양성 종괴로 나타나는 후두 질환 (Unusual Benign Neoplasms of the Larynx)

  • Background and Objectives : Benign neoplasms of the larynx are rare, and papillomas account for approximately 90% of these neoplasms. Other benign neoplasms of the larynx are very rare and form a hetergenous group. We present clinical manifestations of unusual benign neoplasms based on our experiences and review of literatures. Materials and Method : We reviewed retrospectively the clinical records of 14 patients with benign neoplasms of the larynx, excluding papillomas, that were examined in our department during 11-year period from 1995 to 2006. Results : The presenting symptom was most commonly progressive dysphonia. Pathologic diagnosis revealed 5 cases of hemangioma, 3 granular cell tumor, 2 amyloidosis, 2 laryngocele, 1 schwannoma, 1 chondroma. Subsites of the neoplasms were 5 in true vocal cord, 3 in arytenoids, 2 in false vocal cord, 2 in supraglottis, and others were subglottis ; aryepiglottic fold. Treatment was surgical, by a external approach in 1 case of chondroma, and by laryngoscopic approach in other cases. In laryngoscopic approach, carbon dioxide laser was used in 10 cases. Postoperative course was satisfactory. Recurrence was encountered in I case of amyloidosis and revision operation was done 3 times. No recurrence was encountered in other cases. Conclusion : Uncommon benign neoplasms of the larynx require high index of suspicion and histological confirmation. Complete excision with an attempt to maintain normal structures generally results in cure.

보험의학적 악성도평가 (Differentiation between malignant and benign in terms of insurance claims)

  • In case of neoplasm claims, it is important to make a decision of differentiating malignant and benign. In Korean insurance market, there are many insurance products that cover cancer. In the insurance claims adjustment, differentiation between malignant and benign is according to histologic findings. However there are many neoplasms of bad clinical course in spite of benign histopathologic classification. In this article; astrocytoma, thymoma, gastrointestinal stromal tumor, colonic intramucosal carcinoma, gastric high grade adenoma/dysplasia, carcinoid tumor, MALT lymphoma, revision of Korean Classification of Disease-6th edition, and bladder tumors are reviewed in terms of differentiation between malignant and benign in the insurance claims. It may be helpful for claims staff to review important neoplasms in terms of differentiation between malignant and benign.

Sebaceous carcinoma arising from sebaceoma

  • Sebaceous neoplasms are rare adnexal tumors that can present a challenge to clinicians. Only four cases of sebaceous carcinoma with sebaceoma have been reported in the literature. Herein, we describe the case of a sebaceous carcinoma originating from a sebaceoma in a solitary nodule of the posterior neck. Immunohistochemically, the tumor cells were strongly positive for epithelial membrane antigen and p53. It is possible that adnexal carcinomas may arise from malignant transformation of their benign counterparts as well as de novo. Malignant transformation was likely in this case because the lesion was composed of distinct benign and malignant components, and the benign component showed the typical histopathological features of sebaceoma. This case underscores the fact that partial and superficial biopsies sometimes may not provide the correct diagnosis. If a surgeon suspects malignancy based on a clinical examination, then it is mandatory to perform a deep biopsy.

  • Benign fibrous histiocytoma (FH) is a benign tumor composed of fibroblasts and histiocytes in varying proportions. This tumor is usually found in adult extremities but rarely occurs in deep soft tissues of the oral cavity. As it is difficult to diagnose with physical and radiologic exams, deep benign FH can only be diagnosed by histopathology. We report a case of a 36-year-old female patient who came to our department with painless swelling in the right buccal mucosa. This case report reviews the clinical, radiological, and histological aspects of this tumor.

Benign osteoblastoma of the mandible: a case report

  • Osteoblastoma is a rare benign neoplasm formed by osteoid tissue and well-vascularized bone that occurs mainly in children and adolescents. It appears primarily in the long bones, vertebral column, and small bones of the hands and feet, and not typically in the skull and maxillary bones. The purpose of this study is to present the case of an 8-year-old girl with a diagnosis of right mandibular osteoblastoma and a review of the relevant literature. The goals of treatment were to preserve dental occlusion, masticatory function and facial symmetry while minimizing the effects on patient body image and quality of life. Osteoblastoma, although it is benign, can be aggressive, and its treatment will depend on the timing of diagnosis, size and location. Early diagnosis is essential to avoid not only radical surgery as in the case presented, but also to help minimize the risk of possible relapse and potential malignancy of a benign osteoblastoma.

주 타액선 종양에 대한 10년 간의 경험 (Major Salivary Glands Tumors:A 10-Year Experience)

  • Background and Objectives : This study reports our clinicopathological experiences of major salivary gland tumors. Materials and Methods : This study included 302 patients with major salivary gland tumors who had got the diagnosis from January 1995 through December 2004. Medical records were reviewed retrospectively. Results : We found 244 benign and 58 malignant major salivary gland tumors. Among 267 parotid tumors, the most common benign parotid tumor was pleomorphic adenoma. Mucoepidermoid carcinoma, adenoid cystic carcinoma, and acinic cell carcinoma were three most common parotid malignancies. Among 33 submandibular gland tumors, 20 cases were benign and 13 were malignant. There were one benign and one malignant sublingual gland tumors. The duration of symptom of submandibular gland tumors was longer than that of parotid tumors. Most patients presented with asymptomatic mass. Seventy-one percent of salivary gland malignancies underwent postoperative radiation therapy. Five-year disease free survival rate of parotid malignant tumors seemed to be higher than that of submandibular one, although there was no statistical significance. Conclusion : Malignancy rate of each salivary gland followed old axiom that it is inversely related with the size of gland. Submandibular gland tumor tends to be delayed to reach diagnosis. Clinicians must be alert about this finding because submandibular gland tumors are known to have poorer prognosis than parotid tumors.

Salivary Gland Tumors: A Clinicopathologic Study of 366 Cases in Southern Iran

  • Background: Salivary gland tumors (SGT) are one of the most complex human neoplasms, demonstrating variations in their clinicopathological profile related to racial and geographic differences. Few studies with large samples have been reported in Iran. We here investigated a large group of patients in southern Iran. Materials and Methods: In this retrospective study, all cases of primary epithelial salivary gland tumors, which had been recorded in a 5 years period from 2005-2009, were enrolled. Clinical data such as histopathologic type and site of the lesion as well as patients' age and gender were analyzed. Results: Data of 366 cases of SGTs were recorded. Pleomorphic adenoma (80.2%) and adenoid cystic carcinoma (46.6%) were the most common benign and malignant neoplasms. Male to female ratio (M/F) and the mean age of patients were 1:1.05 and 37.7 for benign tumors while they were 1.2:1 and 50.6 for malignant tumors, respectively. Parotid and minor salivary glands were involved more frequently. Conclusions: Although the salivary gland tumours encountered were similar in most of their characteristics to those reported in other countries, some differences such as relative frequency and age and gender prevalence were discovered. These findings should help surgeons and pathologists for more accurate diagnosis, management and treatment.

Primary Perivascular Epithelioid Cell Tumor of the Lung: A Case Report

  • Perivascular epithelioid cell tumors (PEComas) are very rare mesenchymal neoplasms, composed of histologically and immunohistochemically distinctive cells that form a sheet-like appearance around vessel lumens. Although most are benign, a malignant subset does exist, complicating clinical diagnostic efforts. Proper evaluation and management are thus essential for long-term patient survival. Herein, we present a rare case of a benign pulmonary PEComa that was diagnosed postoperatively in a 45-year-old woman.

자궁양성종양 환자의 고강도 초음파 집속술(HIFU) 치료 경험 (Experiences of High Intensity Focused Ultrasound (HIFU) Treatment in Benign Uterus Neoplasms)

  • 본 연구의 목적은 현상학적 접근을 통하여 고강도 초음파 치료를 받은 자궁양성종양 환자에 대한 경험의 본질을 심도 있게 파악하기 위해 이루어졌다. 연구 대상자는 자궁양성종양을 진단받고 고강도 초음파 집속술 치료를 받은 5명의 환자였다. 본 연구의 자료 수집은 2018년 2월부터 3월까지 이루어졌으며, 고강도 초음파 집속술을 시술하고 있는 D시 여성전문병원에서 시행되었다. 연구방법은 일대일 심층 면담을 통해 이루어졌으며, 자료는 비구조적이고 개방된 질문형태를 사용하여 자신의 의견과 경험을 자유롭고 유연하게 표현할 수 있도록 하였다. 인터뷰 자료는 Colaizzi의 현상학적 분석 방법을 사용하였다. 연구결과 5명의 대상자로부터 얻은 고강도 초음파 집속술 치료에 따른 건강문제는 총 3개의 범주로 구성되었고, 각각의 범주는 다시 하부 요인으로 구성되었다. 3개의 범주로는 '불편감', '양가감정', '의지와 요구' 로 나타났다. 본 연구는 고강도 초음파 치료를 받은 자궁양성종양 환자들의 경험을 이해하는데 도움을 줄 것이다. 나아가 본 결과는 향후 여성건강을 위한 새로운 치료법의 개발과 발전에 따라 올바른 간호학적 이해 및 고강도 초음파 치료를 받는 환자에 대한 지속적인 연구와 효과적인 간호 교육프로그램 개발이 이어져야 할 것이다.

췌장 양성 종양의 내시경적 치료 (Endoscopic Therapy for Pancreatic Benign Neoplasms)

  • 내시경초음파가 1980년대에 개발된 이후, 내시경초음파는 췌장 종양 관리에 있어서 진단적인 역할뿐만 아니라 치료적인 역할로 그 기능이 확대되고 있다. 췌장 양성 종양에 대한 전통적인 치료방법은 수술적 절제를 시행하거나 주기적인 경과관찰을 하는 것이었다. 그러나 내시경초음파는 그 대안으로 췌장 양성 종양을 가진 환자에게 최소 침습적인 치료법을 제공한다. 본고에서는 췌장 양성 종양치료에 있어서 내시경초음파의 역할에 대해 알아보고자 한다.
