Purpose: The authors would like to introduce two patients who presented with velopharyngeal inadequacy. We emphasize the importance of nasaopharyngeal endoscopy in evaluating the velopharyngeal function and the usefulness of biofeedback trial therapy. Methods: Two patients visited our clinic due to velopharyngeal inadequacy. Both of the patients showed hypernasality, nasal emission and compensatory articulation such as glottal stop. During oral examination and nasopharyngeal endoscopy both showed no evidence of structural deformities. One inconsistently showed a small gap during articulation. The other showed a rather large gap during compensatory articulation. Both received a simultaneous biofeedback trial therapy using the nasopharyngeal endoscope. Results: Both patients were successfully diagnosed and treated at once using biofeedback trial therapy with nasopharyngeal endoscopy. By giving direct visual feedback to the patient, they were both able to achieve complete velopharyngeal closure during production of 2~3 nonsence syllables and hypernasality was not detected in both of them. Conclusion: The authors were able to help patients with velopharyngeal inadequacy to have velopharyngeal closure through biofeedback trial therapy. The accurate evaluation of velopharyngeal function and the possibility of closure prevented unnecessary operations.

Purpose: To compare the effects of intra-anal biofeedback (BF) and intra-anal electrical stimulation (ES) on pelvic floor muscles (PFMs) activity, nocturnal bladder capacity, and frequency of wet night episodes in children with refractory primary monosymptomatic nocturnal enuresis (PMNE). Methods: Ninety children of both sexes aged 8-12 years with refractory PMNE participated in this study. They were randomly assigned to 3 groups of equal number: control group (CON) that underwent behavioral therapy and PFM training, and 2 study groups (BF and ES) that underwent the same program in addition to intra-anal BF training and intra-anal ES, respectively. PFMs activity was assessed using electromyography, nocturnal bladder capacity was evaluated by measuring the first morning voided volume, and a nocturnal enuresis diary was used for documenting wet night episodes before treatment and after 3 months of treatment. Results: After training, all groups showed statistically significant improvements in all measured outcomes compared to their pretreatment findings. The ES group showed significantly greater improvements in all measured outcomes than the CON and BF groups. Conclusions: Both intra-anal BF training and ES combined with behavioral therapy and PFMs training were effective in the treatment of PMNE, with intra-anal ES being superior to BF training.
Objective: The purpose of this study was to confirm the validity of a respiratory retraining exercise using pressure biofeedback units among individuals with stroke as an effective intervention for improving quality of life. Design: Randomized controlled trial. Methods: Thirty patients with stroke were recruited as subjects. Among them, 15 patients were randomly assigned to an experimental group performing lumbar stabilization exercise and respiratory retraining exercise, and the other 15 patients were randomly assigned to a control group conducting only lumbar stabilization exercises. Exercises were conducted 3 times a week for 6 weeks, and quality of life was evaluated in the pre-test, 3 weeks and 6 weeks periods. The respiratory retraining exercises were performed using a pressure biofeedback unit and the degree of the quality of life was measured using the Stroke- Specific Quality of Life. For data analysis on the study results, a two-way repeated ANOVA was used in order to observe for changes in the measured variables according to time for both groups. If there was a reciprocal action between the groups and the time in the effect test within the entities, a one-way repeated ANOVA was implemented and was statistically processed. Results: There was a significant difference in the main effect test between the 2 populations depending on the duration of the experiment (6 weeks) (p<0.05). Conclusions: The above results showed that respiratory retraining exercises may provide positive effects in the treatment of stroke as the quality of life showed significant differences according to the duration of treatment.
Objective: Forward head posture can caused by deformation of structures and soft tissues around the neck, which has an uncomfortable effect on daily life as well as functional disorders of the neck. However, studies related to direct forward head posture, neck function, and quality of life have not yet been conducted. Therefore, the purpose of this study is to investigate the effect of improving the forward head posture on the function of the neck and to examine the change in the quality of life. Design: A randomized controlled trial Methods: The participants were 41 adults (22.17±2.67 years) recruited and redivided randomly into two group (Biofeedback, BFG vs. Control). The Biofeedback group was proceeded according to the over load principle through 4 steps. (n=21). The control (n=20) was not applied after TENS padding was applied and 20 minute. This study was conducted three times a week for a total of four weeks. Results: Forward head posture for showed significant improvement in the results in the craneocervical angle (p<0.05, 95% CI: 0.130, 2.858). In neck disability index more significant improvement in BFG than group (p<0.05, 95% CI: 14.346, 17.825), and BFG showed significant increased in the results in the Quality of life (p<0.05, 95% CI: 0.392, 9.549) Conclusions: This study suggected that forward head posture treatment and effective for neck function and quality of life

On the other hand, EEG biofeedback treatment studies for ADHD have reported promising results not only in significant reductions in hyperactive, inattentive, and disruptive behaviors, but also improvements in academic performance and IQ scores. However it is too boring for children to finish the whole treatment. The recent increase in computer usage in medicine and rehabilitation has changed the way health care is delivered. Virtual Reality technology provides specific stimuli that can be used in removing distractions and providing environments that get the subjects'attention and increasing their ability to concentrate. VR technology can hold a patient's attention for a longer period of time than other methods can, because VR is immersive, interactive and imaginal. Based on these aspects, we developed Attention Enhancement System (AES) using VR technology, EEG biofeedback, and cognitive training method for enhancing attention and made a clinical trial to people who have attention difficulty and behavioral problems.

PURPOSE: This study examined the changes in the thickness of the abdominal muscles, including the transversus abdominis, according to the set pressure applied by a pressure biofeedback unit during contractions of the hip adductor muscles. METHODS: After randomizing 40 healthy adult males in their 20 s and 30 s, the participants were instructed to match the pressure gauge indication of the pressure biofeedback device to continue contracting the hip adductor while maintaining it at 10 mmHg (low), 40 mmHg (medium), or 70 mmHg (high). The measurement was taken over five seconds using an ultrasound device. RESULTS: According to the contractile pressure applied to the hip adductor muscle, there was a significant difference in the muscle thickness change of the transverse abdominis muscle between 10 mmHg and 70 mmHg and between 40 mmHg and 70 mmHg. The muscle thickness ratio of the external oblique/abdominal muscle was significantly different between 10 mmHg and 70 mmHg and between 40 mmHg and 70 mmHg. CONCLUSION: Increased contraction pressure on the hip adductor muscle increases the muscle thickness of the abdominal transverse muscle. Interbody stability exercise with contractions of the hip adductor muscle is expected to help increase in the muscle thickness of the hip adductor muscle.

Objective: The purpose of this study was to investigate the effects of a three dimensional balance trainer in combination with a video-game system and visual feedback on balance recovery and gait function in subacute stroke patients. Design: Randomized controlled trial. Methods: Twenty-three subacute stroke patients were assigned to either an experimental (n=12) or a control group (n=11) using a random permuted block design and sealed envelopes. The experimental group received additional 3-dimensional balance training combined with visual feedback and a game program for 30 min/day, 5 days/week for 4 weeks. Both groups received 30-min of conservative physical therapy sessions based on neurodevelopmental therapy. Before and after the 20 sessions, walking abilities were evaluated by the the GaitRite system and balances were evaluated using the Berg Balance Scale (BBS). The Trunk Impairment Scale (TIS) was used to assess trunk muscle performances. Results: After the 4-week intervention, BBS and TIS scores were significantly increased in both groups (p<0.05), and increases in these scores were significantly greater in the experimental group (p<0.05). After the 4-week intervention, gait speed and cadence were significantly increased in both groups (p<0.05), and as was observed for BBS and TIS scores, changes of gait speed and cadence were significantly greater in the experimental group (p<0.05). Conclusions: The study shows that the 3-dimensional balance trainer combined with visual biofeedback and a video-game system provides a therapeutic means for improving balance and gait ability in subacute stroke patients.
