일시적 혈류 제한과 함께 수행한 저강도 운동 후 알파운동신경원의 변화 (Changes of α-Motor Neuron Excitability after Low-Intensity Exercise with Transient Restriction of Blood Flow)

  • PURPOSE: Low-intensity exercise with transient restriction of blood flow to muscle could be an alternative rehabilitation method which avoids the problems associated with conventional high-intensity exercise. However, the mechanism of low-intensity exercise with transient restriction of blood flow is not clearly known. Thus, the purpose of this study was to investigate the mechanism of improvement of muscular function after low-intensity exercise with transient restriction of blood flow using H-reflex analysis. METHODS: Twenty one healthy young adults with no medical history of neurological or musculoskeletal disorder voluntarily participated in this study. The ${\alpha}$-motor neuron excitability of the triceps surae was assessed using the H-reflex. The amplitude of the M-wave and H-reflex were measured across three conditions: rest, after low-intensity exercise without restriction of blood flow and after low-intensity exercise with restriction of blood flow. The subjects performed low-intensity ankle plantar flexion exercise at their own pace for one minute without or with transient restriction of blood flow achieved by a sphygmomanometer cuff on popliteal fossa at a pressure of 120mm of mercury(120 mmHg). RESULTS: No significant changes of the excitability of the ${\alpha}$-motor neuron were obtained across three different conditions. CONCLUSION: This study found that low-intensity exercise with transient restriction of blood flow did not influence to ${\alpha}$-motor neuron excitability of the triceps surae. From the results, I could come to the conclusion that further study will be required.

20대의 혈류제한 저항운동이 위팔두갈래근 활성도, 피로도 및 혈역학적 변인에 미치는 영향 (Effect of Blood Flow Restriction Resistance Exercise in Twenties on Biceps Activity, Fatigue and Hemodynamic Variables )

  • PURPOSE: This study examined the effects of lowintensity resistance exercise combined with blood flow restriction on muscle activity and muscle fatigue to determine if such a combination may be an alternative to high-intensity resistance exercise in maintaining the muscle mass and strength and preventing degenerative loss of skeletal muscle and to provide basic data for presenting the effectiveness of exercise. METHODS: The interventions were provided for five weeks, four sessions a week, once a day, 60 minutes a session to Experimental group I (n = 13), in which low-intensity resistance exercise was applied by combining blood flow restriction with the biceps curl and experimental group II (n = 12), in which only high-intensity resistance exercise was applied. As a pre-test, the biceps brachii muscle activity and fatigue were measured by surface electromyography, and the hemodynamic variables, such as blood pressure and heart rate, were measured. The post-test was performed identically to the pre-test and compared and analyzed with the pre-test. RESULTS: A significant difference within-group was observed in the biceps brachii muscle activity and fatigue in experimental group I and only in biceps brachii activity in experimental group II. No significant differences were observed between the two groups. CONCLUSION: Since the low-intensity resistance exercise combined with blood flow restriction has similar effects to high-intensity resistance exercise, it is considered an alternative for improving muscle function in groups unable to perform high-intensity resistance exercise.

헬스케어증진을 위한 하지혈류제한운동이 20대 여대생의 순발력과 균형에 미치는 영향 (Effects of Blood Flow Restriction Exercise on the Alacrity and Balanced Capacity of Female University Students in Their 20s for Health-Care Increase)

  • 본 연구는 하지혈류제한벨트를 한 그룹과 하지 않은 그룹의 균형과 순발력의 변화를 알아보기 위해 광주 지역에 소재한 K대학교 학생 40명을 선정하여 실험군 20명과 대조군 20명을 무작위 배치하여 2019년 9월 16일부터 10월 11일까지 4주간 스쿼트 운동을 실시하였다. 스쿼트 운동 시 혈류제한은 BFR band를 착용하였고 편안하게 선 상태에서 120 mmHg 압력을 넣어 하지혈류제한을 하였다. 실험 결과 앞·중간·뒷부위 발바닥 압력의 시간에 따른 변화에서 두 그룹 모두 통계학적으로 유의한 차이가 나타나지 않았고(p>.05), 실험군과 대조군 간 상호작용 차이도 없었다(p>.05). Height, Maximum Concentric Power, Take Off Speed에서 시간에 따른 유의한 차이를 확인할 수 있었다(p<.05). 이와 같은 결과로 보아 낮은 강도 운동을 통해서도 하지혈류제한을 통해 큰 효과를 얻을 수 있을 것으로 사료된다. 본 논문은 혈류제한운동이 순발력과 균형에 미치는 영향을 알아보고자 하였으며 향후 하지혈류제한 운동을 통해 낮은 강도의 운동으로도 큰 운동 효과를 얻을 수 있는 것의 기초자료로 활용될 수 있을 것이다.

하지 혈류제한 운동이 근육크기와 근력에 미치는 영향 (The Effects of Leg Blood Flow Restriction Exercise on Muscle Size and Muscle Strength)

  • Purpose : The purpose of this study was to investigate thigh muscle-bone CSA and leg strength during low-intensity exercise program with leg blood flow restriction by external compression to reduce muscle outflow. Methods : Eighteen health students gave informed written consent to participate in this investigation. An occlusion cuff was attached to the proximal end of the leg so that blood flow was reduced during the training. The training was conducted one times a day, three times a week, for 8 weeks using one sets of 30 minutes. The training program performed to squat with standing, lunge with standing and heel raise with one leg standing. Measurements of thigh muscle-bone CSA(cross-sectional area) and leg strength were evaluated pre and post-training. Statistical evaluation of these data was accomplished utilizing a paired t-test by SPSS 12.0 program for windows. Significance level was set at p <.05. Results : All data are reported as means and standard deviations(SD) for all variables. The result of the study is followed; After the training, muscle-bone CSA, gluteus maximus m, quadriceps m, hamstring m of both legs were significantly improved but not calf muscle(p<.05). There was no significant difference of change quantity between muscle-bone CSA and leg strength in Lt. and Rt. side. But the variation in leg muscle strength of Rt. leg(dominant) was much more increased than Lt. leg(non-dominant) after 8 weeks training. Conclusion : Low-intensity training with leg blood flow restriction offers a potentially useful method for improving leg muscle strength.

Effects of Low Intensity Blood Flow Restriction Training on Brain Motor Area Activation

  • Purpose: The purpose of this study was to identify the effects of low intensity blood flow restriction training (LBFR) on the central nervous system of healthy adults. Methods: Ten healthy right-handed adults (eight males and two females, mean age of 28.6 ± 2.87 years) were selected as study subjects. Functional magnetic resonance imaging (fMRI) was conducted to measure brain activation (BA) following LBFR and non-LBFR. The primary motor area, premotor area, and supplementary motor area, which are closely related to exercise, were set as the regions of interest. Results: The BA recorded during the LBFR condition was 931.7 ± 302.44 voxel, and the BA recorded during the non-LBFR condition was 1,510.9 ± 353.47 voxel. Conclusion: BA was lower during LBFR than during non-LBFR.

헬스케어 증진을 위한 혈류조절 가압 운동의 효과에 대한 체계적 문헌고찰 ; 국내 연구 중심으로 (Systematic Review of the Effects of Blood Flow Exercise for Health-care Promotion: A Focus on Korean Domestic Research)

  • 본 연구의 목적은 혈류조절 가압운동에 참여한 환자들을 대상으로 헬스케어 증진을 위한 임상적 효과를 알아보고자 문헌들을 분석, 고찰하여 다양한 질환의 재활모델로 모색하는 데 그 목적을 두고 있다. 혈류조절, 혈류제한, 저강도 운동, KAATSU 등의 단어를 사용하여 2010년부터 2019년 10년간 연구된 국내학술 저널에 출간된 것을 바탕으로 문헌 연구를 실시하였다. 혈류조절 가압운동은 1966년 일본의 요사야키 사토 박사가 개발한 운동법으로 혈류조절 밴드를 활용해 운동을 실시하여 성장호르몬 분비를 증가시켜 단시간 내 근육을 발달시키고 혈액순환 및 신진대사를 개선하여 성인병 예방 및 개선, 재활기간 단축 및 심혈관계 기능을 증진 시키는 효율적이고 효과적인 운동법이다. 질환자를 대상으로 한 연구는 총 10편으로 노인 4편, 비만 4편, 뇌졸중 1편, 외상성 수부환자 1명이었다. 본 연구의 결과는 저강도 운동으로 고강도 운동의 효과를 얻을 수 있어 일반적인 운동이 어려운 고령자, 청소년 및 부상환자의 근육 발달 및 재활에 매우 효율적인 운동법이란 근거를 지지할 수 있다. 추후 연구에서는 혈류조절 부위에 따른 효과성 검증과 질환에 따른 고찰 연구가 필요할 것으로 사료된다.

가압훈련의 혈류 압박 정도에 따른 복합근 활동전위의 변화 (Changes in Compound Muscle Action Potential Depending on Pressure Level of Blood Flow During KAATSU Training)

  • Purpose: In recent years, there has been increasing interest in using blood flow-restricted exercise (BFRE) or KAATSU training. The KAATSU training method, which partially restricts arterial inflow and fully restricts venous outflow in the working musculature during exercise at reduced exercise intensities, has been proven to result in substantial increases in both muscle hypertrophy and strength. The purpose of this study was to investigate the proper level of pressure for KAATSU training using compound muscle action potential (CMAP) analysis. Methods: Twenty-two healthy adults voluntarily participated in this study. CMAP was conducted by measuring the terminal latency and amplitude using a motor nerve conduction velocity test. For reference-line, supramaximal electrical stimulation was applied to the median nerves of the participants to obtain CMAP for the abductor pollicis brevis. For baseline, the intensity of the electrical stimulation was decreased to a level at which the CMAP amplitude was about a third of the CMAP amplitude obtained by the supramaximal electrical stimulation. The pressure levels for the KAATSU were set as a systolic blood pressure (strong pressure), the median values of systolic and diastolic blood pressure (intermediate pressure), and diastolic blood pressure (weak pressure). In the KAATSU condition, CMAP was performed under the same conditions as baseline after low-intensity thumb abduction exercises were performed at the subjects' own pace for one minute. Results: As the pressure increased, the CMAP amplitude was significantly increased, signifying that more muscle fibers were recruited. Conclusion: This study found that KAATSU training recruited more muscle fibers than low-intensity exercise without the restriction of blood flow.

혈류제한 유산소운동 프로그램의 웰니스를 위한 효과검정 - 근활성도와 운동신경원을 중심으로 - (Effect for Wellness of Blood Flow Restriction Aerobic Exercise Program - Focusing on Mscle Ativity and Mtor Nurons -)

  • 본 연구는 정상인을 대상으로 건강을 위한 유산소 능력과 관계가 깊은 하지에 혈류제한 유산소 훈련을 시행함으로써 하지 근활성도와 운동신경원을 정량적으로 비교·분석하여 효과적인 혈류제한 운동프로그램의 효용성을 제시하는 기초자료를 제공하고자 한다. 압력 정도가 140 mmHg으로 혈류제한하여 트레드밀 위에서 유산소 운동을 적용한 집단 10명을 실험군I, 트레드밀 위에서 유산소 운동만 적용한 집단을 11명을 대조군으로 각각 무작위 배치한 후, 트레드밀에서 4주간, 주 3회, 1일 1회, 1회 30분간 중재 프로그램을 시행하였다. 중재 전 표면근전도를 활용하여 근활성도와 운동신경원을 측정하여 분석하였다. 연구 결과는 실험군I의 집단 내 전후 비교에서 넙다리곧은근, 넙다리두갈래근, 앞정강근 및 장딴지근의 근활성도가 유의하게 증가하였다(p<.001). 대조군의 집단 내 전후 비교에서는 넙다리곧은근, 넙다리두갈래근, 앞정강근 및 장딴지근의 근활성도가 유의하게 증가하였다(p<.001). 집단 간 변화 비교에서는 넙다리곧은근의 활성도가 유의한 차이가 있었다(p<.05). 하지 혈류제한과 병행하여 유산소운동을 접목하는 것은 엘리트선수 육성과 관절이 약한 노인 등 재활 트레이닝에 기능적인 활동을 회복시킬 수 있는 부상방지 운동프로그램 등으로 발전시킬 수 있을 것이며, 향후 연구에서 이러한 결과를 바탕으로 하여 정상인이 아닌 대상의 영역을 확대하고, 압력강도에 따른 다각적인 연구들이 필요할 것으로 사료된다.

Effects on Mechanical Properties, Joint Range of Motion, and Grip Strength of Forearm Muscles Depending on Wrapping Direction of the Floss Band

  • Objective: The purpose was to investigate changes in mechanical properties, range of motion, and grip strength of the forearm muscle in 13 students depending on the wrapping direction of the floss band. Design: A single-blind, crossover study Methods: Subjects were randomly assigned to the wrapping direction of the flossing band and then performed a concentric exercise of the wrist flexors using a Flex-Bar. Intervention A applied the flossing band in a right spiral direction, Intervention B applied the flossing band in a left spiral direction, and Intervention C performed the exercise alone. All subjects used their dominant right hand, and pre- and post-assessments were conducted between interventions. To analyze differences in changes between groups pre- and post-, all results were subjected to one-way ANOVA, followed by Scheffe's test as a post-hoc analysis. The paired samples t-test was used to analyze the difference between pre- and post-change within groups. Results: First, in the mechanical properties of the Flexor carpi ulnaris (FCU) muscle, interventions A and B significantly improved muscle tone and stiffness than intervention C (p<0.05), and intervention A showed a significant decrease in decrement (muscle elasticity) than intervention B (p<0.05). Second, interventions A and B showed significant improvement in grip strength than intervention C (p<0.05). Conclusions: The right spiral direction of the flossing band tended to increase the elasticity of the muscles compared to the left spiral direction. Therefore, in future studies, it is necessary to choose the direction of the flossing band to increase the elasticity of the muscles.
