The effect of Ca-P coated bovine bone mineral on bone regeneration around dental implant in dogs (개 모델에서의 임플란트 주위 골결손시 Ca-P 표면 처리된 이종골의 효과)

  • 목적 : 최근 발치 후 즉시 임플란트 식립은 널리 사용되는 수술 방식이다. 이 연구의 목적은 임플란트 주위 골결손시 Ca-P으로 표면 처리된 이종골을 사용하여 골재생을 평가하기 위함이다. 재료와 방법 : 두 마리의 개 모델에서 하악 소구치와 제일 대구치를 발치하였다. 발치 6주 후 trephine bur를 이용하여 7.5 mm 지름과 5 mm 깊이를 가진 결손부를 형성하였다. 이 후 이 결손부의 중앙에 3.5 mm 지름과 15mm 길이의 fixture(GS II)를 식립하였다. 결과적으로 임플란트와 주변을 둘러싸고 있는 골 사이에는 2.0 mm정도의 gap이 만들어진다. 준비된 결손부 내로 자기골 또는 $Biocera^{(R)}$를 채웠다. 각각 4주, 8주 후 조직 절편을 제작하였다. 조직학적 평가를 위해 Block biopsy를 시행하였다. 결과 : 두 집단 모두 임상적으로 골이 완전히 채워졌다. 자가골이 이식된 부위(control)의 평균 골-임플란트 접촉(BIC)은 각각 4주째 $28.2{\pm}19%$였고, 8주째 $44.9{\pm}9%$였다. $Biocera^{(R)}$가 이식된 부위(test)의 평균 BIC는 각각 4주째 $34.6{\pm}27%$였고, 8주째 $27.6{\pm}23%$였다. 자가골이 이식된 부위(control)의 평균 골밀도는 각각 4주째 $32.7{\pm}25%$, 8주째 $37.4{\pm}17%$였다. 골-임플란트 접촉(BIC)과 골밀도의 평균 비율(%)은 비슷하였다. 조직학적으로 자가골과 이종골 이식 부위 모두 주변골과 잘 조화를 이루었고 유사한 치유 양상이 관찰되었다. 자가골과 이종골 이식 부위간 유의한 차이는 없었다.(P>0.05) 결론 : 임플란트 주위 2 mm의 골 결손부위에 자가골 또는 이종골로 채운 경우 유사한 결과를 얻었다. 이 결과 임플란트 fixture 주위의 골 결손부 해소를 위해 자가골을 대체할 수 있는 재료로 $Biocera^{(R)}$를 사용할 수 있음을 보여준다.

Histologic evaluation of various membranes on periodontal tissue regeneration of 1-wall intrabony defects in dogs (성견 1벽성 골내낭 결손부의 조직재생에 대한 수종의 차폐막의 조직학적 평가)

  • Purpose: In dental clinical fields, various periodontal membranes are currently used for periodontal regeneration. The periodontal membranes are categorized into two basic types: resorbable and non-resorbable. According to the case, clinician select which membrane is used. Comparing different membranes that are generally used in clinic is meaningful. For this purpose, this study evaluates histological effects of various membranes in canine one wall intrabony defect models and it suggest a valuation basis about study model. Material and Method: The membranes were non-resorbable TefGen $Plus^{(R)}$, resorbable Gore Resolut $XT^{(R)}$ and resorbable $Osteoguide^{(R)}$. One wall intrabony defects were surgically created at the second and the mesial aspect of the fourth mandibular premolars in either right or left jaw quadrants in two dogs. The animals were euthanized 8 weeks post-surgery when block sections of the defect sites were collected and prepared for histological evaluation. Results: 1. While infiltration of inflammatory cells were observed in control, TefGen $Plus^{(R)}$ and Gore Resolut $XT^{(R)}$, it was not observed in $Osteoguide^{(R)}$. 2. TefGen $Plus^{(R)}$ had higher integrity than others and $Osteoguide^{(R)}$ was absorbed with folding shape. Gore Resolut $XT^{(R)}$ was divided everal parts during resorbtion and it was also absorbed from inside. 3. Quantity of new bone and new cementum was not abundant in all membranes. 4. For histologic evaluation of membranes we should consider infiltration of inflammatory, migration of junctional epithelium, integrity of membrane, quantity of new bone and new cementum, connective tissue formation and aspect of resorption. Conclusion: This histologic evaluation suggests that $Osteoguide^{(R)}$ provides periodontal regenerative environment with less inflammatory state. It is meangful that this study model suggests a valuation basis about other study model.

Inhibitory Effects of Deer Antler Herbal-Acupuncture Solution on Cathepsin S and L Activity in RA Model Mice

  • Objective : Dear antler (Cervus korean TEMMINCK var. mantchuricus Swinhoe) used for traditional immunosuppressive and immuno-activating action. The effect of deer antler herbal-acupuncture(DAH) solution, prepared by water extract method, on cathepsin activities in bone tissues (cartilage and synovial) cells from mouse rheumatoid arthritis (RA) model was studied. The cysteine endoprotease cathepsin mediates degradation of the MHC class II invariant chain (Ii) in human and mouse antigen-presenting cells. The studies described here examine the functional significance of cathepsin inhibition on autoantigen presentation and organ-specific autoimmune diseases in a murine model for RA. Methods : An animal model for RA in BALB/c mice thymectomized 3 days after birth (3d-Tx) was constructed All 3d-Tx BALB/c mice developed autoimmune lesions in the bone tissue cells, starting at 3 weeks of age, and the disease mediated by CD4+ T cells was chronic and progressive. Significant inhibitory effects of DAH solution on cathepsin S and L were observed in each organ in a dose-dependent manner. Moreover, we confirmed that cathepsin S and L activity in each organ were clearly inhibited by DAB solution. When we examined the inhibitory effects of DAH solution against autoantigen-specific T cell responses in vitro, in regional lymph node cells, but not in spleens, from model mice, a significant inhibitory effect of DAB solution was observed in a dose-dependent manner. DAH solution do not block T cell proliferation to Con A, indicated that the dose of DAB solution 10 to $20\;{\mu}g/m{\ell}$ was sufficient to inactivate the autoantigen-specific T cell responses in vitro. In vivo therapeutic effects of DAB solution were examined in a murine model for RA, autoantigen-specific (C-II-specific) T cell response were significantly inhibited in LNCs from DAH solution-treated mice. Results : Iinhibition of cathepsin S and L in vivo alters autoantigen presentation and development of organ-specific autoimmunity in RA model. Conclusion : These data identify selective inhibition of cysteine protease cathepsin S and L as a potential therapeutic strategy for autoimmune disease process such RA. Thus, DAH solution will served as a potent anti-inflammatory and anti-arthritic agents for treatment of human RA.

HISTOLOGIC CHANGES IN MANDIBULAR PERIODONTIUM OF THE MONKEY FOLLOWING EXPERIMENTAL EXTRUSION OF ANTERIOR TEETH (실험적 전치 정출시 원숭이 하악 치주 조직의 변화)

  • The purpose of this study was to investigate the histologic changes in mandibular periodontium during overbite closure for openbite treatment by continuous arch wires and anterior vertical elastics. Two female monkey(Macaca nemestrina) with permanent dentition were used. Posterior bite block was fixed to each of their maxillae, which made the animal temporary anterior openbite as well as stabilized the whole maxillary anchorage. In each mandible, all the teeth except the second molars which had been extracted, were prepared for cast crowns. 018 inch Standard brackets were welded on these crowns. After cementation, two types of the $016{\times}022$ inch continuous arch wires, the plain ideal arch to the control animal and the MEAW(multiloop edgewise archwire) to the other experimental one were inserted. Then anterior vertical elastics were applied for two weeks. The overbite depth changes in the monkeys and histologic examinations of the mandibular periodontiums suggested the following conclusions. 1. During two weeks of the experimental period, the overbite increased + 0.3 mm in the control and + 1.3 mm in the experimental one. 2. In both the control and the experimental animal, histologic examinations showed that incisors, canines and first premolars were subject to extrusive force and the rest of posteriors were subject to intrusive one. 3. In periodontiums of the extruded incisors of the experimental one, reorientation of the periodontal fiber structures reflected the direction of force and the alveolar bone surfaces including apical and crestal areas which had been subject to tension, were the front of new bone formation. 4. In periodontiums of the extruded incisors of the experimental one, neither excessive hyalinization nor gross root resorption was observed. 5. Alveolar bone remodeling of anteriors and posteriors was more remarkable in the experimental one than the control.

Effect of Feeding Aflatoxin $B_1$ and Vitamin $D_3$ on Metatarsus Bone Minerals of Broiler Chicks (Aflatoxin $B_1$과 Vitamin $D_3$ 결여가 Broiler병아리의 중족골 무기물함량에 미치는 영향)

  • This study was conducted to investigate the interaction of aflatoxin B$_1$($AFB_1$) and vitamin D$_3$($VD_3$) in broiler chicks. The 336 broiler chicks(Hubbard line) of equally mixed sex were allocated to triplicate 8(2$\times$4 factorial) treatment groups. The 0 or 1ppm of AFB$_1$and 0, 500, 1,000 or 1,500IU/kg of VD$_3$ were supplemented to the basal diet Fourteen broilers of equally mixed sex were allocated to each replica and 24 groups were arranged in a randomized block design After 3 weeks of feeding the metatarsus were collected from the right and left legs of 4 chicks (2 for each sex) per group. The bone ash and minerals were measured. 1. In respect to the fresh weight of metatarsus bone no significant difference was found between 0 and 1ppm $AFB_1$ treatments, however, decreasing trend was recognized when fed increasing level of $VD_3$(P<.01). 2. The ash content in non-fat dry metatarsus bone decreased when fed 1ppm $AFB_1$(P<.01). However, that increased according to the increasing amount of $VD_3$(P<.01). Although there was no interaction between $AFB_1$ and $VD_3$ it was shown that the 1500IU/kg of $VD_3$ was neccessary to cover the decrease in ash content of metatarsus. when fed 1ppm of $AFB_1$. 3. The Ca contents in metatarsus were not influenced by feeding $AFB_1$ but an increasing trend was verified by feeding increasing levels of $VD_3$(P<.05). 4. The P content decreased as $AFB_1$ was fed(P<.01), while no response was found when fed'different levels of $VD_3$ 5. The Cu content decreased when fed $AFB_1$(P<.05). 6. The Na, Mg, K, Zn, Fe and Mn contents were not affected by feeding $AFB_1$ and /or $VD_3$.

  • The ultimate goal of periodontal therapy is the regeneration of the periodontium that have been destroyed as a result of periodontal disease. This study were done in order to determine the healing status of periodontium under Polytetrafluoroethylene and millipore fillter combined with fibrin and the effect of the guided tissue regeneration procedures were performed as follows : 1) flap operation using PTFE membrane(control group) 2) flap operation using PTFE membrane which was fixed with fibrin(experimental group 1) 3) flap operation using millipore filter which was fixed with suture(experimental group II) 4) flap operation using millipore filter which was fixed with fibrin(experimental group II) After 1, 2, 4, 8, 12 weeks, dogs were sacrificed by perfusion technique and tissue block was excised including the tooth and prepared for light microscope with H-E & Masson’s trichrome staining. The result were as follows : In control and experimental group, there is no siginificant difference on epithelial cell down growth within 1st week, but more epithelial cell downgrowth in millipore or millipore combined with fibrin group. In this experiment, there were no significant difference in new cementum and alveolar bone formation whether PTFE membrane was fixed with suture or fibrin. In control and each experimental group, bone maturation appeared in 4 weeks, bone width increased bucco-lingually in control and experimental 1 group especially. Both control group and experimental group showed mild mew cementum formation on root surface and irregular arrangement of collagen fiber at 4 weeks, that showed obvious increased cementum formation at 8 weeks, and that was observed the functional arrangement of collagen fiber between new cementum and new alveolar bone at 12 weeks.

Biological stability of Zirconia/Alumina composite ceramic Implant abutment (지르코니아/알루미나 복합 지대주의 생물학적 안정성에 관한 연구)

  • The purpose of the present study is to evaluate the biological stability of the zirconia/alumina composite abutment by histologic and radiographic examination in clinical cases. 17 partially edentulous patients (5 men and 12 women, mean age 47) were treated with 37 implants. The implants were placed following the standard two-stage protocol. After a healing period of 3 to 6 months, zirconia/alumina composite abutments were connected. All radiographs were taken using paralleling technique with individually fabricated impression bite block, following insertion of the prosthesis and at the 3-, 6-, 12 month re-examinations. After processing the obtained images, the osseous level was calculated using the digital image in the mesial and distal aspect in each implant. An ANOVA and t-test were used to test for difference between the baseline and 3-, 6-, 12 months re-examinations, and for difference between maxilla and mandible. Differences at P <0.05 were considered statistically significant. For histologic examination, sample was obtained from the palatal gingiva which implant functioned for 12 months. Sections were examined under a light microscope under various magnifications. Clinically, no abutment fracture or crack as well as periimplantitis was observed during the period of study. The mean bone level reduction(${\pm}standard$ deviation) was 0.34 rom(${\pm}\;0.26$) at 3-months, 0.4 2mm(${\pm}\;0.30$) at 6-months, 0.62 mm(${\pm}\;0.28$) at 12-months respectively. No statistically significant difference was found between baseline and 3-, 6-, 12-months re-examinations (p > 0.05). The mean bone level reduction in maxilla was 0.33(${\pm}0.25$) at 3-months, 0.36(${\pm}0.33$) at 6-months, 0.56(${\pm}0.26$) at 12-months. And the mean bone level reduction in mandible was 0.35(${\pm}0.27$) at 3-months, 0,49(${\pm}0.27$) at 6-months, 0.68(${\pm}0.30$) at 12-months. No statistical difference in bone level reduction between implants placed in the maxilla and mandible. Histologically, the height of the junctional epithelium was about 2.09 mm. And the width was about 0.51 mm. Scattered fibroblasts and inflammatory cells, and dense collagen network with few vascular structures characterized the portion of connective tissue. The inflammatory cell infiltration was observed just beneath the apical end of junctional epithelium and the area of direct in contact with zirconia/alumina abutment. These results suggest the zirconia/alumina composite abutment can be used in variable intraoral condition, in posterior segment as well as anterior segment without adverse effects.

EFFECT OF FLOCK SIZE ON THE PERFORMANCE OF GOATS FED GLIRICIDIA-SUPPLEMENTED DIET IN DRYLAND FARMING IN BALI, INDONESIA

  • On-farm experiments were carried out in dryland farming in Bali for 48 weeks to study the effect of flock size on the growth and carcass characteristics of cross-bred goat fed gliricidia-supplemented diet. Eighty four bucks with average live weight of 15.87 kg were allocated in a completely randomized block design arrangement, consisted of three treatments and four blocks. The treatments were $3goats/2.7m^2$ (A), $6goats/5.4m^2$ (B) and $12goats/10.8m^2$ (C), while the floor density was the same ($0.9m^2$ per goat). Feed consumed by goat B was similar (p > 0.10), while feed consumed by goat C was lower (p < 0.10) than goat A. Live weight gain of goat B and C were lower (p<0.05) than goat A. FCR of goat B was higher (p < 0.10) than goat A, while FCR of goat C was similar (p > 0.10) with goat A. Goat B has heavier (p < 0.10) head and digestive tract, while goat C has heavier (p<0.10) hindlegs and digestive tract than goat A. Goat B has lighter (p < 0.10) shoulder, while goat C has lighter shoulder and heavier legs (p < 0.10) than goat A. The carcass quality (measured in terms of loin eye muscle area, meat, bone and fat portions) were not affected (p > 0.05) by the flock sizes.

PERFORMANCE OF THE GOAT FED GRASS, SHRUB AND TREE FODDERS DURING THE DRY SEASON IN BALI, INDONESIA

  • An experiment was carried out for 12 weeks to study the feeding behaviour, growth and carcass characteristics of cross-bred goats. Nine bucks with an average live weight of $18.02{\pm}0.9kg$ were allocated in a completely randomized block design arrangement, consisting of three feeding regimes and three blocks. The feeding regimes consisted of 100% natural grass (Treatment A), 100% gliricidia leaf (Treatment B) and 100% ficus leaf (Treatment C). Frequency of turning-over and sniffing the feed and duration and frequency of ruminating, defaecating, and urination of goats in treatment B were longer, while duration of masticating and jaw movement of chewing one bolus was shorter than those of goats in treatments C and A (p < 0.05). Furthermore, goats in treatment B gained more live weight, consumed more feed, consumed less water and was more efficient in utilizing the feed than goats in treatments C or A (p < 0.05). The dressing percentage, carcass length and carcass fat of goats in treatment B were higher and their bone percentages were lower than those in treatments C and A (p < 0.05). There is an indication that feeding gliricidia and ficus leaves during the dry season could not only increase the body weight and improve carcass quality of the goat but also save water for household needs.

Targeting a Safe Entry Point for C2 Pedicle Screw Fixation in Patients with Atlantoaxial Instability

  • Objective : This investigation was conducted to evaluate a new, safe entry point for the C2 pedicle screw, determined using the anatomical landmarks of the C2 lateral mass, the lamina, and the isthmus of the pars interarticularis. Methods : Fifteen patients underwent bilateral C1 lateral mass-C2 pedicle screw fixation, combined with posterior wiring. The C2 pedicle screw was inserted at the entry point determined using the following method : 4 mm lateral to and 4 mm inferior to the transitional point (from the superior end line of the lamina to the isthmus of the pars interarticularis). After a small hole was made with a high-speed drill, the taper was inserted with a 30 degree convergence in the cephalad direction. Other surgical procedures were performed according to Harm's description. Preoperatively, careful evaluation was performed with a cervical X-ray for C1-C2 alignment, magnetic resonance imaging for spinal cord and ligamentous structures, and a contrast-enhanced 3-dimensional computed tomogram (3-D CT) for bony anatomy and the course of the vertebral artery. A 3-D CT was checked postoperatively to evaluate screw placement Results : Bone fusion was achieved in all 15 patients (100%) without screw violation into the spinal canal, vertebral artery injury, or hardware failure. Occipital neuralgia developed in one patient, but this subsided after a C2 ganglion block. Conclusion : C2 transpedicular screw fixation can be easily and safely performed using the entry point of the present study. However, careful preoperative radiographic evaluation, regardless of methods, is mandatory.
