3D Visualization System of Blood Flow Reconstructed using Curvature Estimation (곡률 추정을 이용하여 재건된 혈류의 3차원 가시화 시스템)

  • The methodology to visualize the shape of blood vessel and its blood flow have been attracting as a very interesting problem to forecast and examinate a disease in thrombus precursor protein. May previous visualization researches have been appeared for designing the blood vessel and also modeling the blood flow using a doppler imaging technique which is one of nondestructive testing techniques. General visualization methods are to depict the blood flow obtained from doppler effects with fragmentary stream lines and also visualize the blood flow model using volume rendering. However, these visualizeation techniques have the disadvantage which a set of small line segments does not give the overall observation of blood flows. Therefore, we propose a visualization system which reconstruct the continuity of the blood flow obtained from doppler effects and also visualize the blood flow with the vector field of blood particles. This system will use doppler phase difference from medical equipments such as OCT with low penetration and reconstruct the blood flow by the curvature estimation from vector field of each blood particle.

  • In this study electrical impedance plethysmograph was developed and each ten digital blood flow was measured for eight normal males and a patint with digital arterial reconstruction surgery. Blood flow of the digit with surgery was approximately fourty percent compared to that of the other normal digit. For the normal subjects, right-handed subjects generally have larger blood flow in right fingers than that in left fingers while it was reverse for the left-handed subjects. The relative average magnitude of the digital blood flow is the third, first, second, fourth, and fifth digit in descending order for the left and right hand, respectively. Blood flow of the fifth digit for all subjects except one was the smallest among the five digits.

Mechanical Dilation of the Recipient Vessel with the DeBakey Vascular Dilator in Lower Extremity Reconstruction: A Report of Two Cases

  • In lower extremity reconstruction, the recipient vessel often requires long-range mechanical dilation because of extensive vasospasm or plaque formation induced by concomitant atherosclerosis. While a forceps dilator can be used to manipulate and dilate vessels approximately 1 cm from their end, a DeBakey vascular dilator can dilate long-range vessels. The authors successfully performed free flap reconstruction of the lower extremity using the DeBakey vascular dilator. Of the two patients who underwent lower extremity reconstruction, one had extensive vasospasm, and the other had plaques in the recipient arteries. Irrigation with 4% lidocaine and dilation of the lumen with a forceps dilator were insufficient to restore the normal arterial blood flow. Instead, a DeBakey vascular dilator with a 1-mm diameter tip was gently inserted into the lumen. Then, to overcome vessel resistance, the dilator gently advanced approximately 10 cm to dilate the recipient artery. Normal arterial blood flow was gushed out after dilating the vessel lumen using a DeBakey vascular dilator. The vascular anastomosis was performed, and intravenous heparin 5000 IU was administered immediately after anastomosis. Prophylactic low-molecular-weight-heparin (Clexane, 1 mg/kg) was administered subcutaneously to both patients for 14 days. The reconstructed flap survived without necrosis in either patient.

Distally Based Anterolateral thigh Pedicled Flap in the Reconstruction of Defect Around Knee (역혈류성 전외측대퇴 혈관경피판을 이용한 무릎 주위 결손의 재건)

  • Purpose: As the soft tissue defect around the knee is difficult to reconstruct, local flap or free flap is used. Distally based anterolateral thigh pedicled flap introduced by Zhang uses sufficient reverse flow supplied from the vascular network around the knee. We report successful reconstruction of defect around knee by this method. Methods: Four patients with skin & soft tissue defect around knee have been treated for reconstruction using the distally based anterolateral thigh pedicled flap. First, the doppler was used to check the perforator flap of the descending branch of the lateral circumflex femoral artery and to draw and dissect the perforator flap as much as needed. After the dissection, the proximal of the descending branch was clamped and checked for sufficient supply of blood flow from the reverse flow and then ligated. It was dissected along the descending branch and in order to prevent damage to the joined parts of the descending branch and the lateral superior geniculate artery, a more careful ligation was done starting from 10 cm superior to the knee. The defect was reconstructed after securing enough vascular pedicle to cover all the damaged parts. Results: Not all patients suffered from flap necrosis. In case of the patient with chronic osteomyelitis, slight venous congestion was observed right after the surgery but it disappeared the following day. All three patients had no occurences of additional complications. Conclusion: Distally based anterolateral thigh pedicled flap was enough to provide large flap for knee reconstruction. It had sufficient blood flow and vascular pedicle. It also had taken short operation time compared to the free flap operation. The distally based anterolateral thigh pedicled flap used by the authors is a very useful way of reconstructing the area around knee.

  • In general, the physiological systems have shown nonlinear complex phenomena. This study analyzes nonlinear characteristics of the flow of peripheral blood vessel dynamics in physiological systems using chaos theory. We performed this study by means of several quantity methods and power spectrum. The quantity methods are a phase space reconstruction and a poincare's map. And the power spectrum method is a conventional linear analysis. Experimental data have been acquired from examining 10 diabetic patients, and 10 control subjects in initial stable state. In acquisition experminetal data, we anlysized the differences of nonlinear characteristics between diabetic group and control group. The results of quality analysis methods showed the flow of peripheral blood vessel had the nonlinear and chaotic characteristics, screening a strange attractor on reconstructed phase space. In conclusion, the flow dynamics of peripheral blood vessel had a chaotic behavior of nonlinear dynamic systems, dynamic system, and differences of characteristic of nonlinear dynamic system.

  • The physiological systems have nonlinear complex phenomena. Expecially, the flow of capillary blood vessel has a nonlinear dynamic system. Thus, this study analyzes nonlinear characteristics of the flow of capillary blood vessel in physiological systems using chaotic tools(phase space reconstruction, correlation dimension, largest lyapunov exponent). Experimental data have been acquired from examining 10 rabbits. The results of chaotic analysis showed a decreasing largest lyapunov exponent and correlation dimension according to increasement glocose index. And we also know the chaotic behavior.

Restoring blood flow to the lateral plantar artery after elevation of an instep flap or medialis pedis flap

  • The instep flap and medialis pedis flap are both originate based on the medial plantar artery. The medialis pedis flap is based from the deep branch and the instep flap is based from the superficial branch. To increase the axial rotation, it is acceptable to ligate the lateral plantar artery. However, this can partially affect the blood supply of the plantar metatarsal arch. We restored the blood flow with a vein graft between the posterior tibial artery and the ligated stump. From 2012 to 2020, 12 cases of heel reconstruction, including seven instep flaps and five medialis pedis flaps, were performed with ligation of the lateral plantar artery. The stump of the lateral plantar artery was restored with a vein graft and between the posterior tibial artery and the ligated stump. Patients were followed for 18 months. Long-term results showed the vascular restoration of the lateral plantar artery remained patent demonstrated by doppler ultrasonography. Restoring blood flow to the lateral plantar artery maintains good blood supply to the toes. If the patient in the future develops a chronic degenerative disease, with microvascular complications, bypass surgery can still be performed because of the patency of both branches.

Deep Inferior Epigastric Perforators Topography for "Island Transverse Rectus Abdominis Musculocutaneous Flap" in Breast Reconstruction

  • Background The Island transverse rectus abdominis musculocutaneous (TRAM) flap is well vascularized with very reliable blood flow, because all perforators of the zone I are included when it is harvested. The number of perforators, topographic mapping, and their relationship with reconstructed outcomes were investigated. Methods Fifty patients with Island TRAM breast reconstruction from September 2021 to August 2022 were investigated. The zone I was divided into a total of eight sections. Under the loupe magnification, all perforators larger than 0.5 mm in zone I were counted with fine dissection, and photographs were taken in background of vessel loops. Complications like flap necrosis, seroma, and hematoma were also investigated. Result There are 12 ideal perforators on average in zone I such as one perforator in section I, II, IV, V, VI, VIII, and three perforators in section III and VII. However, two perforators (M6 and L6) below arcuate line were sacrificed in the time of flap harvest to prevent hernia. Island TRAM included 10 perforators on average (5 perforators in each side) above arcuate line to be transferred to the recipient site. Only minor complications were identified. Conclusion The Island TRAM flap includes 10 perforators to get the vigorous blood flow. The periumbilical to upper medial perforators become more dominant in the perfusion of the flap after deep inferior epigastric artery division. Well preserved perforators will guarantee the satisfactory breast reconstruction with the least complication.

  • For the successful guided bone regeneration(GBR) of maxillary bony defect, proper soft tissue coverage is one of the most important things. Soft tissue dehiscence can be most common reason of osseous reconstruction failure. If a vascular supply to the graft should not develop from the host tissue, then the graft may also foil. Both of these prerequisites can be aided by judicious use of the buccal fat pad(BFP). Many methods for adequate soft tissue coverage have been proposed and the use of the BFP is one of them. BFP is useful in posterior maxillary area, can cover larger area and have higher blood flow than other methods. so the use of the BFP may offer protection and early blood supply to maxillary bone graft. This report describes the history, anatomy, blood flow, and clinical usefulness with two clinical cases.
