Prevalence of incidental distal biceps signal changes on magnetic resonance imaging

  • Background: Knowledge of the base rate of signal changes consistent with distal biceps tendinopathy on magnetic resonance imaging (MRI) has the potential to influence strategies for diagnosis and treatment of people that present with elbow pain. The aim of this study is to measure the prevalence of distal biceps tendon signal changes on MRIs of the elbow by indication for imaging. Methods: MRI data for 1,306 elbows were retrospectively reviewed for mention of signal change in distal biceps tendon. The reports were sorted by indication. Results: Signal changes consistent with distal biceps tendinopathy were noted in 197 of 1,306 (15%) patients, including 34% of patients with biceps pain, 14% of patients with unspecified pain, and 8% of patients with a specific non-biceps indication. Distal biceps tendon changes noted on radiology reports were associated with older age, male sex, and radiologists with musculoskeletal fellowship training. Conclusions: The finding that distal biceps MRI signal changes consistent with tendinopathy are common even in asymptomatic elbows reduces the probability that symptoms correlate with pathology on imaging. The accumulation of signal changes with age, also independent of symptoms, suggests that tendon pathology persists after symptoms resolve, that some degree of distal biceps tendinopathy is common in a human lifetime, and that tendinopathy may often be accommodated without seeking care. Magnetic resonance imaging features of the biceps tendon rupture in a Labrador retriever dog

  • A five-year-old spayed female Labrador retriever presented with right forelimb lameness one day earlier. Radiographs and computed tomography of the right shoulder joint revealed conspicuous enthesophytes, bone cyst, and bone fragment at the intertubercular groove. Magnetic resonance imaging showed a loss of continuity of the biceps tendon and inhomogeneous hyperintense signal of the surrounding soft tissue and moderate synovial fluid on T2- and proton density-weighted images with contrast enhancement on the postcontrast T1-weighted images. The dog was diagnosed with tenosynovitis and biceps tendon rupture with osteoarthritis based on a comprehensive evaluation of the computed tomography and magnetic resonance imaging features.

  • 회전근 개 파열과 동반된 관절 내 변화의 전체빈도는 활액막염 (63%), 관절순 이상(54%), 이두박 건 활차의 이상(53%), 이두박 건 이상(41%), 상완골두(30%) 및 관절와(20%) 순 이었으며. 이두박 건 장두, 이두박 건 활차, 관절와의 관절염 변화 등의 경우 2군에서 호발하였다. 또한 수술 시 치료가 필요했던건 주 변화의 경우는 2군에서 이두박 건 활차, 골두, 관절순 및 활액막에서 빈도가 증가하였다.

회전근 개 파열과 동반된 상완 이두 건 장두의 병변 (Long Head of the Biceps Tendon Lesion Associated with Rotator Cuff Tear)

  • 목적: 회전근 개 파열과 동반되어진 상완 이두 건의 병변 및 치료 결과를 알아보고자 하였다. 대상 및 방법: 회전근 개 파열로 수술적 치료를 받고 후향적으로 상완 이두 건의 병변 여부를 확인할 수 있었던 92예를 대상으로 2년 이상 추시하였다. 상완 이두 건 병변은 건막염, 건의 마모 또는 비대, 파열, 불안정성의 4형태로 구분하였다. 회전근 개는 광범위 파열 중 4예를 제외하고는 모두 봉합하였으며 상완 이두 건 병변에 대해서는 변연 절제 30예, 건 절단 10예, 건 고정 8예, 재배치 4예를 시행하였다. 결과는 UCLA 평가 지수를 이용하였다. 결과: 상완 이두 건 병변을 보인 예가 70예 (76%)로 건막염은 19예, 건의 마모 또는 비대 22예, 파열 21예, 불안정성 8예의 병변을 보였다. 회전근 개 파열의 크기에 따라서 중범위 이하에서는 63%, 대범위 이상에서는 88%의 상완 이두 건 병변을 보였다. 치료 결과는 상완 이두 건 병변이 없었던 22예에서는 29.6점, 병변이 있었던 예 중 회전근 개 봉합을 시행하였던 66예에서는 28.3점이었다. 동반된 상완 이두 건을 절단하거나 재고정을 시행한 예에서는 28.2점이었다. 결론: 회전근 개 파열의 크기가 클수록 상완 이두 건 병변이 더 많이 발생하였고 형태도 보다 심하게 나타났다. 따라서 상완 이두 건 병변의 원인이 회전근 개 파열을 유발시키는 원인과 연관될 수 있다고 추측되었다. 동반된 상완 이두 건 병변의 치료로 건 절단이나 건 고정이 동통 완화에 효과적일 수 있다고 사료되었다.

A Case Series of Symptomatic Distal Biceps Tendinopathy

  • Background: The study of conservative and surgical treatment of distal biceps tendinopathy and associated biceps tendon partial rupture. Methods: Twenty-one cases with distal biceps tendonitis and partial ruptures were studied who visited Pohang Semyeong Christianity Hospital from June 2010 to August 2017. The mean age was 57.1 years (39-69 years), 14 males and 7 females. The mean duration of symptom at the time of first visit was 4.9 months (0.2-14 months). Ultrasonography and magnetic resonance imaging were performed for patients with severe symptoms. According to the severity of the symptoms, splint immobilization, oral nonsteroidal anti-inflammatory drugs, and ultrasound-guided steroid injection were performed. Surgical treatment was performed if the patient did not respond to conservative treatment for 3 to 6 months or longer. Results: There were 9 cases of partial rupture of the distal biceps tendon associated with distal biceps tendinopathy on imaging studies. Conservative treatment showed symptomatic improvement in 16 of 21 cases. In 4 cases with a relatively mild symptom, anti-inflammatory analgesics and intermittent splinting showed good result. In 12 cases, symptoms improved after ultrasonography-guided steroid injection. Surgical treatment was performed on 5 cases that did not respond to conservative treatment. Conclusions: Conservative treatment of distal biceps tendinopathy may promise good results. However, in case of partial tear of the distal biceps tendon and refractory to conservative treatment, surgical treatment may be needed.

관절경적 상완 이두근 건고정술의 임상적결과 (Clinical results of arthroscopic tenodesis of the proximal biceps tendon using by interference screw)

  • 목적: 상완 이두근 장두의 고정술을 시행하는데 있어 관절경을 이용한 간섭 나사못(interference screw) 고정술후의 임상적 결과를 분석하고자 하였다. 대상 및 방법: 2008년 1월부터 2010년 1월까지 상완 이두근 장두의 건 고정술을 시행한 환자를 대상으로 하였으며, 최소 1년 이상 추시가 가능하였던 30예를 대상으로 하였다. 그중 회전근개 파열을 동반한 경우는 20예였다. 결과는 VAS, ASES, 상완 이두근 구의 압통, 고정실패 및 변형의 정도로 판단하였다. 결과: 최종 추시시 모든 환자에서 수술 전에 비해서 VAS, ASES 점수는 의미 있게 호전되었으나(p<0.05), 최종 추시시 17%(5예)에서 상완 이두근 구의 통증이 존재하였고, 10%(3예)에서 고정실패 및 BAD (biceps apex distance)의 증가로 인한 Popeye 변형이 존재하였다. 또한 회전근개 파열을 동반한 군과 그렇지 않는 군에서는 최종 추시시 회전근개 파열이 없는 군에서 좋은 결과를 얻을 수 있었다(p<0.05). 결론: 상완 이두근 근위부 병변에 의한 통증이 있는 환자에서 관절경적 상완 이두근 장두의 건고정술은 모두 최종 추시시 좋은 결과를 얻을 수 있으나, 17%에서 상완 이두근 구의 압통이 존재하여 추가적인 연구가 필요할 것으로 사료된다.

Arthroscopic Treatment of Synovial Chondromatosis of the Shoulder Joint with Mini-open Procedure for the Lesions of Biceps Tendon Sheath

  • Synovial chondromatosis is an uncommon condition, and the involvement of the shoulder joint is rare. A 15 year old female patient presented to author's institution for right shoulder pain. We checked the plain radiographs and MRI. And they showed that a diagnosis of synovial chondromatosis in the shoulder, and they also demonstrated that the disease involved the bicipital tendon sheath as well as glenohumeral joint. We removed all loose bodies with total synovectomy by arthroscopic procedure, and a mini-open procedure for the lesions of biceps tendon sheath. Arthroscopic treatment affords excellent visualization of the shoulder joint with less morbidity. However, with current arthroscopic techniques, it is difficult to manage the synovial chondromatosis of biceps tendon in bicipital groove. The authors suggest that the complete elimination of synovial chondromatosis involving shoulder requires a mini-open procedure for the lesions of biceps tendon sheath in addition to the arthroscopic resection of the affected synovium and loose body removal in the glenohumeral joint.

Diagnostic Consistency between Sonoelastography and Conventional Sonography of Long Head of the Biceps

  • Background: Sonoelastography (SE) is a new technique that can assess differences in tissue stiffness, the purpose of this study was to evaluate the ability of SE to assess the long head of biceps tendon alteration. Methods: Forty shoulders of 36 consecutively registered patients with clinical symptoms and conventional ultrasonography findings of biceps tendinitis or tendinosis, and 40 asymptomatic shoulders of 20 healthy volunteers were assessed with SE. Transverse and longitudinal images of long head of biceps tendon were obtained using SE. SE images were performed by one orthopedic surgeon and evaluated by two orthopedic surgeons using an experimentally proven color grading system. Results: The transverse images of SE showed a sensitivity of 87.5%, a specificity of 95.0% and a accuracy of 91.3%, the longitudinal images of SE showed a sensitivity of 92.5%, a specificity of 90.0% and a accuracy of 91.3%. Inter-observer reliability of SE was in 'almost perfect agreement' with a weighted kappa coefficient of 0.83. Conclusions: SE is valuable in the detection of the intratendinous and peritendinous alterations of biceps tendon, and has excellent accuracy and excellent correlation with conventional ultrasound findings.

Determining the incidence and risk factors for short-term complications following distal biceps tendon repair

  • Background: Distal biceps rupture is a relatively uncommon injury that can significantly affect quality of life. Early complications following biceps tendon repair are not well described in the literature. This study utilizes a national surgical database to determine the incidence of and predictors for short-term complications following distal biceps tendon repair. Methods: The American College of Surgeons' National Surgical Quality Improvement Program database was used to identify patients undergoing distal biceps repair between January 1, 2011, and December 31, 2017. Patient demographic variables of sex, age, body mass index, American Society of Anesthesiologists class, functional status, and several comorbidities were collected for each patient, along with 30-day postoperative complications. Binary logistic regression was used to calculate risk ratios for these complications using patient predictor variables. Results: Early postoperative surgical complications (0.5%)-which were mostly infections (0.4%)-and medical complications (0.3%) were rare. A readmission risk factor was diabetes (risk ratio [RR], 4.238; 95% confidence interval [CI], 1.180-15.218). Non-home discharge risk factors were smoking (RR, 3.006; 95% CI, 1.123-8.044) and ≥60 years of age (RR, 4.150; 95% CI, 1.611-10.686). Maleness was protective for medical complications (RR, 0.024; 95% CI, 0.005-0.126). Surgical complication risk factors were obese class II (RR, 4.120; 95% CI, 1.123-15.120), chronic obstructive pulmonary disease (COPD; RR, 21.981; 95% CI, 3.719-129.924), and inpatient surgery (RR, 8.606; 95% CI, 2.266-32.689). Conclusions: Complication rates after distal biceps repair are low. Various patient demographics, medical comorbidities, and surgical factors were all predictive of short-term complications.

Histological Changes in Biceps Muscle after Tenotomizing the Biceps Long Head in a Rat Model

  • Background: Popeye deformity is common after rupture of the biceps muscle's long head tendon. Herein, we report on histological changes in biceps brachii muscles following tenotomy of the long head biceps tendon. Methods: Twelve Sprague-Dawley rats (12-week-old) underwent tenotomy of the long head biceps tendon in the right shoulder. At postoperative weeks 4, 7, and 10, the operative shoulders were removed by detaching the biceps brachii muscle from the glenoid scapula and humerus; the opposite shoulders were removed as controls. H&E staining was performed to elucidate histological changes in myocytes. Oil-red O staining was performed to determine fatty infiltration. Myostatin antibody immunohistochemistry staining was performed as myostatin is expressed by skeletal muscle cells during myogenesis. Results: H&E staining results revealed no changes in muscle cell nuclei. There were no adipocytes detected. Compared with that of the control biceps, the cross-sectional area of the long head biceps was significantly smaller (p=0.00). Statistical changes in the total extent of the 100 muscle cells were significant (p=0.00). Oil-red O staining revealed no fatty infiltration. Myostatin antibody immunohistochemical staining revealed no significant difference between the two sides. Conclusions: Muscular changes after tenotomy of the long head biceps included a decrease in the size of the individual muscle cells and in relative muscle mass. There were no changes observed in muscle cell nuclei and no fatty infiltration. Moreover, there were no changes detected by myostatin antibody immunohistochemistry assay.
