Comparison of the dietary intake and clinical characteristics of obese and normal weight adults

  • Obesity contributes to an increased risk for chronic diseases, including diabetes, cardiovascular diseases, and certain types of cancer. The prevalence of obesity has increased in Korea. We compared the clinical and dietary characteristics of obese adults (n = 30, 17 men and 13 women, mean age 29.9) to those with a normal weight (n = 15, 8 men and 7 women, mean age 26.5). We determined lipid profiles, fasting blood sugar (FBS), blood pressure, and serum free fatty acid (FFA). Dietary intake was estimated using a food frequency questionnaire (FFQ) and a 3-day dietary record. Exercise patterns and average alcohol intake were determined. The average body mass index was 28.3 kg/$m^2$ in the obese and 21.2 kg/$m^2$ in the normal weight groups. The obese group had significantly higher levels of total cholesterol, LDL cholesterol, and triglycerides, lower levels of HDL cholesterol, and higher blood pressures compared to the normal weight group. FBS was not significantly different between the two groups. The obese group had significantly more subjects with metabolic syndrome (26.7%) compared to the normal weight group (0%). Serum FFA levels tended to be higher in the obese (P = 0.087). No significant difference in caloric intake was observed between the two groups. No differences in carbohydrate, protein, or fat intake between two groups were observed from the FFQ. However, results from the 3-day dietary record showed that the percentage of energy from fat was significantly higher in the obese group. The frequency and amount of exercise did not differ between the two groups. Alcohol consumed per drinking episode was significantly higher in the obese group. These results confirm that excessive weight is associated with disturbances in lipid metabolism in these fairly young and otherwise healthy adults. Dietary factors, including higher fat intake and alcohol consumption, seem to be contributing to the obesity of these subjects.

Characteristic Study of the Pulse Position on CHON, KWAN and CHUCK Using the Ultrasonic Waves (초음파 분석을 이용한 촌관척 위치별 혈관의 특성연구)

  • The study aims to measure and analyze the thickness and depth of blood vessel on the pulse diagnosis locations and the blood velocity through the use of ultrasonic waves (LOGIQ5PRO, GE Medical, U.S.) in order to understand the structural difference of pulse diagnosis locations. The subjects included 44 healthy men and women(22.28${\pm}$2.62 age) considered normal in terms of Body Mass Index(BMI). The thickness and depth of the blood vessel and the blood velocity were measured three times on CHON, KWAN and CHUCK to obtain the average value. Results showed there is a statistically significant difference among the variables measured on CHON, KWAN and CHUCK. A difference according to gender was also observed. This explains why an oriental medical doctor can tell the difference in pulses depending on the location of CHON, KWAN and CHUCK. In addition, the difference in pressure between CHON and KWAN was higher than that in pressure between KWAN and CHUCK. The findings explain why oriental medical doctors take pulses by dividing CHON, KWAN and CHUCK in the short length of the three fingers. It can be used to develop a pulse diagnosis device enabling accurate measurement according to the characteristics of blood vessel structure based on where the pulse is taken. Furthermore, the results can be used as basic data for the development of a pulse diagnosis simulator.

An Association between Liver Markers and Physiological Variables: Comparison between Normal and Fatty Liver Subjects

  • We evaluated whether liver markers such as aspartate aminotransferase (AST), alanine aminotransferase (ALT), alkaline phosphatase (ALP), gamma-glutamyltranspeptidase (GGT), and bilirubin have a relationship with other physiological factors in the normal (n=115) and fatty liver subjects (n=122) and there are differences between the two populations. Body indices were higher in the fatty liver group than in the normal group. Liver markers and blood pressure (BP) were greater in the fatty liver group than in the normal group. AST and ALT levels were positively correlated with body indices in the fatty liver group, but not in the normal group. AST, ALT and GGT levels in the fatty liver group had positive relationship with cardiovascular indices (CI). ALP and bilirubin levels were negatively associated with some of CI. Liver markers were negatively or positively correlated with inflammatory markers, thyroid hormones, or several biochemical markers levels. These findings suggest that abnormal changes in liver markers may be useful tool for diagnosis or prognosis of development of cardiovascular and/or inflammatory diseases as well as metabolic syndrome.

Comparing validity of using Body Mass Index, Waist to Hip Ratio, and Waist Circumference to Cardiovascular Risk Factors of Middle Aged Koreans (한국중년층에서 체질량지수, 허리-엉덩이둘레비, 및 허리둘레의 심장혈관계질환 위험인자 타당도 비교)

  • This study was conducted to compare the validity of obese index among body mass index(BMI), waist to hip ratio(WHR), and waist circumference(WC) and to determine which is the best in relation to cardiovascular risk factors of middle aged Korean(40-64yr).Data from the 1998 Korean Health and Nutrition Survey were used(N=3380). Anthropometric indices and cardiovascular risk factors were measured. Chi-square test, analysis of variance following duncan's multiple range test, partial correlation analysis, and Receiver Operator characteristic(ROC) curves were used in the analysis. There was a significant increasing trend in WHR, systolic blood pressure(SBP), high density lipoprotein cholesterol(HDL), and fasting blood sugar(FBS) with age categories of male and in BMI, WC, WHR, diastolic blood pressure(DBP), SBP, total cholesterol(TC), low density lipoprotein cholesterol(LDL), triglycerol(TG), and FBS with those of female. Specially female had the characteristics of upper body fat and systolic blood pressure risk(p<0.05). Proportions of subjects with lifestyle factors related to cardiovascular risk in overweight or upper body fat group were higher than that of normal group. Higher proportions of subjects were practiced exercise in upper body fat group of male than in other groups. Among 7 cardiovascular risk factors in partial correlation analysis, BMI had the highest correlation coefficient in 6 risk factors in male, whereas WC in 4 risk factors in female. Mean of each obese index according to cardiovascular risk groups except smoker was higher than that of normal(p<0.05). These trends were shown in upper body fat group and female. In ROC analysis of 12 risk factors and health conditions, the largest area under curve among obese indices for risk factors were BMI in male and WHR in female. The optimal cutoff values of each index(BMI: WHR: WC) for one or more cardiovascular risk factors were 23.13: 0.89: 85.35 in male and 23.57: 0.84: 78.35 in female. The results showed that cardiovascular risk factors were prevalent in middle aged Koreans within normal limits of obese indices like another Asians. For the identification of cardiovascular risk factors of middle aged Koreans, BMI for men and WHR for women are appropriate indices. But it is recommended that BMI, WHR, and WC, all three indices should be considered, when using these indices.

Effects of $Zhiyin$($BL_{67}$) and $Shangyang$($LI_1$) Reinforcement in Acupuncture on the Changes of Cerebral Blood Flow and Blood Pressure in Rats (지음(至陰)($BL_{67}$).상양(商陽)($LI_1$) 보법(補法) 자침이 백서(白鼠)의 뇌혈류량 및 혈압에 미치는 영향)

  • Objectives : The purpose of this study is to research the effects of acupuncturing $BL_{67}$ and $LI_1$ and determine the mechanism of action of acupuncturing $BL_{67}$ and $LI_1$ by measuring the changes of regional cerebral blood flow(rCBF) and mean arterial blood pressure(MABP) in normal rats and ischemic rats. Method : This study researched the effects of acupuncturing $BL_{67}$ and $LI_1$ on the change of rCBF and MABP. To determine the mechanism of action of acupuncturing $BL_{67}$ and $LI_1$, pretreatment with indomethacine and methylene blue was done. Result : 1. Acupuncturing $BL_{67}$ and $LI_1$ significantly increased rCBF and acupuncturing $BL_{67}$ and $LI_1$ induced increase of rCBF was significantly inhibited by pretreatment with indomethacin(1 mg/kg, i.p.), an inhibitor of cyclooxygenase, and methylene blue(10 ${\mu}g$/kg, i.p.), an inhibitor of guanylate cyclase. 2. Acupuncturing $BL_{67}$ and $LI_1$ decreased MABP and there was no significantly change of decrease of MABP on acupuncturing $BL_{67}$ and $LI_1$ by pretreatment with indomethacin and methylene blue. 3. These result suggested that acupuncturing $BL_{67}$ and $LI_1$ might significantly increase rCBF by dilating arterial diameter and mechanism of acupuncturing $BL_{67}$ and $LI_1$ might be mediated by cyclooxygenase and guanylate cyclase. 4. The rCBF was significantly and stably increased by acupuncturing $BL_{67}$ and $LI_1$ during the period of cerebral reperfusion in cerebral ischemic rats, which contrasted with the rapid and marked increase in the control group. Pretreatment with methylene blue significantly decreased rCBF by acupuncturing $BL_{67}$ and $LI_1$ during the period of ischemic state, increased rCBF during the period of cerebral reperfusion. These results suggested that the mechanism of acupuncturing $BL_{67}$ and $LI_1$ might be mediated by guanylate cyclase. Conclusion : Acupuncturing $BL_{67}$ and $LI_1$ can increase rCBF in normal state, and improve stability of rCBF in ischemic state. In addition, we suggested that mechanisms related with acupuncturing $BL_{67}$ and $LI_1$ was more involved in the guanylate cyclase pathway.

The Effect of Milk Consumption on Blood Lipid Levels of the Korean College Women (여대생의 우유 섭취량이 혈중 지질수준에 미치는 영향)

  • This research was conducted to study the effect of milk consumption on blood lipid levels of Korean college women. According to milk intake from food frequency questionnaire (FFQ), top $20\%$ of subjects were classified as high group (HG) and bottom $20\%$ as low group (LG). Body weight, height and blood pressure were measured and BMI was calculated from the anthropometric data, but there was no significant difference between the groups. Blood samples were collected and analyzed for total cholesterol (TC), triglycerides (TG) and lipoprotein fractions. There was no significant difference in the blood level of albumin, total protein and hemoglobin between two groups, and all blood parameters were in the normal range. Blood lipid levels of two groups were not significantly different, but HDL-cholesterol level were higher in HG (p < 0.05). Therefore, according to the result of the research, it is considered that drinking a pack of milk (200 ml) everyday, the average intake of dairy products of HG, is advisable to promote good health without increasing of cardiovascular disease (CVD) risk.

Effects of Theratainment Physical Factors Application on Blood Pressure, Angle and Circumference in an Female University Students Calf (테라테인먼트 물리적 요소 적용이 대학생 여성 종아리의 혈압, 각도, 둘레에 미치는 영향)

  • This study examined the effect of the application of Theratainment physical factors on the calf blood pressure, ankle angle, and calf circumference. A research sample consisting of normal female university students in their twenties were divided into hot therapy group (n=20), cold therapy group (n=20), and compression therapy group (n=20). By referring to the previous study, 20-minute hot therapy, 15-minute cold therapy, and 30-minute compression therapy with 100mmHg intensity was applied. The therapy was applied to left-side lower limb, followed by the application on the right-side lower limb after an hour of resting time. The calf blood pressure, ankle angle, and calf circumference of each group was assessed. In the study results, hot therapy and compression therapy showed a statistically significant difference on the decrease of blood pressure and increase of ankle angle (p<.05), but cold therapy produced no statistically significant difference. None of the three groups showed a statistically significant difference on the calf circumference. Hence, hot therapy and compression therapy is recommended for reducing calf blood pressure and increasing ankle angle related to the calf. These therapies are expected to be used for solving diverse problems of lower limbs.

An Experiments Analysis of Comparison of Flow Gradients Across Disposable Arterial Perfusion Cannulas (일회용 각종 송혈관의 관류압차에 대한 비교실험)

  • The mean perfusion pressure produced by 20 different design and size of disposable aortic perfusion cannulas [size range: 18F to 20F] at flow rates of 1 to 5 L/min was compared. A roller pump with perfusion tubing 10mm in inner diameter was used with a mixture of 0.9% normal saline and 100% glycerin [mixing ratio of 6.72:13.28] to make the viscosity as normal blood in 20oC as the test solution. Line pressure in these cannulas at a flow rate of 3L/min ranged from 13.2$\pm$4.47 to 157$\pm$1 34 mmHg in a case of absence of resistance, and from 11.4$\pm$1.14 to 227.2$\pm$4.47 mmHg in a case of presence of resistances 14 cannulas had gradients of less than 100Hg at a flow rate of 3L/min and 9 cannulas less than 100mmHg at 4L/min. The following results were obtained from this experimental study: .1] A wide range of line pressure was observed in disposable aortic perfusion cannulas currently in clinical use; 2] Some cannulas currently used for cardiopulmonary bypass in adults generated excessive line pressure; 3] Both the material and design affect function, with some designs safer than others. Cardiac surgeons should base the choice of aortic perfusion cannula on the best performance and safest design available to avoid cannula-related problems during operation.

The Effect of Compression Knee Band and Heat Treatment on Blood Velocity of the Elderly with Osteoarthritis (무릎밴드 가압과 가온요법이 퇴행성 무릎관절염 노인의 혈류속도에 미치는 효과)

  • The purpose of this study was to investigate the change in blood velocity(mm/sec) when compression and/or heat were applied to the knee joint for the elderly who has both normal and painful legs with osteoarthritis(OA). Experimental compression knee band was prepared from the 3D knee data of the average women in 60's. 3D replica of knee was reduced by 7, 10, and 13% from the nude pattern in course direction. Clothing pressure was measured at the front and back of each healthy and painful knee of elderly women for one minute while standing and sitting on the chair. Blood velocity was measured at 13 cm upper from the mid-patella for 15 minutes. Results are as follows: first, compression or heating treatment itself did not change blood velocity of both legs; second, combination treatment with heating and compression was effective to increase blood velocity. In details, for healthy legs, combination treatments with compression by 10% reduced pattern(about 1.3kPa) and heating($43^{\circ}C$) induced the maximal blood velocity, however, for knees with OA, 7% reduced pattern(about 1.0kPa) with simultaneous heating($43^{\circ}C$) was more effective than other cases. These results indicated that pain and spasticity of knee joint with OA could be reduced by applying heat and compression therapy, where the compression level of painful knee should be slightly lower than of healthy leg.

The Effects of Leukocyte Depleted Priming Solution on the Gastric Mucosal $Co_{2}$ Partial Pressure and Serum IL-8 Level during Cardiopulmonary Bypass in Korean Mongrel Dogs (한국산 잡견에서 백혈구 제거 충진액이 체외순환 중 위점막 이산화탄소 농도와 IL-8 수준에 미치는 영향)

  • Background: Cardiopulmonary bypass is an essential process to maintain circulation for saving life during the cardiac surgery, But it is a process in which systemic inflammation was evoked inevitably because of the exposure of blood to foreign surface. The injuries to distal organs during the cardiopulmonary bypass were resulted from systemic inflammation and the disturbances of micro-circulations in the organs. We designed this study to research the effects of leukocyte depletion from pump-oxygenator priming solution on the systemic inflammation, and the micro-circulation of gastric mucosa that is suggested by the gastric mucosal $CO_{2}$ partial pressure and acidity. Material and Method: The dogs were divided into three groups according to the different pump-oxygenator priming solutions; non-hemic crystalloid solution; leukocyte-depleted homologous blood; and non leukocyte-depleted homo-logous blood. Each priming solution group contained five dogs. In all three groups, 2 hours of cardiopulmonary bypass, and 4 consecutive hours of general anesthesia was maintained on the mechanical ventilation. Each dog was evaluated for the gastric mucosal pH, $CO_{2}$ partial Pressure, arterial pH, $CO_{2}$ partial pressure, the exhaled air $CO_{2}$ partial pressure and the level of IL-8 on before the cardiopulmonary bypass, 1 hour after the cardiopulmonary bypass, 2 hours after the cardiopulmonary bypass, 2 hours after the restoration of normal circulation, and 4 hours after the restoration of normal circulation after the cardiopulmonary bypass. The levels of IL-8 were measured with ELISA (enzyme linked immunosorbent assay) technique. Result: 1. There were significant differences of gastric mucosal $CO_{2}$ partial pressure between the leukocyte-depleted homologous blood group and other two groups(vs non leukocyte-depleted homologous blood group; P=0.02, vs non-hemic crystalloid solution group; P=0,01). 2. The gastric mucosal pH of leukocyte-depleted homologous blood group was significantly different from non leukocyte-depleted homologous blood group (p=0.01). 3. The levels of IL-8, which examine the systemic inflammation, showed signi- ficantly better results in leukocyte-depleted homologous blood group and non-hemic crystalloid solution group than non leukocyte-depleted homologous blood group (p=0.01, 0.01). Conclusion: Based upon these results, we concluded that the leukocyte depletion from the pump-oxygenator priming solution has a beneficial effects in reducing systemic inflammation and the preserving of gastric mucosal micro-circulation.
