blood Test Factors at a General Hospital in Gyeonggi-do (경기도 일개 종합병원에서 Pro-Brain Natriuretic Peptide와 통상적 혈액검사 인자간의 상관성 조사)

  • This study investigated retrospectively the correlation between the results of the N-terminal pro-brain natriuretic peptide (NT-proBNP) and a routine blood test using a hospital information system. The NT-proBNP is involved in the pathophysiology of heart failure. The results show that the relationship between age and NT-proBNP was significant (P<0.01) with a positive correlation (r=0.163). The peptide concentration showed a negative correlation between the total protein (r=-0.250) and albumin (r=-0.270), and a negative correlation between the erythrocyte count and hemoglobin and hematocrit (P<0.01). NT-proBNP had a positive correlation with neutrophils (r=0.227) and a negative correlation with lymphocytes (r=-0.236), showing significant results (P<0.01). NT-proBNP and creatinine showed a positive correlation (r=0.594, P<0.01), and it was the most influential factor according to multiple regression analysis (B=0.53, t=7.65). P<0.01). The concentrations of NT-proBNP and uric acid showed a positive correlation (r=0.180, P<0.05). Lactate dehydrogenase was observed as a factor affecting the NT-proBNP (B=0.20, t=3.28, P<0.01). This explanatory power had an influence of 43%. Therefore, the accurate test and related factors of the NT-proBNP have significant clinical value.

Watch and Wait in Lebanese Chronic Lymphocytic Leukemia Patients: How Relevant is it?

  • Background: Chronic lymphocytic leukemia (CLL) is the most frequent form of leukemia in Europe and North America, and it mainly affects older individuals. Many approaches are implemented in the management of CLL from a watch and wait (W&W) strategy to chemotherapeutic regimens. We here reviewed our clinical practice for the relevance of the W&W strategy in Lebanese CLL patients. Materials and Methods: A total of 95 patients with CLL diagnosed in four institutions in Lebanon, between 1992 and 2013, were selected and their files were reviewed. Characteristics of these patients were noted including age, sex, RAI and Binet scores, CBC values, presence of hepatomegaly or splenomegaly, performance of bone marrow biopsy or peripheral blood flux cytometry for diagnosis, adoption of W&W strategy, different chemotherapeutic regimens and the indications for treatment. Results: Some 38 patients (40%) diagnosed with CLL were women and 57 (60%) were men with a mean age of 65.1 years [36-89]. Of the total, 50.5%, 17.2%, 14%, 7.5% and 10.8% had an RAI score at diagnosis of 0, 1, 2, 3 and 4, respectively, while 65.6%, 17.2% and 17.2% had Binet scores of I, II and III. The mean lymphocyte count at diagnosis was $39885/mm^3$ [1596-290000], the mean hemoglobin level was 12.7 g/dl [6.2-17] and the mean platelet count was $191255/mm^3$ [14000-458000]. While 26.3% of patients with CLL had splenomegaly, only 7.4% had hepatomegaly. Some 33.7% had undergone a bone marrow biopsy, 66.3% flow cytometry of circulating blood and 5.3% a lymph node biopsy. Overall, the W&W was adopted in 62.4% (58) of patients with a mean duration of 37.7 months [3-216]. The W&W was used in 82.6%, 73.3%, 46.2%, 14.3% and 0% of patients having RAI scores of 0, 1, 2, 3 and 4, respectively, and, it was used in 80%, 46.7% and 6.25% with Binet scores of I, II and III. The most frequent indication for treatment was anemia and thrombocytopenia, accounting for 32.7% of cases. The most frequently used chemotherapeutical regimens were chlorambucil until the end of the last century and flufarabine-cyclophosphamide-rituximab during the last decade. Conclusions: This retrospective review of CLL clinical practice showed an important implementation of the W&W strategy with a long duration, especially in early stage cases with low RAI or Binet scores.

Antropometric and Health Status of the Elderly Women Attending a Health Promotion Program in an Urban Community (건강증진프로그램을 이용하는 도시지역 여자노인의 신체 및 건강수준)

  • This study was conducted to evaluate the health status of elderly women who attended in a Health Promotion Program of the Seo-gu Health Center in Daegu. The study subjects were 158 elderly women (over 65 years) in an urban community. The subjects were investigated by means of individual interviews using a questionnaire, Blood tests for analyzing their biochemical status were carried out. The average age of the study subjects was 70.9 $\pm$ 2.3 years. Of the subject group 79.1% ranged in age from 65 to 74 year and 20.9% were over 75 years. With respect to health related factors, 23.4% of subjects drank alcoholic beverages and 15.2% of subjects currently smoke.The prevalence with chronic diseases was 51.9%, and 26.6% of the subjects were healthy. The diseases most frequently reported as having been or being treated were arthritis (38.1%), hypertension (21.4%), and diabetes (17.9%). The average height of subjects was below the standard established in the Korean Recommended Dietary Allowances, while the average weight was close to the standard. The means of systolic and diastolic blood pressure were 130.2 mmHg and 71.9 mmHg, respectively. The mean serum albumin level was 4.5 mg/d$\ell$ and the value of hemoglobin and hematocrit was 12.5 mg/d$\ell$ and 0.7%, respectively. The mean serum cholesterol level was 207.1 mg/d$\ell$ and the mean triglyceride level was 187.7 mg/d$\ell$. The serum lipid levels were higher then in those reported in research. The serum cholesterol levels of 71.5% of subjects were within the normal range. The GOT and GTP levels were within the moderate range. In conclusion, the health status of the elderly who attended the Health Promotion Program in the Seo-gu Health Center were very average. However, it was necessary to prepare a health management program to deal with the serum lipids so as to establish and maintain good health. When we carry out the health promotion program in a community, individual program Of adequate to health status should be developed more.

Effects of Feeding Rate and Pellet Water-Soaking on Growth, Blood Components, and Histology of Olive Flounder Paralichthys olivaceus (배합사료의 수침과 공급량이 넙치(Paralichthys olivaceus)의 성장, 혈액 및 조직 성상에 미치는 영향)

  • Two consecutive feeding trials investigated the effects of feeding rate and pellet expansion by water-soaking on the growth performance, blood components and histology of olive flounder Paralichthys olivaceus. The first two experiments were carried out to determine the effects of pellet expansion and feeding rate. In the first experiment, growth performance, feed utilization and survival of fish were not significantly affected by pellet expansion for six weeks. There were no significant differences in hematocrit, hemoglobin, aspartate aminotransferase, alanine aminotransferase, glucose and total protein of fish fed the expanded pellet. However, whole-body lipid content of fish in the non-expanded group was significantly higher than that in the expanded group. Histological analysis of the anterior intestine revealed that fish in the expanded group had shorter and smaller mucous folds. These results indicate that pellet expansion had no beneficial effect in terms of growth performance, feed utilization and fish health. In the second experiment, weight gain and feed efficiency were significantly increased as feeding rate increased from 0 to 2.5% body weight per day (BW/d), but there were no significant differences in weight gain or feeding efficiency in fish fed the expanded pellet at ratios of 2.5% BW/d and satiation for three weeks. Plasma aspartate aminotransferase activity of fish fed the expanded pellet at a ratio of 2.5% was significantly lower than that of starved fish. Histological analysis of the anterior intestine revealed that fish in the 0% group had shorter mucous folds. Broken-line regression analysis suggested that the optimum juvenile olive flounder feeding rate was 3.5% BW/d during the low temperature season ($16-17^{\circ}C$).

A comparative study on iron deficiency anemia based on feeding patterns of nine-month-old infants (9개월 영아의 수유방법에 따른 철결핍빈혈에 대한 연구)

  • Purpose : This study was conducted to evaluate the red cell indices and frequency of iron deficiency anemia based on the feeding patterns of nine-month-old infants. Methods : Blood tests were performed on 253 nine-month-old infants, who visited Il Sin Christian Hospital for health check-ups from January to December 2007. Their parents answered telephonic questions regarding their feeding patterns and weaning foods. Results : Three infants groups were created according to feeding patterns before they started weaning foods. One group was exclusively breast-fed (48.6%), another had mixed feeding (27.3%), and the third had artificial milk feeding (24.1%). Red cell indices (hemoglobin, hematocrit, MCV, MCH) of the breast-fed group were comparatively lower than those of the other two groups (P<0.05). Twenty-five infants (9.9%) were diagnosed with iron-deficiency anemia. According to feeding patterns, the frequency of anemia was highest in the breast-fed group. Six infants who started weaning foods before six months of age (113 infants) were diagnosed with iron-deficiency anemia (5.3%), and nineteen who started after six months of age (140 infants) were diagnosed with iron-deficiency anemia (13.6%). Conclusion : When nine-month old infants visit hospitals for health check-ups, pediatricians must consider their feeding pattern and weaning foods histories, and then recommend screening blood tests for iron-deficiency anemia.

Factors affecting Diabetic Eye disease and Kidney disease Screening in Diabetic Patients (당뇨병 환자의 당뇨성 안질환 및 신장질환 합병증 검사 수검 여부에 영향을 주는 요인)

  • This study was undertaken to investigate factors that affect the assessment of complications in diabetic eye and kidney diseases. Data was obtained from the National Community Health Survey, 2017. The subjects included were 25,829 respondents who had been diagnosed with diabetes. Logistic regression analysis was applied to determine the factors affecting associated diabetic eye disease (fundus examination) and kidney disease (microalbuminuria examination) complications. The diabetic eye disease complication rate was 35.6%, and diabetic kidney disease complication rate was 39.8%. Complications arising due to diabetes were determined to be 35.6% for eye diseases and 39.8% for kidney related diseases. Ed. Notes: The original sentence is not very lucid. Hematologic Features of Children with Inflammatory Bowel Disease (염증장병 환아의 혈액학적 소견)

  • Purpose: The aim of this study was to evaluate hematologic features in Korean children with inflammatory bowel disease (IBD) in order to determine appropriate management strategies. Methods: We retrospectively analyzed the complete blood count (CBC) profiles and assessed the incidence of anemia in 25 children with IBD (Crohn disease, 16; and ulcerative colitis, 9). The correlation between hemoglobin (Hb) and duration of illness and the activity index of IBD were also investigated. Results: The incidence of anemia was 76% in IBD, 88% in Crohn disease, and 56% in ulcerative colitis. Leukocytosis and thrombocytosis were only found in Crohn disease (56% and 25%, respectively). No statistically significant correlation was observed between Hb and duration of illness in Crohn disease and ulcerative colitis or between Hb and the activity index in Crohn disease and ulcerative colitis. Conclusion: The incidence of anemia found in this study was slightly higher than in previous reports. Additionally, there was no correlation between Hb and the duration of illness or the activity index. Therefore, even if the duration of illness is short, or the activity index is low, appropriate management of children with IBD should include careful evaluation for anemia.
