The management of cardiac arrhythmias by cardiac pacing has increased greatly since the treatment of complete heart block with an external transcutaneous pacemaker in 1952, followed by the use of myocardial wires connected to an external pulse generation, by external transvenous pacing, and then by transvenous pacing with implantable components in thoracic wall.By now, the three bases of modern cardiac pacing for bradyarrhythmias had been established [1] an implantable device [2] the transvenous approach [3] the ability of the pacemaker to sense cardiac activity and modify its own function accordingly. In transvenous implantation of a pacemaker, any one of four vessels at the root of the neck is suitable for passage of the electrode - cephalic vein, external jugular vein, internal jugular vein, costo-axillary branch of the axillary vein. The new technique of direct puncture of the subclavian vein, either percutaneously or after skin incision only has been made, is invaluable & is used routinely. We have experienced one 25 years old patient who had rheumatic mitral stenosis & minimum aortic regurgitation with sinus bradycardia associated with premature atrial tachycardia & another 54 years old female patient who was suffered from sick sinus syndrome with sinus bradycardia & sinus arrest. The 1st patient was taken open mitral commissurotomy & aortic valvuloplasty and then was taken atrlal pace-maker implantation through If subclavian puncture method in post-op 14 days, and the second patient was taken atrial pacemaker implantation through If subclavian puncture method. Their postop course was in uneventful & were discharged, without complication. Their condition have been good to now.

There exist many types of pipe and component fatigue through vibrations, thermal fatigues or shifting. In some cases of thermal stratification/thermal fatigue, pipes & components are receiving thermal stress by means of material expansion and shrinkage by continuous thermal repetitive variation. Small cracks initially occur on the inside surface by thermal stress. These cracks grow in depth the pipe wall and finally come to a rupture. Pipe parts of susceptibility to thermal stratification and thermal fatigue are now being examined by conventional UT(ultrasonic test) as volumetric examination. It is difficult to fully satisfy the code & standards requirements because 3" weldolet weldments of RCS 16" pipe to 3" branch connection lines have complex structural shape. To solve the problems of conventional UT examination, we made a realistic mock-up and UT calibration block. We performed a simulation of phased array UT utilizing CIVA as NDE(Non-Destructive Examination) simulation software. Also we designed phased array UT transducer and wedge, optimal frequency by using simulation data. We performed phased array UT experiment through mock-up including artificial flaws(notch). The phased array UT technique is finally developed to improve the reliability of ultrasonic test at RCS 16" pipe to 3" branch connection weld.

The experiment was carried out to determine the optimum application rate of liquid waste from methane fermentation (LW) and its effect on botanical composition, dry matter yields and nutrient quality of pasture mixtures. Experimental fields was designed as a randomized block treated with NPK chemical fertilizer (NPK = 28-20-24 kg/lOa), NPK + Water 28 ton, 112 NPK + LW 28 ton, 112 NPK + LW 42 ton, LW 28 ton, LW 42 ton and LW 56 ton/lOa at Livestock Experiment Station in Suweon, 1985. The results obtained are summarized as follows: 1. Vegetation of introduced pastures, both in grasses and legumes, was markedly increased in the plots treated with methane-liquid waste. However, heavy application of liquid waste tended to increase of native weeds such as Polygronum spp., Rumex spp. and Lactuca spp. 2. Crude protein contents was increased in the plants applied with liquid waste, but NFE was decreased compared with those of chemical fertilizer applied. The concentrations of crude fat and crude fibre were, however less affected by the fertilizer resource. Among cell-wall constituents, cellulose content was decreased as the liquid waste application rate increased, while hemicellulose showed a negative association. 3. Productivity of the pasture was increased as the liquid waste application rate increased. The highest dry matter yields was obtained in the plot treated with LW 42 ton/lOa by 71 1 kg/lOa, which shows about 71% increments compared with those of chemical fertilizer treated. Net energy yields, both in starch value and NEL, were also markedly increased under liquid waste application. As a results, the optimum application rate of methane-liquid waste was found to be 42 ton in 10 a.

The local heat transfer coefficient is experimentally investigated for the reflux condensation in a countercurrent flow between the steam-air mixture and the condensate. A single vertical tube has a geometry which is a length of 2.4m, inner diameter of 16.56mm and outer diameter of 19.05mm and is made of stainless steel. Air is used as a noncondensible gas. The secondary side is installed in the form of coolant block around vertical tube and the heat by primary condensation is transferred to the coolant water. The local temperatures are measured at 15 locations in the vertical direction and each location has 3 measurement points in the radial direction, which are installed at the tube center, at the outer wall and at the coolant side. In three different pressures, the 27 sets of data are obtained in the range of inlet steam flow rate 1.348 -3.282kg/hr, of inlet air mass fraction 11.8 -55.0%. The local heat transfer coefficient increases as the increase of inlet steam flow rate and decreases as the decrease of inlet air mass fraction. As an increase of the system pressure, the active condensing region is contracted and the heat transfer capability in this region is magnified. The empirical correlation is developed represented with the 165 sets of local heat transfer data. As a result, the Jacob number and film Reynolds number are dominant parameters to govern the local heat transfer coefficient. The rms error is 17. 7% between the results by the experiment and by the correlation.
Purpose: In accordance to an increased interest in facial appearance and the popularization of computed tomography scanning, the number of diagnosis and treatment of blowout fractures has been increased. The purpose of this article is to review pure blowout fracture surgery through transconjunctival incision focusing on complications and their management. Methods: In this retrospective study, 583 patients, who had been treated for pure blowout fracture through transconjunctival incision from 2000 to 2009, were evaluated. Their hospital records were reviewed according to their sex, age, fracture site, preoperative presentations, time interval between trauma and surgery, and postoperative complications. Results: According to postoperative follow-up results, there were early complications that included wound dehiscence and infection (0.2%), hematoma (insomuch as extraocular movement is limited) (0.7%), lacriminal duct injury (0.5%), and periorbital nerve injury (0.7%). In addition, there were late complications that lasted more than 6 months, that included persistent diplopia (1.7%), extraocular movement limitation (0.9%), enophthalmos (1.0%), periorbital sensation abnormalities (1.0%), and entropion (0.5%). Conclusion: We propose the following guidelines for prevention of postoperative complications: layer by layer closure; bleeding control with the epinephrine gauzes, Tachocomb, and Tisseel; conjunctival incision 2 to 3 mm away from punctum; avoidance of excessive traction; performing surgical decompression and high dose corticosteroid therapy upon confirmation of nerve injury; atraumatic dissection and insertion of Medpor Barrier implant after securing a clear view of posterior ledge; using Medpor block stacking technique and BioSorb FX screw fixation; performing a complete resection of the anterior ethmoidal nerve during medial wall dissection; and making an incision 2 to 3 mm below the tarsal plate.
A 60-year-old man was diagnosed with locally advanced non-small cell lung cancer. He refused treatment with a curative aim and was treated conservatively. Pain had developed on his shoulder and chest wall, which became worse as the cancer progressed. Although his pain initially appeared to be relieved with weak opioids and analgesics, it became more severe Strong opioids (transdermal fentanly patch and oxycodone), antidepressant or epidural block were introduced, However, the background pain became more intense and reached up to 8~9/10 on the visual analog scale (VAS). The dose of the transdermal fentanl patch was gradually increased to $600{\mu}g/hr$, which resulted in a dramatic improvement in his pain (9/10 of VAS) to 3/10 for most of the time. We described the successful experience with a high dose transdermal fentanyl patch for cancer pain relief, which might be an alternative option for cancer patients suffering from severe pain.

The paper presents a case study addressing the design and construction aspects for DCM(Deep Cement Mixing) method employed as the foundation of a caisson type breakwater with heavy weight(10,700 ton/EA) and a high design wave height($H_{1/3}$=8.7m). The DCM was designed for the project(Ulsan New Port North Breakwater Phase 1) by optimizing the pattern of DCM columns with a combination of short and long columns (i.e., block type(upper 3m)+wall type(lower)) and considering overlapped section between columns as a critical section against shear force where the coefficient of effective width of treated column($\alpha$) was estimated with caution. It was shown that the value can be 0.9 under the condition with the overlapped width of 30cm. In addition to that, a field trial test was performed after improving conventional DCM equipment (e.g., mixing blades, cement paste supplying pipes, multi auger motor, etc.) to establish a standardized DCM construction cycle (withdrawal rate of mixing blades) which can provide the prescribed strength. The result of the field strength test for cored DCM specimens shows that the averaged strength is larger than the target strength and the distribution of the strength(with a defect rate of 7%) also satisfies with the quality control normal distribution curve which allows defect rate of 15.9%.

According to the increase of power demand and expansion of downtown, it is necessary to install transformers additionally in operating substations and construct substations in residential area. But the public complaint has been increased due to the transformer noise of the substation. KEPCO has used a vibration preventing pad, various soundproof walls and an enclosure to transformers in outdoor substations, and a soundproof door, shutter and wind-path soundproof equipment in indoor substations to block the sound propagation from the transformers. But these noise reduction methods are not satisfied. It should be considered to reduce transformer noise itself. To shut out sound emitted from transformers, we developed a soundproof technology that wall up reinforcing bar by steel sheet. For the practical application of it, we analyzed the vibration characteristics of a transformer tank and reduction of noise according to soundproof plate. As a result, it is confirmed that the technology can reduce 11 [dB] of transformer noise.
In this paper, a numerical study is carried out to investigate the turbulent flow around a twin-skeg container ship model with rudders including propeller effects. A commercial CFD code, FLUENT is used with body forces distributed on the propeller disk to simulate the ship stem and wake flows with the propeller in operation. A multi-block, matching, structured grid system has been generated for the container ship hull with twin-skegs in consideration of rudders and body-force propeller disks. The RANS equations for incompressible fluid flows are solved numerically by using a finite volume method. For the turbulence closure, a Reynolds stress model is used in conjunction with a wall function. Computations are carried out for the bare hull as well as the hull with appendages of a twin-skeg container ship model. For the bare hull, the computational results are compared with experimental data and show generally a good agreement. For the hull with appendages, the changes of the stem flow by the rudders and the propellers have been analyzed based on the computed result since there is no experimental data available for comparison. It is found the flow incoming to the rudders has an angle of attack due to the influence of the skegs and thereby the hull surface pressure and the limiting streamlines are changed slightly by the rudders. The axial velocity of the propeller disk is found to be accelerated overall by about 35% due to the propeller operation with the rudders. The area and the magnitude of low pressure on the hull surface enlarge with the flow acceleration caused by the propeller. The propellers are found to have an effect on up to the position where the skeg begins. The propeller slipstream is disturbed strongly by the rudders and the flow is accelerated further and the transverse velocity vectors are weakened due to the flow rectifying effect of the rudder.
This study deals with a physical properties of Larch(Larix kaemferi Carr.) treated at temperatures above 100℃. Treatment conditions of this experiment were operated at regular intervals of 20℃ at temperature up to 180℃ for 10, 30, 60 and 90 minutes by using the bomb, respectively. The results of this study were as follows : 1) The density was decreased with increasing the times and temperatures of steaming. 2) It was considered that the steaming treated specimen's higher shrinkage compared to control was due to change of composition and structure in cell wall. 3) The warpage of half edge grain specimen was decreased by high temperature steaming.
