배부근과 복부근에 대한 테이핑 적용이 체간 굴곡, 신전 근력에 미치는 영향 (The Change of Strength of Trunk Flexion and Extension by Intervention of Taping on Abdominal and Back muscles)

  • Purpose : The purpose of this study was to measure the change of strength of trunk flexion and extension on abdominal and back muscles. Methods : Each taping group 1(n=14), taping group 2(n=14), taping group 3(n=14) and control group(n=15) were measured a muscle strength by Biodex at first test, second test, third test and forth test in 3 days. Results : The strength of trunk flexion and extension was significantly different among the experimental groups (p<.05). It also shows that significant increasing of muscle strength at 48 hours after application of taping (p<.05). Conclusion : In conclusion, we were found that taping technique base on taping type show immediately assistance of muscle strength and enduring effect for 48 hours.

Effects of Closed-Sling Exercise on Muscle Activity and Balance; Football Club Player with Chronic Ankle Instability

  • The purpose of this study was to investigate the effects of sling exercise on muscle activity and balance on football players with chronic ankle instability. Eight subjects performed Biodex and sling exercises for four weeks and compared the muscle activity and balance ability of the uninjured and injured feet. Stable and unstable foot % MVIC did not change significantly after treatment in all muscles. The total balance ability was not significantly different between the two groups, however, increased only in the unstable side. The anterior/posterior balance ability also represented no significant difference between the groups and was significantly improved only in the unstable side. The medial/lateral balance ability was not significantly different between the groups and was significantly improved only in the unstable side. This study suggests that sling exercise contributes to improving % MVIC, total balance ability, anterior/posterior balance ability, and medial/lateral balance ability of the unstable side.

슬링 운동이 무릎관절 전치환술을 시행한 여성 환자의 근력과 관절가동범위에 미치는 영향 (The Effect of Sling Exercise on Muscular Strength and Range of Motion in Female Patients who Received Total Knee Replacement)

  • 본 연구의 목적은 무릎관절 전치환술을 시행한 여성 환자에게 슬링 운동을 적용하여 무릎관절의 근력과 관절가동범위에 미치는 영향을 비교하기 위해 실시하였다. 참여자는 2그룹 나누어 슬링 운동군 15명, 대조군 15명으로 무작위로 배정하였다. 대상자들은 근력 검사를 위해 Biodex system를, 관절가동범위 검사를 위해 각도기를 사용하여 평가하였다. 데이터의 분석은 통계적 유의 수준을 결정하기 위해서 대응표본 t-검정과 독립 t-검정을 이용하였다. 본 연구의 결과 슬링 운동군은 중재 전후 굽힘 각, 넙다리네갈래근, 뒤넙다리근 근력이 통계학적으로 유의한 증가를 보였고, 대조군은 중재 전후 굽힘 각, 폄 각, 넙다리네갈래근, 뒤넙다리근 근력이 통계학적으로 유의한 증가를 보였다. 무릎관절 근력과 굴곡 관절 가동범위는 슬링 운동군이 대조군에 비해 통계학적으로 유의하게 증가하였다. 슬링 운동법이 무릎관절 전치환술 환자의 근력과 관절 가동범위에서 긍정적인 영향이 있음을 알 수 있었다.

등속성 운동검사를 이용한 정상 한국인 성인에서의 발목관절 근력 측정 (Measurement of Muscle Strength of Ankle Joint Using Isokinetic Dynamometer in Normal Korean Adults)

  • Purpose: Restoration of ankle stability through the strengthening exercise of peroneus muscles is considered an important factor for achievement of successful outcomes, in the rehabilitation program following ankle ligament injuries. However, there were few definitive data on normal muscle strength, including eversion power by peroneus muscles. This study was conducted to evaluate the muscle strength of ankle joint measured using an isokinetic dynamometer in normal Koreans. Materials and Methods: Sixty adults (120 ankles) were recruited and divided into three groups (20 in their twenties, 20 in thirties, and 20 in forties). Each group consisted of 10 males and 10 females. The selection criteria were no history of ankle injury and no evidence of instability. The peak torque, total work, and deficit ratio were measured using the Biodex$^{TM}$ (Biodex Medical Systems). Differences in muscle strength by age, gender and dominant versus non-dominant side were analyzed. Results: The peak torque of dorsiflexion was average 31.5 Nm at $30^{\circ}/s$ of angular velocity and 18.8 Nm at $90^{\circ}/s$; average 69.3 Nm ($30^{\circ}/s$) and 42.4 Nm ($90^{\circ}/s$) on plantarflexion; average 19.6 Nm ($30^{\circ}/s$) and 10.8 Nm ($90^{\circ}/s$) on inversion; average 12.9 Nm ($30^{\circ}/s$) and 8.0 Nm ($90^{\circ}/s$) on eversion. The deficit ratio of strength in women was average 61.1% of men on dorsiflexion; average 66.2% on plantarflexion; average 48.5% on inversion; average 55.4% on eversion. The deficit ratio in non-dominant foot was average 88.6% of dominant foot on dorsiflexion; average 90.1% on plantarflexion; average 85.1% on inversion; average 85.6% on eversion. Conclusion: The muscle strength of the ankle joint showed a tendency to weaken with age. There were significant differences in muscle strength by gender and dominancy. Further studies for comparison of patients with ankle instability, a comparison between before and after surgery for instability, the correlation between clinical outcomes and the recovery in muscle strength will be needed.

로잉운동의 부하편차 방식 적용에 따른 상하지 생체 역학적 평가 (Evaluation of Bio-Mechanical Character istics According to Loading Deviation Methods dur ing Rowing Exercise)

  • The purpose of this study was to verify the validation of exercise effect with loading deviation during rowing exercise. We performed evaluation of based physical fitness and joint torque before the experiment for finding muscle unbalance. So we recruited twenty four subjects who have bigger muscle strength in more 20% than average one. Subjects divided two groups. One is dominant upper limbs(DU) and the other was dominant lower limbs(DL). Subjects performed rowing exercise using electric equipment (Robo.gym.Humonic.korea). Exercise is performed four sets a day including 25 times a set, and three days a week. Measurement consist of evaluation of based physical fitness and joint torque using biodex(biodex system3.USA). Evaluation of exercise effect performed each week in joint torque of shoulder, lumbar and knee joint and each month in based physical fitness. Also we adapted 30% of 1RM for muscular endurance and 70% of 1RM for muscle strength as exercise load. The results showed that the difference of maximal peak torque were getting increase significantly during exercise. Also difference of various factor in based physical fitness were getting increase significantly except flexibility and agility. This interpreted that rowing exercise with loading deviation types could provide muscle strength and muscular endurance exercise in same time. These results could be interpret to two ways. One is effect of improving physical fitness for rowing exercise and the other meaned validation of loading deviation in rowing exercise. Our study is going to verify the validation of loading deviation during rowing and we found out that loading deviation could provide muscle strength and muscular endurance exercise for improving muscle unbalance. Our study can be used development of exercise equipment and program for normal people with muscle unbalance. Also that provide effect of whole body exercise to anybody.

만성뇌졸중 환자에 시각적 되먹임 트레드밀 보행 훈련이 보행과 균형 능력에 미치는 효과 (The Effect of Treadmill Gait Training Accompanied by Visual Feedback on the Gait and Balance of Chronic Stroke Patients)

  • Purpose: This study compares the effects of treadmill gait training accompanied by visual feedback and general treadmill gait training on the gait and balance ability of patients with chronic stroke. Methods: A total of 11 patients with chronic stroke were randomly divided into either the treadmill gait training accompanied by visual feedback group (six patients) or the general treadmill gait training group (five patients). The gait and balance ability of the two groups were measured before and after the interventions using the functional reach test, the Timed Up and Go (TUG) test, Berg's balance test, and the Biodex balance test. The treadmill gait training accompanied by visual feedback group performed the exercise under the supervision of a therapist after first being provided with a hat and a goal that was devised for the purpose of providing visual feedback information. The interventions were applied to the respective groups for four weeks. For the statistical analysis, we conducted a Mann-Whitney test to compare the results between the two groups. Additionally, the Wilcoxon test was used to compare the results from before and after the intervention in each group. Results: The treadmill gait training accompanied by visual feedback group showed a significant difference in terms of the functional reach test after the intervention when compared to the general treadmill gait training group (p<0.05). Although there was no significant difference, the treadmill gait training accompanied by visual feedback group showed a larger improvement in the TUG test, Berg's balance test, and the Biodex balance test than the general treadmill gait training group (p>0.05). Conclusion: The results of this study suggest that treadmill gait training accompanied by visual feedback can be used as a beneficial intervention scheme for the recovery of the gait and balance ability of patients with chronic stroke.

기능적 발목 불안정성시 하지 근피로에 의한 동적균형이 족저압에 미치는 영향 (The Effect of the Plantar Pressure on Dynamic Balance by Fatigue of Leg in the Subjects with Functional Ankle Instability)

  • 본 연구는 기능적 발목 불안정성시 하지 근피로도에 따른 동적균형이 족저압에 미치는 영향에 대해 알아보고자 하였다. 본 연구는 설문조사를 통해 기능적 발목 불안정성으로 진단된 14명의 발목불안정집단과 16명의 발목안정집단 총 30명을 대상으로 실시하였다. 하지의 근피로는 Biodex system III, 족저압은 Zebris FDM-S system으로 측정하였고, 한발로 점프 후 착지 검사로 동적균형을 실시하였다. 동적균형시 족저압 분포비율은 발목불안정집단의 전족부(p2, p3, p4)에서 높게 나타났다(p<0.05). 동적균형시 근피로 유발 전후 족저압 분포 비율은 발목 불안정집단에서 발목 안정집단보다 전족부(p2, p3, p4)와 외측 중족부(p6)에서 유의하게 높았으나 종골부(p7)에서는 낮았다(p<0.05). 동적균형시 족저압 중심(CoP)의 전후이동거리(AP)와 좌우이동 폭(ML)은 발목 불안정집단이 발목 안정집단에 비해 근피로 유발 전보다 유발 후 유의하게 길었다(p<0.05). 기능적 발목불안정시 하지 근피로도에 따른 동적균형은 족저압 분포와 족저압 중심이동에 영향을 준 것으로 사료된다. 향후, 기능적 발목 불안정이 있는 다양한 연령대를 대상으로 족저압 차이를 객관화, 척도화 시키는 연구가 이루어져야 할 것으로 사료된다.

서울 일부 지역 노인들의 등속성 근기능, 유연성, 균형성 측정 및 평가 (Assessment of Elderly's Isokinetic Muscle Function, Flexibility and Balance in a Region of Seoul)

  • 이 연구는 고령화 사회를 대비하여 노인 체력 향상 프로그램 설계 및 관련 정책을 수립하기 위한 기초 자료를 수집하기 위한 목적으로 실시되었다. 이를 위해 2013년, 서울 동대문구 소재 노인종합복지관에 등록 중인 노인 52명과 동대문구 소재 대학교 학부 과정에 재학 중인 학생 46명을 연구 대상으로 하여 하지의 등속성 근기능과 발목 관절가동범위, 유연성, 균형성 및 신체조성 등을 측정하였으며, Biodex system IV, Biodex stabilizer system, CNP-5403, Inbody 3.0이 사용되었다. 남자 노인 집단의 평균 연령은 77.13세였으며, 45.85kg의 근육량을 가지고 있었고, 체전굴은 2.97cm로 유연성이 아주 결여되어 있었다. 좌측발의 균형 능력이 우측보다 좋지 않았으며, 발바닥쪽굽힘의 관절 가동 범위는 35도 내외로 정상 범위에서 벗어났다. 전반적인 무릎 폄근과 굽힘근의 근력은 남자 대학생의 50% 정도 수준이었으며, 뒤넙다리근의 결손율이 18.55%로 균형이 깨어져 있었다. 발목 가쪽번짐-안쪽번짐 결손율 또한 23.08%, 19.19%로 나타났다. 여자 노인 집단의 평균 연령은 75.46세로 근육량은 35.68kg이었으며, 체전굴은 11.69cm로 여자 대학생 집단과 비슷하게 좋은 편이었다. 무릎의 동측 근력비가 좌우 모두 50% 미만이었고, 결손율 또한 폄 14.32%, 굽힘 19.73%로 좌우 근력 균형이 좋지 않았다. 발목의 발등굽힘력은 여자 대학생 집단의 62%(좌), 73%(우) 정도였다. 발등굽힘-발바닥쪽굽힘의 결손율이 각각 25.05%, 26.86%, 가쪽번짐-안쪽번짐 결손율이 19.97%, 21.09%로 나타나 발목의 좌 우 근력 불균형이 상당히 심한 것으로 나타났다. 상기 연구 결과들이 실질적인 노인 체력 향상 훈련 프로그램을 제공하고 관련 정책을 수립하는 데 도움이 되기를 바란다.

무릎 슬리브 착용과 골지 건 기관 자극이 한 발 서기 동작과 고유수용성 감각에 미치는 영향 (The Effects of Knee Sleeve with a Stimulation of Golgi Tendon Organ on One Leg Standing and Proprioception)

  • 무릎 슬리브는 재활뿐만 아니라 스포츠 활동 시 손상 예방의 목적으로도 사용되고 있다. 무릎 슬리브의 스포츠 손상 예방을 정의하기 위해서는 고유수용성감각으로부터 기인하는 균형을 평가하는 것이 중요한 요소이다. 본 연구의 목적은 골지 건 기관 자극 패드 무릎 슬리브의 효과를 분석하는 것이다. 5명의 건강한 성인 남성이 본 연구의 실험대상자로 참여하였다. 실험대상자들은 일반 무릎 슬리브와 자극 패드 슬리브를 착용한 상태에서 각각 BIODEX에서 설정된 타겟(Target) 각도를 재연하였고, 한 발 서기 동작을 수행하였다. 각도 재연성 평가의 결과를 보면, 5명의 실험대상자 중 4명의 실험대상자는 무릎 슬리브를 착용했을 때 보다 자극 패드 슬리브 착용 시 %Target angle이 증가하는 결과를 보였다. 한 발 서기 동작 수행 시 무릎 슬리브 착용 시 보다 자극 패드 슬리브 착용 시 유지 시간(duration time)이 증가했으며 x, y 방향에서 COP 길이가 모두 감소했다(COPx - 무릎 슬리브: $162.06{\pm}58.99mm$, 자극 패드 무릎 슬리브: $149.03{\pm}45.30mm$; COPy - 무릎 슬리브: $310.79{\pm}115.89mm$, 자극 패드 무릎 슬리브: $291.57{\pm}76.53mm$). 초기 전이구간, 정적 구간, 말기 전이구간이 차지하는 비율은 무릎 슬리브 착용 시와 자극 패드 무릎 슬리브 착용 시에 유의한 차이를 보였다 (all, p < 0.05). 본 연구의 결과로부터 골지 건 기관 자극 패드 무릎 슬리브 착용이 고유수용성 감각과 균형에 긍정적인 영향을 미치는 것을 확인할 수 있었다.

정상성인의 인솔 높이가 균형에 미치는 영향 (Effect of Heel-heights of Insole on Balance in Healthy Adults)

  • Purpose:The purpose of this study was to investigated the effect of stability on heel-heights of insole in healthy adults. Methods:Subjects of 39 males measured stability index of the Biodex Stability System(BSS) by wearing shoes of 2cm, 5cm insoles including bare feet. The BSS was consisted of a movable balance platform and the platform was interfaced with computer software that enables the device to serve as an assessment of balance performance. Stability index of the BSS included overall stability index(OSI), anteroposterior stability index(APSI), mediolateral stability index(MLSI). In the BSS, subjects were asked to step on to the platform of the BSS and assume a comfortable position while maintaining slight flexion in the knees, looking straight ahead. Subjects were trained for 10min for adaptation to the heel-height of insole and then they maintained double limb stance for 2min. Biodex platform set to convert Lv.8 into Lv.1 gradually. Results:There were statistically significant differences between heel-heights of insole and stability index of OSI, APSI, and MLSI(p<.05). The result of post-hoc test were as follows; 1)OSI had significant differences between bare feet and 2cm, 5cm. 2)APSI had significant differences between bare feet and 2cm, 5cm. 3)MLSI had significant differences between bare feet and 2cm, 5cm(p<.05). Conclusions:We found that the more heels of insole high, the more stability index increases. In particular, balance index of insole above 5cm more increased and we could acknowledge that the insole above 5cm more effected balance of healthy adults.
