3차원 뉴튼(3D-NEWTON)을 이용한 척추 주위근 기능 평가의 유용성 (The Utility of Measuring Paravertebral Muscle Function with 3D-NEWTON)

  • Purpose: This study evaluated the validity of measuring paravertebral muscle function with 3D-NEWTON (Hanmed, Korea) by assessing the correlation between Biodex (Biodex, USA) and surface electromyography (EMG). Methods: Nineteen healthy adults participated. The function of their paravertebral muscle was measured in three ways. Maximum endurance time was measured in seconds when 3D-NEWTON was inclined forward for assessing extensor function, and inclined backward for assessing flexor function. Using surface EMG, maximum muscle activity was obtained from the eractor spinae and rectus abdominis during the 3D-NEWTON measurement. Maximum muscle activity was the mean activity from 10 seconds when the root mean squared firing data was highest. Through Biodex the peak torques of the extensor and flexor were measured during isometric exercises. The Spearman correlation coefficiencts from 3D-NEWTON, surface EMG, and Biodex were calculated. Results: The data from surface EMG and Biodex were statistically correlated when measured for flexor function, but less so for extensor function. In the case of 3D-NEWTON, the correlation coefficient with Biodex was 0.50 (p=0.05), while the coefficient with surface EMG was 0.53 (p=0.02) when measured for extensor function. Similarly, the correlation coefficienct with Biodex was 0.60 (p=0.01), while the surface EMG was 0.51 (p=0.03) for flexor function. Conclusion: 3D-NEWTON was a useful method for measuring paravertebral muscle function and can give helpful information for treating people with diseases associated with the lumbar spine.

The Effect of Proprioceptive Exercise on Knee Active Articular Position Sense Using Biodex System 3pro®

  • In order to assess the impact of proprioceptive exercises on the position sense ability of the knee joint, we conducted an analysis using Biodex System $3pro^{(R)}$, targeting 42 ordinary people (male=22, female=20). After applying proprioceptive exercise, we measured changes in balance and flexibility, and active articular position sense (AAPS), depending on gender. To find out the change in each measurement item variable, we carried out dependent t-tests. The statistical significance level was set to 0.05. The research showed that after applying the proprioceptive exercise, AAPS was significantly improved for both men and women (p<.01). In the case of women, the flexibility was significantly improved (p<.01). Also, the balance was significantly improved for both men and women (p<.01). Therefore, the proprioceptive exercise program is considered to be very useful in improving muscle and joint function, and preventing injuries. Thus, continuous clinical studies using Biodex system $3pro^{(R)}$ are required for a variety of scientific evaluations of proprioceptive skills.

등속운동을 이용한 경직성 관절장애 정량화 (Identification of Spastic Joint Pathologies using Isokinetic Movement)

  • 본 연구의 목적은 관절계 댐핑특성을 상용 에르고미터(BIODEX)의 등속운동을 이용하여 파악할 수 있는지를 평가하는 것이다. 제안된 방법은 등속운동을 유지하기 위해 인가한 외부토크와 하퇴의 관성토크의 차로부터 슬관절의 댐핑토크를 계산하는 것이다. 댐핑토크는 여러 가지 관절각속도에서 산출되었고, 이로부터 댐핑특성을 유도하였다. 실험장치는 BIODEX 시스템과 별도의 힘센서 및 Labview 시스템으로 구성되었다. 댐핑특성의 해석에는 Matlab을 사용하였다. 실험결과로부터 등속운동의 기동 및 종료를 위한 크랭크 암의 가속 및 감속으로 인한 미세한 각속도의 변화가 댐핑토크의 영향보다 큰 관성토크를 유발하는 것을 알 수 있었다. 따라서, 현재의 등속운동장비로는 댐핑특성을 추정하기가 어렵다고 보여진다.

스마트폰을 이용한 동적균형능력 측정의 타당도 연구 (Validity Study of Dynamic Balance Abilities Measure using a Smartphone)

  • PURPOSE: The purpose of this study was to investigate the validity of dynamic balance measurements using a smartphone. METHODS: Thirty subjects were selected out of Y-university students without fractures, operation history, and inflammatory arthritis who had not started regular exercise during the past three months. Their dynamic balance ability was measured by the Biodex Balance System (Biodex Medical Systems, Inc., USA) using smartphones. The smartphone utilized in this study was the Galaxy Note4 LTE (SM-N910K, Samsung, Korea), and the application was the Sensor Kinetics pro (Ver.2.1.2, INNOVENTIONS Inc, US). The dynamic balance ability was measured in triplicate. RESULTS: With eyes closed, a low level of correlation (r>0.30, p<0.05) and low reliability (ICC>0.60) were determined between the roll value of the smartphone gyroscope versus the medial/lateral score of the Biodex Balance System. With eyes closed, a low level of correlation (r>0.30, p<0.05) and low reliability (ICC>0.60) were determined between the total value of the smartphone gyroscope versus the total score of the Biodex Balance System. CONCLUSION: This study indicated that using a smartphone can generate highly limited data regarding balance ability. They are unlikely to replace the existing more expensive devices entirely. However, smartphones may be highly useful in environments in which an expensive device is not available or when dynamic balance ability should be measured immediately or within a few hours.

Biodex System3을 사용한 근력측정과 체조성의 양적조건과의 관련성 (Quantitative Relationship between Body Composition and Muscular Strength Measured with the Biodex System 3)

  • This study measured body composition and muscular strength, to examine their relationship, using the Bioelectrical Impedance (IB) method and biodex system3, respectively, in 44 healthy male(20) and female(24) university students. 1. Muscular strength of the upper extremities correlated significantly with fat-free mass(FFM) with $r=.604{\sim}.630$ and intracellular fluid(ICF) with $r=.672{\sim}.668$ in males and FFM with $r=.416{\sim}.552$, ICF with $r=.432{\sim}.564$ and extracellular fluid(ECF) with $r=.429{\sim}.463$ in females. 2. Muscular strength of the lower extremities correlated significantly with FFM with $r=.522{\sim}.785$, ICF with $r=.501{\sim}.739$ and ECF with $r=.498{\sim}.796$ in males and FFM with $r=.642{\sim}.660$, ICF with $r=.627{\sim}.671$ and ECF with $r=.572{\sim}.623$ in females. 3. Muscular strength of the trunk correlated with FFM with $r=.595$, ICF with $r=.627$, ECF with $r=.448$ in males, but did not correlate significantly with body composition in females. These results suggest that total body water(TBW) and ICF may be factors directly associated with muscular strength as well as physical fitness.

The Effects of Isokinetic Exercise on Knee Joint Combined Russian Current Stimulation

  • Purpose: This study was conducted to determine how stimulation using Russian current changes isokinetic exercise performance of quadriceps muscle and its antagonist muscles. Methods: Subjects were 20 20-year-old healthy adults who were instructed in the flexion to extension of knees in angular velocity $60^{\circ}/sec$ and $180^{\circ}/sec$ using a Biodex (Biodex system 3). We measured the difference in muscle performance between the Russian current stimulated at the same time during the flexion to extension of knees and not stimulated. Results: The results showed that when Russian current stimulation was applied at the angular velocity $60^{\circ}/sec$, the flexed and extended muscles and the angular velocity $180^{\circ}/sec$ increased significantly, but the peak torque of flexing muscles at the angular velocity $180^{\circ}/sec$ did not increase. Conclusion: These findings suggest that Russian current stimulation with isokinetic exercise of the knee joint could affect the quadriceps muscle and its antagonist muscle performance of muscle strength and endurance.

  • The purpose of this study is to evaluate the possibility of identifying joint damping property through commercially available isokinetic ergometer (BIODEX). The proposed method is to estimate the damping torque of the knee joint from the difference between the external joint torque for maintaining isokinetic movement and the gravity torque of the lower leg. The damping torque was estimated at various joint angular velocities, from which the damping property would be derived. Measurement setup was composed of the BIODEX system with an external force sensor and Labview system. Matlab was used in the analysis of the damping property. The experimental result showed that the small variation in angular velocity due to acceleration and deceleration of the crank arm resulted in greater change of inertial torque than the damping torque, so that the estimation of damping property from the isokinetic movement is difficult.

앞십자인대 재건술 후 엉덩관절 벌림근 강화운동이 근력과 보행에 미치는 영향 (The Effects of Strengthening Exercise of Hip Abductors on Muscle Strength and Ambulation in Patient with ACL Reconstruction)

  • Purpose: The purpose of this study was to investigate the effect of strengthening exercises of hip abductors on muscle strength and ambulation for patients with ACL reconstruction. Methods: The subjects were randomly assigned to the intervention group (general exercise plus strengthening of hip abductors) or the control group (general exercise without the strengthening of hip abductors). Both groups participated in a six-week exercise protocol after the surgery for ACL reconstruction. The knee strength test (quadriceps, hamstring, hip abductor) and gait analysis were performed in pretest and post-test. Results: Muscle strength was measured using the Biodex system III model (Biodex Medical System, Inc, NY, US). The results showed no significant difference in knee strength (quadriceps, hamstring) between the two groups (p>0.05), however, there was a significant difference in hip abductors (p<0.05). The gait analysis was measured with Gaitrite system (CIR System Inc, US). Results of comparison between groups showed a significant increase in the step length of both groups. (p<0.05), and there was a significant difference in the affected leg of the intervention group(p<0.05). Conclusion: Exercise of hip abductors for patients with ACL reconstruction provides significant benefits with respect to improvement of muscle strength in hip abductors, increasing the stride. This result indicates that a training program designed for ACL reconstruction patients should include strengthening of the hip abductor. We think that further study is needed to determine the relationship of hip muscles and knee joint for patients with ACL reconstruction.

고등학교 엘리트 카약 선수들의 등속성 근기능·균형성·유연성 측정 및 평가 (Assessment of Isokinetic Muscle Function, Balance and Flexibility in Elite High School Kayakers)

  • 본 연구는 고등학교 엘리트 카약 선수들의 등속성 근기능, 균형성, 유연성, 신체 조성 등을 검사 및 진단하여 선수들의 경기력 향상과 부상 예방을 위한 훈련 방법론에 대한 조언을 하고자 실시되었다. 11명의 선수들을 대상으로, Biodex system IV, Biodex stabilizer system, Inbody 3.0, CNP-5403을 사용하여 근기능(peak torque, average power, work fatigue) 동적 균형 신체 성분 유연성 검사를 하였다. 선수들의 BMI 평균은 $22.57kg/m^2$, 체지방률은 10.90%, 체전굴은 16.95cm, 밸런스 점수는 좌우 각각 1.55, 1.49로 나타났다. S4, S5, S8, S10, S11은 어깨 근력과 파워 훈련이, S5, S6, S10, S11은 어깨 근지구력 훈련이 필요한 것으로 평가되었다. S5, S7, S8, S10, S11은 무릎 근력과 파워 훈련이, S1, S4, S5, S10은 무릎 근지구력 훈련이 요구되었다. S3, S10, S11은 손목 근지구력 강화 훈련이 필요했다. 추후 근기능 불균형으로 인한 경기력 저하와 잠재적 부상의 위험을 줄이기 위해선 획일화된 근력 훈련 프로그램을 지양하고, 정기적인 근기능 검사와 함께 개인별로 특화된 훈련 프로그램을 수립해야 한다.

대학 엘리트 카나디언 선수들의 어깨·허리·무릎 관절의 등속성 근기능 평가 및 진단 (Assessment of Isokinetic Muscle Function in Elite Canadian College Paddlers)

  • 본 연구는 대학 엘리트 카나디언 선수들의 등속성 근기능을 검사 및 진단하여 선수들의 기능 향상 및 상해예방을 위한 훈련 방법론에 대한 조언을 하고자 함에 목적이 있다. 경주 소재 D 대학의 평균 8.5년의 경력을 가진 8명의 엘리트 선수들이 실험에 참가했으며, Biodex system III, Biodex stabilizer system, THP 2, Inbody 720이 근기능 검사(peak torque, average power, work fatigue), 동적 균형 검사, 신체성분 검사를 위해 사용되었다. 등속성 근기능 검사는 어깨와 허리, 무릎 관절에 대하여 실시하였으며, 측정 후 동측 불균형과 좌 우측 불균형 정도를 평가하였다. 검사 결과 A3, A4, A5, A6, A8 선수는 어깨의 근력과 파워 훈련이, A1, A2, A7, A8 선수는 어깨의 근지구력 훈련이 필요한 것으로 평가되었다. A2, A5, A8 선수는 허리의 근력과 파워 강화 훈련이 필요하였고, A1, A8 선수는 허리의 근지구력 강화 훈련이 필요하였다. 대부분의 선수들은 무릎과 허리의 동측 근력비가 정상적이지 못했는데, 이의 개선을 위해 A2, A3, A4, A8 선수는 복근의 강화가 필요했고 A3, A5, A6, A7, A8 선수는 햄스트링의 강화가 요구되었다. 선수들의 BMI 평균은 24.90 $kg/m^2$, 체지방율은 12.23%, 반응시간 평균은 324 msec, 밸런스 점수는 각각 2.0 미만으로 나타났다. 이러한 결과를 종합해 볼 때, 카나디언 엘리트 선수들은 근력 불균형에 의한 경기력 저하와 상해 잠재 위험성이 있으므로 정기적인 근기능 검사를 통한 관리와 함께 적절한 훈련 처방이 필요할 것이라 생각된다.
