생물학적 보철판막의 조직실패 (Primary Tissue Filure of Bioprosthetic Valves)

  • Boprosthetic cardiac valves fail from biological and metabolic as well as mechanical reasons, and the limited durability is the main factor of marked withdrawal in their clinical use. Starting the use of bioprosthetic valves in 1976, up to the end of 1992, the consecutive 178 patients have undergone re-replacement of glutaraldehyde-treated xenograft valves for primary tissue failure [PTF]among the patients who had initial valve replacement at Seoul national University Hospital. The explanted valves were 69 porcine aortic [51 Hancock, 12 Angell-Shiley and 6 Carentier-Edwards] and 141 bovine pericardial [129 standard-profile and 12 low-profile ionescu-Shiley] valwes, with an overall incidence of PTF of 15.2%. The operative mortality rate of re-replacement was 5.1%. Calcific degeneration and tissue damage in relation to calcification were the most frequent modes of PTF on gross examinatin of the explanted valves resulting hemodynamically in valvular regurgitation. The number of Hancocg porcine and the standard-profile Ionescu-Shiley valves in valves in mitral position failed more often from tissue damage [tears, holes, and loss or destruction of cuspal tissue] than calcification [68.3% vs. 39.0%, p<0.01] with resultant regurgitation in 61%, the Ionescu-Shiley valves in the same position in 53%. The tendency of more calcification than tissue damage[71.3% vs. 33.3%, p<0.001]with stenosis in 53%. The tendency of more calcification and immobility of cusps in the latter group was partly explainable by the inclusion of patients of pediatric age. Observation made in this study suggest : many of bioprosthetic valves would fail from calcification and tissue damage : some fail prematurely because of mechanical stress probably owing to the valve design in construction ; andeven those valves escaped early damage would be subject to calcify in the prolonged follow-up period. In conclusion, at the present time, the clinical use of bioprostheticxenograft valves seems to be quite limited until further improvement in biocompatibility and refinement in valve design in manufacture are achieved.

난소절제 쥐에서 비만과 지방조직 염증에 대한 제니스테인과 운동의 영향 (The effects of genistein and exercise on obesity and adipose tissue inflammation in ovariectomized mice)

  • 본 연구는 고지방식이를 섭취한 난소절제 쥐의 지방조직에서 제니스테인에 의한 지방조직감소와 항염증에 대한 운동의 영향을 조사하였다. 고지방식이를 섭취한 쥐(C)에 비해 제니스테인 단독처리(G)와 운동 단독처리(Ex)는 지방조직무게, 지방세포 크기 및 혈청 속 포도당과 유리 지방산 수치를 감소시켰으며, 이러한 현상은 제니스테인과 운동을 동시처리(G/Ex) 하였을 경우 더 효과적으로 감소하였다. 지방조직에서 C에 비해 G와 Ex의 지방생성 유전자와 염증성 사이토카인 유전자의 발현은 모두 감소하였고, 특히 이러한 유전자의 감소 현상은 G와 Ex 각각 보다 G/Ex에서 더욱 효과적이었다. 따라서 본 연구는 고지방식이를 섭취한 난소절제 쥐에서 제니스테인과 운동의 동시처리는 제니스테인 단독처리에 비해 더 효과적으로 비만과 지방조직 염증을 개선하였으며 이것은 지방생성 유전자를 긍정적으로 조절한 결과임을 밝혔다. 본 연구는 제니스테인의 항비만과 지방조직 항염증에 대해 운동이 유익한 영향을 미친다는 것을 제안한다.

Overexpression and Selective Anticancer Efficacy of ENO3 in STK11 Mutant Lung Cancers

  • Oncogenic gain-of-function mutations are clinical biomarkers for most targeted therapies, as well as represent direct targets for drug treatment. Although loss-of-function mutations involving the tumor suppressor gene, STK11 (LKB1) are important in lung cancer progression, STK11 is not the direct target for anticancer agents. We attempted to identify cancer transcriptome signatures associated with STK11 loss-of-function mutations. Several new sensitive and specific gene expression markers (ENO3, TTC39C, LGALS3, and MAML2) were identified using two orthogonal measures, i.e., fold change and odds ratio analyses of transcriptome data from cell lines and tissue samples. Among the markers identified, the ENO3 gene over-expression was found to be the direct consequence of STK11 loss-of-function. Furthermore, the knockdown of ENO3 expression exhibited selective anticancer effect in STK11 mutant cells compared with STK11 wild type (or recovered) cells. These findings suggest that ENO3-based targeted therapy might be promising for patients with lung cancer harboring STK11 mutations.

Sustained release of alginate hydrogel containing antimicrobial peptide Chol-37(F34-R) in vitro and its effect on wound healing in murine model of Pseudomonas aeruginosa infection

  • Background: Antibiotic resistance is a significant public health concern around the globe. Antimicrobial peptides exhibit broad-spectrum and efficient antibacterial activity with an added advantage of low drug resistance. The higher water content and 3D network structure of the hydrogels are beneficial for maintaining antimicrobial peptide activity and help to prevent degradation. The antimicrobial peptide released from hydrogels also hasten the local wound healing by promoting epithelial tissue regeneration and granulation tissue formation. Objective: This study aimed at developing sodium alginate based hydrogel loaded with a novel antimicrobial peptide Chol-37(F34-R) and to investigate the characteristics in vitro and in vivo as an alternative antibacterial wound dressing to treat infectious wounds. Methods: Hydrogels were developed and optimized by varying the concentrations of crosslinkers and subjected to various characterization tests like cross-sectional morphology, swelling index, percent water contents, water retention ratio, drug release and antibacterial activity in vitro, and Pseudomonas aeruginosa infected wound mice model in vivo. Results: The results indicated that the hydrogel C proved superior in terms of cross-sectional morphology having uniformly sized interconnected pores, a good swelling index, with the capacity to retain a higher quantity of water. Furthermore, the optimized hydrogel has been found to exert a significant antimicrobial activity against bacteria and was also found to prevent bacterial infiltration into the wound site due to forming an impermeable barrier between the wound bed and external environment. The optimized hydrogel was found to significantly hasten skin regeneration in animal models when compared to other treatments in addition to strong inhibitory effect on the release of pro-inflammatory cytokines (interleukin-1β and tumor necrosis factor-α). Conclusions: Our results suggest that sodium alginate -based hydrogels loaded with Chol-37(F34-R) hold the potential to be used as an alternative to conventional antibiotics in treating infectious skin wounds.

Suppression of Migration and Invasion by Alnus hirsuta in Human Hepatocellular Carcinoma Cells

  • Hepatocellular carcinoma (HCC) has a poor prognosis and high metastasis and recurrence rates. Although extracts of Alnus hirsuta (Turcz. ex Spach) Rupr. (AH) have been demonstrated to possess potential anti-inflammatory and anti-cancer activities, the underlying mechanism of AH in HCC treatment remains to be elucidated. We investigated the effects and potential mechanisms of AH on migration and invasion of Hep3B cells. Within the non-cytotoxic concentration range, AH significantly inhibited motility and invasiveness of Hep3B cells in a concentration-dependent manner. Inhibitory effects of AH on cell invasiveness are associated with tightening of tight junctions (TJs), as demonstrated by an increase in transepithelial electrical resistance. Immunoblotting indicated that AH decreased levels of claudins, which form major components of TJs and play key roles in the control and selectivity of paracellular transport. Furthermore, AH inhibited the expression and activity of matrix metalloproteinase (MMP)-2 and MMP-9 and simultaneously increased the levels of tissue inhibitor of metalloproteinase (TIMP)-1 and TIMP-2. These effects were related to inactivation of the phosphoinositide 3-kinase (PI3K)/AKT pathway in Hep3B cells. Therefore, AH inhibits migration and invasion of Hep3B cells by inhibiting the activity of MMPs and tightening TJs through suppression of claudin expression, possibly by suppressing the PI3K/AKT signaling pathway.

온침 시술 시 생체 조직 내 열분포 분석에 관한 연구 (Thermal Distribution in Living Tissue during Warm Needling Therapy)

  • Objectives This study aims to analyze a thermal distribution in biological living tissue during warm needling therapy by using a finite element method. The analysis provides an understanding of warm needling's efficacy and safety. Methods A model which consisted of four-layered tissue and stainless steel needle was adopted to analyze the thermal distribution in living tissue with a bioheat transfer analysis. The governing equation for the analysis was a Pennes' bioheat equation. A heat source characteristic of warm needling therapy was obtained by previous experimental measurements. The first analysis of the time-dependent temperature distribution was conducted through points on a boundary between the needle and the tissue. The second analysis was conducted to visualize the horizontal temperature distribution. Results When heat source's peak temperatures was above $500^{\circ}C$ and temperature rising rates were relatively slow, the peak temperature at skin surface exceeded a threshold of pain and tissue damage ($45^{\circ}C$), whereas when the peak temperature was around $400^{\circ}C$, the peak temperature at the skin surface was within a safe limit. In addition, the conduction of combustion energy from the moxa was limited to the skin layer around the needle. Conclusions The results suggest that the skin layer around the needle can be heated effectively by warm needling therapy, but it appears to have little effect at the deeper tissue. These findings enhance our understanding of the efficacy and the safety of the warm needling therapy.
