This study aimed at observing the effect of diazepam on the contractility of trachealis muscle isolated from canine trachea, possible involvement of central or peripheral type benzodiazepine receptor, and the calcium related mechanism of action of diazepam. Trachealis muscle strips of 15 mm long were suspended in an isolated organ bath containing 1 ml of physiologic salt solution maintained at $37^{\circ}C$, and aerated with 95% $O_2$ /5% $CO_2$. Isometric myography was performed. Diazepam reduced the basal tone concentration dependently, and this inhibitory action was not affected by neither flumazenil, a central benzodiazepine receptor antagonist, nor PK11195, a peripheral benzodiazepine receptor antagonist. Pretreatment with diazepam showed the inhibitory effect on the concentration-response curves to agonists such as bethanechol, 5-hydroxytryptamine and histamine. Diazepam also caused concentration-related inhibition of contraction with potassium chloride 30 mM. The effect of diazepam on the basal tone and potassium chloride-induced contraction with calcium channel blockers were compared. Similar results were obtained in canine trachealis with verapamil, nifedipine and diltiazem. These results suggest that diazepam relax an airway muscle not via specific receptors but by a similar action as calcium channel blockers in canine trachealis muscle.

A growing number of people are concerned about their sleep. There are many people with chronic sleep disorders. Sedativehypnotics including benzodiazepine and non-benzodiazepine have been widely used in chronic insomniacs. It is widely accepted that current hypnotics are efficient in alleviating subjective symptoms of insomnia. Non-benzodiazepine hypnotics include zolpidem, zopiclone, and melatonin. These novel non-benzodiazepine hypnotics that have efficacy comparable to benzodiazepines were developed with more understanding of benzodiazepine receptor pharmacology. Their unique pharmacologic profiles may offer few significant advantages in terms of adverse effects of benzodiazepines. However, most of hypnotics including non-benzodiazepine have some of dependence, tolerance, impaired daytime function and rebound insomnia. Currently, it is accepted that combination therapy with pharmacologic and behavioral intervention is the most effective for chronic insomniacs.
세계적으로 그 처방률이 높은 약물들 중 하나인 benzodiazepine계 약물은, 강력한 불안해소작용, 항경련작용, 최면작용 및 근이완작용을 가지고 있다. Benzodiazepine계 약물의 약리작용기전과 관련하여 불안장애의 병인에서 각종 신경전달계의 역할은 밝혀지고 있다. Benzodiazepine 약물은 편도체와 같은 변연계, 우측 시상하부 및 raphe nuclei에서 GALA성 신경전달을 조절함으로써 불안해소 작용을 나타내는 것으로 보고되었으며, 공포와 스트레스로 인해 유발되는 행동에 있어서 편도체의 역할은 이미 여러 보고를 통해 시사된 바 있다. 본 종설에서는 GABA성 신경전달 조절작용이 있다고 보고된 내인성 benzodiazepine 수용체 배위자의 불안 및 스트레스 반응의 인지 조절작용 및 기억 형성과정 조절작용 등에 관한 연구결과들을 종합하여 본 결과 내인성 benzodiazepine 수용체 배위자들이 불쾌한 학습과정에 동반되는 스트레스와 불안의 정도에 영향을 미침으로써 기억 형성과정을 조절한다는 생각을 뒷받침해 주었다. 또한 각종 행동 검사후 나타나는 뇌조직의 benzodiazepine 유사 물질의 부위별(편도체, 해마, 중격) 농도 감소는 행동 검사에 수반되는 스트레스 및 불안 정도에 비례하므로, 이러한 물질의 감소는 스트레스 및 불안에 대한 변연계의 일반적인 반응으로 생각된다. 부가적으로 이러한 결과는 변연계의 $GABA_A$/benzodiazepine 수용체 복합체는 생체의 스트레스 및 불안반응의 생리, 생화학적 면에서 중추적인 역할을 한다는 생각을 할 수 있게 하는 결과라 할 수 있다.
홍삼사포닌분획의 항불안작용의 양상을 검색하고자 사포닌분획을 사용하여 실험 동물에서 항불안작용을 검색하고 이들의 작용을 benzodiazepine의 대표적 약물인 diazepam의 효과와 비교하여 보았다. 실험동물에서 항불안효과의 검정을 위하여 생쥐에게 각각 상이 한 단일성분 함량을 지닌 여러분획들을 투여 후 elevated plus maze법을 사용하여 항불안효과를 비교, 관찰하였다. 실험결과 홍삼 ginsenoside Rg1, Rf 및 Re 등의 함량이 보다 높은 TSI 분획에서 항불안 효과를 관찰하였다. 중추신경계에 작용하여 진정효과를 나타내는 약물 중 특히 항불안제의 작용에 관여하는 신경전달물질 수용체로 현재까지 가장 주목을 받고 있는 것은 GABA A-benzodiazepine 수용체-chloride통로 복합체에 있는 benzodiazepine 수용체이다. 본 실험결과 관찰한 홍삼 사포닌의 항불안작용의 기전 규명을 위한 접근방법으로 홍삼 사포닌의 benzodiazepine 수용체에 대한 수용체 결합력을 관찰하고자, 흰쥐의 대뇌 피질을 검체로 하여 benzodiazepine 수용체에 대한 [$^3H$] Ro15-1788 결합 실험을 실시하였다. 홍삼 사포닌 분획들은 benzodiazpine 수용체에 결합하는 반응성을 보였으며, ginsenoside Rb, Re 및 Rd등의 함량이 높은 TSIV 분획이 가장 높은 결합력을 나타내었는데, 이 분획에서는 항불안 작용을 관찰할 수 없었다. 이상의 결과에서 볼 때 홍삼사포닌은 항불안작용을 나타내었으며, 이 항불안작용과 benzodiazepine 수용체에 대한 결합력과의 연관성은 관찰되지 않았다.
목 적 : 스트레스 유발 고혈압을 일으키는데 말초성 benzodiazepine수용체가 중요한 역할을 하리라 추정되어 왔다. 반복적 부동화 스트레스에 의한 신장의 말초성 benzodiazepine수용체의 변화 양상을 Sprague-Dawley rats와 boderline hypertensive rats의 두 실험동물군에서 비교, 관찰하여 고혈압을 유발하는데 신장의 말초성 benzodiazepine 수용체의 병태생리학적 기능을 규명하고자 하였다. Benzodiazepine수용체의 변화 양상은 방사성 동위원소를 사용한 수용체 결합 반응으로 검색하였으며 elevated plus maze검사로 각 실험동물의 불안도를 측정하여 각 군간의 결과를 비교, 관찰하였다. 방 법 : 불안도를 보기 위하여 측정한 plus-maze performance에서 percent open crosses는 Sprague-Dawley rats ($34.7{\pm}2.2$)에 비해 boderline hypertensive rats ($16.2{\pm}1.7$)가 유의하게 낮았고(P<0.05), percent time in open도 Sprague-Dawley rats ($22.5{\pm}1.0$)에 비해 boderline hypertensive rats ($12.1{\pm}1.2$)가 유의하게 낮아 불안도가 높은 상태임을 나타내었다(P<0.05). 스트레스를 주지 않은 Sprague-Dawley rats의 신장 말초성 benzodiazepine수용체의 수(Bmax: $5.5{\pm}0.6$pmol/mg protein)에 비하여 boderline hypertensive rats의 수용체의 수($3.1{\pm}0.7$pmol/mg protein)는 유의하게 낮았다(P<0.05). 하루 2시간씩 14일간 부동화 스트레스를 부하하였을 때, Sprague-Dawley rats와 boderline hypertensive rats에서 신장의 말초성 benzodiazepine 수용체의 수($7.4{\pm}0.7$ 및 $5.9{\pm}1.2$ pmol/mg protein)는 스트레스를 주지 않았을 때보다 증가하였으며(P<0.05), 스트레스에 노출된 boderline hypertensive rats는 스트레스에 노출된 Sprague-Dawley rats에 비하여 신장 말초성 benzodiazepine수용체의 수가 여전히 낮은 수준임을 관찰할 수 있었다(P<0.05). 결 론 : 이상의 결과로부터 신장의 말초성 benzodiazepine수용체는 스트레스 조절작용을 매개하며, 본 수용체의 수적 감소는 스트레스에 의한 고혈압 발생에 중요한 역할을 할 것으로 생각되었다.
Etifoxine (etafenoxine, $Stresam^{(R)}$) is a non-benzodiazepine anxiolytic with an anticonvulsant effect. It was developed in the 1960s for anxiety disorders and is currently being studied for its ability to promote peripheral nerve healing and to treat chemotherapy-induced pain. In addition to being mediated by $GABA_A{\alpha}2$ receptors like benzodiazepines, etifoxine appears to produce anxiolytic effects directly by binding to ${\beta}2$ or ${\beta}3$ subunits of the $GABA_A$ receptor complex. It also modulates $GABA_A$ receptors indirectly via stimulation of neurosteroid production after etifoxine binds to the 18 kDa translocator protein (TSPO) of the outer mitochondrial membrane in the central and peripheral nervous systems, previously known as the peripheral benzodiazepine receptor (PBR). Therefore, the effects of etifoxine are not completely reversed by the benzodiazepine antagonist flumazenil. Etifoxine is used for various emotional and bodily reactions followed by anxiety. It is contraindicated in situations such as shock, severely impaired liver or kidney function, and severe respiratory failure. The average dosage is 150 mg per day for no more than 12 weeks. The most common adverse effect is drowsiness at the initial stage. It does not usually cause any withdrawal syndromes. In conclusion, etifoxine shows less adverse effects of anterograde amnesia, sedation, impaired psychomotor performance, and withdrawal syndromes than those of benzodiazepines. It potentiates $GABA_A$ receptor-function by a direct allosteric effect and by an indirect mechanism involving the activation of TSPO. It seems promising that non-benzodiazepine anxiolytics including etifoxine will replenish shortcomings of benzodiazepines and selective serotonin reuptake inhibitors according to animated studies related to TSPO.
The use of the classification and regression tree (CART) methodology was studied in a quantitative structure-activity relationship (QSAR) context on a data set consisting of the binding affinities of 39 imidazobenzodiazepines for the α1 benzodiazepine receptor. The 3-D structures of these compounds were optimized using HyperChem software with semiempirical AM1 optimization method. After optimization a set of 1481 zero-to three-dimentional descriptors was calculated for each molecule in the data set. The response (dependent variable) in the tree model consisted of the binding affinities of drugs. Three descriptors (two topological and one 3D-Morse descriptors) were applied in the final tree structure to describe the binding affinities. The mean relative error percent for the data set is 3.20%, compared with a previous model with mean relative error percent of 6.63%. To evaluate the predictive power of CART cross validation method was also performed.
