The Effects of Trunk Exercise on Mobility, Balance and Trunk Control of Stroke Patients

  • PURPOSE: The initiation of the trunk muscles in stroke patients is delayed because the muscles involved in reach arm are activated earlier than the trunk muscles. The objective of this study was to examine the effects of mobility, balance, and trunk control ability through selective trunk exercise (STE) in patients with chronic stroke. METHODS: A randomized pre-test and post-test control group design was initially used, with subjects randomly assigned to the STE group (n=15) and a control group (n=14). All groups underwent physical therapy based on the neuro-developmental therapy (NDT) for 30 minutes a day, five times per week for four weeks. Additionally, the STE group did the trunk exercise for 30 minutes a day, three times per week for four weeks. The timed up and go test (TUG), Berg balance scale (BBS), and trunk impairment scale (TIS) were used for assessment. RESULTS: The scores of the TUG, BBS, dynamic sitting balance subscale, and coordination subscale of TIS improved significantly in both groups but the improvement was more pronounced in the STE group (p<.05). This study showed a large effect on the scores of the TIS coordination subscale (d=.93) (p<.05), TIS dynamic balance subscale (d=.81) (p<.05), TUG (d=.75) (p<.05), and BBS (d=.73) (p<.05). CONCLUSION: The combined STE and NDT program showed improvements in measures of mobility, balance, and trunk control in chronic stroke patients. These results suggest that STE should be considered to be included in the treatment program for patients with chronic stroke.

  • Purpose: The purpose of this study was to determine the effect of symmetrical-reciprocal pattern of scapula and pelvis exercises using proprioceptive neuromuscular facilitation (PNF) on gait speed and balance in patients with hemiplegia. Methods: Among the adult patients with hemiplegia that were hospitalized at Michuhol Rehabilitation Center after being diagnosed with stroke, 10 that were capable of independent walking for more than five minutes and that understood and cooperated with the therapy and test methods of this research, were selected as subjects. The therapy was implemented based on the concept of PNF, and it was performed on a low mat and a height-adjustable mat, as proscribed by the fundamental procedure for PNF. Symmetrical-reciprocal pattern of scapula and pelvis exercises were applied to the patients in the decubitus position. The therapy scheme included stabilizing reversals, rhythmic stabilization, and a combination of isotonics, rhythmic initiation, and dynamic reversals. To investigate gait speed and body trunk mobility before and after the symmetrical-reciprocal pattern of scapula and pelvis exercises were applied, walking speed for a distance of 10 m was measured and balance was tested based on the Berg-Balance scale test table. The Berg-Balance scale test was performed by one therapist to minimize any error that could occur from the subjective evaluation method used by therapists. Results: Gait speed increased by 8.97 seconds after applying the symmetrical-reciprocal pattern of scapula and pelvis exercises using the concept of PNF, showing a significant difference (p<0.01). However, balance showed no significant difference after the therapy (p>0.14). Conclusion: Exercise therapy that uses the symmetrical-reciprocal pattern of scapula and pelvis with the concept of PNF can be said to be a useful therapeutic technique that can enhance the walking speed of patients with hemiplegia.

  • Purpose: Deficits in lower-extremity function and trunk control ability have a negative impact on individuals with hemiplegia. This case report aimed to describe the effect of trunk stability exercises using proprioceptive neuromuscular facilitation (PNF) on trunk control ability, balance, and gait in a patient with hemiplegia. Methods: A 77-year-old man with hemiplegia and trunk and lower extremity impairment participated in this four-week training intervention. Results: The patient demonstrated improvements in trunk control ability, balance, and gait performance. Outcome measures (Fugl-Meyer Assessment Lower Extremity (FMA-LE), Trunk Control Test, Berg Balance Scale, Timed Up and Go test, 10 Meter Walk test) were measured before and after the training program. Conclusion: The results of this case suggest that a trunk stability exercise using a PNF program may improve trunk control ability, balance, and gait in a patient with hemiplegia.

  • Purpose: This study was to find a correlation between the bilateral reciprocal leg press test and a the balance in chronic stroke patients. Methods: Eighteen patients performed an isokinetic leg press test consisting of a bilateral reciprocal and unilateral mode. Following the isokinetic leg press testing, subjects performed the balance test: Berg Balance Scale (BBS), Timed Up & Go (TUG) test, and stability limit. Pearson product moment correlation coefficients were used to determine the correlation between the mean score of the isokinetic leg press test, balance test in both affected and non-affected side. Results: This study indicated a significant correlation between the bilateral reciprocal leg press test and stability limit. There were significant correlation between non-affected side bilateral leg press(NBL) and BBS (r=0.501), affected side bilateral leg press (ABL) and non-affected side stability limit(NS) (r=0.614), ABL and total stability limit (TS) (r=0.493), NBL and affected side stability limit(AS) (r=0.480), NBL and NS (r=0.560), NBL and TS (r=0.563), among the patients. Conclusion: Measurement of the lower extremity strength using the bilateral reciprocal leg press test can be used as an evaluating tool of the balance test.

  • Purpose: The aim of this study was to identify the effects of the lower trunk stabilization exercise using neurodevelopmental technique (NDT) on muscle activity in lumbar region and balance in the patients with hemiplegia. Methods: Fifteen participants were allocated in two groups: NDT group (n=8) or control group (n=7). NDT group performed NDT exercise program, while control group conducted walking exercise. Both interventions were given for 30 minutes a day, 3 times a week, for 5 weeks. To measure the muscle activity in rectus abdominis (RA), external oblique (EO) and internal oblique (IO), electromyography (EMG) was used. And, Timed-Up and Go (TUG) test and Berg-Balance Scale (BBS) were performed to assess balance before and after intervention. Results: NDT group showed a significant improvement of muscle activity in RA and EO, while control group did not show significant changes in three muscles. Also, there was a significant difference in muscle activity of RA and EO between two groups. In BBS and TUG test, participants in two groups showed significant improvements after intervention. Especially, significant difference was observed in TUG test between two groups (p<.05). Conclusion: This study demonstrated that NDT exercise is an effective intervention to improve the muscle activity in trunk region and to increase balance in patients with stroke. Thus, we suggested that NDT exercise program would be a treatment intervention in stroke rehabilitation.

  • 본 연구는 만성뇌졸중 편마비 환자에게 적용한 동작관찰 훈련이 보행과 균형에 미치는 영향을 알아보고자 하였다. 연구에 동의한 만성 뇌졸중 편마비 환자 4명중 동작관찰훈련을 시행한 실험군 2명 일반과제지향훈련을 시행한 대조군 2명에게 총 4주간 하루 30분씩 훈련하였다. 훈련효과를 알아보기 위해 훈련전 보행은 10m검사를 시행하였고 균형은 버그균형척도로 검사하고 훈련 후 다시 재측정하였다. 이에 동작 관찰 훈련군이 일반과제지향훈련군에 비해 보행과 균형에서 유의미한 차이를 보였다. 이에 동작관찰훈련이 만성뇌졸중 편마비환자의 보행과 균형에 효과적인 프로그램임을 알 수 있다.

복합적 균형 훈련이 만성 뇌졸중 환자의 균형에 미치는 영향 (The Effects of Combined Balance Exercise on Balance in Chronic Stroke Patients)

  • Purpose: The study investigated the effect of combined balance exercise using visual feedback and balance pads in rehabilitation of chronic stroke patients. Methods: The participants were 30 patients diagnosed with stroke who met the study selection criteria. Participants were randomly divided into 3 groups of 10: a balance pad exercise (BPE) group, a visual feedback exercise (VFE) group, and a combined balance exercise (CBE) group. All three groups engaged in 30 minutes of exercise, 3 times per week, for 6 weeks. Results: Pre-test and post-test results were analyzed using the functional reach test (FRT), the Berg balance scale (BBS), the timed up & go test (TUG), and the Korean version of the activities-specific balance confidence (K-ABC) scale. The study yielded the following results. Pre- and post-program FRT measures showed significant differences between the BPE and CBE groups (p < 0.05). BBS, TUG, and K-ABC scores showed clear differences in all three groups. Secondly, the CBE group and the BPE group a differed significantly on the BBS before and after the 6-week program (p < 0.05). Finally, the CBE group and the VFE group differed significantly on the BBS before and after the 6-week program (p < 0.05). Conclusion: The study results indicate that combined balance exercise improves balance more effectively than the balance pad and visual feedback exercises. This finding should help to improve rehabilitation in the future.

아급성 뇌졸중 환자에게 무릎 신전 보조기기가 균형과 보행에 미치는 효과 및 유용성 : 사례 연구 (The effect and feasibility of knee extension assist orthosis on balance and gait in subacute stroke patients : case study)

  • Background: This study was to confirm the effect and feasibility of knee extension assist orthosis (KEAO) on balance and gait in subacute stroke patients. Design: Case study. Methods: The subjects of the study were 4 subacute stroke patients, who had an onset period of less than 6 months. The limit of stability (LOS) and berg balance scale (BBS), timed up and go test (TUG) were used to verify the dynamic balance ability, static balance ability, and gait ability pre and post and after wearing the knee extension assist orthosis (KEAO). In addition, the satisfaction survey was to confirm the feasibility of the knee extension assist orthosis (KEAO) through the to Korean quebec user evaluation of satisfaction assistive technology 2.0 (K-QUEST 2.0). Results: After the wearing on KEAO, the distance for the limit of stability decreased by mean 541.25±240.46 mm2, and the score on the berg balance scale improved by mean 5±2.71 point, and the time for the timed up and go test deceased by mean 3.75±1.71 second. The stability and durability were found to be full score, and the control, ease, effectiveness were some high score, and the size, weight, comfort were some low score in the satisfaction and feasibility. Conclusion: The knee extension assist orthosis (KEAO) produce in this study was improved the static balance ability, dynamic balance ability and gait ability of subacute stroke patients, and the satisfaction and feasibility were high in the stability, durability and effectiveness of the user.

뇌졸중 환자의 균형, 기능적 보행, 시지각, 일상생활 평가도구의 상관성 (The Correlations between the Balance Test, functional movement, Visual Perception Test and Functional Independent Measure in Stroke Patients)

  • Purpose: The purpose of this study was to determine correlations between the Berg Balance Scale (BBS), Functional Reach Test (FRT), Timed Up & Go (TUG), Motor-Free Visual Perception Reach Test Vertical format (MVPT-V), Functional Independence Measure (FIM). Methods: The subjects used in this study were 35 stroke patients from Cheongju ST. Mary's hospital. Balance was measured by BBS, FRT. Functional mobility was measured by TUG. Visual perception was measured by MVPT-V. FIM was used to evaluate the activities of daily living. Data was analyzed using pearson product correlation. The TUG and MVPT-V index were analyzed by linear regression. Results: There was a statistically significant difference between FRT and BBS (r=0.89, p<0.01), FIM (r=0.74, p<0.05), MVPT-V (r=0.40, p<0.05), and TUG (r=-0.36, p<0.05). There was significant statistical differences between TUG and MVPT-V (r=-0.64, p<.01). However, statistically significant differences were observed between BBS and FIM (r=0.79, p<0.01). The visual close item of the MVPT-V showed the strongest variance in predicting TUG. Conclusion: The use of both quantitative and qualitive scales was shown to be a good measuring instrument for the classification of general clinical performances of stroke patients. In particular, the results suggest that the visual perception test may be able to predict functional locomotion in stroke patients.

뇌졸중환자의 반웅크리기 동안에 하지근활성도와 BBS, TUG와의 상관관계 (Correlation Between BBS, TUG and Lower Extremity Muscle Activity during Semi-Squat in Stroke Patients)

  • Purpose : We studied the correlation between reference voluntary contraction (% RVC) of vastus lateralis (VL) and vastus medialis (VM), Berg balance scale (BBS), and timed-up & go (TUG) test. Methods : We recruited 30 stroke patients from a rehabilitation center at a hospital.? All subjects could walk with or without an assisting device. Subjects were evaluated with % RVC of VL and VM, BBS, and TUG. The data were analyzed using a Pearson correlation coefficient. Results : The % RVC of VL and VM and BBS (p<0.01) showed a significant positive correlation. TUG negatively correlated with % RVC of VL and VM and BBS (p<0.01). Conclusion : Lower extremity muscle activity increases balance and walking ability. We recommend the implementation of lower extremity strength exercises in the rehabilitation of stroke patient.
