흉부CT 검사 시 HRCT 영상 재구성의 유용성 (Usefulness Evaluation of HRCT using Reconstruction in Chest CT)

  • Purpose : Skip the repetitive HRCT axial scan in order to reduce the exposure of patients during chest HRCT scan, Helical Scan Data into a reconstructed image, and exposure of the patient change and visually evaluate the usefulness of the HRCT images. Materials and method : Patients were enrolled in the survey are 50 people who underwent chest CT scans of patients who presented to the hospital from January 2015 to March 2015. 50 people surveyed 22 people men and 28 people women people showed an average distribution of 30 to 80 years age was 48 years. 50 patients to Somatom Sensation 64 ch (Siemens) model with 120 kVp tube voltage to a reference mAs tube current to mAs (Care dose, Siemens) as a whole, including the lungs and the chest CT scan was performed. Scan upon each patient CARE dose 4D (Automatic exposure control, Siemens Medical Solution Erlangen, Germany) was to maintain the proper radiation dose scan every cross-section through a device that automatically adjusts the tube current of. CT scan is the rotation time of the Tube slice collimation, slice width 0.6 mm, pitch factor was made under the terms of 1.4. CT scan obtained after the raw data (raw data) to the upper surface of the axial images and coronal images for each slice thickness 1 mm, 5 mm intervals in the high spatial frequency calculation method (hight spatial resolution algorithm, B60 sharp) was the use of the lung window center -500 HU, windows were reconstructed into images in the interval -1000 HU to see. Result : 1. Measure the total value of DLP 50 patients who proceed to chest CT group A (Helical Scan after scan performed with HRCT) and group B (Helical Scan after the HR image reconstruction to the original data) compared with the group divided, analysis As a result of the age, but show little difference for each age group it had a decreased average dose of about 9%. 2. A Radiation read the results of the two Radiologist and a doctor upper lobe and middle lobe of the lung takes effect the visual evaluation is not a big difference between the two images both, depending on the age of the patient, especially if the blood vessels of the lower lobe (A: 3.4, B: 4.6) and bronchi(A: 3.8, B4.7) image shake caused by breathing in anxiety (blurring lead) to the original data (raw data) showed that the reconstructed image is been more useful in diagnostic terms. Conclusion : Scan was confirmed a continuous, rapid motion video to get Helical scan is much lower lobe lung reduction in visual blurring, Helical scan data to not repeat the examination by obtaining HRCT images reorganization reduced the exposure of the patient.

Effects of Controlled Photoperiod on Body Development in Growing Juvenile Rats

  • Melatonin is induced by light information through the retina and leads to growth factor activation. Thus, we investigated the effects of melatonin by controlling the photoperiod of growing young rats. Male Sprague-Dawley rats (n=6; 4 weeks old) were divided into two experimental groups: the L/D group (normal photoperiod; light/dark: 12/12 h; lights on at 9:00 a.m.) and the L/L group (light/light: 24 h). Rat body weight and food consumption were measured daily for 8 weeks. After 8 weeks, the rats were anesthetized with a mixture of ketamine (50 mg/kg) and xylazine (10 mg/kg) and sacrificed. Tissue was then collected for RNA isolation (from brain, heart, liver, kidney, adrenal gland, testis, tibia, hind limb muscles). Also, serum was isolated from blood using a centrifugal separation. The L/L group had significantly lower body weight than the L/D group from 4 to 6 weeks (p<0.05). The L/D group had increased tissue mass, compared with the L/L group, but the difference was not statistically significant. The L/D group had a significantly higher melatonin concentration than the L/L group between the hours of midnight and 2:00 a.m (p<0.01). These results indicate that photoperiod length may affect the secretion of melatonin from the pineal gland. Also, the reduction of nocturnal melatonin secretion may retard the development of growing young rats. In future studies, we plan to compare exogenous melatonin administration with endogenous melatonin concentration induced by photoperiod control. Moreover, we will confirm whether the effects seen in pathological animal models can be reversed by controlling the photoperiod.

Diagnostic Mutational Analysis of MECP2 in Korean patients with Rett syndrome

  • Purpose: Rett syndrome (RTT) is an X-linked dominant neurodevelopmental disorder affecting 1 per 10,000~15,000 female births worldwide. The disease-causing gene has been identified as MECP2 (methyl-CpG-binding protein). In this study, we carried out diagnostic mutational analysis of MECP2 gene in RTT patients. Methods: We analyzed four exons and putative promoter of MECP2 gene from the peripheral blood of 43 Korean patients with RTT by PCR-RFLP and direct sequencing. Results: Mutations were detected in MECP2 gene about 60.5% of patients. The mutations consisted of 14 different types including 9 missense mutations, 4 nonsense mutations and 1 frameshift mutation. Of these, three mutations (G161E, T311M, P385fsX409) were newly identified and these were determined as disease-causing mutations by PCR-RFLP and direct sequencing analysis. Most of the mutations were located within MBD (42.3%) and TRD (50%). T158M, R270X, and R306C mutations were identified with high frequency. An intronic SNP (IVS3+23C>G) was newly identified in only three of the patients. It may be a disease-related and Korea-specific SNP with RTT. The L100V and A201V have been reported to be unclassified variant and SNP. However, these mutations were not found in more than 100 normal Korean control samples. These base substitutions seem to be the disease-causing mutations in Korean RTT contrary to previous studies. Conclusion: Disease-causing mutations and polymorphisms would be very important for diagnosing of RTT in Korean. The experimental procedure used in this study might be considered for molecular biologic diagnosis used in clinical field.

당뇨병이 없는 비만 성인의 hs-CRP 수준과 FBG, Fructosamine 및 HbA1c와의 관련성 (Relationship of the hs-CRP Levels with FBG, Fructosamine, and HbA1c in Non-diabetic Obesity Adults)

  • 본 연구는 국민건강영양조사 자료를 이용하여 당뇨병이 없는 성인 4,734명을 대상으로 비만 성인의 hs-CRP와 혈당조절 지표들과의 관련성에 대해 알아보고자 하였다. 연구결과 BMI가 증가할수록 혈당조절지표인 FBG와 fructosamine 및 $HbA_1c$가 증가하였으며, hs-CRP는 비만 군에서 가장 높은 수치를 보였고, 인슐린저항성의 지표인 HOMA-IR도 비만 군에서 통계적으로 유의하게 증가하였다. 또한 hs-CRP 수준을 구분하여 혈당조절지표들과의 관련성을 확인한 결과 hs-CRP가 증가할수록 FBG, fructosamine 및 $HbA_1c$가 증가됨을 확인하였고, 여러 관련 변수를 보정한 후에도 hs-CRP가 증가할수록 FBG, fructosamine 및 $HbA_1c$가 통계적으로 유의하게 증가하였다. 결과적으로 비만과 hs-CRP, 혈당조절지표는 각각에서 연관성을 보여 당뇨병이 없는 성인 중 높은 BMI에서 증가된 hs-CRP에 의해 당뇨병의 주요한 병인인 인슐린 저항성을 증가시켜 혈당조절지표인 FBG, fructosamine 및 $HbA_1c$가 통계적으로 유의하게 증가한 것으로 사료된다. 이에 당뇨병이 없는 성인에서도 올바른 식습관 및 규칙적인 운동으로 비만을 예방하여 당뇨병 발생을 감소시키는 노력이 강구되어야 하겠다.

Treadmill 운동과 DNA 및 칡 Catechin 섭취가 흰쥐 생체내 지방조성과 항산화 활성에 미치는 영향 (Effect of the Treadmill Exercise and the Intake of DNA and Crude Catechin (from Puerariae thunbergiana Roots) on the Body Fat Composition and the Antioxidant Activity in Rats)

  • 본 연구는 운동부하와 DNA 및 칡 crude catechin의 급여에 의한 생체내 성분 변화를 알아보기 위하여 흰쥐를 대조군, 운동부하군, 운동부하+DNA 및 운동부하+DNA+crude catechin 첨가군으로 나누어 사육하였다. 각각 0.4% DNA와 0.1% 칡crude catechins을 해당 횐쥐에 급식시켰으며, 운동부하군은 Treadmill에서 20 m/min의 속도로 6주 동안 운동시켰다. 사육실험종료 후 체중 증체량과 체지방무게를 먼저 칭량한 후, 항산화활성과 체지방조성을 kit assay를 이용하여 측정하였다. 6주후 각 군간의 최종 체중에는 유의적인 차이가 없었으나 간장, 부고환주위지방 및 신장주위지방은 운동부하군, 운동부하+DNA 첨가군 및 운동부하+DNA+crude catechin 첨가군이 대조군에 비해 유의하게 낮은 값(p<0.05)을 나타냈다. 또, 혈중 phospholipid, cholesterol 및 triglyceride농도는 운동부하군, 운동부하+DNA 첨가군 및 운동부하+DNA+crude catechin 첨가군이 대조군에 비해 유의하게 낮았으나(p<0.05), HDL-cholesterol은 유의하게 높은 값을 나타냈다. GPT 활성은 각 군간에 유의차가 없었으나, GOT활성은 운동부하를 처리한 군에서 적은 수치를 나타냈으며, TBA가는 각 군간에 유의 차가 없었다. 또, 혈중 NEFA 수준은 운동부하+DNA+crude catechin 첨가군이 대조군에 비해 유의하게 높은 값을 나타냈다.(p<0.05). 이상의 실험에서 운동과 더불어 crude catechin 섭취가 유의한 차이를 나타내지는 않았지만 혈청지질의 성분의 향상에 보조역할을 할 것으로 사료된다.

손바닥선인장 보충이 고지방식이 흰쥐 골격근의 PPAR-γ 와 PGC-1α 단백질 발현 증가에 미치는 영향 (Opuntia humifusa Supplementation Reduces Fat Weight by Increasing PPAR-γ and PGC-1α Protein Expression in the Skeletal Muscle of Rats)

  • 본 연구는 안정 시 고지방식이 흰쥐의 골격근에서 PPAR-${\delta}$, PPAR-${\gamma}$ 그리고 PGC-$1{\alpha}$ 단백질 발현에 손바닥선인장 보충이 미치는 효과에 대하여 연구하였다. SD계 수컷 흰쥐 16마리를 무작위로 대조군(CG, n=8)과 실험군(EG, n=8)으로 분류하였다. 8주 동안 대조군은 고지방식이를 부하하였으며, 실험군은 5% 손바닥선인장을 보충식이하였다. 본 실험결과, 복부지방과 고환부 지방 중량은 EG군이 CG군에 비해 유의하게 낮게 나타났다(p<0.01). 또한 혈당, 중성지방, 총콜레스테롤의 농도도 EG군이 CG군에 비해 유의하게 낮게 나타났다(p<0.01). 한편, 골격근에서 PPAR-${\gamma}$와 PGC-$1{\alpha}$ 단백질 발현은 EG군이 CG군에 비해 유의하게 높게 나타났다(p<0.05). 이상의 결과로부터 손바닥선인장 보충이 고지방식이 흰쥐의 혈당과 중성지방 농도의 감소와 골격근에서 PPAR-${\gamma}$와 PGC-$1{\alpha}$ 단백질 발현을 증가시킴으로서 체지방을 감소시켜 체중증가 억제에 긍정적인 영향을 미치는 것으로 나타났다.

당삼 열수 추출물이 마우스의 체중 변화에 미치는 영향 (The Effects of an Aqueous Extract from the Dried Roots of a Codonopsis pilosula (Franch.) Nannf. (CP) on the Body-weight Gains of Mice)

  • 본 연구에서는, 만삼(Codonopsis pilosula)의 뿌리를 말린 한약재인 당삼을 마우스에 매일 장기간 투여할 경우 실험 동물의 체중 변화에 어떠한 영향을 미치는지 알아보고자 하였다. 당삼 열수 추출물(CP-W) 1 g/kg을 매일 1회씩 10주 동안 마우스에 경구 투여하였으며, 전체 실험 기간 동안 체중 및 사료 섭취량을 측정하였다. 실험 종료시점에서 마우스로부터 채혈하여 각종 혈액학적/혈청 생화학적 지표들을 분석하였으며, 안락사 후 부검을 실시하고 복부 내 장기들의 무게를 측정하였다. 그 결과, CP-W 투여군의 체중이 대조군에 비해 약 10% 가벼웠던 반면, 사료 섭취량은 군 간의 유의적인 차이가 없었다. 실험 기간 내내 어느 개체에서도 임상적으로 이상 징후가 관찰되지 않았으며, 부검 시 어떠한 이상 증상도 발견되지 않았다. 혈액 시료 분석 및 장기 무게 측정 결과 역시 어느 지표에서도 군 간의 차이를 보이지 않았다. 이상의 연구 결과들을 요약하면, CP-W 1 g/kg을 마우스에 매일 1회씩 10주 동안 경구 투여한 결과, 사료 섭취량의 변화 없이 체중 증가가 유의적으로 억제되었으며, 장기간 투여에 따른 뚜렷한 독성 증상은 발견되지 않았다.

노령 흰쥐의 뇌허혈 손상시 뇌대사활성의 변화 및 량격산화탕의 영향에 대한 [$^{14}C$2-Deoxyglucose Autoradiography 연구 (Alteration of Neural Activity and Effect of Yanggyuksanhwa-tang(Lianggesanhuo-tang) on Cerebral Ischemia of Aged BCAO Rats; [$^{14}C$]2-Deoxyglucose Autoradiography Study)

  • Objective : This study investigated the alteration of neural activity and effect of Yanggyuksanhwa-tang (Lianggesanhuo-tang) on cerebral ischemia of rats. Methods : Considering age-related impact on cerebral ischemia, aged rats (18 months old) were used for this study. Ischemic damage was induced by the transient occlusion of bilateral common carotid arteries (BCAO) with hypotension. Yanggyuksanhwa-tang (Lianggesanhuo-tang) was administered twice a day orally. Then alterations of neural activities in the brain of aged BCAO rats were measured by the [$^{14}C$]2-deoxyglucose autoradiography method. Results : The BCAO in aged rats led to significant decrease of neural activity in the whole brain. Treatment with Yanggyuksanhwa-tang (Lianggesanhuo-tang) significantly attenuated the decrease of neural activity in the whole brain following BCAO ischemia. Treatment significantly attenuated the decrease of neural activity in the CA1, CA2, CA3, dentate gyrus of the hippocampus, activated barrel, barrel cortex, somatosensory cortex, cingulate cortex, caudate putamen, and medial septal nucleus following BCAO in aged rats. Treatment with Yanggyuksanhwa-tang (Lianggesanhuo-tang) also significantly attenuated the decrease of neural activity in the anteroventral thalamic nucleus, ventral anterior thalamic nucleus, arcuate nucleus, posterior hypothalamic area, medial mammillary nucleus, lateral periaqueductal gray, dorsal raphe nucleus, interpeduncular nucleus, median raphe nucleus, and medial pontine nucleus. Conclusion : It can be suggested that Yanggyuksanhwa-tang (Lianggesanhuo-tang) has a neuroprotecuve effect on cerebral ischemia through the control of glucose metabolic rate and cerebral blood flow.

임상 간호사의 당뇨병에 대한 지식, 중요도 및 인지도 조사 (Research into Clinical Nurses' Knowledge, Importance and Perception of Diabetes Mellitus)

  • The purpose of this study is to determine the clinical nurses' knowledge of DM and diabetes patient education aptitude so as to provide a basis for clinical nurse training with respect to diabetes patient education program. The data has been collected through the questionnaires of 42 items from the Knowledge on DM and each 16 item from the Importance on the elements of diabetes patient and the Recognition for clinical nurses, respectfully. 166 nurses from a general hospital who had participated in clinical nurse training in Sep. 28 and Oct. 4, 2001 were subject to respond the questionnaires. Analysis has been done by using statistical method such as percentage, average, standard deviation, t-test, ANOVA, Duncan test and Pearson correlation coefficients. The findings are as follows: 1. Clinical nurses' knowledge levels of diabetes 1) The average level of nurses' knowledge about diabetes is 29.37 (right answer- finding rate: 70%), which is intermediate. 2) Amongst the nurses classified by their knowledge levels about diabetes, the group with less than one year career and that with more than 5 year careers are found to have higher knowledge levels. The item with the highest right answer-finding rate was 'Please find the wrong out of the following examples about foot care'. Meanwhile, the item with highest incorrect answer-finding rate was 'what does blood sugar control aims for amongst gestational diabetes?'. 2. Clinical nurses' importance and perception levels of educational training about diabetes 1) There were no differences amongst nurses' importance level about diabetes. 2) Nurses usually had high scores(4.30) in terms of the items related to the importances about educational training. 3) There were quite high recognitions of general characteristics and symptoms about diabetes, amongst the nurse cohorts working more than one year and less than 5 years, and over 5 years, the group belonging to the internal department, that having the previous experiences of dealing with diabetes, and that having their diabetic relatives and other close people. Meanwhile, strangely, the group who identified themselves as 'not good at treating diabetes' had a high recognition level of educational training about diabetes. 3. Relationship between knowledge levels and importance & perception levels of diabetes 1) The higher knowledge about diabetes nurses had, the more importance they recognized. 2) It is found that there was no relationship between knowledge and perception of diabetes. 3) The more importance about diabetes nurses had, the higher perception they obtained. In conclusion, there is an urgent need for systematic educational programs about diabetes including technical aspects, in order to upgrade and improve nurses knowledge levels. In addition, re-educational training should be provided at regular intervals. Further, we believe the nurses with high knowledge about diabetes and interests in the provision of educations for patients can be far more confident, and in return, patients can have better self-management about diabetes obtained through educations. Based on the above-mentioned findings, we would like to make the suggestion: re-evaluation about nurses' knowledge and cognition levels should be carried out after job training programs about diabetes.

Effects of zinc bearing palygorskite supplementation on the growth performance, hepatic mineral content, and antioxidant status of broilers at early age

  • Objective: This study was conducted to investigate effects of zinc (Zn) bearing palygorskite (ZnPal) supplementation on growth performance, hepatic mineral content, and antioxidant status of broilers at early age. Methods: A total of 240 1-day-old Arbor Acres broiler chicks were allocated into 5 treatments with 6 replicates of 8 chicks each. Birds in 5 treatments were fed a basal diet supplemented with 0 (Control group; Analyzed Zn content: 81 mg/kg), 20, 40, 60, and 80 mg/kg Zn as ZnPal for 21 days, respectively. Blood, liver and intestinal mucosa were collected at 21 days of age. Results: Treatments did not affect growth performance of broilers during the 21-day study (p>0.05). The contents of hepatic Zn and magnesium (Mg) were linearly increased (p<0.001) by ZnPal supplementation. ZnPal inclusion linearly (p = 0.007) reduced malondialdehyde (MDA) concentration in serum. The activity of total superoxide dismutase (T-SOD) in liver increased linearly (p = 0.001) with concentration of ZnPal in diet. ZnPal inclusion linearly (p = 0.036) and quadratically (p = 0.005) increased T-SOD activity, and linearly (p = 0.012) increased copper/zinc superoxide dismutase (Cu/Zn SOD) activity in jejunal mucosa. The maximum responses of hepatic and jejunal antioxidant enzymes activities (T-SOD and Cu/Zn SOD) were found when supplementing the basal diet with 60 mg/kg Zn as ZnPal. Furthermore, ZnPal supplementation quadratically (p = 0.001) increased Cu/Zn SOD activity in ileal mucosa, and its maximum activity was observed in the diet supplemented with 20 mg/kg Zn as ZnPal. Conclusion: ZnPal supplementation did not alter growth performance of broilers. Dietary ZnPal inclusion could increase concentrations of hepatic trace minerals (Zn and Mg) and inhibit lipid peroxidation by reducing serum MDA accumulation, with the optimal dosage of Zn from ZnPal being 80 mg/kg diet (analyzed Zn content in the diet: 165 mg/kg), and 60 mg/kg Zn as ZnPal (analyzed Zn content in the diet: 148 mg/kg) was the optimum dosage for broilers to achieve maximum antioxidant enzyme activities.
