Complication and management of implant-assisted removable partial denture with distal extension: a clinical report (양측성 후방연장 임플란트 보조 국소의치의 합병증과 관리: 증례보고)

  • Implant supported removable partial denture (ISRPD) using the implants enables favorable rehabilitation by complementing biomechanical limitations of the conventional removable partial denture (RPD). However, continuous recall check is necessary for evaluation of the mechanical and biological complications to ensure good long-term prognosis of ISRPD. This clinical report describes the complication and management in patient of Kennedy class I edentulism with ISRPD using healing abutment. The wear and fracture of healing abutment occurred at 36 months after delivery. So, healing abutment was replaced by connecting $Locator^{(R)}$ abutment for altering into the implant retained partial overdenture.

Operative Treatment of Distal Clavicle Fracture Nonunion (원위 쇄골 불유합의 수술적 치료)

  • Purpose: The distal clavicle has a biomechanical structure different from that of the proximal or middle 1/3 clavicle, and delayed union or nonunion occurs frequently in a distal clavicle fracture. The authors obtained favorable results from an open reduction and bone grafting of the distal clavicle nonunion. We report the results together with review of the relevant literature. Materials and Methods: The subjects were 8 patients(average age, 38.9) who had undergone surgery for distal clavicle nonunion from August 2003 to May 2006. Nonunion occurred after surgical treatment in 4 cases, and after conservative treatment in the other 4. In all cases, the patients complained of pain. Results: The mean follow-up duration was 14 months, and radiological union was observed in 8 weeks on average. In all cases, the range of shoulder joint motion was normal at the end of the follow-up observation. In the functional evaluation, 7 cases showed excellent results and 1 case showed good results. Conclusion: Surgical treatment is a safe and reliable treatment for distal clavicle fracture nonunion because it can achieve early rehabilitation and union.

Evaluation of Insole-equipped Ankle Foot Or thosis for Effect on Gait based on Biomechanical Analysis (인솔 장착형 단하지 보조기의 생체 역학적 분석을 통한 보행 영향성 평가)

  • The purpose of this study was to evaluate the effects of insole-equipped ankle-foot-orthoses (AFO) on gait. 10 healthy males who had no history of injury in the lower extremity participated in this study as the subjects. The foot of each subject was first scanned, and the insole fit to the plantar was made using BDI-PCO(Pedcad Gmbh, Germany). The subject then was made to walk on a treadmill under four experimental conditions: 1) normal walking, 2) walking wearing AFO, 3) walking wearing AFO equipped with the insole, 4) walking wearing pneumatic-ankle-foot-orthosis (pAFO) equipped with the insole. During walking, foot pressure data such as maximum force, contacting area, peak pressure, and mean pressure was collected using Pedar-X system (Novel Gmbh, Germany) and EMG activity of lower limb muscles such as gastrocnemius medial head, gastrocnemius lateral head, and soleus was recorded using MP150 EMG module (BIOPAC System Inc., USA). Collected data was then analyzed using paired t-test in order to investigate the effects of the insole. As a result of the analysis, when insole was equipped, overall contacting area was increased while both the highest peak pressure and the mean pressure were significantly decreased, and EMG activity of the lower limb muscles was decreased. On the contrary, the cases of wearing AFO showed the decreased contacting area and the increased pressures. Therefore, the AFO equipped with a proper insole fit well to the foot can help comfortable walking by spreading the pressure over the entire plantar.

Comparison of removal torque of dual-acid etched and single-acid etched implants in rabbit tibias (단일, 이중 산처리 임플란트의 회전제거력 비교)

  • Purpose: Chemically strong-acids (HF and $HCl/H_2SO_4$) dual etching implant surfaces have higher strengths of osseointegration than machined implant surfaces. However, the dual acid treatment deteriorates the physical properties of the titanium by weakening the fatigue resistance of the implant and causing microcracks. The removal torque comparison between the dual-acid etched (hydrochloric acid, sulfuric acid, HS) and single-acid etched implants (hydrochloric acid, H) could reveal the efficiency of implant surface acid treatment. Materials and methods: Nine $3.75{\times}4mm$ dual-acid etched SLA implants and nine single-acid etched SLA implants were inserted into New Zealand rabbit tibias. After 10 days, removal torque, roughness, and wetting angle were measured. Results: Mean removal torque values were as follows: Mean removal torque were 9.94 Ncm for HS group and 9.96 Ncm for H group (P=.995). Mean surface roughness value were $0.93{\mu}m$ for HS group and $0.84{\mu}m$ for H group (P=.170). Root mean square roughness (RSq) values were $1.21{\mu}m$ for HS group and $1.08{\mu}m$ for H group (P=.294), and mean wetting angle values were $99^{\circ}$ for HS group and $98^{\circ}$ for H group (P=.829). Statistical analysis showed no significant difference between the removal torques, roughness, or wetting angles of the two groups. Conclusion: In this experiment, we found no significant difference in removal torque, roughness, or wetting angle between dual-acid etched and single-acid etched implants.

A comparison between the modified Brostrom procedure using single and double suture anchor for chronic lateral ankle instability (발목관절 외측 불안정성에서 단일 봉합나사와 이중 봉합나사를 이용한 변형 Brostrom 술식간의 임상결과 비교)

  • Purpose: This study was retrospectively performed to compare the clinical outcomes of modified Brostrom procedures using the single suture anchor and the double suture anchor for chronic lateral ankle instability. Materials and Methods: Thirty-seven patients were followed up for more than 1 year after the modified Brostrom procedures using suture anchor. Single surgeon treated seventeen cases with single suture anchor and 20 cases with double suture anchor. The clinical evaluation was performed according to the Karlsson scale and Sefton grading system. Radiologic measurement of the talar tilt and anterior talar translation was performed through anterior and varus stress radiographs using Telos device. Results: The Karlsson scale had improved significantly from preoperative average of 45.2 points to 89.4 points in single suture anchor group, and from 46.4 points to 90.5 points in double suture anchor group. According to the Sefton grading system, 15 cases (88.2%) in single suture anchor group and 18 cases (90%) in double suture anchor group achieved satisfactory results. The talar tilt angle and anterior talar translation had improved significantly from preoperative average of $13.6^{\circ}$ and 8.6 mm to $5.4^{\circ}$ and 4.1 mm in single suture anchor group, from $14.1^{\circ}$ and 8.4mm to $3.9^{\circ}$ and 4 mm in double suture anchor group. Double suture anchor technique was significantly superior in postoperative talar tilt. Conclusion: Single and double suture anchor techniques produced similar clinical and functional outcomes except for talar tilt, which was significantly superior in double suture anchor group. Both modified Brostrom procedures using the single and double suture anchor appear to be effective treatment methods for chronic lateral ankle instability. Further evaluation of clinical outcomes and biomechanical studies in athletes are needed.
